BACKGROUND: Overweight and obesity after retirement are likely to be caused by unhealthy eating habits and the energy intake exceeding the energy expenditure. OBJECTIVES: This study was designed to assess the effects of two 12-week interventions involving, respectively, either regular physical activity or a modified lower-calorie diet on the anthropometric parameters and blood lipid profiles in overweight and obese retired miners with lipid disorders. DESIGN: The study participants (n = 30, aged 58.7 ± 4.1 years, body height 174.8 ± 7.3 cm, body weight 96.6 ± 13.9 kg) were randomly assigned to 2 intervention groups: the Nordic walking group (NW), which exercised with intensity from 60 to 70% of participants' maximal heart rates for 1 h 3 times a week, and the modified diet group (MD). Modification of the diet consisted of reducing the daily energy intake by 30%, increasing the dietary content of mono- and polyunsaturated fatty acids and dietary fiber, and reducing the proportion of saturated fatty acids. The variables assessed at baseline and after 6 and 12 weeks were: anthropometric parameters (body weight, fat mass content [FM], fat percentage [BF], BMI, waist circumference [WC], hip circumference [HC], and waist-to-hip ratio [WHR]) and blood lipid indicators (total cholesterol [TC], triglycerides [TG], low density lipoprotein cholesterol [LDL-C], and high density lip-oprotein cholesterol [HDL-C]). RESULTS: The body weight of the participants in the NW was lower at week 12 by an average of 5 kg, BMI by 6%, FB by 19%, FM by 15%, WC by 8%, HC by 6%, and WHR by 3%. In the MD, the respective decreases were 8 kg and 8, 25, 20, 6, 2, and 7%. In the MD, the postintervention concentrations of TC and TG were within the reference range. CONCLUSION: Both 12-week interventions improved the anthropometric parameters and blood lipid profiles of retired heavy manual workers, with the improvements being more pronounced in the dieting group.

OBJECTIVE: Obesity is a multifactorial disease. This non-infectious epidemic has reached pandemic proportions in the 21 century. Posture is a dynamic process referring to an active maintenance of body movement segments against the action of external forces. The aim of the study was to investigate the effect of comprehensive group therapy for obese persons on selected anthropometric and postural parameters. METHODS: The study comprised 53 females with a mean age of 44.5 years (range 29–65 years, standard deviation 9.42 years, median 44 years), who completed a controlled weight loss programme. At the beginning and at the end of the programme, anthropometric parameters (Body Mass Index (BMI), weight and waist circumference) were measured and the posturography tests Limits of Stability (LOS) and Motor Control Test (MCT) were performed using the NeuroCom's SMART EquiTest system. The data were statistically analyzed using R software at a level of significance of 0.05. RESULTS: There were positive changes after the controlled weight loss programme in anthropometric parameters (BMI reduction, with p<0.001; waist circumference reduction, with p<0.001; and weight loss, with p<0.001), postural stability with statistically significant (p<0.05) improvements in both postural activity (LOS test parameters) and reactions (MCT parameters). CONCLUSION: The study showed a statistically significant effect of comprehensive group therapy for obesity in terms of reductions in waist circumference, body weight and BMI, and thus the overall reduction of both cardiovascular and metabolic risks, as well as improved postural skills (activity and reactions).

