Primary hemophagocytic lymphohistiocytosis (pHLH) is a life-threatening hyperinflammatory syndrome that develops mainly in patients with genetic disorders of lymphocyte cytotoxicity and X-linked lymphoproliferative syndromes. Previous studies with etoposide-based treatment followed by hematopoetic stem cell transplantation (HSCT) resulted in 5-year survival of 50% to 59%. Contemporary data are lacking. We evaluated 88 patients with pHLH documented in the international HLH registry from 2016-2021. In 12 of 88 patients, diagnosis was made without HLH activity, based on siblings or albinism. Major HLH-directed drugs (etoposide, antithymocyte globulin, alemtuzumab, emapalumab, ruxolitinib) were administered to 66 of 76 patients who were symptomatic (86% first-line etoposide); 16 of 57 patients treated with etoposide and 3 of 9 with other first-line treatment received salvage therapy. HSCT was performed in 75 patients; 7 patients died before HSCT. Three-year probability of survival (pSU) was 82% (confidence interval [CI], 72%-88%) for the entire cohort and 77% (CI, 64%-86%) for patients receiving first-line etoposide. Compared with the HLH-2004 study, both pre-HSCT and post-HSCT survival of patients receiving first-line etoposide improved, 83% to 91% and 70% to 88%. Differences to HLH-2004 included preferential use of reduced-toxicity conditioning and reduced time from diagnosis to HSCT (from 148 to 88 days). Three-year pSU was lower with haploidentical (4 of 9 patients [44%]) than with other donors (62 of 66 [94%]; P < .001). Importantly, early HSCT for patients who were asymptomatic resulted in 100% survival, emphasizing the potential benefit of newborn screening. This contemporary standard-of-care study of patients with pHLH reveals that first-line etoposide-based therapy is better than previously reported, providing a benchmark for novel treatment regimes.

BACKGROUND: Niemann-Pick disease type C is a rare lysosomal storage disorder. We evaluated the safety and efficacy of N-acetyl-l-leucine (NALL), an agent that potentially ameliorates lysosomal and metabolic dysfunction, for the treatment of Niemann-Pick disease type C. METHODS: In this double-blind, placebo-controlled, crossover trial, we randomly assigned patients 4 years of age or older with genetically confirmed Niemann-Pick disease type C in a 1:1 ratio to receive NALL for 12 weeks, followed by placebo for 12 weeks, or to receive placebo for 12 weeks, followed by NALL for 12 weeks. NALL or matching placebo was administered orally two to three times per day, with patients 4 to 12 years of age receiving weight-based doses (2 to 4 g per day) and those 13 years of age or older receiving a dose of 4 g per day. The primary end point was the total score on the Scale for the Assessment and Rating of Ataxia (SARA; range, 0 to 40, with lower scores indicating better neurologic status). Secondary end points included scores on the Clinical Global Impression of Improvement, the Spinocerebellar Ataxia Functional Index, and the Modified Disability Rating Scale. Crossover data from the two 12-week periods in each group were included in the comparisons of NALL with placebo. RESULTS: A total of 60 patients 5 to 67 years of age were enrolled. The mean baseline SARA total scores used in the primary analysis were 15.88 before receipt of the first dose of NALL (60 patients) and 15.68 before receipt of the first dose of placebo (59 patients; 1 patient never received placebo). The mean (±SD) change from baseline in the SARA total score was -1.97±2.43 points after 12 weeks of receiving NALL and -0.60±2.39 points after 12 weeks of receiving placebo (least-squares mean difference, -1.28 points; 95% confidence interval, -1.91 to -0.65; P<0.001). The results for the secondary end points were generally supportive of the findings in the primary analysis, but these were not adjusted for multiple comparisons. The incidence of adverse events was similar with NALL and placebo, and no treatment-related serious adverse events occurred. CONCLUSIONS: Among patients with Niemann-Pick disease type C, treatment with NALL for 12 weeks led to better neurologic status than placebo. A longer period is needed to determine the long-term effects of this agent in patients with Niemann-Pick disease type C. Niemann‐Pickova choroba typu B (NPB) představuje vzácnou dědičnou metabolickou chorobu, která má velmi široké spektrum příznaků s různým věkem nástupu a tíží. Přesná diagnostika je založena na enzymologickém a současně genetickém vyšetření. V současné době je již dostupná specifická léčba pomocí enzymové substituční terapie olipudázou alfa. V tomto článku prezentujeme dospělého pacienta s náhodným nálezem splenomegalie, u kterého byla následně potvrzena diagnóza NPB.
Niemann-Pick disease type B (NPB) is a rare inherited metabolic disease that has a very wide spectrum of symptoms with different ages of onset and severity. Accurate diagnosis is based on an enzymological and genetic examination. Currently, a specific treatment using enzyme replacement therapy with olipudase alfa is already available. In this article, we present an adult patient with a random finding of splenomegaly, in whom the diagnosis of NPB was subsequently confirmed.

