Obličej, akrální části končetin, nehty a sliznice považujeme z hlediska dermatoskopického vyšetření za „netypické“ lokalizace. Anatomická stavba kůže se v těchto oblastech liší, a proto i dermatoskopické nálezy jsou odlišné. Na obličeji starších pacientů od sebe často odlišujeme ploché seboroické veruky (senilní lentiga), pigmentované aktinické keratózy a lentigo maligna. Tato diferenciální diagnostika patří z pohledu dermatoskopie v klinické praxi k nejobtížnějším. V současné době se doporučuje tzv. „inverzní přístup“, kdy nejprve hledáme kritéria dermatoskopického obrazu typická pro seboroickou veruku či aktinickou keratózu (šupiny, bílé a rozšířené folikuly, erytém, hnědé linie v paralelním nebo retikulárním uspořádání, ostré ohraničení léze a struktury běžné pro seboroickou veruku). Při jejich absenci považujeme projev za rizikový s ohledem na lentigo maligna. V nejasných případech volíme biopsii a histopatologické vyšetření.
From the point of view of dermatoscopic examination, we consider the face, acral parts of the limb, nails and mucous membrane to be "atypical" locations. The anatomical structure of the skin differs in these areas, and therefore the dermatoscopic findings are also different. On the face of elderly patients, we often diferentiate flat seborrheic keratosis (senile lentigo), pigmented actinic keratosis and lentigo maligna. From the point of view of dermatoscopy, this differential diagnosis is one of the most difficult in clinical practice. Currently, the so-called "inverse approach" is recommended, where we look for dermatoscopic criteria typical of seborrheic keratosis or actinic keratosis (scales, white and enlarged follicles, erythema, brown lines in a parallel or reticular arrangement, sharp lesion boundaries and structures common to seborrheic keratosis). In their absence, we consider the manifestation to be risky with respect to lentigo maligna. In unclear cases, we choose biopsy and histopathological examination.

AIM: The evaluation of the trend in the occurrence of melanoma nodulare, melanoma superficiale, lentigo maligna and melanoma in situ in the period of 2002-2017 in East Bohemia region in the Czech Republic. We examine if the annual numbers of hours of sunshine could affect the number of patients with melanoma. METHOD: In the peridod of 2002-2017, altogether 2230 patients with new diagnosis of melanoma were examined. We studied 1) If there is some trend in the occurrence of lentigo maligna and melanoma in situ, melanoma superficiale, and melanoma nodulare and if there is a difference in the age of patients with this diagnosis (adjusted calculation of specific kind of melanomas and adjusted calculation of age). 2) If the annual numbers of hours of sunshine affect the trend in the occurrence of melanoma and if the annual numbers of hours of sunshine affect the body site of melanoma. RESULTS AND CONCLUSION: Our study confirmed that the number of patients with lentigo maligna and melanoma in situ had increased in East Bohemia region in the period of 2002-2017. The number of melanomas of nodular and superficial type does not increase. The total number of melanomas in this period does not increase either. No difference of the age of patients with melanoma nodulare, superficiale, lentigo maligna and melanoma in situ was confirmed. We confirmed no relation of the annual numbers of hours of sunshine to the number of melanoma and to the body site of melanoma.

AIM: To evaluate the occurrence of melanoma in the period 1996-2017 in East Bohemia region in the Czech Republic. METHOD: We studied the incidence of melanoma and the age of diagnosis (adjusted calculation) and the parameters such as histology, body site of lesions, the length of the duration of lesions in 2810 patients. RESULTS AND CONCLUSION: No change in the occurrence of melanoma and in age of melanoma during this period was found. The difference between men and women was not confirmed in histology, but the difference between men and women was confirmed in the body site of lesion and in the length of duration of lesion. No relation between the length of duration of lesions from which melanoma had originated and its histology was confirmed. The relation was confirmed between histology and body site of melanoma. The relation between the body site and the length of duration of previous lesions was confirmed also. The increasing occurrence of melanoma on the trunk according to the duration of the previous lesions was confirmed.

