Dexmedetomidin je účinný, vysoko selektívny a špecifický α2-adrenergný agonista, ktorý má sedatívny, analgetický a sympatolytický účinok. Vyznačuje sa vysokou selektivitou α2:α1 v pomere 1600 : 1. Hypnotický účinok dexmedetomidinu je sprostredkovaný hyperpolarizáciou noradrenergných neurónov lokalizovaných v locus coeruleus. Dexmedetomidin pôsobí cez s G-proteínom spriahnutý receptor, ktorý navodzuje inhibíciu adenylátcyklázy, čo v konečnom dôsledku vedie k poklesu tvorby cAMP. Výsledkom je inhibícia uvoľňovania noradrenalínu, ktorá vedie k poklesu excitácie, najmä v locus coeruleus. Dexmedetomidin zmierňuje neurokognitívnu dysfunkciu a znižuje prevalenciu delíria. Taktiež znižuje vyplavovanie noradrenalínu do systémového obehu, čím zlepšuje hemodynamickú stabilitu a pozitívne vplýva na zásobenie myokardu kyslíkom, čím sa podieľa na ochrane myokardu. Protektívny efekt α2-adrenergných agonistov na myokard spočíva vo zvýšení hladín cAMP, čo umožní adenozínom navodenú vazodilatáciu koronárnych artérií. α2-adrenergné agonisty zmierňujú stresovú odpoveď organizmu na chirurgickú intervenciu, a tým počas operácie srdca znižujú počet ischemických epizód a ich podanie je asociované so zníženým rizikom vzniku infarktu myokardu. Svojim protizápalovým účinkom, stabilizáciou sympatikového nervového systému a zmierňovaním ischemicko-reperfúzneho poškodenia sa dexmedetomidin vyznačuje ochranným účinkom na funkciu srdca, mozgu, obličiek a imunitného systému. Hoci má dexmedetomidin sedatívny, analgetický a anxiolytický účinok, na rozdiel od iných sedatív nespôsobuje útlm dýchania a poskytuje respiračnú stabilitu počas anestézie. Primárnym miestom pôsobenia α2-adrenergných agonistov je locus coeruleus a nie mozgová kôra, ako tomu je v prípade použitia GABA-mimetík. To je dôvod, prečo táto skupina účinných látok navodzuje odlišný typ sedácie v porovnaní s benzodiazepínmi a propofolom. Nezvyčajná subkortikálna forma sedácie navodená dexmedetomidinom sa vyznačuje rýchlou a ľahko dosiahnuteľnou stratou vedomia, ktorá pripomína prirodzený spánok.
Dexmedetomidine is a potent, highly selective and specific α2-adrenergic agonist with sedative, analgesic and sympatholytic effect. It is characterized by high selectivity of α2:α1 at a ratio of 1600:1. Dexmedetomidine acts through the G-protein coupled receptor, which induces inhibition of adenylate cyclase, which in turn leads to a decrease in cAMP. The result is inhibition of the release of norepinephrine, which leads to a decrease of excitation in particular in the locus coeruleus. It also reduces the release of norepinephrine into the systemic circulation, thereby improving the haemodynamic stability, and has a positive effect on the myocardial oxygen supply, which contributes to the protection of the myocardium. α2-adrenergic agonists alleviate the stress response of the organism to the surgical intervention, thus during heart surgery they reduce the number of ischemic episodes and their administration is associated with a decreased risk of myocardial infarction. By its anti-inflammatory effect, stabilization of the sympathetic nervous system and the mitigation of ischemia-reperfusion damage, dexmedetomidine is characterized by a protective effect on the function of the heart, brain, kidney and the immune system. Although dexmedetomidine has sedative, analgesic and anxiolytic effects, in contrast to other sedatives it does not cause respiratory depression and provides respiratory stability during anaesthesia. The primary site of action of α2-adrenergic agonists is the locus coeruleus and not the cortex, like it is in the case of GABA-mimetics. The unusual subcortical form of sedation induced by dexmedetomidine is characterized by quickly and easily reached loss of consciousness, which resembles natural sleep.

We assessed blood-brain barrier (BBB) disruption in early stage of photothrombotic focal cerebral ischemia in the rat. We specifically looked for contralateral changes in BBB permeability and tested the influence of two anesthetics on the results. Adult Wistar rats were randomly anesthetized with pentobarbital (PB) or ketamine-xylazine (KX). Rats received intravenously (i.v.) Rose Bengal followed by Evans Blue (EB). Stereotactically defined spots on denuded skull were irradiated by laser (532 nm) for 18 min. Twenty four hours later, rats were killed, brains perfused, fixated, sectioned and slices analyzed by fluorescence microscopy. Volume of necrosis and volume of EB-albumin extravasation were calculated. Evidence of BBB breakdown in remote brain areas was sought and compared to sham handled controls. BBB disruption was consistently present, frequently with EB-albumin accumulating cells. Total lesion volume did not significantly differ among groups (TLVPB=9.4±1.3 mm³ vs. TLVKX=8.3±2.1 mm³); same was true for the volume of necrosis (NVPB=5.1±0.7 mm³ vs. NVKX=6.3±1.9 mm³). However, volume of EB-albumin extravasation area was significantly smaller in KX group (EBEVPB=4.3±0.8 mm³ vs. EBEVKX=2.0±0.5 mm³; p=0.0293). Median background EB-fluorescence signal density was higher in PB group (p<0.0001). Furthermore, regional increase in EB-fluorescence was found in two animals in PB group. Our study shows that anesthesia with NMDA-antagonist ketamine and α2-adrenergic agonist xylazine may reduce BBB breakdown in photothrombosis. Pentobarbital anesthesia lead to increased BBB permeability in the contralateral hemisphere.

Mesenteric veins are more sensitive than arteries to the constrictor effects of sympathetic nerve stimulation and alpha-adrenoceptor agonists. We tested the hypothesis that alpha(1)- and alpha(2)-adrenoceptors interact to enhance adrenergic reactivity of mesenteric veins. We studied neurogenic and agonist-induced constrictions of mesenteric veins and arteries in vitro. Norepinephrine concentration-response curves were left-shifted in veins compared to arteries. UK 14,304 (0.01-1 microM, alpha(2)-adrenoceptor receptor agonist) did not constrict arteries or veins but enhanced constrictions and Ca(2+) signals mediated by alpha(1)-adrenoceptor stimulation in veins. Yohimbine (alpha(2)-adrenoceptor receptor antagonist) and MK912 (alpha(2C)-adrenoceptor receptor antagonist), but not alpha(2A)- or alpha(2B)-adrenoceptor antagonists, produced rightward shifts in norepinephrine concentration-response curves in veins. Pharmacological studies revealed that alpha(1D)-adrenoceptors mediate venous constrictions. Norepinephrine responses in veins from alpha(2C)-adrenoceptor knock-out (KO) mice were not different from wild type veins. Yohimbine inhibited norepinephrine constrictions in alpha(2C)-adrenoceptor KO veins suggesting that there is upregulation of other alpha(2)-adrenoceptors in alpha(2C)-KO mice. These data indicate that alpha(1D)- and alpha(2C)-adrenoceptors interact in veins but not in arteries. This interaction enhances venous adrenergic reactivity. Mesenteric vein-specific alpha(2)-adrenoceptor linked Ca(2+) and perhaps other signaling pathways account for enhanced venous adrenergic reactivity.
