syndrom krátkého střeva je hlavní příčinou chronického intestinálního selhání a představuje skupinu od mírné intestinální insuficience, kterou lze zvládnout dietními a farmakologickými postupy, až po velmi těžké formy, vyžadující domácí parenterální výživu. intestinální selhání je definováno jako redukce absorpční funkce trávicího systému pod nezbytné minimum potřebné ke vstřebání makronutrientů a/nebo elektrolytů a vody vyžadující parenterální suplementaci k udržení zdraví a růstu. Závažnost syndromu krátkého střeva závisí na etiologii, reziduální délce tenkého střeva základním onemocnění a anatomii po resekci střeva. dlouhodobá a domácí parenterální výživa je vitální metodou volby, ale má své závažné komplikace a je spojena se zhoršenou kvalitou života. léčba GlP-2 představuje nový koncept vedoucí k redukci domácí parenterální výživy a může vést k jejímu ukončení. intestinální rehabilitační chirurgie se vždy zvažuje u všech nemocných s cílem obnovení kontinuity trávícího traktu a zlepšení digestivní funkce. intestinální transplantace nebo multiviscerální transplantace je pak určena pro nemocné s komplikacemi domácí parenterální výživy a neřešitelnými abdominálními stavy.
short bowel syndrome is the main cause of chronic intestinal failure and represents a complex, from mild intestinal insufficiency that can be overcome with dietary and pharmacological treatment, to a severe condition required home parenteral nutrition support. intestinal failure is defined as the reduction of gut function below the minimum necessary for the absorption of macronutrient and/ or water and electrolytes, such that intravenous supplementation is required to maintain health and growth. the severity of short bowel syndrome is related to its etiology, remnant small bowel length, underlying disease and bowel anatomy after resection. long-term/ home parenteral nutrition is the vital and reference treatment for this condition that can lead to chronic complications in addition to a significantly low quality of life. however, GlP-2 treatment represents a new concept associated with parenteral nutrition reduction and in some cases even directed for weaning of Pn. intestinal rehabilitation surgery should be always proposed, that can restore digestive continuity and absorption. intestinal transplantation/ multivisceral transplantation is reserved for patients with life threatening complications of long-term parenteral nutrition or untreatable abdominal diseases.

Cílem tohoto sdělení je souhrn postupů u nemocných s chronickým intestinálním selháním. Syndrom krátkého střeva představuje nejčastější příčinu chronického selhání střeva. Etiologie vzniku SBS je rozmanitá a léčba zahrnuje komplexní mezioborový přístup. Pacienti často mají píštěle a stomie a jsou odkázáni na domácí parenterální výživu, jež je jako jediná schopna zvládnout těžkou malnutrici. Zásadním postupem je pokus o rekonstrukci zažívacího traktu s obnovením anatomické kontinuity. Pokud po rekonstrukci nedojde k independenci na domácí parenterální výživě, je na místě farmakologická léčba s cílem zvýšení resorpční kapacity střeva a provedení autologní střevní rekonstrukce. Při rozvoji komplikací domácí parenterální výživy je indikována transplantace střeva nebo multiviscerální transplantace, které se v posledních letech staly součástí léčby ireverzibilního střevního selhání.
This presentation aims to summarize the course of action in patients with chronic intestinal failure. Short bowel syndrome (SBS) is the most common cause of chronic bowel failure. The etiology of SBS is diverse. SBS treatment involves a complex multidisciplinary approach. Patients often have stomas and intestinal fistulas. They are very often dependent on home parenteral nutrition (HPN). HPN is the only way to manage their severe malnutrition. The most crucial process is attempting to reconstruct their digestive tract with the restoration of its anatomical continuity. If independence on HPN fails after digestive tract reconstruction, pharmacological treatment is used to increase intestine tract resorptive capacity. Furthermore, autologous intestinal reconstruction is planned. In recent years, intestinal or multi-visceral transplantation has become part of treating irreversible intestinal failure in patients who developed HPN complications.

