Colorectal cancer, one of the most frequent types of cancer worldwide, has a high mortality rate. Irinotecan (CPT-11) has been approved for the treatment of advanced or metastatic disease either as a single agent or, more commonly, as part of combined chemotherapeutic regimens. Treatment with irinotecan is often accompanied by severe toxicity (e.g. neutropenia and diarrhea) that can result in treatment interruption or cessation, thus jeopardizing the patient's prognosis and quality of life. Irinotecan is bioactivated into its metabolite SN-38, which is subsequently detoxified by uridine diphosphate-glucuronosyl transferases (mainly UGT1A1). Further, ABC transporters (i.e. ABCB1, ABCC1-ABCC6, and ABCG2) are responsible for drug efflux into bile and urine whereas OATP transporters (SLCO1B1) enable its influx from blood into hepatocytes. Genetic polymorphisms in these enzymes/pumps may result in increased systemic SN-38 level, directly correlating with toxicity. Contemporary research is focused on the clinical implementation of genetic screenings for validated gene variations prior to treatment onset, allowing tailored individual doses or treatment regimens.

Drug-induced cardiotoxicity is a serious problem associated with the administration of many drugs. MicroRNAs (miRNAs) have been reported to be affected by drugs and other xenobiotics, and the potential of miRNAs as biomarkers and diagnostic tools has been considered. In recent years, an association of certain miRNAs with the cardiotoxicity of some drugs, namely anthracyclines, bevacizumab, cyclosporine A and isoprenaline, has already been found. This review article summarizes available information about the changes in miRNA levels induced by cardiotoxic drugs. Three aspects are discussed: the altered expression of miRNAs in the heart upon treatment with cardiotoxic drugs, circulating miRNAs as promising early biomarkers of cardiotoxicity, and the potential of miRNAs in the prevention and/or attenuation of drug-induced cardiotoxicity. The targeted changes in the level of certain miRNAs by antagomiRs and miRNA mimics are also described and evaluated. In addition, the cardioprotective mechanism of various natural compounds via their effect on miRNA levels are examined.

Haloperidol (HP) je látka ze skupiny incizivních antipsychotik (dříve označovaných neuroleptika). Je užíván k symptomatické léčbě schizofrenních, schizoafektivních a afektivních psychóz, k potlačení neklidu, agitovanosti a agresivity. Při dlouhodobém užívání se u pacientů objevovaly závažné vedlejší účinky. Jednalo se zejména o těžké postižení extrapyramidových nervových drah a následné poruchy mobility pacientů, tardivní dyskinezi (TD), prodloužení QT intervalu, výskyt arytmií a náhlé srdeční smrti. Bylo zjištěno, že v incidenci extrapyramidových poruch existují výrazné interindividuální rozdíly. Možným mechanismem cytotoxicity HP je působení oxidativního stresu. U pacientů s extrapyramidovými příznaky někdy dochází k vymizení příznaků po podání vitaminu E a jiných v tucích rozpustných antioxidantů, což je v souladu s teorií, že za vedlejší účinky HP je zodpovědná zejména peroxidace membránových lipidů. HP je antagonista dopaminu a jeho hlavním místem účinku je dopaminový receptor D2 (DRD2). HP nemá vliv pouze na signalizaci pomocí tohoto receptoru, ale ovlivňuje také jeho expresi a rovněž expresi dalších genů.
Haloperidol (HP) is the most potent neuroleptic agent which has been widely used not only in psychiatric patients but also in patients with other diagnoses, mainly geriatric and surgical, for the control of severe agitation, e.g. after general anaesthesia. Haloperidol was considered to be a safe drug, but unfortunately its therapeutic effects also come with severe extrapyramidal side effects, resulting in movement disorders, tardive dyskinesia, prolonging of the corrected QT interval which can cause severe arrhythmias such as Torsade de Pointes (TdP) and eventually result in sudden cardiac death. Large interindividual differences in the incidence of side effects have been demonstrated. The possible mechanism of HP cytotoxicity is an effect of oxidative stress, especially peroxidation of membrane lipids. Support for lipid peroxidative damage in patients with movement disorders comes from reports of amelioration of TD after treatment with vitamin E, or other lipid soluble antioxidants. HP is a dopamine antagonist and its major site of action is the dopamine D2 receptor (DRD2), which exhibits high affinity for the drug. It was found that HP affects not only DRD2 signalisation, but also the expression of DRD2 and other genes.

PURPOSE: Decreased 5-fluorouracil catabolism has been considered a major factor contributing to fluoropyrimidine (FP)-related toxicity. Alterations in the dihydropyrimidine dehydrogenase gene coding for the first and rate-limiting enzyme of FP catabolic pathway could explain toxicity in only a limited proportion of FP-treated patients. The importance of gene variants in dihydropyrimidinase (DPYS) coding for subsequent catabolic enzyme of FP degradation is not fully understood. METHODS: We performed genotyping of DPYS based on denaturing high-performance liquid chromatography in 113 cancer patients including 67 with severe FP-related toxicity and 46 without toxicity excellently tolerating FPs treatment. RESULTS: We detected nine DPYS variants including four located in non-coding sequence (c.-1T>C, IVS1+34C>G, IVS1-58T>C, and novel IVS4+11G>T), four silent (c.15G>A, c.216C>T, and novel c.105C>T and c.324C>A), and one novel missense variant c.1441C>T (p.R481W). All novel alterations were detected once only in patients without toxicity. The c.-1T>C and IVS1-58T>C variants were found to modify the risk of toxicity. The CC carriers of the c.-1C alleles were at higher risk of mucositis (OR = 4.13; 95% CI = 1.51-11.31; P = 0.006) and gastrointestinal toxicity (OR = 3.54; 95% CI = 1.59-7.88; P = 0.002), whereas the presence of the IVS1-58C allele decreased the risk of gastrointestinal toxicity (OR = 0.4; 95% CI = 0.17-0.93; P = 0.03) and leucopenia (OR = 0.29; 95% CI = 0.08-1.01; P = 0.05). CONCLUSIONS: Our results indicate that missense and nonsense variants in DPYS are infrequent, however, the development of serious primarily gastrointestinal toxicity could be influenced by non-coding DPYS sequence variants c.-1T>C and IVS1-58T>C.
