BACKGROUND AND AIMS: Fabry disease (FD) is a rare X-linked lysosomal storage disorder caused by disease-associated variants in the alpha-galactosidase A gene (GLA). FD is a known cause of stroke in younger patients. There are limited data on prevalence of FD and stroke risk in unselected stroke patients. METHODS: A prospective nationwide study including 35 (78%) of all 45 stroke centers and all consecutive stroke patients admitted during three months. Clinical data were collected in the RES-Q database. FD was diagnosed using dried blood spots in a stepwise manner: in males-enzymatic activity, globotriaosylsphingosine (lyso-Gb3) quantification, if positive followed by GLA gene sequencing; and in females GLA sequencing followed by lyso-Gb3. RESULTS: 986 consecutive patients (54% men, mean age 70 years) were included. Observed stroke type was ischemic 79%, transient ischemic attack (TIA) 14%, intracerebral hemorrhage (ICH) 7%, subarachnoid hemorrhage 1% and cerebral venous thrombosis 0.1%. Two (0.2%, 95% CI 0.02-0.7) patients had a pathogenic variant associated with the classical FD phenotype (c.1235_1236delCT and p.G325S). Another fourteen (1.4%, 95% CI 0.08-2.4) patients had a variant of GLA gene considered benign (9 with p.D313Y, one p.A143T, one p.R118C, one p.V199A, one p.R30K and one p.R38G). The index stroke in two carriers of disease-associated variant was ischemic lacunar. In 14 carriers of GLA gene variants 11 strokes were ischemic, two TIA, and one ICH. Patients with positive as compared to negative GLA gene screening were younger (mean 60±SD, min, max, vs 70±SD, min, max, P = 0.02), otherwise there were no differences in other baseline variables. CONCLUSIONS: The prevalence of FD in unselected adult patients with acute stroke is 0.2%. Both patients who had a pathogenic GLA gene variant were younger than 50 years. Our results support FD screening in patients that had a stroke event before 50 years of age.

Background and Purpose- The RE-SPECT ESUS trial (Randomized, Double-Blind, Evaluation in Secondary Stroke Prevention Comparing the Efficacy and Safety of the Oral Thrombin Inhibitor Dabigatran Etexilate Versus Acetylsalicylic Acid in Patients With Embolic Stroke of Undetermined Source) tested the hypothesis that dabigatran would be superior to aspirin for the prevention of recurrent stroke in patients with embolic stroke of undetermined source. This exploratory subgroup analysis investigates the impact of age, renal function (both predefined), and dabigatran dose (post hoc) on the rates of recurrent stroke and major bleeding. Methods- RE-SPECT ESUS was a multicenter, randomized, double-blind trial of dabigatran 150 or 110 mg (for patients aged ≥75 years and/or with creatinine clearance 30 to <50 mL/minute) twice daily compared with aspirin 100 mg once daily. The primary outcome was recurrent stroke. Results- The trial, which enrolled 5390 patients from December 2014 to January 2018, did not demonstrate superiority of dabigatran versus aspirin for prevention of recurrent stroke in patients with embolic stroke of undetermined source. However, among the population qualifying for the lower dabigatran dose, the rate of recurrent stroke was reduced with dabigatran versus aspirin (7.4% versus 13.0%; hazard ratio, 0.57 [95% CI, 0.39-0.82]; interaction P=0.01). This was driven mainly by the subgroup aged ≥75 years (7.8% versus 12.4%; hazard ratio, 0.63 [95% CI, 0.43-0.94]; interaction P=0.10). Stroke rates tended to be lower with dabigatran versus aspirin with declining renal function. Risks for major bleeding were similar between treatments, irrespective of renal function, but with a trend for lower bleeding rates with dabigatran versus aspirin in older patients. Conclusions- In subgroup analyses of RE-SPECT ESUS, dabigatran reduced the rate of recurrent stroke compared with aspirin in patients qualifying for the lower dose of dabigatran. These results are hypothesis-generating. Aspirin remains the standard antithrombotic treatment for patients with embolic stroke of undetermined source. INTRODUCTION: Many different factors may have an impact on clinical outcome after mechanical thrombectomy (MT) for acute ischemic stroke (AIS). We aimed to investigate levels of serum glycemia (GLY) within the first 48 hours after MT. SUBJECTS AND METHODS: Consecutive AIS patients were enrolled in the retrospective bi-center study. Neurological deficit was assessed with National Institutes of Health Stroke Scale (NIHSS) and functional outcome after 3 months with modified Rankin scale with a score 0-2 for good outcome. Presence of symptomatic intracerebral hemorrhage was assessed according to the SITS- MOST criteria. RESULTS: In total, 868 patients (442 males, mean age 69.7 ± 12.2 years) with a median of admission NIHSS 17 points were enrolled in the study and 253 (29.1%) of them were diabetics. Recanalization was reached in 758 (87.3%) patients. Patients with good outcome (412, 47.5%) had lower median of GLY (6.5 versus 7.4 mmol/L, P < .0001) within the first 48 hours after MT. Similar results were found also in diabetics (8.1 versus 9.6 mmol/L, P < .0001) and in patients with achieved recanalization (6.5 versus 7.5 mmol/L, P < .0001). Multivariate regression analysis with adjustment for potential confounders showed median of GLY (P = .0001, odds ratio: 0.830, 95% confidence interval: 0.755-0.913) as a predictor of good outcome after MT. CONCLUSION: Lower levels of GLY within the first 48 hours after MT may be associated with better functional outcome after 3 months.

