The aim of the article is to present a novel method for fuzzy medical image retrieval (FMIR) using vector quantization (VQ) with fuzzy signatures in conjunction with fuzzy S-trees. In past times, a task of similar pictures searching was not based on searching for similar content (e.g. shapes, colour) of the pictures but on the picture name. There exist some methods for the same purpose, but there is still some space for development of more efficient methods. The proposed image retrieval system is used for finding similar images, in our case in the medical area - in mammography, in addition to the creation of the list of similar images - cases. The created list is used for assessing the nature of the finding - whether the medical finding is malignant or benign. The suggested method is compared to the method using Normalized Compression Distance (NCD) instead of fuzzy signatures and fuzzy S-tree. The method with NCD is useful for the creation of the list of similar cases for malignancy assessment, but it is not able to capture the area of interest in the image. The proposed method is going to be added to the complex decision support system to help to determine appropriate healthcare according to the experiences of similar, previous cases.

BACKGROUND: Leadless cardiac pacemakers have emerged as a safe and effective alternative to conventional transvenous single-chamber ventricular pacemakers. Herein, we report a multicenter experience on the feasibility and safety of acute retrieval (<6 weeks) and chronic retrieval (>6 weeks) of the leadless cardiac pacemaker in humans. METHODS AND RESULTS: This study included patients enrolled in 3 multicenter trials, who received a leadless cardiac pacemaker implant and who subsequently underwent a device removal attempt. The overall leadless pacemaker retrieval success rate was 94%: for patients whose leadless cardiac pacemaker had been implanted for <6 weeks (acute retrieval cohort), complete retrieval was achieved in 100% (n=5/5); for those implanted for ≥ 6 weeks (chronic retrieval cohort), retrieval was achieved in 91% (n=10/11) of patients. The mean duration of time from implant to retrieval attempt was 346 days (range, 88-1188 days) in the chronic retrieval cohort, and nearly two thirds (n=7; 63%) had been implanted for >6 months before the retrieval attempt. There were no procedure-related adverse events at 30 days post retrieval procedure. CONCLUSIONS: This multicenter experience demonstrated the feasibility and safety of retrieving a chronically implanted single-chamber (right ventricle) active fixation leadless pacemaker. Over the course of history, human beings have never stopped seeking effective methods for information storage. From rocks to paper, and through the past several decades of using computer disks, USB sticks, and on to the thin silicon "chips" and "cloud" storage of today, it would seem that we have reached an era of efficiency for managing innumerable and ever-expanding data. Astonishingly, when tracing this technological path, one realizes that our ancient methods of informational storage far outlast paper (10,000 vs. 1,000 years, respectively), let alone the computer-based memory devices that only last, on average, 5 to 25 years. During this time of fast-paced information generation, it becomes increasingly difficult for current storage methods to retain such massive amounts of data, and to maintain appropriate speeds with which to retrieve it, especially when in demand by a large number of users. Others have proposed that DNA-based information storage provides a way forward for information retention as a result of its temporal stability. It is now evident that DNA represents a potentially economical and sustainable mechanism for storing information, as demonstrated by its decoding from a 700,000 year-old horse genome. The fact that the human genome is present in a cell, containing also the varied mitochondrial genome, indicates DNA's great potential for large data storage in a 'smaller' space.
BACKGROUND: The growing use of leadless pacemaker (LP) technology requires safe and effective solutions for retrieving and removing these devices over the long term. OBJECTIVES: This study sought to evaluate retrieval and removal of an active helix-fixation LP studied in worldwide regulatory clinical trials. METHODS: Subjects enrolled in the LEADLESS II phase 1 investigational device exemption, LEADLESS Observational, or LEADLESS Japan trials with an attempted LP retrieval at least 6 weeks postimplantation were included. The retrieval success rate was evaluated for all attempted retrievals. Site-reported complications associated with LP retrievals were quantified, with the severity and relationship adjudicated by an independent clinical events committee. RESULTS: There were 1,423 successful LP implants and 234 chronic LP retrieval attempts in 233 subjects. The overall retrieval success rate was 87.6% (205 of 234 attempts). The most common reason for failed retrieval (86.2%, 25 of 29 attempts) was inability to access the LP docking button. The mean time from implantation to attempted retrieval was 3.2 years (range 0.2-9.0 years) and did not significantly influence the retrieval success (P = 0.71). The clinical events committee adjudicated 11 complications in 9 subjects as being retrieval related (3.9%, 9 of 233 subjects). CONCLUSIONS: The safety and efficacy of chronic LP retrieval of an active helix-fixation device was demonstrated in an international clinical trial setting, with implant durations up to 9 years.

Objetivos:Com o incremento da disponibilidade de documentos eletrônicos em bases bibliográficas e de documentação clínica, a recuperação dessas informações requer a adoção de técnicas e interfaces amigáveis ao usuário. Métodos:Apresenta-se neste artigo uma estratégia específica para a recuperação de documentos, no domínio da indexação semântica de textos. A estratégia é baseada em um thesaurus de subwords e no mapeamento de textos, em diferentes linguagens, para uma representação única, interlingual, permitindo a busca entre diferentes coleções de documentos. Resultados:Três casos são apresentados empregando a metodologia desenvolvida: uma base de dados bibliográfica, um sistema departamental de prontuário eletrônico de pacientes e um portal Web. Conclusões:A utilização da metodologia de indexação e recuperação semântica revelou-se útil em diferentes protótipos e rotinas de trabalho, tendo sido bem aceita por diferentes grupos de usuários. Por fim, ressalta-se que a avaliação da performance da indexação já fora realizada, empiricamente, em estudos anteriores.
Objectives: The increasing amount of electronically available documents in bibliographic databases and the clinical documentation requires user-friendly techniques for content retrieval. Methods: A domain-specific approach on semantic text indexing for document retrieval is presented. It is based on a subword thesaurus and maps the content of texts in different European languages to a common interlingual representation, which supports the search across multilingual document collections. Results: Three use cases are presented where the semantic retrieval method has been implemented: a bibliographic database, a department EHR system, and a consumer-oriented Web portal. Conclusions: It could be shown that a semantic indexing and retrieval approach, the performance of which had already been empirically assessed in prior studies, proved useful in different prototypical and routine scenarios and was well accepted by several user groups.

AIM: Prolonged temporary implantation of retrievable vena cava filter is little discussed in literature. The aim of our article is to present our experience with a new type of retrievable "Recovery IVC Filter" with prolonged period of placement in the inferior vena cava (IVC). MATERIALS AND METHODS: "Recovery IVC Filter" was implanted in another department for a 27 year old patient with congenital deficiency of anti thrombin III with deep vein thrombosis (DVT) complicated by pulmonary embolism (PE). After 383 days from implantation of vena cava filter, the patient was indicated for removal of the filter from the right jugular vein access. RESULTS: Filter was removed with the help of a special extraction system without significant technical complications in our department. On control phlebography after extraction of cava filter, there were no signs of injury to the wall of inferior vena cava, extravasation or stenosis. CONCLUSION: Removal of "Recovery IVC Filter" after long term temporary implantation is possible and safe.
