Immunoglobulin light chain amyloidosis (ALA) is a haematological disorder and may coexist with other plasma cell (PC) dyscrasias. It has been reported that 10% of ALA patients may have multiple myeloma (MM) at the same time. Nevertheless, it is still unclear if such clinical manifestation is defined by aberrant PC clones architecture or subsequent clone evolution. Due to the development in next generation sequencing (NGS) technology, it is possible now to model clonal differentiation during tumour progression. We suspect that, presence of different ALA and MM clones can be explained as a result of clone diversification during disease progression (branched evolution). Alternatively, severe genomic changes in aberrant PC can cause forming of new unique clone responsible for mentioned clinical manifestation (linear evolution). Learning outcomes of the offered project would mean certainly progress in the terms of diagnosis and subsequent treatment of ALA patients.
Amyloidóza lehkých řetězců imunoglobulinů (ALA) je hematologické onemocnění, které se může vyskytovat současně s jinými dyskraziemi plazmatických buněk (PC). Udává se, že zhruba 10% pacientů s ALA má současně i mnohočetný myelom (MM). Přesto je stále nejasné, zda je tato klinická manifestace způsobena evolucí pouze jednoho aberantního klonu plazmatických buněk či koexistencí více aberantních klonů plazmatických buněk současně. Díky rozvoji techniky sekvenování nové generace (NGS) je dnes možné modelovat diferenciaci buněčných klonů v průběhu progrese onemocnění. Domníváme se, že přítomnost různých klonů ALA a MM lze chápat jako výsledek diverzifikace jednoho klonu během progrese nemoci (rozvětvená evoluce). Popřípadě mohou kritické genomické změny v aberantních plazmatických buňkách dát vzniknout novému unikátnímu klonu (lineární evoluce), který je odpovědný za klinickou manifestaci onemocnění. Získané poznatky tohoto projektu budou znamenat zásadní posun ve stanovení diagnózy onemocnění a volbě následné léčby.

AIMS: Amyloidosis is caused by deposition of abnormal protein fibrils, leading to damage of organ function. Hereditary amyloidosis represents a monogenic disease caused by germline mutations in 11 amyloidogenic precursor protein genes. One of the important but non-specific symptoms of amyloidosis is hypertrophic cardiomyopathy. Diagnostics of hereditary amyloidosis is complicated and the real cause can remain overlooked. We aimed to design hereditary amyloidosis gene panel and to introduce new next-generation sequencing (NGS) approach to investigate hereditary amyloidosis in a cohort of patients with hypertrophic cardiomyopathy of unknown significance. METHODS: Design of target enrichment DNA library preparation using Haloplex Custom Kit containing 11 amyloidogenic genes was followed by MiSeq Illumina sequencing and bioinformatics identification of germline variants using tool VarScan in a cohort of 40 patients. RESULTS: We present design of NGS panel for 11 genes (TTR, FGA, APOA1, APOA2, LYZ, GSN, CST3, PRNP, APP, B2M, ITM2B) connected to various forms of amyloidosis. We detected one mutation, which is responsible for hereditary amyloidosis. Some other single nucleotide variants are so far undescribed or rare variants or represent common polymorphisms in European population. CONCLUSIONS: We report one positive case of hereditary amyloidosis in a cohort of patients with hypertrophic cardiomyopathy of unknown significance and set up first panel for NGS in hereditary amyloidosis. This work may facilitate successful implementation of the NGS method by other researchers or clinicians and may improve the diagnostic process after validation.

