Treatment with cyclosporin A in dermatology includes an ever increasing spectrum of diseases. Further indications include the use (along with corticoids) in the group of bullous diseases. A 57 year-old patient who was never sick except for a mild ethylic hepatopathy developed a major eruption of bullae (on a non-inflamed or inflamed backgroud) on the lower half of the trunk, beneath the knees, on the thighs and rarely on the forearms. The condition was complicated by the formation of extensive necrotic foci at the sites where the surface of the bullae was damaged and by the general alteration of the patient. The assumed diagnosis was bullous pemphigoid. The histological examination revealed the diagnosis more accvirately as pemphigus vulgaris. The treatment used was cyclosporin A, 5 mg/kg/day. Already on the fifth day dramatic improvement of the local finding was recorded. Laboratory monitoring revealed however a steady increase of creatinine levels. Therefore cyclosporm A was gradually discontinued for six weeks and the local finding was cured. Revision of the histological examination and of the final diagnosis were pemphigoid buUosus. After more than one year the patient has no signs of dermatosis and laboratory parameters are within normal limits.

Jedny z nejčastěji využívaných imunosupresiv v terapii myastenie jsou azathioprin (Imuran) a cyklosporin (Sandimmun Neoral, Equoral). Jedná se o velmi účinné léky, která významně zvyšují kvalitu života pacientů, ale mají i určitá rizika. Alespoň části z nich se dá předcházet nastavením správné dávky, což ale vyžaduje dostatečnou zkušenost. Cílem článku je poskytnout přehled nežádoucích účinků obou léků, hlavních metabolických interakcí, možností terapeutické monitorace a v neposlední řadě praktické doporučení pro zahájení terapie.
Azathioprin (Imuran) and ciclosporin (Sandimmun Neoral, Equoral) are among the most widely used immunosuppressive drugsfor myasthenia gravis. They are very efficient and help to improve patient's quality of life. However, such therapy has some risks. Atleast part of the risks can be avoided by managing the right dose, which requires certain experience. The aim of this article is a reviewof their side effects, metabolic pathways and interactions, therapeutic drug monitoring and some dosage recommendations.

During the long period of cyclosporine-containing dosage forms development a lot of significant findings have been done especially in the field of drug delivery systems. Currently available drugs are based, from technological point of view, on self-dispersible drug delivery systems, which contain cyclosporine solved in pharmaceutically acceptable vehicle. One can find difference among particular systems a) at particle size distribution after dispergation, b) at composition and c) at bioavailability of cyclosporine. As far as improvement of bioavailability between original brand leader formulation Sandimmune and recent brand leader formulation Neoral was followed by significant improvement in particle size distribution it was generally assumed that the reason for this improvement have been found. Several other formulations e.g. Consupren or SangCyA--self-dispersible systems, more or less correspond with above mentioned theory that smaller is better and by this principle bridged liquid based dosage forms with solid dosage forms. Bioavailability of novel drug delivery system which gives coarse dispersion with average particle size between 1-150 microns when dispersed have been tested on healthy volunteers. No difference among pharmacokinetic parameters of novel drug delivery system and microemulsion system have been observed in spite of fact that average particle size of novel system is almost 1000x bigger.
