Objectives: Body composition (BC) analysis is a routine part of comprehensive public health care. Assessment of BC is more important source of information than BMI. Adherence to the standard measurement conditions is essential for the correct results. Our study aimed to examine the effect of acute fluid consumption on measures of body mass (BM), percentage of body fat (%BF), visceral fat (VF), percentage of body water (%BW), and impedance at 100 kHz (I100) and 20 kHz (I20) using segmental multi-frequency bioelectrical impedance analysis (SMF-BIA) in a general healthy population. Methods: 95 consecutive healthy normal-weight adults (42 men; 53 women) were involved in the study (mean ± s.d.; age 23.9±1.6 years; body mass 68.3±14.1 kg). All subjects underwent two separate series of body composition (BC) measurements at 0 (BASELINE), 30, 60, 90 min (POST): the first series after drinking 600 ml of isotonic carbohydrate/electrolyte drink (IST) and the second after no fluid administration (CON). Individual measurements were performed in the morning on two consecutive days. Results: In the IST group, BM, VF (both P<0.001), and %BF (P<0.05) increased significantly at 30 min POST compared to BASELINE. BM and VF remained elevated at 90 min POST (both P<0.001). %BW decreased significantly at 30 min POST (P<0.01) then increased at 60 min (P<0.001) and 90 min (P<0.01) POST. There were no significant changes in I100. I20 increased significantly at 30 min POST (P<0.001) then decreased at 60 min (P<0.001) and 90 min POST (P<0.01) compared to BASELINE. In the CON group, BM and VF decreased below BASELINE at 90 min POST (P<0.001), %BF, %BW and I100 did not change significantly. The difference between IST and CON was statistically significant for all POST measurement times only in BM and VF (both P<0.001). The VF results are also underlined by the detected impedance changes in the trunk area at 20 kHz (B20) and 100 kHz (B100) at 60 min and 90 min (both P<0.001).Conclusions: Our study suggests that segmental impedances and BC measurement in healthy young normal-weight adults requires strict adherence to fluid restriction at least 90 min before the measurement to avoid false impedance values and overestimation of BM and VF.

The model of electromigration of a multivalent weak acidic/basic/amphoteric analyte that undergoes complexation with a mixture of selectors is introduced. The model provides an extension of the series of models starting with the single-selector model without dissociation by Wren and Rowe in 1992, continuing with the monovalent weak analyte/single-selector model by Rawjee, Williams and Vigh in 1993 and that by Lelièvre in 1994, and ending with the multi-selector overall model without dissociation developed by our group in 2008. The new multivalent analyte multi-selector model shows that the effective mobility of the analyte obeys the original Wren and Row's formula. The overall complexation constant, mobility of the free analyte and mobility of complex can be measured and used in a standard way. The mathematical expressions for the overall parameters are provided. We further demonstrate mathematically that the pH dependent parameters for weak analytes can be simply used as an input into the multi-selector overall model and, in reverse, the multi-selector overall parameters can serve as an input into the pH-dependent models for the weak analytes. These findings can greatly simplify the rationale method development in analytical electrophoresis, specifically enantioseparations.
AIMS: In the IN-TIME trial, automatic daily implant-based multiparameter telemonitoring significantly improved clinical outcomes in patients with chronic systolic heart failure and implantable cardioverter-defibrillator (ICD) or cardiac resynchronization therapy defibrillator (CRT-D). We compared IN-TIME results for ICD and CRT-D subgroups. METHODS: Patients with LVEF ≤ 35%, NYHA class II/III, optimized drug treatment, no permanent atrial fibrillation, and a dual-chamber ICD (n = 274) or CRT-D (n = 390) were randomized 1:1 to telemonitoring or no telemonitoring for 12 months. Primary outcome measure was a composite clinical score, classified as worsened if the patient died or had heart failure-related hospitalization, worse NYHA class, or a worse self-reported overall condition. RESULTS: The prevalence of worsened score at study end was higher in CRT-D than ICD patients (26.4% vs. 18.2%; P = 0.014), as was mortality (7.4% vs. 4.1%; P = 0.069). With telemonitoring, odds ratios (OR) for worsened score and hazard ratios (HR) for mortality were similar in the ICD [OR = 0.55 (P = 0.058), HR = 0.39 (P = 0.17)] and CRT-D [OR = 0.68 (P = 0.10), HR = 0.35 (P = 0.018)] subgroups (insignificant interaction, P = 0.58-0.91). CONCLUSION: Daily multiparameter telemonitoring has a potential to reduce clinical endpoints in patients with chronic systolic heart failure both in ICD and CRT-D subgroups. The absolute benefit seems to be higher in higher-risk populations with worse prognosis.

