syndrom krátkého střeva je hlavní příčinou chronického intestinálního selhání a představuje skupinu od mírné intestinální insuficience, kterou lze zvládnout dietními a farmakologickými postupy, až po velmi těžké formy, vyžadující domácí parenterální výživu. intestinální selhání je definováno jako redukce absorpční funkce trávicího systému pod nezbytné minimum potřebné ke vstřebání makronutrientů a/nebo elektrolytů a vody vyžadující parenterální suplementaci k udržení zdraví a růstu. Závažnost syndromu krátkého střeva závisí na etiologii, reziduální délce tenkého střeva základním onemocnění a anatomii po resekci střeva. dlouhodobá a domácí parenterální výživa je vitální metodou volby, ale má své závažné komplikace a je spojena se zhoršenou kvalitou života. léčba GlP-2 představuje nový koncept vedoucí k redukci domácí parenterální výživy a může vést k jejímu ukončení. intestinální rehabilitační chirurgie se vždy zvažuje u všech nemocných s cílem obnovení kontinuity trávícího traktu a zlepšení digestivní funkce. intestinální transplantace nebo multiviscerální transplantace je pak určena pro nemocné s komplikacemi domácí parenterální výživy a neřešitelnými abdominálními stavy.
short bowel syndrome is the main cause of chronic intestinal failure and represents a complex, from mild intestinal insufficiency that can be overcome with dietary and pharmacological treatment, to a severe condition required home parenteral nutrition support. intestinal failure is defined as the reduction of gut function below the minimum necessary for the absorption of macronutrient and/ or water and electrolytes, such that intravenous supplementation is required to maintain health and growth. the severity of short bowel syndrome is related to its etiology, remnant small bowel length, underlying disease and bowel anatomy after resection. long-term/ home parenteral nutrition is the vital and reference treatment for this condition that can lead to chronic complications in addition to a significantly low quality of life. however, GlP-2 treatment represents a new concept associated with parenteral nutrition reduction and in some cases even directed for weaning of Pn. intestinal rehabilitation surgery should be always proposed, that can restore digestive continuity and absorption. intestinal transplantation/ multivisceral transplantation is reserved for patients with life threatening complications of long-term parenteral nutrition or untreatable abdominal diseases.

Cílem tohoto sdělení je souhrn postupů u nemocných s chronickým intestinálním selháním. Syndrom krátkého střeva představuje nejčastější příčinu chronického selhání střeva. Etiologie vzniku SBS je rozmanitá a léčba zahrnuje komplexní mezioborový přístup. Pacienti často mají píštěle a stomie a jsou odkázáni na domácí parenterální výživu, jež je jako jediná schopna zvládnout těžkou malnutrici. Zásadním postupem je pokus o rekonstrukci zažívacího traktu s obnovením anatomické kontinuity. Pokud po rekonstrukci nedojde k independenci na domácí parenterální výživě, je na místě farmakologická léčba s cílem zvýšení resorpční kapacity střeva a provedení autologní střevní rekonstrukce. Při rozvoji komplikací domácí parenterální výživy je indikována transplantace střeva nebo multiviscerální transplantace, které se v posledních letech staly součástí léčby ireverzibilního střevního selhání.
This presentation aims to summarize the course of action in patients with chronic intestinal failure. Short bowel syndrome (SBS) is the most common cause of chronic bowel failure. The etiology of SBS is diverse. SBS treatment involves a complex multidisciplinary approach. Patients often have stomas and intestinal fistulas. They are very often dependent on home parenteral nutrition (HPN). HPN is the only way to manage their severe malnutrition. The most crucial process is attempting to reconstruct their digestive tract with the restoration of its anatomical continuity. If independence on HPN fails after digestive tract reconstruction, pharmacological treatment is used to increase intestine tract resorptive capacity. Furthermore, autologous intestinal reconstruction is planned. In recent years, intestinal or multi-visceral transplantation has become part of treating irreversible intestinal failure in patients who developed HPN complications.
