Bolesti zad patří mezi nejčastější příčiny návštěvy u praktického lékaře. Nejčastější příčinou bolestí zad jsou v současné době mechanické poruchy a degenerativní změny pohybového aparátu. Praktický lékař, jako lékař první linie, by měl aktivně působit na prevenci, včasnou diagnostiku a adekvátně pomoci či alespoň ulevit v akutním stavu. Pokud je podezření na závažné onemocnění (tumor, systémové onemocnění apod.), je na místě minimalizovat prodlevy a provést vyšetření specialistou co nejdříve.
Back pain is one of the most common reasons for a visit general practicioner. The most common cause of back pain is currentlymechanical disturbances and degenerative changes in the locomotive apparatus. Practitioner, as a first-line doctor, should activelywork on prevention, early diagnosis, and adequately help or at least relieve the acute condition. If a serious illness (a tumor,a systemic disease, etc.) is suspected, it is advisable to minimize delays and perform a specialist examination as soon as possible.

OBJECTIVE: Obesity is a multifactorial disease. This non-infectious epidemic has reached pandemic proportions in the 21 century. Posture is a dynamic process referring to an active maintenance of body movement segments against the action of external forces. The aim of the study was to investigate the effect of comprehensive group therapy for obese persons on selected anthropometric and postural parameters. METHODS: The study comprised 53 females with a mean age of 44.5 years (range 29–65 years, standard deviation 9.42 years, median 44 years), who completed a controlled weight loss programme. At the beginning and at the end of the programme, anthropometric parameters (Body Mass Index (BMI), weight and waist circumference) were measured and the posturography tests Limits of Stability (LOS) and Motor Control Test (MCT) were performed using the NeuroCom's SMART EquiTest system. The data were statistically analyzed using R software at a level of significance of 0.05. RESULTS: There were positive changes after the controlled weight loss programme in anthropometric parameters (BMI reduction, with p<0.001; waist circumference reduction, with p<0.001; and weight loss, with p<0.001), postural stability with statistically significant (p<0.05) improvements in both postural activity (LOS test parameters) and reactions (MCT parameters). CONCLUSION: The study showed a statistically significant effect of comprehensive group therapy for obesity in terms of reductions in waist circumference, body weight and BMI, and thus the overall reduction of both cardiovascular and metabolic risks, as well as improved postural skills (activity and reactions).

Obezita a její komplikace jsou celosvětově řazeny mezi tři nejčastější příčiny úmrtí u populace ve věku 50 až 70 let. Je spojena s množstvím zdravotních rizik, které výrazně zasahují do kvality života jednotlivce. Postura označuje aktivní držení pohybových segmentů těla proti působení zevních sil. Cílem studie byl průkaz vztahu mezi reakcí postury a tělesnou hmotností. V práci bylo využito posturografických testů LOS (Limits of Stability) a MCT (Motor Control Test) modulu Smart Equitest System posturografu firmy NeuroCom®. Soubor tvořilo 20 žen průměrného věku 21,75 let, (10 obézních žen s průměrným BMI 34,7 a 10 neobézních žen s průměrným BMI 20,3). Výsledky byly statisticky zpracovány a vyhodnoceny na hladině signifikance p < 0,05. U obézních žen v MCT testu u podtrhu dopředu byly prokázány statisticky významně vyšší hodnoty v parametru Small F (p = 0,007). V testu LOS pak byly prokázány statisticky významné vyšší hodnoty parametru Reaction Time, statisticky významně nižší hodnoty parametru Movement Velocity, parametru Endpoint Excursion, parametru Directional Control a parametru Maximum Excursion (p < 0,05). Závěr: Výsledky práce potvrdily horší posturální stabilitu obézních, což s sebou nese zvýšené riziko pádů při překročení limitů stability a traumat vlivem inadekvátní svalové aktivace a koordinace.
Obesity and its complications are globally ranked among the three most common causes of death in the population aged 50 to 70 years. It is associated with a number of health risks that significantly affect the quality of life of the individual. Posture indicates the active movement of body segments against the influence of external forces. The aim of the study was to demonstrate the relationship between the response posture and body weight. The work was used posturographic tests LOS (Limits of Stability) and MCT (Motor Control Test) module Smart Equitest System from posturograph of NeuroCom® companie. The kohort consisted of 20 women with a mean age of 21.75 years, (10 obese women with a mean BMI 34.7 and 10 non-obese women with a mean BMI 20.3). The results were statistically processed and evaluated at the level of significance p <0. 05. In obese women MCT test - forward direction - have been demonstrated significantly higher values in the parameter Small F (p = 0.007). In the LOS test were statistically significant higher value of the parameter Reaction Time, significantly lower values of Movement Velocity, Endpoint Excursion, Directional Control and Maximum Excursion parameters (p <0.05). Conclusion: The results confirmed the worse postural stability of obese people, which carries an increased risk of falls, when stability limits are exceeded, and trauma due to inadequate muscle activation and coordination.

