Srdeční selhání je definováno jako komplexní klinický syndrom s typickými symptomy (dušnost, únava) a známkami (zvýšená náplň krčních žil, poslechový nález chrůpků nad plícemi a otoky končetin). Současná aktivace neurohumorální osy (sympatický nervový systém a systém renin-angiotenzin-aldosteron) s cílem kompenzovat nízký srdeční výdej vede k nepříznivé srdeční remodelaci a další progresi srdeční dysfunkce. Farmakologická léčba je zaměřena na inhibici neurohumorální osy a pomocí modulace systému natriuretických peptidů podporuje kontraregulační děje. Nové terapeutické postupy přináší zapojení inhibitorů SGLT2 transportérů, léků primárně určených k terapii diabetu mellitu. Jejich příznivý efekt na snížení celkové úmrtnosti a rozšíření farmakoterapie o tyto preparáty tak přináší nový pohled na patofyziologii srdečního selhání. Tento přehledový článek poskytuje informace o klasické patofyziologii chronického srdečního selhání s neurohumorální aktivací, ale zdůrazňuje také recentní poznatky o kontraregulačních dějích a významu inhibitorů SGLT2 s ohledem na moderní doporučovanou léčbu.
Heart failure is determined as a complex clinical syndrome with typical symptoms (dyspnea, fatigue) and signs (jugular vein distension, crackles on auscultation, and leg swelling). Simultaneous activation of the neurohumoral axis (sympathetic nervous system and system renin-angiotensin-aldosterone), aimed to compensate for low cardiac output, leads to inappropriate cardiac remodeling and heart dysfunction. The pharmacotherapy of heart failure inhibits the neurohumoral axis and supports counterregulatory actions by modulation of the natriuretic peptides system. New therapeutic options involve the inhibitors of SGLT2 transporters, which were primarily supposed to treat diabetes mellitus. Their benefit in reducing overall mortality and an extension of treatment possibilities with these drugs bring a new perspective to the pathophysiology of heart failure. This review provides information about classical neurohumoral activation and emphasizes current knowledge about counterregulatory pathways. Furthermore, it highlights the significance of SGLT2 inhibitors involved in the contemporary recommended treatment.

Moxonidin je perorální antihypertenzivum ze skupiny sympatolytik II. generace. U pacientů s mírnou až středně těžkou hypertenzí snižuje moxonidin krevní tlak (TK) stejně efektivně jako většina antihypertenziv první volby, pokud je použit v indikovaných případech v monoterapii, a je také účinnou doplňkovou léčbou v kombinaci s jinými antihypertenzivy. Zlepšuje metabolický profil u pacientů s hypertenzí a diabetes mellitus nebo zhoršenou glukózovou tolerancí, je velmi dobře snášen, má nízký potenciál pro lékové interakce a je podáván v jedné denní dávce. Moxonidin je dobrou volbou především v léčbě pacientů s těžšími formami hypertenze, zejména jako doplňková léčba u pacientů s metabolickým syndromem a nemocných s výraznější psychickou tenzí.
Moxonidine is an oral antihypertensive drug from the group of 2nd generation sympatholytics. In patients with mild to moderate hypertension, moxonidine lowers blood pressure (BP) as effectively as most first-line antihypertensives when used as monotherapy - if appropriate, and is also an effective adjunctive therapy in combination with other antihypertensives. It improves metabolic profile in patients with hypertension and diabetes mellitus or impaired glucose tolerance, is very well tolerated, has a low potential for drug interactions and is administered in a single daily dose. Thus, moxonidine is a good choice especially in the treatment of patients with more severe forms of hypertension, especially as adjunctive therapy in patients with metabolic syndrome or with mental stress.
Beta-blokátory v současné medicíně stále patří mezi velmi důležité léky v celé řadě indikací. Měli bychom ale zvažovat okolnost, že i mezi jednotlivými beta-blokátory existují rozdíly, které mohou ovlivnit výsledný léčebný efekt či nežádoucí účinky. Proto bychom se měli při jejich výběru zaměřovat hlavně na látky, které jsou vysoce beta-1-selektivní, mají dobré farmakokinetické vlastnosti a jejich účinek je potvrzený řadou klinických studií.
Current clinical practice continues to regard beta blockers as very important drugs in a number of indications. What should be taken into account, however, is that individual beta blockers vary and that the differences between them may infl uence the overall treatment effect or side effects. In choosing the appropriate beta blockers, we should focus on the beta-1 selective agents which have good pharmacokinetic properties and whose effi cacy has been proved by a whole range of clinical trials.