BACKGROUND: Tuberculosis (TBC) in solid organ transplant recipients represents a severe complication. The incidence among transplant recipients is higher than in the general population, and the diagnosis and treatment remain challenging. We present a case of active disseminated tuberculosis in a kidney transplant recipient treated with an anti-CD40 monoclonal antibody, who had been previously exposed to an active form of the disease, but latent tuberculosis (LTBI) was repeatedly ruled out prior to transplantation. To the best of our knowledge, no other case has been reported in a patient treated with the anti-CD40 monoclonal antibody. CASE PRESENTATION: A 49-year-old patient, 1.5 years after primary kidney transplantation, presented with vocal cord problems, a dry irritating cough, and a sore throat. A detailed investigation, including a high-resolution chest CT scan, revealed the diagnosis of disseminated tuberculosis. The antituberculosis treatment consisting of rifampicin, isoniazid, pyrazinamide, and ethambutol was started immediately. The patient's condition became complicated by relapsing diarrhoea. The colonoscopy revealed a circular stenosis above Bauhin's valve. Microscopical findings showed active colitis and vaguely formed collections of epithelioid macrophages without fully developed caseous granulomas and were consistent with the clinical diagnosis of tuberculosis. The antituberculosis treatment was subsequently enhanced by moxifloxacin and led to a great improvement in the patient's condition. CONCLUSION: In this case, false negativity of interferon-γ release assays and possibly higher risk for intracellular infections in patients on costimulatory signal blockers are discussed.

Domácí parenterální výživa (DPV) je život zachraňující léčba v případech chronického intestinálního selhání různé etiologie. Hepatopatie indukovaná domácí parenterální výživou nebo hepatopatie spojená s intestinálním selháním (PNALD/IFALD) zahrnuje široké spektrum stavů od steatózy, cholestázy, ikteru až po konečné selhání jater (ESLD). Diagnóza a monitorace PNALD/ IFALD zahrnuje komplexní klinické, biochemické, radiologické a někdy histologické vyšetření. Důležitým postupem je vyloučení jiné příčiny jaterního poškození před zahájením DPV. Management péče je založen na multidisciplinárním přístupu zahrnujícím nutricionistu, hepatologa, radiologa a chirurga. ESLD s jaterním selháním je indikací k transplantaci tenkého střeva/multiviscerální transplantaci. Izolovaná transplantace střeva je indikací pro nemocné bez významné formy PNALD/IFALD. Cílem tohoto sdělení je problematika prevence, managementu a léčby IFALD.
Home parenteral nutrition (HPN) is life saving therapy for patients with chronic intestinal failure caused by different etiology. Parenteral nutrition associated liver disease or intestinal failure associated liver disease (PNALD/IFALD) emcompasses a wide range of disease from steatosis, cholestasis, jaundice to end stage liver disease (ESLD). Diagnosis and monitoring of liver disease requires a complex of clinical, biochemical, radiological and where appropriate histology examination. It is important that other causes of derranged liver function are excluded before HPN. The management of PNALD/IFALD is based on multidisciplinary cooperation of nutritionist, hepatologist, radiologist and surgeron. ESLD with overt liver failure is an indication for small bowel and liver transplatation/multiviceral transplantation. Isolated small bowel transplantion is indicated for mild PNALD/IFALD cases. The purpose of this article is to discuss the current data dealing with IFALD prevention management and therapy.

