Alterations in PBP2a have been recognized in cefotaxime-resistant laboratory mutants and β-lactam-resistant clinical isolates of Streptococcus pneumoniae. DNA sequencing revealed fundamental differences between these two settings. Internal stop codons in pbp2a occurred in all three laboratory mutants analyzed, caused by a mutation in pbp2a of mutant C604, and tandem duplications within pbp2a resulting in premature stop codons in another two mutants C403 and C406. In contrast, mosaic PBP2a genes were observed in several penicillin-resistant clinical isolates from South Africa, the Czech Republic, Hungary, and in the clone Poland23F-16, with sequence blocks diverging from sensitive strains by over 4%. Most of these pbp2a variants except pbp2a from the South African strain contained sequences related to pbp2a of Streptococcus mitis B6, confirming that this species serves as reservoir for penicillin-resistance determinants.

AIMS: Critically ill patients undergoing aggressive fluid resuscitation and treated empirically with hydrosoluble time-dependent beta-lactam antibiotics are at risk for sub-therapeutic plasma concentrations. The aim of this study was to assess the impact of two covariates - creatinine clearance (Clcr) and cumulative fluid balance (CFB) on pharmacokinetics/pharmacodynamics (PK/PD) target attainment within a week of treatment with meropenem (ME) or piperacillin/tazobactam (PIP/TZB). METHODS: In this prospective observational pharmacokinetic (PK) study, 18 critically ill patients admitted to a surgical Intensive Care Unit (ICU) were enrolled. The primary PK/PD target was free antibiotic concentrations above MIC at 100% of the dosing interval (100%fT>MIC) to obtain maximum bactericidal activity. Drug concentration was measured using liquid chromatography-tandem mass spectrometry. RESULTS: The treatment of both 8 septic patients with IV extended ME dosing 2 g/3 h q8 h and 10 polytraumatized patients with IV intermittent PIP/TZB dosing 4.0/0.5 g q8 h was monitored. 8/18 patients (44%) manifested augmented renal clearence (ARC) where Clcr ≥130 mL/min/1.73 m2. Maximum changes were reported on days 2-3: the median positive CFB followed by the large median volume of distribution: Vdme=70.3 L (41.9-101.5), Vdpip = 46.8 L (39.7-60.0). 100%fTme>MIC was achieved in all patients on ME (aged ≥60 years), and only in two patients (non-ARC, aged ≥65 years) out of 10 on PIP/TZB. A mixed model analysis revealed positive relationship of CFBpip with Vdpip (P=0.021). CONCLUSION: Assuming that the positive correlation between CFB and Vd exists for piperacillin in the setting of the pathological state, then CFB should predict Vdpip across subjects at each and every time point.

The nucleotide (p)ppGpp is a key regulator of bacterial metabolism, growth, stress tolerance, and virulence. During amino acid starvation, the Escherichia coli (p)ppGpp synthetase RelA is activated by deacylated tRNA in the ribosomal A-site. An increase in (p)ppGpp is believed to drive the formation of antibiotic-tolerant persister cells, prompting the development of strategies to inhibit (p)ppGpp synthesis. We show that in a biochemical system from purified E. coli components, the antibiotic thiostrepton efficiently inhibits RelA activation by the A-site tRNA. In bacterial cultures, the ribosomal inhibitors thiostrepton, chloramphenicol, and tetracycline all efficiently abolish accumulation of (p)ppGpp induced by the Ile-tRNA synthetase inhibitor mupirocin. This abolishment, however, does not reduce the persister level. In contrast, the combination of dihydrofolate reductase inhibitor trimethoprim with mupirocin, tetracycline, or chloramphenicol leads to ampicillin tolerance. The effect is independent of RelA functionality, specific to β-lactams, and not observed with the fluoroquinolone norfloxacin. These results refine our understanding of (p)ppGpp's role in antibiotic tolerance and persistence and demonstrate unexpected drug interactions that lead to tolerance to bactericidal antibiotics.