Cílem příspěvku je vymezení strategických východisek a návrh strategie preventivního působení na příslušníky romské minority Jihočeského kraje vedoucí ke snížení obezity a nadváhy. Výzkum kvantitativní povahy byl realizován formou řízeného polostrukturovaného rozhovoru týkajícího se jídelního chování respondentů, jejich psychického stavu, stresu, reflexe a sebereflexe, fyzické aktivity, kouření, konzumace alkoholu, socioekonomických podmínek respondentů, primární prevence i přístupu zdravotníků. Výzkumný výběrový soubor zahrnoval 600 respondentů (z toho 302 příslušníků romské minority a 298 osob z většinové populace). Výběrový soubor romské minority i majoritní populace lze považovat z hlediska pohlaví za reprezentativní. Byly zjištěny statisticky významné rozdíly mezi respondenty z minoritní a majoritní společnosti spojené s životním stylem příslušníků romské minority, které mohou způsobovat obezitu a nadváhu a následně ovlivňovat zdravotní stav jedinců. Jedná se o odlišnosti v jídelním chování včetně složení jídelníčku, fyzických aktivit, kouření nebo částce věnované nákupu potravin pro dospělého člena rodiny na měsíc. Na základě těchto zjištěných východisek – výsledků získaných v tomto reprezentativním výzkumném šetření patřícím v provedené SWOT analýze ke slabým stránkám životního stylu romské minority byl vytvořen návrh strategie pro preventivní působení u romské minority, jejímž cílem bude snížení rizikových faktorů ovlivňujících obezitu a nadváhu příslušníků romské minority žijících na území Jihočeského kraje.
The aim of this paper is to define strategic resources and strategy design of preventive measures for the Roma minority in the South Bohemian Region to reduce obesity and overweight. Quantitative research was conducted via a controlled semi-structured interview concerning the eating behaviour of the respondents, their mental state, stress, self-reflection, physical activity, smoking, alcohol consumption, socioeconomic condition, primary prevention and the approach of medical professionals. The research sample included 600 respondents (302 from the Roma minority and 298 from the majority population). The sample of the Roma minority and the majority population can be considered representative in terms of gender. There were statistically significant differences between the respondents from the minority and the majority population. The lifestyle of the Roma minority can cause obesity and overweight, and consequently influence the health of individuals. These are differences in eating behaviour, including the composition of diet, physical activity, smoking, or the money spent on food for an adult family member per month. The results obtained in this representative research survey within the SWOT analysis, which represents the weaknesses of the life style of the Roma minority, laid the foundations of a strategy for preventive measures for the Roma minority, which aims to reduce the risk factors affecting obesity and overweight in the Roma minority living in the South Bohemian Region.

Předkládaná studie se zabývala vlivem edukace v oblasti pohybových aktivit (PA) na modulaci zdraví. Za výzkumnou techniku byl zvolen experiment, do kterého bylo zahrnuto celkem 54 záměrně vybraných probandů ve věkovém rozmezí 20–71 let. Výzkumná skupina byla rozdělena na skupinu experimentální, která čítala 20 probandů, a skupinu kontrolní, do které spadalo 34 probandů. V rámci šestiměsíčního výzkumu vykonávali probandi experimentální skupiny předem definovanou PA. Všichni účastníci výzkumného šetření byli podrobeni analýze složení lidského těla bioelektrickou impedancí před začátkem a po ukončení experimentu. Konkrétně byly hodnoceny ukazatele obsahu tělesného tuku, aktivní tělesné hmoty jak s obsahem vody, tak bezvodé, bazální metabolismus a obsah vody. Z výsledků je patrný statisticky významný vzestup aktivní tělesné hmoty u experimentální skupiny oproti skupině kontrolní. Příznivé výsledky jsou patrné i v oblasti bazálního metabolismu, kde po cílené intervenci došlo u experimentální skupiny k jeho zvýšení. V hodnotách BMI a obsahu tělesného tuku došlo k předpokládanému snížení hodnot po šesti měsících. Lze tedy konstatovat, že i velmi obecná edukace sestry v oblasti PA má signifikantně pozitivní dopad na složení lidského těla.
The submitted study dealt with the influence of education in the area of motor activities (MA) on the modulation of health. The selected research method was an experiment which included a total of 54 purposely selected respondents from the age of 20–71 years. The experimental group was divided into an experimental group of 20 respondents and a control group of 34 respondents. Within 6 months of the research the respondents of the experimental group performed the predefined MA. All participants of the research survey underwent an analysis of human body constitution by means of bioelectric impedance, before and after the experiment. The assessed indices were the content of body fat, active body mass with water content and without water content, basal metabolism and water content. The results show a statistically significant increase of active body mass in the experimental group compared with the control group. Favourable results are also found in the area of basal metabolism, which increased after the targeted intervention in the experimental group. The values of BMI and the content of body fat decreased after six months as predicted. It can also be stated that a very general education of nurses in the area of MA has a significant and positive impact on the constitution of the human body.