Congenital disorders of glycosylation (CDG) and Niemann-Pick type C (NPC) disease are inborn errors of metabolism that can both present with infantile-onset severe liver disease and other multisystemic manifestations. Plasma bile acid and N-palmitoyl-O-phosphocholineserine (PPCS) are screening biomarkers with proposed improved sensitivity and specificity for NPC. We report an infant with ATP6AP1-CDG who presented with cholestatic liver failure and elevated plasma oxysterols and bile acid, mimicking NPC clinically and biochemically. On further investigation, PPCS, but not the bile acid derivative N-(3β,5α,6β-trihydroxy-cholan-24-oyl) glycine (TCG), were elevated in plasma samples from individuals with ATP6AP1-, ALG1-, ALG8-, and PMM2-CDG. These findings highlight the importance of keeping CDG within the diagnostic differential when evaluating children with early onset severe liver disease and elevated bile acid or PPCS to prevent delayed diagnosis and treatment.

Protože Erdheimova-Chesterova nemoc (Erdheim-Chester disease, ECD), známá i pod označením polyostotická sklerotická histiocytóza, působí i na cévní struktury, může být mylně diagnostikována jako ischemická choroba dolních končetin. Postižení aorty při ECD obvykle odhalí rentgenové vyšetření. Dosud však nemáme k dispozici dostatek informací o vzhledu postižení během chirurgického výkonu ani o léčbě postižení aorty při ECD. V tomto článku popisujeme případ pacienta s postižením vzestupné aorty zjištěným náhodně při provádění koronárního bypassu a dalšího pacienta s postižením břišní aorty připomínajícím aortoiliackou okluzivní nemoc.
Erdheim-Chester disease (ECD), known as histiocytic neoplastic disease, can be diagnosed as a peripheral arterial disease because of its effects on vascular structures. Aortic involvement in ECD is usually recognised radiologically. However, there is insufficient information about the surgical appearance and treatment of aortic involvement in ECD. In this report, we presented a patient with ascending aortic involvement detected incidentally during coronary bypass and a patient with abdominal aortic involvement mimicking aortoiliac disease.

Hemofagocytární syndrom, difuzní alveolární hemoragie, katastrofický antifosfolipidový syndrom a různé typy trombotických mikroangiopatií patří mezi vzácné stavy s významnou morbiditou a mortalitou. Společným rysem bývá pozdní stanovení diagnózy, což může ovlivnit úspěšnost léčby. Cílem přehledného článku je shrnutí základních diagnostických a léčebných kroků předmětné subpopulace kriticky nemocných.
Haemophagocytic syndrome, diffuse alveolar haemorrhage, catastrophic antiphospholipid syndrome and various types of thrombotic microangiopathies are rare conditions with significant morbidity and mortality. A common feature is late diagnosis, which can affect the success of treatment. The aim of this review article is to summarize the basic diagnostic and therapeutic steps of the present subpopulation of critically ill patients.

Východiska: Rosai-Dorfman-Destombesova choroba (Rosai-Dorfman-Destombes disease – RDD) je vzácné histiocytární onemocnění charakterizované akumulací aktivovaných histiocytů v ložisku choroby se zánětlivou reakcí. Ačkoliv imunofenotyp patologických buněk je znám, etiopatogeneze této nemoci není stále objasněna. Původně byla nemoc považována za reaktivní, v posledních letech byly identifikovány četné mutace provázející RDD: NRAS, KRAS, MAP2K1 a ARAF, což zase svědčí pro klonální (maligní) etiologii této nemoci. RDD je heterogenní onemocnění s velmi odlišnými klinickými průběhy. Izolované nodální či kožní postižení má podstatně lepší prognózu než případy s extranodálním postižením, které jsou spojené se závažnou morbiditou i mortalitou. Nemoc se vyskytuje izolovaně anebo v asociaci s maligními či autoimunitními chorobami. Terapie je odvislá od lokalizace choroby. Při nevelkém rozsahu nemoci se používá monoterapie glukokortikoidy. V případech pokročilejšího onemocnění je nutná intenzivní léčba. Ve starších publikacích byly popsány léčebné odpovědi po antimetabolitech, vinka alkaloidech a prednisonu, v novějších pak po kladribinu, příp. thalidomidu, lenalidomidu či sirolimu, ale také rituximabu. V několika případech byl prokázán efekt kobimetinibu. Cíl: Text přináší přehled současných znalostí o tomto raritním onemocnění a uvádí dosavadní zkušenosti s léčbou RDD.
Background: Rosai-Dorfman-Destombes disease (RDD) is a rare histiocytosis characterized by accumulation of activated histiocytes within affected tissues. Although the immunophenotype of this disease was described, the pathophysiology of this disease is still not sufficiently understood. Recent studies have found NRAS, KRAS, MAP2K1, and ARAF mutations in RDD lesions, raising the possibility of a clonal origin in some forms of RDD while in other cases reactive origin or association with other malignant and autoimmune disease is supposed. RDD is a widely heterogeneous entity with a range of clinical phenotypes occurring in some patients in association with autoimmune or malignant diseases. Its therapy should reflect the localization of the disease. Monotherapy with glucocorticoids is sufficient only in limited disease. In patients with advanced disease, combined nodal and extranodal forms of RDD need more intensive therapy. In older publications, antimetabolites, vinca alkaloids and prednisone were used; in recent publications, remissions after cladribine, rituximab, sirolimus, thalidomide, lenalidomide and cobimetinib were described. Purpose: This text summarizes current knowledge about this rare disease and reviews the therapeutic options.
