V kazuistice prezentujeme případ velmi vzácného systémového onemocnění p-ANCA pozitivní nekrotizující vaskulitidy u 11letého dítěte. Jedná se o polyangiitis microscopica, která patří do skupiny ANCA vaskulitid. Onemocnění je vedle přítomnosti nespecifických příznaků charakterizované orgánovým postižením nejen ledvin s typickým histologickým obrazem, ale i plic. Současné postižení plic (tzv. pulmorenální syndrom) je prognosticky velmi závažné. V léčbě progresivního onemocnění se nejčastěji používá imunosuprese kortikoidy (prednison, metylprednisolon) nejčastěji v kombinaci s cytotoxickými léky (cyklofosfamid, azathioprin). Plazmaferéza je indikovaná v akutní fázi a při pulmorenálním syndromu. Kombinace rituximab+kortikoidy byla v roce 2011 schválena FDA v léčbě dospělých pacientů. Léčba je dlouhodobá vzhledem k častým relapsům onemocnění.
We present a very rare case of systemic disease p-ANCA positive necrotizing vasculitis in an 11-year-old child. Polyangiitis microscopica belongs to the group of ANCA vasculitis. The disease is characterized beside presence of non-specific symptoms with typical kidney involvement based on characteristic histological finding. Coincident lung and kidney involvement, the so called pulmorenal syndrome has much more poor prognosis. The most common treatment of progressive forms of the disease is based on immunosuppressants: corticosteroids (prednisolon, methylprednisolon) in combination with cytotoxic drugs (cyclophosphamide, azathioprine). Plasmapheresis is indicated in an acute progressive phase of the disease resistant to the immunosuppression and in case of pulmorenal syndrome. Combination of rituximab+corticosteroids has been proved by FDA in 2011 in the treatment in adulthood. The treatment should be long-lasting because of frequent relapses of the disease.

Renální biopsie (RB) se stala v posledních letech relativně bezpečnou metodou vyšetření renálního parenchymu. Přesto je třeba mít na paměti, že se jedná o invazívní zákrok, který může být zatížen závažnými vedlejšími účinky. RB je indikovaná v diagnostice onemocnění glomerulů i intersticia, je nezbytná pro posouzení aktivity renálního onemocnění, rozsahu změn renálního parenchymu, k rozhodnutí o zahájení dlouhodobé imunosupresivní terapie, k hodnocení úspěšnosti léčby, ke stanovení progrese renálního onemocnění atd. Sami máme zkušenosti se skoro 2000 renálními biopsiemi u dětí a dorostu. V následujícím přehledu shrnujeme výsledky 200 renálních biopsií provedených v posledních 4 letech s podrobnějším klinicko-histologickým rozborem; biopsie štěpů po transplantaci ledviny nejsou zahrnuty.

A mother and her son, both with tubulointerstitial nephritis and uveitis syndrome (TINU) are reported. The nephritis presented itself at 13 years in the mother and at 10 years in her son. Glomerular filtration (GFR) decreased in both, and renal biopsies confirmed the diagnosis. Nephritis preceded the onset of uveitis in both. Clinical course and renal function improved quickly on oral steroids in the boy. The mother's hyperazotemia decreased spontaneously (without steroids), but not to normal range, and remained stable for 35 years of follow-up. Local steroids due to recurrences of uveitis were repeatedly needed in both. We believe this is the first report on familial occurrence of inherited TINU syndrome in two generations.

BACKGROUND/AIMS: In adults, nighttime hypertension and hyperuricemia are new risk factors for progression of IgA nephropathy (IgAN). In children, nighttime blood pressure (BP) and serum uric acid have never been investigated. The aim of the study was to investigate nighttime BP and uric acid in children with IgAN. METHODS: Data on children with IgAN from two pediatric nephrology centers were retrospectively reviewed (renal biopsy - subclasses according Hass I-V, ambulatory blood pressure monitoring ABPM, serum uric acid, proteinuria). RESULTS: Twenty-eight untreated children with IgAN were included. Hypertension was diagnosed on the basis of ABPM in 54% of children, 50% were nondippers and 25% have isolated nighttime hypertension. The mean ambulatory BP was higher at nighttime than during daytime (systolic nighttime BP 1.11 +/- 0.79 SDS vs. daytime 0.59 +/- 0.79, diastolic nighttime BP 1.16 +/- 0.95 vs. daytime 0.52 +/- 1.10, p < 0.01 for systolic and p = 0.01 for diastolic). Children with severe histological subclasses (III-IV) tended to have higher prevalence of hypertension than children with mild subclasses (I-II), 67% vs. 38%, p = 0.13. Hyperuricemia was diagnosed in 14% of children. A significant correlation was found between proteinuria and histopathological subclasses (r = 0.44, p < 0.05). CONCLUSION: Children with IgAN have often nighttime hypertension. Hypertension and proteinuria are associated with severe histopathological findings. Hyperuricemia is a rare finding in children.

Idiopathic focal segmental glomerulosclerosis (FSGS) is believed to be caused by a circulating permeability factor. FSGS recurrence is common after transplantation. The treatment is still a matter of debate; plasmapheresis (PE) and immunoadsorption (IA) are often used. We report on PE and IA in the treatment of two children with recurrent nephrotic proteinuria. Patient 1 was a 16-year-old girl who had recurrence of nephrotic proteinuria on the first day after transplantation (proteinuria-19 g/d). Primary immunosuppressive therapy was changed to high-dose cyclosporine and cyclophosphamide; plasmapheresis was started on day 4. Altogether we performed 53 PE and 38 IA procedures. During the first month, PE procedures were performed with no more than a 2-day interval between sessions, and the girl achieved partial remission (proteinuria 3 g/d). PE was then stopped. After 2 months, a relapse of heavy proteinuria occurred. This relapse was successfully treated again with intensified PE treatment. After achieving remission, a chronic PE regimen was started (PE once a week), similar to the previous series. The child remained in partial remission. Seven months after renal transplantation, she was switched from PE to IA, because of severe hypoproteinemia. Graft biopsy performed at 4 months showed effacement of the foot processes. At the present time she has a good graft function and 3 g/d proteinuria. Patient 2 was a 13-year-old girl with FSGS since 9 years. On the second day after renal transplantation she developed nephrotic proteinuria (proteinuria-14 g/d), which was treated with 39 PE and 16 IA treatments. She went into complete remission on the intensified PE regimen, had one relapse, and was switched to chronic IA. Graft biopsy performed at 2 weeks after transplantation showed effacement of the foot processes. At the present time she has good graft function and low proteinuria (0.3 g/d). In conclusion, intensified PE or IA treatments induced remission of recurrent nephrotic range proteinuria. Chronic PE or IA can maintain patients with frequent relapses in long-term remission.