Proteasome inhibitors are the backbone in the treatment of multiple myeloma with 3 of its representatives (bortezomib, carfilzomib, and ixazomib) having already been approved. There is a different situation altogether in the treatment of amyloid light chain (AL) amyloidosis where owing to the rarity of this entity neither of these drugs has currently gained approval. Amyloid light chain plasma cells are possibly more vulnerable to bortezomib than myeloma plasmocytes because of a slightly distinct mechanism of action, which is described in depth in this manuscript. Bortezomib is highly active and rapidly effective as a single agent and even more potent in combination with dexamethasone and alkylators. Bortezomib-based regimens have become a standard part of the initial treatment of AL amyloidosis in the majority of centers. We have reviewed all available data on bortezomib in various combinations and settings. Carfilzomib seems to be effective but also toxic in these fragile patients with a high rate of cardiac events. Oral ixazomib has shown a surprisingly high efficacy with manageable toxicity and has received the Food and Drug Administration Breakthrough Therapy designation in 2014 for relapsed AL amyloidosis patients. In this review we have comprehensively described the current available knowledge of these 3 proteasome inhibitors and their use in AL amyloidosis.
Amyloidóza lehkých řetězců imunoglobulinů (AL amyloidosis – ALA) je monoklonální gamapatie charakteristická přítomností aberantních plazmatických buněk produkujících amyloidogenní lehké řetězce imunoglobulinů. To vede k tvorbě amyloidních fibril v cílových orgánech a tkáních, především v srdci a ledvinách, což způsobuje jejich dysfunkci. Jelikož tvorba amyloidních depozit je nezvratný proces, je kladeno velké úsilí k nalezení biomarkeru, který by odlišil ALA od ostatních monoklonálních gamapatií v časném stadiu onemocnění, kdy amyloidní depozita ještě nemají fatální následky. Vysoce výkonné technologie přinášejí nové možnosti v rámci moderního výzkumu nádorů, jelikož umožňují studovat nemoc v rámci jeho komplexnosti. Moderní metody, jako jsou sekvenování nové generace, genové expresní profilování a profilování cirkulujících mikroRNA u aberantních buněk ALA pacientů a příbuzných onemocnění patří mezi nové přístupy využívané ke studiu aberantních plazmatických buněk amyloidózy lehkých řetězců a jiných příbuzných onemocnění. Zatímco obecně známé mutace u pacientů s mnohočetným myelomem (KRAS, NRAS, MYC, TP53) nebyly u ALA pacientů nalezeny, počet mutovaných genů u jednotlivých diagnóz není rozdílný. Transkriptom ALA pacientů se jeví být podobnější pacientům s monoklonální gamapatií nejasného významu, a zároveň exprese cirkulujících mikroRNA, pro které je známá korelace s poškozením srdce je zvýšená právě u ALA pacientů, u nichž je poškození srdce typickým projevem.
Immunoglobulin light chain amyloidosis (AL amyloidosis – ALA) is a monoclonal gammopathy characterized by presence of aberrant plasma cells producing amyloidogenic immunoglobulin light chains. This leads to formation of amyloid fibrils in various organs and tissues, mainly in heart and kidney, and causes their dysfunction. As amyloid depositing in target organs is irreversible, there is a big effort to identify biomarker that could help to distinguish ALA from other monoclonal gammopathies in the early stages of disease, when amyloid deposits are not fatal yet. High throughput technologies bring new opportunities to modern cancer research as they enable to study disease within its complexity. Sophisticated methods such as next generation sequencing, gene expression profiling and circulating microRNA profiling are new approaches to study aberrant plasma cells from patients with light chain amyloidosis and related diseases. While generally known mutation in multiple myeloma patients (KRAS, NRAS, MYC, TP53) were not found in ALA, number of mutated genes is comparable. Transcriptome of ALA patients proves to be more similar to monoclonal gammopathy of undetermined significance patients, moreover level of circulating microRNA, that are known to correlate with heart damage, is increased in ALA patients, where heart damage in ALA typical symptom.