It has been suggested that sympathetic activity, measured as changes in electrical skin impedance (SI), can be used to assess the adequacy of general anesthesia. Our prospective study investigated if measurements of skin impedance can determine levels of sedation induced by midazolam. Twenty-seven patients scheduled for arthroscopy requiring general anesthesia were served as their own control. These were blinded to the order of injections by telling them that they will be randomly administered a placebo (saline) orsedative agent. A DM 3900 multimeter was used for SI measurements. The degree of sedation was measured using the modified Observer's Assessment of Alertness and Sedation (mOAAS) scale. Resting SI values were noted, and all participants were then administered the placebo followed 5 min later by midazolam 2 mg i.v. Five min after that, patients were administered standard general anesthesia with propofol, oxygen, nitrous oxide 60 %, and isoflurane 1 MAC via a laryngeal mask, and sufentanil 5 - 10 μg. SI significantly increased after administration of midazolam and induction of anesthesia. There were no significant differences between pre-administration (baseline) and placebo and end of surgery and end of anesthesia with closed eyes. There were highly significant differences (p<0.001) between pre-administration vs. midazolam, placebo vs. midazolam, pre-administration vs. induction of anesthesia. We found slight correlation between mOAAS and SI. There were no significant changes between the end of surgery and the end of anesthesia with closed eyes, but SI significantly decreased (p<0.01) after eyes opened.

Obesity is associated with increased sympathetic nervous system activation, possibly contributing to higher cardiovascular risk. The aim of this study was to assess the relationship between body adiposity and sympathoadrenergic contractions in rat isolated mesenteric arteries, and the modulatory effect of mesenteric perivascular adipose tissue (PVAT). Experiments were performed on male 38-week-old Wistar, Zucker lean (ZL) and Zucker diabetic fatty (ZDF) rats. Paired rings of isolated rat superior mesenteric arteries with or without PVAT were prepared and connected to a force-displacement transducer for the recording of isometric tension. Contractile responses were elicited by increasing doses of exogenous noradrenaline and by endogenous noradrenaline released during electrical stimulation of perivascular adrenergic nerves. In ZDF rats, mesenteric PVAT had marked anticontractile effect leading to significant reduction in adrenergic contractions of their superior mesenteric arteries; however, in arterial preparations without PVAT, obese rats showed significantly increased sensitivity in their contractile responses to adrenergic stimulation when compared to other rat groups. In Wistar rats, ranging in the level of body adiposity between ZL and ZDF rats, neurogenic contractions in arterial preparations with preserved PVAT were higher compared to those without PVAT. No vasomodulatory effect of PVAT was detected in mesenteric arteries from ZL rats. The results of this study indicate that the modulatory effect of mesenteric PVAT on arterial adrenergic contractions did not change in proportion with increasing adiposity; however, it could be influenced by the rat strain-specific distribution of sympathetic nerves between PVAT and the proper mesenteric arterial wall. In ZDF rats, characterized by higher vascular sympathetic tone, the mesenteric arteries might be specifically regulated by the anticontractile effect of PVAT, leading to higher mesenteric blood flow. This could be associated with hyperphagia and increased nutrient-induced mesenteric vasodilatation in this rat strain.