OBJECTIVES: To describe the prevalence and diversity of IncX plasmids with antibiotic resistance genes in Enterobacteriaceae and to identify the most disseminated lineages of the plasmid family. METHODS: IncX plasmids were screened in 1894 Enterobacteriaceae isolates resistant to cefotaxime (2 mg/L) or with reduced susceptibility to ciprofloxacin (0.05 mg/L) obtained from various sources in five continents using PCR. IncX plasmid-harbouring isolates were identified using MALDI-TOF or biochemical tests, and screened for antibiotic resistance genes using PCR and sequencing; their clonality was determined by PFGE. Horizontal transfer of plasmids was tested using transformation and conjugation. IncX plasmids were characterized by S1-nuclease and PFGE, RFLP and hybridization. RESULTS: A total of 164 Escherichia coli isolates (8.7%, n = 1894) carried at least one IncX subgroup. Seven isolates harboured two distinct subgroups. IncX1 subgroup was found in 93 isolates, followed by IncX2 (35 isolates), IncX4 (28) and IncX3 (15). IncX4 plasmids were not transferred horizontally as single plasmids and therefore excluded from further analysis. The most disseminated lineages of IncX plasmids included IncX1 harbouring qnrS1 and blaTEM-1,-135 found in 36 E. coli from different sources in Europe and Australia and IncX2 carrying qnrS1 and tet(A) detected in nine E. coli from wildlife in Europe. IncX3 plasmids harboured predominantly blaSHV-12 and qnrS1 or qnrB7. CONCLUSIONS: IncX plasmids were widely distributed in E. coli from wildlife in Europe and were predominantly associated with fluoroquinolone resistance genes. Plasmids showing indistinguishable restriction profiles were identified in E. coli from different sources and countries suggesting wide dissemination of certain plasmid lineages.

V současné době se v důsledku vysoké rezistence bakteriálních patogenů k širokospektrým penicilinům a cefalosporinům zvyšuje aplikace karbapenemů. Cílem předložené práce byla analýza vztahu spotřeby karbapenemů a rezistence vybraných gramnegativních patogenů k meropenemu. Na základě lineární regresní analýzy byla prokázána statisticky významná závislost vztahu mezi spotřebou karbapenemů a rezistencí Pseudomonas aeruginosa.
As a result of high resistance of bacterial pathogens to broad-spectrum penicillins and cephalosporins, carbapenems have been increasingly used recently. The presented study aimed at analyzing the association between carbapenem consumption and resistance of selected Gram-negative pathogens to meropenem. Using linear regression analysis, a statistically significant association was found between carbapenem consumption and resistance of Pseudomonas aeruginosa.

Ertapenem je širokospektré baktericidní antibiotikum ze skupiny karbapenemů. Od ostatních látek této skupiny se odlišuje především absencí účinku vůči gramnegativním nefermentujícím tyčkám a dlouhým biologickým poločasem, který umožňuje jeho podávání jednou denně. Podává se v dávce 1 gram intravenózně jednou denně. Obvykle je dobře tolerován, jeho nejčastějšími nežádoucími účinky jsou průjem a flebitida v místě podání. Indikován je k léčbě komunitních pneumonií, intraabdominálních a gynekologických infekcí, infekcí kůže a měkkých tkání, včetně diabetické nohy.
Ertapenem is a broad-spectrum bactericidal carbapenem antibiotic. It differs from the other substances of this group by the absence of action against Gram-negative non-fermenting bacilli and by a long elimination half-hfe, which allows once-daily administration. It is administered once daily in a dose of 1 gram intravenously. Ertapenem is generally well-tolerated, with the most common side effects being diarrhea and phlebitis at the injection site. It is used for the treatment of community-acquired pneumonia, intra-abdominal and gynecological infections, and skin and soft tissue infections, including diabetic foot.

