Patients with autoimmune polyendocrinopathy syndrome type 1 (APS-1) caused by autosomal recessive AIRE deficiency produce autoantibodies that neutralize type I interferons (IFNs)1,2, conferring a predisposition to life-threatening COVID-19 pneumonia3. Here we report that patients with autosomal recessive NIK or RELB deficiency, or a specific type of autosomal-dominant NF-κB2 deficiency, also have neutralizing autoantibodies against type I IFNs and are at higher risk of getting life-threatening COVID-19 pneumonia. In patients with autosomal-dominant NF-κB2 deficiency, these autoantibodies are found only in individuals who are heterozygous for variants associated with both transcription (p52 activity) loss of function (LOF) due to impaired p100 processing to generate p52, and regulatory (IκBδ activity) gain of function (GOF) due to the accumulation of unprocessed p100, therefore increasing the inhibitory activity of IκBδ (hereafter, p52LOF/IκBδGOF). By contrast, neutralizing autoantibodies against type I IFNs are not found in individuals who are heterozygous for NFKB2 variants causing haploinsufficiency of p100 and p52 (hereafter, p52LOF/IκBδLOF) or gain-of-function of p52 (hereafter, p52GOF/IκBδLOF). In contrast to patients with APS-1, patients with disorders of NIK, RELB or NF-κB2 have very few tissue-specific autoantibodies. However, their thymuses have an abnormal structure, with few AIRE-expressing medullary thymic epithelial cells. Human inborn errors of the alternative NF-κB pathway impair the development of AIRE-expressing medullary thymic epithelial cells, thereby underlying the production of autoantibodies against type I IFNs and predisposition to viral diseases.

Over the past years, eosinophils have become a focus of scientific interest, especially in the context of their recently uncovered functions (e.g. antiviral, anti-inflammatory, regulatory). These versatile cells display both beneficial and detrimental activities under various physiological and pathological conditions. Eosinophils are involved in the pathogenesis of many diseases which can be classified into primary (clonal) and secondary (reactive) disorders and idiopathic (hyper)eosinophilic syndromes. Depending on the biological specimen, the eosinophil count in different body compartments may serve as a biomarker reflecting the underlying pathophysiology and/or activity of distinct diseases and as a therapy-driving (predictive) and monitoring tool. Personalized selection of an appropriate therapeutic strategy directly or indirectly targeting the increased number and/or activity of eosinophils should be based on the understanding of eosinophil homeostasis including their interactions with other immune and non-immune cells within different body compartments. Hence, restoring as well as maintaining homeostasis within an individual's eosinophil pool is a goal of both specific and non-specific eosinophil-targeting therapies. Despite the overall favourable safety profile of the currently available anti-eosinophil biologics, the effect of eosinophil depletion should be monitored from the perspective of possible unwanted consequences.

SAPHO syndrom představuje zastřešující termín pro autoinflamatorní kostní zánětlivé procesy spojené s osteolýzou a pro současně se vyskytující patologické morfy na kůži. SAPHO syndrom je akronym pro synovitis, akné, pustulózu, hyperostózu a osteitidu. Popisujeme 40letého muže, který si stěžoval na bolesti čelisti a v zádech vedoucí k výraznému snížení fyzické zdatnosti, úbytku hmotnosti a synovitidu četných kloubů, u něhož se objevily kožní změny typu akné. Laboratorní vyšetření odhalilo abnormálně zvýšené známky zánětu. Předchozí četná zobrazovací vyšetření odpovídala obrazu multifokální nebakteriální osteomyelitidy s osteolytickými změnami a periostálními infiltráty a synovitidy velkých kloubů. Pacient splňoval kritéria SAPHO syndromu, jiné příčiny těchto změn nebyly potvrzeny. Zpočátku byl léčen glukokortikoidy a nesteroidními antiflogistiky (NSAIDs). Účinné byly pouze vysoké dávky dexametazonu nebo prednisonu, což vedlo ke změně léčebné strategie a zahájení léčby anakinrou. Pro inhibici zvýšené aktivity osteoklastů, odbourávajících kostní tkáň, jsme upřednostnili denosumab před bisfosfonáty, protože je plánována osteosyntéza patologické fraktury mandibuly. Laboratorní zánětlivé markery se snížily, kostní bolesti ustoupily, ale kožní projevy ve formě akné vykázaly pouze částečný ústup. Kontrolní PET/MR vyšetření po pěti měsících léčby anakinrou a denosumabem prokázalo výraznou regresi metabolické aktivity a signifikantní zmenšení patologických perioseálních infiltrátů. Léčebnou odpověď na anakinru hodnotíme tedy jako parciální remisi. Text přináší přehled možností biologické terapie.