Zdravá strava je důležitá pro udržování celkového zdraví a pocitu celkové spokojenosti, prevenci chronických onemocnění, optimalizaci očekávané délky života a klinickou péči o prakticky všechny chorobné stavy. Dietní mýty (tj. představy týkající se výživy, které nejsou podloženy vědeckými důkazy nebo jim odporují) mohou být překážkou pro konzumaci zdravé stravy. Dietní mýty existují v oblasti mikronutrientů, makronutrientů, nevýživových složek a energie získávané z potravy. Příklady mýtů z každé z výše uvedených skupin zahrnují tvrzení, že se pacienti musejí zaměřit na příjem dostatečného množství kalcia, aby měli zdravé kosti, že tuk v potravě vede k obezitě a má škodlivý vliv na zdraví cévních stěn, že veškerá vláknina (ať už přirozeně se vyskytující, nebo dodaná uměle) je prospěšná a že energie přijatá stravou se lineárním vztahem s jednoduchou aritmetikou přeměňuje na kilogramy tělesné hmotnosti. Běžnou oblastí dietních mýtů jsou některé názory výživových poradců, které zdůrazňují vybrané složky stravy v protikladu ke stravě jako celku. Doporučení týkající se zdravé stravy představují celostnější pohled; odpovídající důkazy podporují doporučení omezit příjem výrazně technologicky upravených potravin a naopak doporučují konzumaci minimálně zpracovaných potravin, obecně ve formě, která se co nejvíce blíží té, v jaké se potrava vyskytuje v přírodě. Praktičtí lékaři mohou být pacientům nápomocni vyvracením těchto mýtů a poskytováním rozumných rad týkajících se stravy s tím, že se zaměří na současnou stravu a širší výživové modely.
Healthy dietary intake is important for the maintenance of general health and wellness, the prevention of chronic illness, the optimization of life expectancy, and the clinical management of virtually all disease states. Dietary myths (i.e., concepts about nutrition that are poorly supported or contradicted by scientific evidence) may stand in the way of healthy dietary intake. Dietary myths exist about micronutrients, macronutrients, non-nutrients, and food energy. Representative myths of each type include following: that patients need to focus on consuming enough calcium to ensure bone health, dietary fat leads to obesity and is detrimental to vascular health, all fiber (whether naturally occurring or artificially added) is beneficial, and food calories translate to pounds of body weight through a linear relationship and simple arithmetic. A common theme for dietary myths is a reductionist view of diet that emphasizes selected food constituents as opposed to whole foods. Healthy dietary advice takes a more holistic view; consistent evidence supports recommendations to limit the consumption of ultraprocessed foods and to eat whole or minimally processed foods, generally in a form that is as close to what occurs in nature as possible. Family physicians can help dispel myths for patients and give sound nutritional advice by focusing on actual foods and broader dietary patterns.

Elevated levels of glucocorticoids lead to the development of obesity and metabolic syndrome. Local glucocorticoid levels are regulated through the enzyme 11beta-hydroxysteroid dehydrogenase 1 (11beta-HSD 1), an enzyme that regenerates active cortisol from inert cortisone. Increased expression of 11beta-HSD 1 in adipose tissue promotes higher body mass index (BMI), insulin resistance, hypertension, and dyslipidemia. Human 11beta-HSD 1 is also responsible for inter-conversion of 7-hydroxylate metabolites of dehydroepiandrosterone (7-OH-DHEA) to their 7-oxo-form. To better understanding the mechanism of the action, we focused on 7-OH- and 7-oxo-DHEA, and their circulating levels during the reductive treatment in adolescent obese patients. We determined plasma levels of 7alpha-OH-DHEA, 7beta-OH-DHEA, and 7-oxo-DHEA in 55 adolescent patients aged 13.04-15.67 years, BMI greater than 90th percentile. Samples were collected before and after one month of reductive therapy. Circulating levels of 7alpha-OH-DHEA decreased during the reductive therapy from 1.727 (1.614; 1.854, transformed mean with 95 % confidence interval) to 1.530 nmol/l (1.435; 1.637, p<0.05) in girls and from 1.704 (1.583; 1.842) to 1.540 nmol/l (1.435; 1.659, p<0.05) in boys. With regard to the level of 7-oxo-DHEA, a significant reduction from 1.132 (1.044; 1.231) to 0.918 nmol/l (0.844; 1.000, p<0.05) was found after the treatment, but only in boys. No significant difference in 7beta-OH-DHEA levels was observed. In conclusions, diminished levels of 7alpha-OH-DHEA indicate its possible effect on activity of 11beta-HSD 1. Further studies are necessary to clarify whether competitive substrates for 11beta-HSD 1 such as 7alpha-OH-DHEA could inhibit production of glucocorticoids and may be involved in metabolic processes leading to reduction of obesity.