BACKGROUND: Aldehyde dehydrogenase (ALDH) is highly active in physiological stem cells as well as in tumor-initiating cells of some malignancies including multiple myeloma (MM). Finding higher activity of ALDH in some cell subsets in monoclonal gammopathies (MG) could identify potential source of myeloma-initiating cells (MICs). METHODS: Bone marrow of 12 MM, 9 monoclonal gammopathy of undetermined significance (MGUS), and 10 healthy donors (HD) were analyzed by flow cytometry. ALDH activity of B-cells and plasma cells (PC) was analyzed using Aldefluor. RESULTS: Similar changes of ALDH activity were found during B-cell development in HD and MG. Decreasing of ALDH activity from immature to naïve B-cells was found. In postgerminal stages, the activity started to increase, and in PCs, the ALDH activity was the same as in immature B-cells. Increased ALDH activity of all PC subsets compared to naïve B-cells was found in MM as well as in HD, while in MGUS, only CD19(-) PCs have higher ALDH activity. In HD, ALDH activity was higher in CD19(+) PCs compared with MG. CONCLUSIONS: Our results indicate that changes of ALDH activity are the natural phenomenon in B-cell development; thus, high ALDH activity as a single marker is not appropriate for MICs identification.

AIMS: Some types of monoclonal gammopathies are typified by a very limited availability of aberrant cells. Modern research use high throughput technologies and an integrated approach for detailed characterisation of abnormal cells. This strategy requires relatively high amounts of starting material which cannot be obtained from every diagnosis without causing inconvenience to the patient. The aim of this methodological paper is to reflect our long experience with laboratory work and describe the best protocols for sample collection, sorting and further preprocessing in terms of the available number of cells and intended downstream application in monoclonal gammopathies research. Potential pitfalls are also discussed. METHODS: Comparison and optimisation of freezing and sorting protocols for plasma cells in monoclonal gammopathies, followed by testing of various nucleic acid isolation and amplification techniques to establish a guideline for sample processing in haemato-oncology research. RESULTS: We show the average numbers of aberrant cells that can be obtained from various monoclonal gammopathies (monoclonal gammopathy of undetermined significance/light chain amyloidosis/multiple myeloma (MM)/MM circulating plasma cells/ minimal residual disease MM-10 123/22 846/305 501/68 641/4000 aberrant plasma cells of 48/30/10/16/37×106 bone marrow mononuclear cells) and the expected yield of nucleic acids provided from multiple isolation kits (DNA/RNA yield from 1 to 200×103 cells was 2.14-427/0.12-123 ng). CONCLUSIONS: Tested kits for parallel isolation deliver outputs comparable with kits specialised for just one type of molecule. We also present our positive experience with the whole genome amplification method, which can serve as a very powerful tool to gain complex information from a very small cell population.

Monoklonální protilátky jsou standardní součástí léčby celé řady onkologických onemocnění, u mnohočetného myelomu byly však po dlouhou dobu neúspěšné. Daratumumab v monoterapii byl schválen v roce 2015 americkou FDA na základě bezprecedentních výsledků u značně předléčené skupiny pacientů, a stal se tak novou nadějí pro tyto nemocné. Následně byl v roce 2016 schválen i v kombinaci s lenalidomidem a dexametazonem nebo s bortezomibem a dexametazonem v prvním relapsu onemocnění. Jako prakticky jediný nežádoucí účinek byly pozorovány tzv. reakce spojené s infuzí, které se objevily u cca poloviny léčených. Vzhledem k tomu, že mnohočetný myelom je typický přítomností paraproteinu (monoklonální protilátky) a že CD38 je všudypřítomný antigen, vyvstaly nečekané potíže, a to zejména pro pracovníky klinických laboratoří. V tomto souhrnném sdělení jsou rozebrány situace, které lze očekávat při podání daratumumabu, stejně jako jejich řešení. Interference s elektroforézou a imunofixací může komplikovat hodnocení léčebné odpovědi, interference v transfuzním lékařství může způsobit potíže s včasným výdejem kompatibilních transfuzních přípravků a problémy lze rovněž očekávat při hodnocení minimální reziduální nemoci pomocí flow cytometrie. Vzhledem k potenciálu daratumumabu lze předpokládat jeho široké použití, a je proto nutné počítat s jeho specifickými efekty. Rovněž je zásadní informovat své kolegy z klinických laboratoří o léčbě daratumumabem u konkrétního pacienta.
Monoclonal antibodies represent a standard part in the treatment of oncologic patients, but their efficacy in multiple myeloma used to be unsatisfactory. Daratumumab monotherapy was approved by the American FDA in 2015, after unprecedented results were obtained in a heavily pre-treated group of patients. In 2016 daratumumab was approved in combination with lenalidomide and dexamethasone, or bortezomib and dexamethasone, for the treatment of myeloma patients who have received at least one prior therapy.The toxicity of the drug is low, and is dominated by infusion-related reactions in more or less half of patients. The development as well as the management of these sometimes urgent reactions is described in depth in this review. As multiple myeloma is characterized by the presence of paraprotein (monoclonal antibody) and CD38 is a ubiquitous antigen, several unexpected complications have been reported during the administration of the drug. In this review, we aim to describe and offer some solutions for the complications that may be encountered during daratumumab treatment, such as interference with serum protein electrophoresis and immunofixation assays that may confuse the assessment of the hematological response, interference with blood compatibility testing that may cause a delay in the delivery of compatible transfusions, and difficulties that may occur in flow cytometric analysis of minimal residual disease. Because of the high activity of daratumumab and its expected widespread use, clinicians should be aware of its side effects and their management. It is also very important to inform colleagues in clinical laboratories about the initiation of daratumumab treatment in particular patient.