SAPHO is an acronym derived from capital letters of Synovitis, Acne, Pustulosis, Hyperostosis, and Osteitis (SAPHO). SAPHO syndrome is an umbrella term covering a constellation of bone lesions and skin manifestations. A 40-year-old male complained about his jaw and back pain, swelling of multiple joints and weight loss accompanied by physical deterioration and acne type skin lesions. Laboratory tests revealed abnormal elevation of inflammatory markers. Imaging studies illustrated multiple osteolytic bone lesions and paraosseal infiltrates. According to the set of criteria diagnosis of SAPHO syndrome was stated. The patient was treated with glucocorticoids and non-steroidal anti-inflammatory drugs (NSAIDs), but only high dose dexamethasone and prednisone were effective. Daily subcutaneous administration of anakinra at the dose of 100 mg was initiated due to limited response to more classical therapies. Because of planned mandibular osteosynthesis initiation of denosumab was preferred before bisphosphonates. Therapeutic response was confirmed by FDG-PET/MR after 5 months of anakinra and denosumab therapy, showing decreased accumulation of FDG in periosteal and paraosseal infiltrates. Inflammatory markers significantly decreased, bone pain deferred but skin manifestation receded only partially. Therefore the response was evaluated as partial remission.

Problematika očkování pacientů se sekundárním imunodeficitem po chemoterapii, imunosupresivní léčbě a po transplantaci krvetvorných kmenových buněk je dlouhodobě studována. Po vlastních klinických zkušenostech s očkováním a pravidelným vyšetřováním postvakcinační imunity u těchto rizikových pacientů jsme vytvořili po společné dohodě s dětskými hematology a onkology doporučení, jak postupovat v bezpečné, časné a efektivní obnově imunitní ochrany vůči závažným infekčním onemocněním.
The issue of vaccination of patients with secondary immunodeficiency after chemotherapy, immunosuppressive treatment and after hematopoietic stem cell transplantation has been studied for a long time. After our own clinical experience with vaccination and regular follow-up of post-vaccination immunity in these high-risk patients, we created, by collective agreement with pediatric hemato-oncologists, recommendation on how to proceed to safe, early and effective restoration of immune protection against serious infectious diseases.
