Úvod: Disekce axily (ALND) představuje standardní postup zajišťující lokoregionální kontrolu při metastatickém postižení sentinelové uzliny (SN). Řada studií již dokazuje, že její vynechání nezhoršuje celkovou prognózu. Metody: Retrospektivně zpracovaný soubor 249 pacientek v 3letém období s biopsií sentinelové uzliny byl rozdělen do 2 skupin – s negativní, resp. pozitivní SN. Ve sledované skupině s pozitivní SN byly pacientky rozděleny s, resp. bez ALND a hodnoceny další nonsentinelové uzliny při ALND a onkologická léčba. Předmětem bylo srovnání, zda ALND a onkologická léčba ovlivní bezpříznakový interval (DFI) a celkové přežití (OS) u pacientek s pozitivní SN a zda budou vykazovat rozdíly s kontrolní skupinou. Hodnocením histopatologických a biologických vlastností primárního nádoru, charakteru metastázy v SN a počtu metastatických SN bylo snahou určit ty faktory, které lze využít jako prediktory pozitivity nonsentinelových uzlin. Výsledky: Pozitivní SN je zatížena vysokým procentem pozitivity nonsentinelových uzlin (63 %). Doplnění ALND neprodlužuje DFI ani DFS, není-li zajištěna lokoregionální kontrola pooperační radioterapií a systémovou léčbou, které ovšem dokážou zajistit srovnatelný DFI i DFS bez doplnění ALND. Pacientky s pozitivní SN s, resp. bez ALND dosahují 70, resp. 72 měsíců DFI, 5leté přežití 84 %, resp. 80 %. Za prediktory pozitivity nonsentinelových uzlin lze považovat tumory nad 2 cm, s vysokou proliferační aktivitou a vysokým gradem. Závěr: Na malém souboru lze dokladovat, že vynechání ALND při metastatickém postižení 1−2 SN bez ohledu na prognostické ukazatele tumoru neovlivňuje DFI ani DFS při následném onkologickém zajištění. Nádory s vysokým gradem, vysokou proliferační aktivitou a rozměrem nad 2 cm mají významně vyšší riziko postižení nonsentinelových uzlin. Vynechání ALND při pozitivní SN není v současnosti léčebnou metodou lege artis, pro vytvoření odborných směrnic jsou požadovány další studie na velkých souborech.
Introduction: Axillary lymph node dissection (ALND) is a standard procedure for locoregional control in metastatic sentinel lymph nodes (SN). A number of studies have provided evidence that avoiding ALND does not worsen the general prognosis. Methods: A group of 249 female patients with sentinel lymph node biopsy was analysed retrospectively for a 3-year period. The patients were divided into two groups – with non-metastatic SN and with metastatic SN. In the metastatic SN group, the patients were further divided into a group with ALND and a group without ALND, and additional lymph nodes (non-sentinel) in ALND and oncological treatment were evaluated. The goal was to find out whether ALND and oncological treatment affect the disease-free interval (DFI) and overall survival (OS) in the group of patients with metastatic SN and to compare the results with the control group. The histopathology and biology of the primary tumour, its size and the number of metastatic SN were subsequently evaluated as the factors that may be useful for predicting metastatic non-sentinel lymph node positivity. Results: There was a high risk of metastatic non-sentinel lymph nodes in the cases of metastatic SN (63%). Addition of ALND does not prolong either DFI or DFS without post-operative radiotherapy and systemic oncological treatment, both of which can provide a comparable length of DFI as well as DFS without ALND. Patients with metastatic SN with and without ALND had a DFI of 70 and 72 months, respectively, and a 5-year survival of 84% and 80%, respectively. Tumours over 2 cm, tumours with high proliferative activity and a high grade can be regarded as predictors of metastatic non-sentinel lymph nodes. Conclusion: In a small patient group it has been demonstrated that the avoidance of ALND in 1 or 2 metastatic SN, regardless of the prognostic factors, does not affect either DFI or DFS if adjuvant oncological treatment is administered. High-grade tumours, tumours with high proliferative activity and tumours larger than 2 cm carry a significantly higher risk of metastatic non-sentinel lymph nodes. Nowadays, the avoidance of ALND in metastatic SN is not a lege artis procedure; further large studies are needed to create scientific guidelines.