Multiple myeloma (MM) is composed of an array of multiple clones, each potentially associated with different clinical behavior. Previous studies focused on clinical implication of centrosome amplification (CA) in MM show contradictory results. It seems that the role of CA as well as CA formation in MM differ from other malignancies. This has brought about a question about the role of CA positive clone which is--is it going to be a more aggressive clone evolutionally arising under pressure of negative conditions or can CA serve as a marker of cell abnormality and lead to cell death and further elimination of this damaged subpopulation? This current review is devoted to the discussion of the existence of MM subclones with centrosome amplification (CA), its evolutionary behaviour within intraclonal heterogeneity as well as its potential impact on the disease progression and MM treatment.

Monoclonal antibodies (mAbs) are currently the most investigated therapeutic compounds in oncology, but there is no monoclonal antibody approved in the treatment of multiple myeloma (MM). Nevertheless several really promising molecules are under investigation in phase III clinical trials. Dominantly daratumumab (anti-CD38) and elotuzumab (anti-CS1) showed extraordinary effectiveness in phase I/II trials. The toxicity was acceptable which is important for their addition to standard anti-myeloma agents like proteasome inhibitors or immunomodulatory drugs. Monoclonal antibodies such as denosumab (anti-RANKL) or BHQ880 (anti-DKK-1) are investigated also in the management of myeloma bone disease. This review is focused on the most promising mAbs, their mechanisms of action and the rationale of use. Practically all available results have been described. If the ongoing trials confirm the efficacy and safety of mAbs, they would become an important part of MM treatment that would be translated in the further improvement of therapeutic outcomes.

Immunoglobulin light chain amyloidosis (ALA) is a plasma cell dyscrasia characterized by deposition of amyloid fibrils in various organs and tissues. The current paper is devoted to clarify if ALA has a unique gene expression profile and to its pathogenetic argumentation. The meta-analysis of ALA patients vs. healthy donors, monoclonal gammopathy of undetermined significance, smoldering and multiple myeloma patients' cohorts have revealed molecular signature of ALA consists of 256 genes representing mostly ribosomal proteins and immunoglobulin regions. This signature appears pathogenetically supported and elucidates for the first time the role of ribosome dysfunction in ALA. In summary of our findings with literature overview, we hypothesize that ALA development is associated not only with changes in genes, coding amyloidogenic protein itself, but with post-transcriptional disbalance as well. Based on our data analysis in ALA, ribosome machinery is impaired and the affected link mainly involves translational initiation, elongation and co-translational protein folding.