Detailed records are an integral part of managing the training process. A number of theoretical and practical complications are encountered in the selection of appropriate parameters and the flawless processing of training data. The use of information technology to record and analyze training offers a significant simplification of this complicated process. In the text we define key general and special indicators for training for multi-endurance sports, which are summarized in a theoretical model. On the basis of that model we developed a specialized software application for the processing, recording and analysis of training for multi-endurance sports, which was then verified in a pilot test. The application allows the user to work with the data entered to structure training for multi-endurance sports.

Cíl: Mechanická trombektomie (MT) symptomatického uzávěru mozkové tepny u pacientů s akutní ischemickou cévní mozkovou příhodou (iCMP) se stala standardní léčbou. Cílem studie METRICS (MechanicalThrombectomy Quality Indicators Study in Czech Stroke Centers) bylo ověřit, zda centra v České republice provádějící endovaskulámí léčbu iCMP plní indikátory kvality navržené multioborovým konsenzem, a tak v mezinárodním kontextu ověřit kvalitu péče napříč naší republikou. Metodika: Národní prospektivní observační studie METRICS probíhala v České republice v roce 2019. Studie se zúčastnilo 12 z 15 center provádějících mechanickou trombektomii. Všechna technická a klinická data byla sbírána prospektivně a následně byla srovnána s doporučenými hodnotamijednotlivých parametrů multioborového konsenzu. Výsledky: V práci jsou analyzována data z 12 center, ve kterých bylo mechanickou trombektomii ošetřeno celkem 1178 pacientů (50,3% mužů) s mediánem věku 72 roky (18-98 let). Intravenóznitrombolýzou (IVT) bylo před MT léčeno 827 nemocných (70,2%) a 445 (37,8%) bylo sekundárně transponováno k MT z jiného centra. Získaná data z národní prospektivní observační studie METRICS byla hodnocena pro jednotlivá centra. Shoda výsledků péče s doporučeními navrženými multioborovým konsenzem byla velmi vysoká. Pět center splnilo parametry všech doporučení. Ostatní centra nesplnila pouze ojedinělá kritéria a ve většině případů jen hraničně. Závěr: Výsledky studie METRICS ukazují, že lze splnit doporučované hodnoty parametrů uvedených v mezinárodním multioborovém konsenzu i v podmínkách běžné klinické praxe napříč Českou republikou.
Aim: Mechanical thrombectomy (MT) of symptomatic occlusion of cerebral artery has become a standard treatment in acute ischemic stroke (IS) patients. The aim of the presented study was to evaluate if centers performing endovascular treatment of acute ischemic stroke meet the recommendations established by the international multi-society consensus. Methods: The national prospective observational multicenter study METRICS (Mechanical Thrombectomy Quality Indicators Study in Czech Stroke Centers) was conducted in Czech Republic in year 2019. Twelve of 15 centers performing mechanicaltrombectomy were participating on study. Alltechnicaland clinical data were collected prospectively and achieved results were subsequently compared with the recommendations established by international multi-society consensus. Results: In 12 centers 1178 (86%) patients (50.3% males, median of age 72 years, range 18-98 years) by MT were treated in 2019 and the collected data of these patients were analyzed. IV thrombolysis prior MT was performed in 827 (70.2%) patients and 445 (37.8%) patients were transferred secondarily for MT from primary stroke centers to dedicated comprehensive centers. Achieved data from the study METRICS were subsequently evaluated for individual participating centers. The concordance of results with the recommendations of multi-society consensus was very high. Five centers met parameters of all recommendations. Remaining centers did not meet sporadic criteria only and most parameters had borderline values. Conclusion: The results of METRICS study showed that it is possible meet the recommended values of parameters stated in the international multi-society consensus even in a real world practice across the Czech Republic.

In contrast to conventional diffusion magnetic resonance imaging (MRI), multi-b-value diffusion MRI methods are able to separate the signal from free water, pseudo-diffusion, and non-Gaussian components of water molecule diffusion. These approaches can then be utilised in so-called intravoxel incoherent motion imaging and diffusion kurtosis imaging. Various parameters provided by these methods can describe additional characteristics of the tissue microstructure and potentially help in the diagnosis and classification of various pathological processes. In this review, we present the basic principles and methods of analysing multi-b-value diffusion imaging data and specifically focus on the known possibilities for its use in the diagnosis of brain lesions. We also suggest possible directions for further research.