Cíl. Zhodnotit častost indikací zobrazení srdce a věnčitých tepen pomocí CT, MRI, PET a SPECT na jednom pracovišti. Metoda. Byla provedena retrospektivní analýza indikací neinvazivních zobrazovacích metod srdce na jednom pracovišti, které sdružuje pracoviště radiologická a pracoviště nukleární medicíny. Zhodnoceny byl jednak indikace, jednak absolutní poměr prováděných vyšetření. Výsledky. Nejčastěji prováděnou metodou je SPECT myokardu, která tvoří více než 64 % všech provedených vyšetření, CT tvoří méně než jednu čtvrtinu vyšetření, magnetická rezonance tvoří méně než 7 % vyšetření a PET méně než 1 %. Nejvýznamnějšími indikacemi je ischemická choroba srdeční, která zcela převládá v indikacích, a to nejen z důvodu velkého zastoupení SPECT vyšetření, ale i díky indikacím k CT. U vyšetření magnetické rezonance dominovaly indikace při podezření na myokarditidu. Závěr. Vyšetření srdce tvoří významnou část vyšetření prováděných na radiologicko-nukleárně-medicínském pracovišti. Počty vyšetření v souhrnu převyšují ve srovnatelných časových úsecích počet katetrizačních výkonů na srdci a věnčitých tepnách.
Aim. To evaluate the indications in cardiac imaging using CT, MRI, PET and SPECT performed in one department. Methods. The retrospective analysis of the indications in cardiac imaging in one performing department including radiology and nuclear medicine was done. The proper indications with the particular ratio of imaging methods such as CT, MRI, SPECT and PET were assessed. Results. Myocardial SPECT is the most frequent imaging modality of the heart, it forms more than 64% of all procedures; CT examinations count less than one quarter of imaging procedures, MRI less than 7% and PET less than 1%. The most important indication is the ischemic heart disease due to the leading number of SPECTs, also due to the indications of CT of the heart. The suspected myocarditis is the leading indication of MRI procedures. Conclusion. The heart imaging is the important part of all procedures performed in department including both radiology and nuclear medicine. The number of all no-invasive procedures exceeds the number of catheter-based examinations in interventional cardiology.

Cíl. Zhodnotit možnosti hybridního zobrazení a frekvenci používaných výkonů v detekci okultního muskuloskeletálního pyogenního zánětu jako příčiny sepse. Metoda. Retrospektivně byly zhodnocena hybridní zobrazení indikovaná během 3 let k detekci příčiny septického stavu nejasného původu, vyšetření byla vyhledána mezi všemi provedenými 18F-FDG-PET/CT a SPECT/CT. Výsledky. Bylo provedeno celkem 27 vyšetření 18F-FDG-PET/CT z indikace sepse nejasného původu, kdy v šesti případech byla nalezena spondylodiscitida, pyogenní artritida ve třech případech, svalový absces ve dvou případech a infekce endoprotézy ve čtyřech případech. Muskuloskeletální příčina sepse tvořila 55,5 % případů. Bylo provedeno celkem 16 vyšetření SPECT/CT, kdy v deseti případech byly použity techneciem značené fragmenty myších protilátek a v šesti případech leukocyty značené 99mTc-HMPAO. Ve dvou případech byla prokázána akutní spondylodiscitida, v jednom případě chronická spondylitida a v jednom případě mnohočetné abscesy svalů původem v infekci endoprotézy. Muskuloskeletální příčina tvořila 25 % nálezů. Závěr. Za dobu současného používání PET/CT a SPECT/CT v detekci okultních infekcí tvoří větší díl vyšetření pomocí 18F-FDG-PET/CT, neboť nabízí vícečetné výhody před zobrazeními leukocytárními.
Aim. To asses a role and frequency of indications of hybrid imaging methods in the detection of occult pyogenous musculoskeletal inflammation as a cause of the septic state of unknown origin. Method. The retrospective evaluation of the hybrid imaging indicated due a sepsis of unknown origin during three years was performed. All procedures of 18F-FDG-PET/CT and the SPECT/CT using labeled leucocytes were enrolled into the assessment. Results. 27 procedures of 18F-FDG-PET/ CT were performed due to the searching of the cause in cases of the sepsis of unknown origin; the spondylodiscitis was found in 6 cases, pyogenous arthritis in 3 cases, muscular abscesses in 2 cases and 4 cases of pyogenous infection of implanted joint prosthesis. Muskuloskeletal inflammations formed 55,5% of all findings. 16 procedures of SPECT/CT were performed, 10 using 99mTc-labeled antigranulocyte antibodies, and 6 using 99mTc-HMPAO labeled leukocytes. In 2 cases, the acute spondylodiscitis was found, one case of chronic spon-dylitis and one case of soft tissue abscesses due to the infection of endoprosthesis were found. Muskuloskeletal inflammations formed 25% of all findings. Conclusion. During the period of the parallel use of PET/CT and SPECT/CT in detection of occult infection, the majority of procedures were performed using 18F-FDG-PET/CT, offering multiple advantages over leukocyte imaging.