Primary sclerosing cholangitis (PSC) frequently associates with inflammatory bowel diseases (IBD), in particular ulcerative colitis (UC) and colorectal cancer. Although different clinical course and features (higher carcinogenic potential) suggest that IBD in PSC represents disease entity distinct to IBD without hepatobiliary component underlying molecular mechanisms remain unknown. In this project we will analyze transcriptome of colon biopsy samples collected in a prospective study of patients diagnosed with PSC without UC, UC without PSC, PSC with concurrent UC, and control group. Using whole-genome transcriptome profiling, we will identify expression signatures of the different diagnoses and corroborate these further in expanded patient cohorts. In parallel we will characterize available experimental PSC mouse models with respect to alterations in colonic tissues and genes identified in the transcriptome analysis. Primární sklerózující cholangitida (PSC) je často asociována s idiopatickými střevními záněty (IBD) a to zejména ulcerózní kolitidou (UC) a kolorektálním karcinomem. Odlišný klinický průběh a další charakteristiky, jako zvýšený karcinogenní potenciál, naznačují, že PSC-UC představuje specifický podtyp IBD, který se liší od IBD bez hepatobiliární komponenty. Molekulární mechanismy vedoucí k tomuto rozdílnému klinickému obrazu nejsou v současnosti známy. V tomto projektu se v prospektivní studii zaměříme na zjištění změn v transkriptomu bioptických vzorků tlustého střeva, odebraných pacientům s PSC bez UC, UC bez PSC a PSC-UC a kontrolního souboru. Pomocí celogenomové analýzy transkriptomu určíme unikátní expresní signatury, které budou dále potvrzeny na rozšířených kohortách pacientů. Souběžně budeme charakterizovat dostupné myší modely PSC s ohledem na změny střevní sliznice a geny identifikované v transkriptomové analýze. Pochopení mechanismu patogeneze PSC-UC je významné pro definování nových cílů v diagnostice, prevenci a léčbě tohoto závažného onemocnění.

BACKGROUND AND AIMS: Ulcerative colitis (UC) is a chronic inflammatory disease. Fecal microbial transplantation (FMT) is a promising alternative treatment. METHODS: This multicenter, open-label, noninferiority trial randomized patients with active left-sided UC (Mayo score 4-10) equally to FMT or 5-aminosalicylic acid (5-ASA) enemas. FMT enemas were administered five times in the first week and then once weekly for 5 weeks. 5-ASA enemas were administered daily for 2 weeks and then every other day. The primary study endpoint was clinical remission, with a total Mayo score ≤2 at week 12 with no subscore >1. RESULTS: Sixty-one patients were screened; 45 were enrolled and randomized to FMT (n = 23) or 5-ASA (n = 22). Twenty-one FMT and 22 5-ASA patients completed at least the week 4 study visit and were included in the mITT analysis. Twelve FMT (57%) and eight 5-ASA patients achieved the primary study endpoint. FMT noninferiority with 10% margin was confirmed (95% CI: -7.6%, 48.9%). Adverse events occurred in 12 FMT (57%) and 13 5-ASA (59%) patients. Increased microbial diversity persisted 3 months after FMT. CONCLUSION: FMT is an effective treatment for left-sided UC and increased recipient microbiome diversity. Targeted microbiome modification may improve FMT efficacy. Further investigation is needed to guide donor and patient selection.

Plectin, a highly versatile cytolinker protein, provides tissues with mechanical stability through the integration of intermediate filaments (IFs) with cell junctions. Here, we hypothesize that plectin-controlled cytoarchitecture is a critical determinant of the intestinal barrier function and homeostasis. Mice lacking plectin in an intestinal epithelial cell (IEC; PleΔIEC) spontaneously developed colitis characterized by extensive detachment of IECs from the basement membrane (BM), increased intestinal permeability, and inflammatory lesions. Moreover, plectin expression was reduced in the colons of ulcerative colitis (UC) patients and negatively correlated with the severity of colitis. Mechanistically, plectin deficiency in IECs led to aberrant keratin filament (KF) network organization and the formation of dysfunctional hemidesmosomes (HDs) and intercellular junctions. In addition, the hemidesmosomal α6β4 integrin (Itg) receptor showed attenuated association with KFs, and protein profiling revealed prominent downregulation of junctional constituents. Consistent with the effects of plectin loss in the intestinal epithelium, plectin-deficient IECs exhibited remarkably reduced mechanical stability and limited adhesion capacity in vitro. Feeding mice with a low-residue liquid diet that reduced mechanical stress and antibiotic treatment successfully mitigated epithelial damage in the PleΔIEC colon.
