Brexpiprazol je nové antipsychotikum ze skupiny parciálních receptorových agonistů dopaminu (DRPA). Prezentujeme sedm případových studií, které jsou příkladem vhodného využití brexpiprazolu v komplexní léčbě schizofrenie. Kromě účinku na pozitivní a negativní příznaky je nejenom lékaři, ale i pacienty oceňován při léčbě brexpiprazolem efekt na redukci hmotnosti, absence sedace, akatizie či neuroleptické dysforie, sociální oživení a návrat ke studijní či pracovní činnosti. Kazuistiky dokládají, že brexpiprazol není zaměnitelný s jinými antipsychotiky a znamená obohacení léčby, kterou lze dosáhnout funkční remise pacientů a podstatného zlepšení kvality života.
Brexpiprazole is a new antipsychotic that belongs to dopamine receptor partial agonists (DRPA). We present seven case studies that represent prosperous use of brexpiprazole in complex treatment of schizophrenia. Not only psychiatrists but patients appreciate weight loss, absence of sedation, akathisia, neuroleptic dysphoria and return to society, study or working activities together with improvement of positive and negative symptoms with brexpiprazole treatment. The case studies documented that brexpiprazole is not exchangeable with other antipsychotics. Brexpiprazole meants enrichment of treatment, because patients might reach the functional remision of schizophrenia and substantial quality of life improvement.

BACKGROUND: Since 2010, the Czech Republic has been experiencing a spontaneous phenomenon in which General Practitioner (GPs) practices have been transferred to a legal limited liability company as defined by the Commercial Code. OBJECTIVES: Our research attempted to ascertain the reasons that induced GPs to change their legal form, thereby opting for an entrepreneurial orientation (EO). A second objective was to measure the impact of this change on the healthcare performance of GP practices. METHODOLOGY/APPROACH: We used exploratory research to examine this phenomenon in healthcare service provision. Data from 131 questionnaires was evaluated using an exploratory survey with descriptive statistics. To assess the healthcare performance of GP practices, we used secondary data from the Institute of Health Information and Statistics of the Czech Republic. FINDINGS: The GPs interviewed considered their practices to be businesses. Their decision to change to a limited company form was influenced by the possibility of selling or transferring the business, employment sustainability and job security, and advantages related to cost structure and taxation. Our study shows that, in a generational exchange, the change in legal form enhances the sustainability and operation of the practice within the context of the current demands for high-quality healthcare.

The present study examines the hypothesis that adult attachment orientation, specifically anxious attachment, is related to children's diminished ability to self-regulate their food intake, and that this relationship is mediated by parents' persuasive-controlling feeding practices. Two hundred and sixty five mothers and fathers of preschool children completed online questionnaires that included measures of Adult Attachment Orientation, Parental Persuasive-Controlling Feeding Practices, and Child Self-Regulation of Eating. Structural equation modeling revealed a significant relationship between parental anxious attachment and child self-regulatory abilities, which was fully mediated by parental persuasive-controlling feeding. Also as predicted, parents' avoidant attachment was found to be unrelated to persuasive-controlling feeding and child self-regulated eating. Findings suggest that parents with an anxious attachment orientation may be more likely than other parents to try to use persuasive techniques to control their children's food intake, which may impair children's ability to regulate their food intake, increasing their obesity risk. Implications for intervention are discussed.