The integration of fluoro-deoxy-glucose positron emission tomography (PET) and computer tomography (CT) provides precise localization of the non-Hodgkin´s lymphoma lesions and increases specificity of the examination. Definition of integrated PET-CT system role in primary staging of various NHL subtypes and correlation the immunohistological and proliferation lymphoma markers with PET-CT results will be investigated. It will be assesed a contribution PET-CT imaging after 2-3 cycles and after the end oftherapy with regard to dose intensity of treatment, prognostic factors, achievement of conventional complete/partial remission and survival probability. The project will lead to decreasing of malignant lymphoma mortality due to complex, modern and well timed diagnostic and also to therapeutic effectiveness increasing and patient´s management individualization.
Integrace fluoro-deoxy-glukózové pozitronové emisní tomografie (PET) a počítačové tomografie umožňuje precizní lokalizaci patologických lézí u ne-hodgkinských lymfomů a zvyšuje specificitu vyšetření. Bude definována role integrovaného PET-CT systému v primárním stážování různých subtypů NHL a korelovány imunohistologické a proliferační markery NHL s PET-CT výsledky. Bude zkoumán přínos PET-CT vyšetření po 2-3 cyklech a po skončení terapie s přihlédnutím k dávkové intenzitě terapie, prognostickým faktorům, dosažení konvenčně definované remise choroby a pravděpodobnosti jejího trvání. Projekt povede ke snížení mortality NHL díky komplexnímu, modernímu, dynamickému a vhodně načasovanému posouzení prediktivních faktorů a ke zvýšení efektivity terapie díkyindividualizaci léčby nemocného.

AIM: Staging of head and neck tumors is one of the most difficult tasks in imaging techniques, due to the very complicated head and neck anatomy and serious problems with the differentiation of reactive enlarged lymph nodes and lymph nodes involved with metastases. The aim of the study was to evaluate the validity of the whole-body approach in the assessment of head and neck malignancies using (18)F-FDG-PET/CT. MATERIALS AND METHODS: The analysis of a group of 1750 consecutive whole-body procedures in all indications of (18)F-FDG-PET/CT was made according to: the presence of orofacial tumors; their histology; findings concerning the spread outside head and neck region; and findings concerning the primary staging or restaging. The examinations of head and neck tumors were performed after intravenous application of the (18)F-FDG and its accumulation for one hour. Drinking and speaking is restricted during this accumulation to prevent artificial muscle (18)F-FDG uptake and to minimize false positive findings. In our hospital, high resolution PET is followed by the sub-millimeter isotropic acquisition of CT data after intravenous application of an iodinated contrast material. The acquisitions of head and neck region and trunk are performed separately to obtain optimal resolution in both regions. RESULTS: 105 examinations of the orofacial tumors were performed on 87 patients in a group of 1750 consecutive PET/CT examinations. The ratio between primary staging and restaging was 3:7. The most frequent indications were carcinomas of the tongue (19 examinations) and carcinomas of the salivary glands (19 examinations). The metastatic spread of the tumor outside the region of the head and neck was noted in 12 cases. CONCLUSION: Our findings of distant metastases confirmed the importance of the use of whole-body PET/CT in this indication.

AIM: The aim of presented work is to evaluate the clinical value of (18)F-FDG-PET/CT in patients with fever of unknown origin (FUO) and to compare PET/CT finding with the results of the following investigation. MATERIAL AND METHOD: 48 patients (24 men, 24 women, mean age 57.6 years with range 15-89 years) underwent (18)F-FDG-PET/CT due to the fever of unknown origin. All examinations were performed using complex PET/CT protocol combined PET and whole diagnostic contrast enhanced CT with sub-millimeter spatial resolution (except patient with history of iodine hypersensitivity or sever renal impairment). CT data contained diagnostic images reconstructed with soft tissue and high-resolution algorithm. PET/CT finding were compared with results of biopsies, immunology, microbiology or autopsy. RESULTS: The cause of FUO was explained according to the PET/CT findings and followed investigations in 44 of 48 cases-18 cases of microbial infections, nine cases of autoimmune inflammations, four cases of non-infectious granulomatous diseases, eight cases of malignancies and five cases of proved immunity disorders were found. In 46 cases, the PET/CT interpretation was correct. Only in one case, the cause was overlooked and the uptake in atherosclerotic changes of arteries was misinterpreted as vasculitis in the other. The reached sensitivity was 97% (43/44), and specificity 75% (3/4) respectively. CONCLUSION: In patients with fever of unknown origin, (18)F-FDG-PET/CT might enable the detection of its cause.