K velmi důležitým bakteriálním patogenům, především v případě nozokomiálních infekcí, patří kmeny Pseudomonas aeruginosa. Frekvence tohoto species jako původce infekcí, včetně život ohrožujících, se zvyšuje a současně stoupá jeho odolnost k účinku antibiotické léčby. Cílem předložené práce bylo stanovení rezistence Pseudomonas aeruginosa k antibiotikům a analýza souboru meropenem-rezistentních izolátů. V období od 1. 1. 2008 do 30. 6. 2013 bylo izolováno celkem 11 342 kmenů Pseudomonas aeruginosa z klinického materiálu pacientů hospitalizovaných ve Fakultní nemocnici Olomouc a 1 341 kmenů v případě pacientů Vojenské nemocnice Olomouc. Rezistence k antimikrobiálním přípravkům byla stanovena standardní diluční mikrometodou. U 57 meropenem-rezistentních kmenů Pseudomonas aeruginosa byla stanovena produkce metalo-beta-laktamáz, serinových karbapenemáz a AmpC enzymů fenotypovými i genotypovými metodami. Byla prokázána stoupající četnost kmenů Pseudomonas aeruginosa rezistentních k meropenemu a piperacilinu s tazobaktamem. U řady izolátů zůstala zachována citlivost pouze ke kolistinu a amikacinu. Produkce metalo-beta-laktamáz a serinových karbapenemáz nebyla prokázána a rezistence k meropenemu je podmíněna především produkcí AmpC a ztrátou stěnových porinů.
Pseudomonas aeruginosa strains are among very important bacterial pathogens, particularly in the case of nosocomial infections. The frequency of this species as a causative agent of infections, including life-threatening ones, is increasing, and so is its resistance to the effect of antibiotic therapy. The aim of the present paper was to determine the resistance of Pseudomonas aeruginosa to antibiotics and to analyze the group of meropenem-resistant isolates. In the period from 1 January 2008 to 30 June 2013, a total of 11,342 strains of Pseudomonas aeruginosa were isolated from the clinical material of patients hospitalized in the Olomouc University Hospital and another 1,341 strains were isolated in the case of patients from the Olomouc Military Hospital. Resistance to antimicrobial agents was assessed using the standard dilution micromethod. In 57 meropenem-resistant strains of Pseudomonas aeruginosa, the production of metallo-beta-lactamases, serine carbapenemases, and AmpC enzymes was determined by means of both phenotype and genotype methods. An increasing frequency of Pseudomonas aeruginosa strains resistant to meropenem and piperacillin with tazobactam was demonstrated. In a number of isolates, sensitivity only to colistin and amikacin remained preserved. The production of metallo-beta-lactamases and serin carbapenemases was not confirmed, and resistance to meropenem is particularly related to the production of AmpC and the loss of the cell wall porins.

In this study, extended-spectrum β-lactamase (ESBL)-producing Enterobacteriaceae isolates in children with malignancies hospitalised at a paediatric oncology department in the Czech Republic were investigated. From June 2009 to January 2010, a total of 50 ESBL-producing faecal isolates of Enterobacteriaceae were obtained from 28 patients. These isolates were characterised with regard to ESBL enzymes, plasmid-mediated quinolone resistance genes, multilocus sequence typing (MLST) and plasmids conferring resistance to cephalosporins and fluoroquinolones. ESBL-producing isolates included Klebsiella pneumoniae (n=36), Escherichia coli (n=7), Klebsiella oxytoca (n=3), Enterobacter cloacae (n=2) and Citrobacter freundii (n=2). Klebsiella pneumoniae isolates belonged to 7 MLST types, including sequence types ST280, ST321, ST323 and ST416 as well as the novel types ST626, ST627 and ST628. The multiresistant epidemic clone E. coli B2-O25b-ST131 was detected in one patient. The gene bla(CTX-M-15) was found on large conjugative IncFII(K) plasmids along with bla(TEM-1), bla(OXA-1), qnrB1, aac(6')-Ib-cr, strA, sul2, aac(3')-II and tet(A) genes in most isolates. Dissemination of IncFII(K) plasmids among various Enterobacteriaceae isolates was considered an important aspect of nosocomial colonisation in the wards by Enterobacteriaceae species producing ESBLs. This is the first study documenting multiple antibiotic resistance elements, including qnr genes, in IncFII(K) plasmids in various bacterial species isolated in a single hospital department. The results highlight the evolution of IncFII(K) plasmids into new variants containing novel antibiotic resistance elements and their important role in spreading ESBL-producing bacteria among hospitalised patients.