Domácí parenterální výživa (DPV) je život zachraňující léčba v případech chronického intestinálního selhání různé etiologie. Hepatopatie indukovaná domácí parenterální výživou nebo hepatopatie spojená s intestinálním selháním (PNALD/IFALD) zahrnuje široké spektrum stavů od steatózy, cholestázy, ikteru až po konečné selhání jater (ESLD). Diagnóza a monitorace PNALD/ IFALD zahrnuje komplexní klinické, biochemické, radiologické a někdy histologické vyšetření. Důležitým postupem je vyloučení jiné příčiny jaterního poškození před zahájením DPV. Management péče je založen na multidisciplinárním přístupu zahrnujícím nutricionistu, hepatologa, radiologa a chirurga. ESLD s jaterním selháním je indikací k transplantaci tenkého střeva/multiviscerální transplantaci. Izolovaná transplantace střeva je indikací pro nemocné bez významné formy PNALD/IFALD. Cílem tohoto sdělení je problematika prevence, managementu a léčby IFALD.
Home parenteral nutrition (HPN) is life saving therapy for patients with chronic intestinal failure caused by different etiology. Parenteral nutrition associated liver disease or intestinal failure associated liver disease (PNALD/IFALD) emcompasses a wide range of disease from steatosis, cholestasis, jaundice to end stage liver disease (ESLD). Diagnosis and monitoring of liver disease requires a complex of clinical, biochemical, radiological and where appropriate histology examination. It is important that other causes of derranged liver function are excluded before HPN. The management of PNALD/IFALD is based on multidisciplinary cooperation of nutritionist, hepatologist, radiologist and surgeron. ESLD with overt liver failure is an indication for small bowel and liver transplatation/multiviceral transplantation. Isolated small bowel transplantion is indicated for mild PNALD/IFALD cases. The purpose of this article is to discuss the current data dealing with IFALD prevention management and therapy.

Úvod: Anastomotická pseudoaneurysmata (APSA) jsou pozdní komplikací cévních rekonstrukcí. Přesná prevalence APSA není známa, protože mnoho případů není v literatuře publikováno. Ruptura APSA je však spojena s vysokou morbiditou a mortalitou. Metoda: Operační výkon byl proveden v celkové anestezii. Byl proveden řez v levém třísle současně s resekcí nekrotické kůže nad APSA. APSA bylo pečlivě vypreparováno s potvrzením nálezu CT angiografie (CTA) - masivní trombotizované APSA vystupující z distální anastomózy aortofemorálního bypassu (AFB). APSA bylo plně resekováno. AFB byl částečně resekován a za pomoci protetického interpozitu prodloužen. Společná stehenní tepna byla podvázána pod tříselným vazem. Výsledky: Pacient byl propuštěn 7. pooperační den s nízkými parametry zánětu. V současné době máme pa- cienta ve sledování jedenáct měsíců. Pacient má průchodnou cévní rekonstrukci bez známek anastomotického APSA na Dopplerově ultrasonografii. Závěr: Nejčastějším místem výskytu APSA po AFB je femorální oblast. Endovaskulární léčbu lze použít pouze ve velmi vybraných případech s velmi vysokým rizikem komplikací souvisejících s endovaskulárními intervencemi.
Introduction: Anastomotic pseudoaneurysms (APSA) are late complications of vascular reconstructions. The precise prevalence of APSA is not known, as many cases are not published in the literature. However, the rupture of APSA is associated with high morbidity and mortality. Method: The procedure was performed under full anesthesia. An incision was made in the left groin resect- ing the necrotic skin above APSA. APSA was carefully dissected, confirming the CT angiography finding of a massive thrombosed APSA arising from the distal anastomosis of the aorto-femoral bypass (AFB). The APSA was fully resected. An extension of the AFB with the use of prosthetic vascular graft was performed. The common femoral artery was ligated below the inguinal ligament. Necrotic skin from the groin was resected, and the wound was closed with a patent vascular reconstruction. Results: The patient was discharged on the 7th postoperative day with low infection's parameters. Currently we have eleven months of follow-up on the patient. The patient has a patent vascular reconstruction with- out any signs of anastomotic APSA on Doppler's ultrasonography. Conclusion: The most frequent site of APSA occurrence after AFB is the femoral region. Endovascular treat- ment can be used only in very selected cases with a very high risk of complications related to the endovascular devices.