Odvrácenou stranou zvyšujícího se počtu implantovaných přístrojů je narůstající počet pacientů s akumulací výbojů pro komorové arytmie. Ovlivnění aktivity sympatiku pomocí blokády ganglion stellatum navigované ultrazvukem přináší možnost zasáhnout do klíčového spouštěcího a zřejmě i udržujícího mechanismu arytmie.
Malignant ventricular storm is a life-threatening condition. The increasing use of implantable cardioverter-defibrillators has significantly improved the related mortality. On the other hand, the increasing number of implanted devices has led to a growing number of patients with ventricular arrhythmias treated with shocks. Altering the activity of the sympathetic system with ultrasound-guided stellate ganglion blockade may target the crucial initiating as well as sustaining mechanisms of ventricular tachyarrhythmia.
Our previous studies demonstrated that chronic systemic blockade of renin-angiotensin system (RAS) lowered blood pressure (BP) of Ren-2 transgenic rats (TGR) by the attenuation of both angiotensin II-dependent and sympathetic vasoconstriction. Since systemic RAS blockade also inhibits brain RAS, we were interested which effects on these two types of vasoconstriction will have the central RAS blockade in hypertensive TGR rats. Adult male heterozygous TGR rats and their Hannover Sprague Dawley (HanSD) controls were subjected to chronic systemic or intracerebroventricular administration of either angiotensin type 1 receptor blocker losartan or direct renin inhibitor aliskiren for 4 weeks. Additional groups of TGR and HanSD rats were used for the evaluation of acute peripheral and brain effects of angiotensin II. Both chronic systemic and intracerebroventricular administrations of losartan or aliskiren normalized BP of TGR animals. BP effect of brain RAS blockade was based solely on the reduced sympathetic vasoconstriction, while systemic RAS blockade attenuated both angiotensin II-dependent and sympathetic vasoconstriction. Surprisingly, neither peripheral nor central pressor effects of acute angiotensin II administration were enhanced in TGR compared to HanSD rats. In conclusion, sympathoinhibition represents the main mechanism of BP reduction in heterozygous TGR rats subjected to chronic brain or systemic RAS blockade, while peripheral attenuation of angiotensin II-dependent vasoconstriction during systemic RAS blockade is less important. Our data suggest that the participation of angiotensin II in BP control of adult heterozygous TGR rats is shifted from peripheral vasoconstriction to central sympathoexcitation. Similar mechanisms cannot be excluded in human essential hypertension.

Ligands of auxiliary α2δ subunit of voltage-dependent calcium channels (VDCCs) decrease elevated L-type VDCCs surface expression in arterial myocytes and arterial constriction in spontaneously hypertensive rats (SHR). However, their effect on blood pressure (BP) is unclear. In this study, we investigated the hemodynamic response to acute and chronic administration of gabapentin, a ligand of auxiliary α2δ subunit of VDCCs, in adult SHR with established neurogenic hypertension. The acute gabapentin administration lowered BP and heart rate more in conscious SHR than Wistar-Kyoto rats. Both nifedipine (L-type VDCCs blocker) and ω-conotoxin GVIA (N-type VDCCs blocker) also decreased BP more in SHR, but only gabapentin and ω-conotoxin GVIA abolished the nitroprusside-induced reflex tachycardia of baroreceptor-heart rate control. Hypotensive effect of gabapentin was accompanied by a reduction of (1) plasma norepinephrine level, (2) depressor response to ganglionic blocker pentolinium, (3) power of low frequency component of systolic BP variability, and (4) pressor response of mesenteric vascular bed to periarterial nerve stimulation, suggesting the decrease of peripheral sympathetic nerve transmission. Moreover, gabapentin effects on BP and baroreflex were absent in sympathectomized rats. In conclusion, the acute (but not chronic) administration of gabapentin lowered BP more in SHR than in Wistar-Kyoto rats. Besides the known L-type VDCCs involvement in the vascular effect of gabapentin, our data revealed the important role of N-type VDCCs in acute gabapentin effect on sympathetic control of BP. Gabapentin-induced changes of sympathetic nerve transmission indicated major hemodynamic mechanism of the acute response to this drug.