Activated phosphoinositide 3-kinase delta (PI3Kδ) syndrome (APDS) is an inborn error of immunity with clinical manifestations including infections, lymphoproliferation, autoimmunity, enteropathy, bronchiectasis, increased risk of lymphoma, and early mortality. Hyperactive PI3Kδ signaling causes APDS and is selectively targeted with leniolisib, an oral, small molecule inhibitor of PI3Kδ. Here, 31 patients with APDS aged ≥12 years were enrolled in a global, phase 3, triple-blinded trial and randomized 2:1 to receive 70 mg leniolisib or placebo twice daily for 12 weeks. Coprimary outcomes were differences from baseline in the index lymph node size and the percentage of naïve B cells in peripheral blood, assessed as proxies for immune dysregulation and deficiency. Both primary outcomes were met: the difference in the adjusted mean change (95% confidence interval [CI]) between leniolisib and placebo for lymph node size was -0.25 (-0.38, -0.12; P = .0006; N = 26) and for percentage of naïve B cells, was 37.30 (24.06, 50.54; P = .0002; N = 13). Leniolisib reduced spleen volume compared with placebo (adjusted mean difference in 3-dimensional volume [cm3], -186; 95% CI, -297 to -76.2; P = .0020) and improved key immune cell subsets. Fewer patients receiving leniolisib reported study treatment-related adverse events (AEs; mostly grades 1-2) than those receiving placebo (23.8% vs 30.0%). Overall, leniolisib was well tolerated and significant improvement over placebo was notable in the coprimary endpoints, reducing lymphadenopathy and increasing the percentage of naïve B cells, reflecting a favorable impact on the immune dysregulation and deficiency seen in patients with APDS. Multisystem inflammatory syndrome in children (MIS-C) is a rare, but severe complication of coronavirus disease 2019 (COVID-19). It develops approximately 4 weeks after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and involves hyperinflammation with multisystem injury, commonly progressing to shock. The exact pathomechanism of MIS-C is not known, but immunological dysregulation leading to cytokine storm plays a central role. In response to the emergence of MIS-C, the European Academy of Allergy and Clinical Immunology (EAACI) established a task force (TF) within the Immunology Section in May 2021. With the use of an online Delphi process, TF formulated clinical statements regarding immunological background of MIS-C, diagnosis, treatment, follow-up, and the role of COVID-19 vaccinations. MIS-C case definition is broad, and diagnosis is made based on clinical presentation. The immunological mechanism leading to MIS-C is unclear and depends on activating multiple pathways leading to hyperinflammation. Current management of MIS-C relies on supportive care in combination with immunosuppressive and/or immunomodulatory agents. The most frequently used agents are systemic steroids and intravenous immunoglobulin. Despite good overall short-term outcome, MIS-C patients should be followed-up at regular intervals after discharge, focusing on cardiac disease, organ damage, and inflammatory activity. COVID-19 vaccination is a safe and effective measure to prevent MIS-C. In anticipation of further research, we propose a convenient and clinically practical algorithm for managing MIS-C developed by the Immunology Section of the EAACI.

Obor imunologie prochází v posledních desetiletích velmi výrazným vývojem, který se odrazil zvláště v počátku tohoto tisíciletí ve významných pokrocích v porozumění imunitnímu systému a v uplatnění těchto znalostí v praxi. Nečekaný pokrok a zrychlení výzkumů a pokroků na poli imunologie způsobil nečekaný nástup pandemie onemocnění covid-19 v roce 2020. Intenzivní vědecká práce vedla nejenom k rozvoji našeho porozumění imunitní odpovědi vůči virům, ale i k rychlé konverzi těchto znalostí do praktického zvládání pandemie ve světovém měřítku, jak je patrné na příkladu rychlého vývoje vakcín proti viru SARS‐CoV-2. Doba pandemie dále přispěla ke zrychlení aplikace nejenom biologických objevů, ale i technologických přístupů, jako jsou možnosti matematických a technických věd, informatiky a nově i umělé inteligence, které výrazně posouvají celé odvětví. V tomto sdělení přinášíme konkrétní pokroky v jednotlivých oblastech imunopatologických stavů, jimiž jsou hlavně alergie, imunodeficience, imunita a infekce, očkování, autoimunitní onemocnění a imunologie nádorových onemocnění.
The field of immunology has undergone a very significant development in recent decades, which has been reflected especially in the beginning of this millennium in significant advances in the understanding of the immune system and in the application of this knowledge in practice. The progress and acceleration of research and advances in the field of immunology was further prompt by the unexpected onset of the COVID-19 pandemic in 2020. The intense scientific work has not only led to the development of our understanding of the immune response to viruses, but also to the rapid conversion of this knowledge into practical pandemic management on a global scale, as exemplified by the development of vaccines against SARS-Cov-2 virus. The pandemic era has further contributed to the acceleration of the application of not only biological discoveries but also technological approaches into practical applications, such as use of advanced mathematics, computer science and, more recently, artificial intelligence which are all are adding to the advances that are significantly moving the field of immunology forward. In this communication, we present specific advances in particular areas of immunopathology, which are mainly allergy, immunodeficiency, immunity and infection, vaccination, autoimmune diseases and cancer immunology.