OBJECTIVES: Lentigo maligna is a common in situ malignancy in elderly patients, with a low risk of progression to an invasive tumor. Surgical treatment may be complicated or refused. Non-surgical treatment options (such as lasers) may therefore be needed. PATIENTS AND METHODS: We report on 17 patients treated with a 2940-nm Er:YAG (erbium-doped yttrium aluminum) laser for histopathologically confirmed lentigo maligna. The lesions were ablated with a 5 mm margin of adjacent skin under local anesthesia with a 3.5 mm overlapping spot, energy density of 6.5 J/cm2 and a repetition rate of 5 Hz. If clinically visible pigmentation was seen in the ablated area during the following three months, the lesion was re-treated. All patients were followed up for residual or recurrent tumors. RESULTS: We achieved clinical clearance in all 17 patients. There were three recurrences during the follow-up period (9, 30 and 36 months after laser therapy). All patients were satisfied with the treatment course and cosmetic outcome. The mean follow-up duration was 28 months. CONCLUSIONS: Laser ablation is an interesting alternative method for treatment of lentigo maligna. It can be used for selected cases where surgery is contraindicated, complicated or declined by the patient.

Východisko: Maligní melanom je nádorové onemocnění kůže, které vzniká nekontrolovanou proliferací melanocytů. Nejčastěji se objevuje na kůži, ale může postihnout i sliznice, meningy či oko. Část melanomů se vyvíjí z melanocytárního névu. Akrolentiginózní melanom je typ melanomu lokalizovaný na dlaních, chodidlech, prstech, pod nehty, a je nejčastějším typem melanomu u fototypu VI. Nejdůležitějším faktorem úspěšné léčby je včasná detekce a excize tumoru s histologickým stagingem. Klíčová je chirurgická léčba v počátečním stadiu onemocnění, mnoho pacientů, ve větší míře mužů, vyšetření u lékaře ale odkládá a přichází až s pokročilým onemocněním. Popis případu: Autoři popisují případ 59letého pacienta trpícího syndromem bílého pláště, kterému byla provedena amputace článku prstu ruky pro údajnou gangrénu. Následně došlo k výsevu hnědočerných nodulů na předloktí a paži. Histologické vyšetření prokázalo metastázy maligního melanomu. Až v této fázi pacient anamnesticky přiznal na špičce amputovaného prstu névus, ve kterém postupně vznikla ulcerace. Závěr: Případ demonstruje kombinaci několika nepříznivých faktorů, které vedly k oddálení stanovení diagnózy a zahájení terapie.
Background: Melanoma is a malignant skin disease. The tumor development is caused by an uncontrollable proliferation of melanocytes. The most common occurrence is on the skin, but melanoma may also develop on the mucous membrane, meninges, and eyes. Some melanomas develop from melanocytic nevus. Acral lentiginous melanoma occurs on palms, feet, fingers and under nails, and is the most common type of melanoma for phototype VI. The most important factor for successful treatment of malignant melanoma is an early detection, excision of the primary tumor and histological staging. Surgical treatment of an early-stage melanoma is a key to successful therapy; however, many patients (mostly men) do not seek medical attention before it istoo late. Case report: This case study presents a 59-year-old patient, who suffers from white coat syndrome and whose finger was amputated for alleged gangrene. Subsequently, brownish black nodules appeared across his arm. Histological examination proved metastases of malignant melanoma. It was only at this phase, when the patient admitted a nevus at the tip of his amputated finger, from which ulceration and gangrene gradually emerged. Conclusion: This case demonstrates a combination of multiple unfavorable factors, which led to delayed diagnosis and therapy. Maligní melanom je nádor s trvale narůstající incidencí. Na 100 000 obyvatel již připadá víc jak 20 nově hlášených melanomů ročně. V České republice stoupla incidence melanomu za posledních 30 let 4násobně. Tvoří asi 5 % všech zhoubných nádorů kůže a je jedním z nejzhoubnějších nádorů vůbec. Může se vyskytovat na kůži (v 90 %), na sliznicích (1 %, u asijské populace 5–8 %) nebo v oku (3–5 %). Základní léčebnou možností je chirurgická léčba, dřívější aktinoterapie a systémová chemoterapie (u metastatického onemocnění) je v dnešní době nahrazována biologickou léčbou.
Malignant melanoma is a tumour with a steadily increasing incidence. There are already more than 20 newly reported melanomas per 100,000 population a year. In the Czech Republic, the incidence of melanoma has increased four-fold in the last 30 years. It accounts for about 5% of all malignant skin cancers and is one of the most malignant tumours there is. It can occur on the skin (90%), the mucosae (1%; in the Asian population 5–8%), or in the eye (3–5%). The principal therapeutic option is surgical treatment; the previously used actinotherapy and systemic chemotherapy (in metastatic disease) have nowadays been replaced by biological therapy.