Cílem tohoto sdělení je poukázat na rizika současného živoního stylu. V současné době se neustále poukazuje na rizika hypomobility a imobilizace na lůžku. Málokdo si však uvědomuje, jak blízko k této hypomobilitě díky životnímu stylu jsme. Předkládané sdělení shrnuje účinky pohybové aktivity a poukazuje na rizika hypoaktivity. V druhé části sdělení jsou prezentovány možnosti prevence a sebeošetření.
The aim of this review is to highlight the risks of contemporary lifestyle.Currently continually points to the risks of hypomobility and immobilisation. Few, however, realize how close to this hypomobility through our lifestyle we are. The present review summarizes the effects of physical activity and points to the risks of hypoactivity. In the second part of the review are presented the possibilities of prevention and self-treatment.

AIM: Using functional scales and face video analysis, changes in central facial paresis are monitored in patients with stroke after orofacial therapy and correlations between changes in mimicry, mental function and overall quality of life of patients after stroke are made. MATERIALS AND METHODS: A prospective randomized study of patients after stroke with facial paresis. The functional status of the experimental group of 50 cases with orofacial regulation therapy and 49 control cases without mimicry therapy is observed after four weeks of rehabilitation. RESULTS: Changes in mimicry functions evaluated by the House-Brackmann Grading System (HBGS) clinical range and using 2D video analysis of the distance between the paretic corner of the mouth and earlobe at rest and during smiling were statistically better in the experimental group than in controls. Changes in mental function - depression using Beck Depression Inventory and changes in the quality of life using Bartle index and modified Rankin score (scale) were significantly greater in the experimental group. There was a very close relationship between the changes in mimicry, mental state and overall quality of life according to the Spearman correlative coefficient. CONCLUSION: Orofacial rehabilitation therapy for patients with paresis after stroke has a significant influence on the adjustment of mimicry, mental functions and overall quality of life after 4 weeks of treatment.

Intervenční rehabilitace je moderní postup v terapii poruch pohybového ústrojí tam, kde klasická konzervativní kinezioterapie selhává. Využívá aplikace lokálního anestetika pomocí speciálních anesteziologických postupů k ovlivnění neuromuskulárního aparátu. Velkou výhodou je cílené ovlivnění periferního nervového systému při minimálním ovlivnění vitálních funkcí a dále možnost spolupráce s pacientem během výkonu i během následné rehabilitace. Jako příklad je uvedena kazuistika mladého muže s posttraumatickou limitací dolní končetiny, kdy byla využita intervenční rehabilitace s promptním efektem.
Interventional rehabilitation is a modern approach in the treatment of musculoskeletal disorders where classical conservative physiotherapy fails. It uses the application of the local anesthetic using special anesthetic technique affecting the neuromuscular system. The big advantage is targeted affecting the peripheral nervous system with minimal influence on vital functions and the possibility of cooperation with the patient during the procedure and during rehabilitation. As an example, there is the case of a young man with post-traumatic lower extremity limitations, where the interventional rehabilitation was used with prompt effect.

Cíl: Cílem práce je předložit přehlednou analýzu možné asociace poruch posturální stability a dětské obezity. Metody: Užitými metodami byly kompilace a komparace dostupné odborné literatury, využití dosavadních terapeutických zkušeností autorů, které byly v týmu podrobeny kritické diskuzi. Výsledky: Práce prokázala negativní vliv obezity na posturální stabilitu dětí s obezitou. Autoři doporučují vyšetření posturální stability jako součást komplexního vyšetření obézního dítěte a jeho výsledek pak zohlednit v preskripci pohybové aktivity.
Aim: The aim of the thesis is to lay out a detailed analysis of the probable association of postural stability with a child obesity. Methods: The used methods were compilations and comparisons of the available expert literature. Furthermore, the authors‘ experience in current therapeutical field were also used. These were thoroughly examined within a tight group discussion. Results: The thesis proved the negative impact of obesity on the postural stability of children suffering from obesity. The authors rightly recommend the examination of the postural stability as part of the overall medical evaluation of an obese child. This result should then be taken into account while suggesting the child‘s expected physical activity.
V posledních letech se úrazy zápěstí stále častěji řeší jak konzervativně, tak operačně. Povědomí o rehabilitaci je však velmi omezené, ale je třeba si uvědomit, že rehabilitace se na dobrém výsledku podílí minimálně 50 % z hlediska znovuzískání ztracené či traumatem omezené funkce. Péče o končetinu v pooperačním období má velký význam, a pokud není důsledně prováděná, může mít závažné důsledky. S terapií je třeba začít již v období, kdy je ruka ve fixaci, aby se minimalizoval otok a bolest. Cílem této práce je představit základní zásady pro rehabilitaci po úrazech zápěstí a ruky.
In recent years, wrist injuries are increasingly addressed both conservatively and surgically. Awareness about proper rehabilitation is sadly very limited, even when we should be keeping in mind that rehabilitation contributes at least 50% towards achieving a good result, i.e. restoring the function lost or impaired as a result of the trauma. Proper care for the limb in the postoperative period is of great importance, and if it is not performed consistently and diligently, it can have serious consequences. The therapy should be commenced at a time when the hand is in a cast to minimise swelling and pain. The aim of this paper is to present the basic principles for rehabilitation after injuries to the wrist and the hand.