Úvod: Nádor z testikulárních germinálních buněk (GCT) je nejčastějším maligním solidním nádorem u mužů s nejvyšší incidencí mezi 15-45 lety. GCT prokázal rostoucí výskyt za posledních 30 let. Metoda: Operace byla provedena přes střední laparotomii. V břiše byla nalezena masivní retroperitoneální lymfadenopatie (RL). Intrarenální aorta (IA) a dolní dutá žíla (IVC) byly pečlivě vypreparovány a zcela zbaveny RL. Část levé renální žíly (LRV) byla suspektně infiltrovaná RL. Proto jsme provedli částečnou resekci LRV. Zbytková LRV byla krátká a reanastomóza na IVC nebyla možná. LRV byla reanastomozována na splenickou žílu. Vzhledem k velké velikosti resekovaného RL v kombinaci s blízkým kontaktem s IA a IVC byly do dutiny břišní vloženy chirurgické roušky pro vysoké riziko pooperačního krvácení. "Second look" byl proveden 24 hodin po zákroku a střední laparotomie byla uzavřena standardním způsobem. Výsledky: Pooperační období proběhlo bez komplikací. Pacient byl propuštěn osmý pooperační den s dobrými renálními funkcemi a dobrou perfuzí levé ledviny dle Dopplerovy ultrasonografie. Závěr: U pacientů s GCT a velkými retroperitoneálními masami je nutný multidisciplinární přístup kombinující chemoterapii a chirurgický zákrok, aby se významně zvýšil léčebný úspěch.
Introduction: Testicular germ cell tumour (GCT) is the most common malignant solid tumour among Caucasian men with the highest incidence between 15-45 years. GCT had shown an increasing incidence in the past 30 years. Method: The procedure was performed through the midline laparotomy. Massive retroperitoneal lymphadenopathy (RL) was found in the abdomen. The infrarenal aorta (IA) and inferior vena cava (IVC) were carefully dissected and entirely freed from the RL. Part of the left renal vein (LRV) showed suspicious infiltration by the RL. Therefore we performed a partial LRV resection. The residual LRV was short, the reanastomosing to the IVC was not possible. We reanastomosed the residual LRV to the splenic vein. Due to the large size of the resected RL combined with a close encounter with IA and IVC, surgical swabs were placed into the abdomen for high risk of postoperative bleeding. The second look was performed 24 hours after the procedure, and the midline laparotomy was closed in a standard manner. Results: Postoperative period was uneventful. The patient was discharged on the 8th postoperative day with good renal functions and good left kidney perfusion and drainage on Doppler's ultrasonography. Conclusion: A multidisciplinary approach combining chemotherapy and surgical intervention is needed in patients with GCT and large retroperitoneal masses in order to significantly increase the curative success.

Úvod: Chronická ohraničená ruptura břišního aneurysmatu (AAA) erodující do obratlů je relativně neobvyklá. Jedním z projevů kromě pulsující břišní rezistence může být nespecifická bolest v dolní části zad. Kazuistika: Sedmdesátiletý pacient, muž po pravém aortofemorálním protetickém bypassu (RAFB) před jedenácti lety, byl předán do našeho centra s nespecifickou bolestí dolní části zad, která trvala posledních 12 hodin. Výpočetní tomografická angiografie (CTA) odhalila prasklé AAA 57 × 63 mm erodující do obratle L4. U pacienta byla naplánována chirurgická intervence. Mikrobiologické vyšetření kultury z AAA následně prokázalo infekci Staphylococcus hominis. Po zákroku byla u pacienta zahájena třítýdenní antibiotická terapie linezolidem. Výsledky: V současné době máme tříměsíční sledování u pacienta bez známek infekce v protetické náhradě infrarenální aorty na základě ultrazvuku. Závěr: Bolest dolní části zad je častá lékařská diagnóza. Jejím důvodem však nemusí být vždy primární onemocnění páteře. Chronické prasklé AAA s erozí vertebrálních obratlů je neobvyklým projevem prasklého AAA.
Chronic contained rupture of the abdominal aneurysm (AAA) eroding into the vertebrae is relatively uncommon. One of its manifestations besides pulsatile abdominal mass can be unspecific low back pain. Case presentation: A 70-year-old male patient after right aorto-femoral prosthetic bypass (RAFB) eleven years ago was referred to our center with non-specific lower back pain lasting for the past 12 hours. Computed tomography angiography (CTA) revealed contained ruptured 57 × 63 mm AAA protruded into the L4 vertebra. The patient was scheduled for an open repair through a midline laparotomy. Microbiological examination from the AAA sack revealed positive culture of Staphylococcus hominis. After the procedure a three-week antibiotic treatment with Linezolid was started in the patient.