Cíl: Bylo prokázáno, že C-reaktivní protein stanovený vysoce senzitivní metodou (high-sensitive C-reactive protein; hs-CRP) má dobrou senzitivitu při diagnostice cerebrovaskulárních příhod. Cílem studie bylo vyhodnotit spojitosti mezi hs-CRP a rizikovými faktory CMP, neurologickou poruchou, ischemií mozku a prognózou. Materiál a metody: Studie zahrnula 153 pacientů rozdělených do dvou skupin: pacienti s akutní ischemickou CMP a pacienti s rizikovými faktory, kteří CMP neprodělali. V odebraných vzorcích krve byla stanovena hladina hs-CRP v séru. Neurologické postižení bylo hodnoceno při přijetí do nemocnice a při propuštění pomocí NIHSS (National Institute of Health Score Scale) škály. Ve stejné skupině pacientů bylo v průběhu prvních 24 h po rozvinutí CMP provedeno vyšetření CT a byly zaznamenány známky časných ischemických lézí. Případy CMP byly rozděleny do podskupin pomocí TOAST klasifikace. Výsledky: Hladiny hs-CRP v séru pacientů s CMP byly významně vyšší v porovnání s pacienty, kteří CMP neměli. Nejvyšší podíl pacientů s hladinami hs-CRP > 3 mg/ l byl pozorován u pacientů s CMP nejasné etiologie. U hs-CRP byla ukázána významná diagnostická schopnost identifikovat CMP; optimálním kritériem byla hladina 4,29 mg/ l. Poměr šancí u hs-CRP pozitivních pacientů ukázal zvýšené riziko CMP a ischemických lézí. Hladina hs-CRP pozitivně korelovala s NIHSS a časem testování a negativně s vývojem CMP. Závěr: Hs-CRP je spolehlivý marker, který může být nápomocen při diagnóze CMP, její etiologické klasifikaci, klinických projevech a prognóze. Naše výsledky přispívají k identifikaci charakteristik CMP v populaci Bulharska.
Aim: High-sensitive C-reactive protein (hs-CRP) has demonstrated good sensitivity in the diagnosis of cerebrovascular events. The study aimed to explore the as sociations between hs-CRP and stroke risk factors, neurological impairment, cerebral ischaemia and prognosis. Materials and methods: The study involved 153 patients categorized into two groups: patients with acute ischaemic stroke and patients with risk factors who had not had a stroke. Blood samples were collected to analyze the serum level of hs-CRP. The National Institutes of Health Stroke Scale (NIHSS) was used to determine the neurological disability of the stroke patients upon hospital admission and upon discharge. Cerebral CT was performed on the same group of patients dur - ing the fi rst 24 h after stroke onset and evidence of early ischaemic lesions was recorded. The stroke cases were divided into subgroups according to the TOAST classification. Results: The serum levels of hs-CRP of the stroke patients were significantly higher than in the non-stroke patients. The highest percentage of patients with hs-CRP values > 3mg/ L was observed in cases with stroke of undetermined aetiology. Hs-CRP showed a significant diagnostic ability in identifying stroke with an optimal criterion of 4.29 mg/ L. The odds ratios for hs-CRP positive patients indicated an increased risk of stroke and ischaemic lesions. Hs-CRP cor related positively with NIHSS at the time of test ing and negatively with stroke evolution. Conclusion: Hs-CRP is a reliable marker that can be helpful for stroke diagnosis, aetiological classification, clinical manifestation and prognosis. Our results contribute to identifying the characteristics of stroke among the Bulgarian population.
Available data from observational studies on the association of admission hyperglycemia (aHG) with outcomes of patients with acute ischemic stroke (AIS) treated with intravenous thrombolysis (IVT) are contradictory, especially when stratified by diabetes mellitus (DM) history. We assessed the association of aHG (≥144 mg/dL) with outcomes stratified by DM history using propensity score-matched (PSM) data from the SITS-ISTR. The primary safety outcome was symptomatic intracranial hemorrhage (SICH); 3-month functional independence (FI) (modified Rankin Scale [mRS] scores 0-2) represented the primary efficacy outcome. Patients with and without aHG did not differ in baseline characteristics both in the non-DM (n = 12,318) and DM (n = 6,572) PSM subgroups. In the non-DM group, patients with aHG had lower 3-month FI rates (53.3% vs. 57.9%, P < 0.001), higher 3-month mortality rates (19.2% vs. 16.0%, P < 0.001), and similar symptomatic intracerebral hemorrhage (SICH) rates (1.7% vs. 1.8%, P = 0.563) compared with patients without aHG. Similarly, in the DM group, patients with aHG had lower rates of 3-month favorable functional outcome (mRS scores 0-1, 34.1% vs. 39.3%, P < 0.001) and FI (48.2% vs. 52.5%, P < 0.001), higher 3-month mortality rates (23.7% vs. 19.9%, P < 0.001), and similar SICH rates (2.2% vs. 2.7%, P = 0.224) compared with patients without aHG. In conclusion, aHG was associated with unfavorable 3-month clinical outcomes in patients with and without DM and AIS treated with IVT.

Předkládaný článek shrnuje základní algoritmus vyšetření etiologie ischemické cévní mozkové příhody (iCMP). Rozebírá obecné principy etiologické klasifikace, příčinný a fenotypický. Detailně je rozebrána aktuálně pro denní klinickou nejvhodnější mezinárodně používaná příčinná klasifikace SSS-TOAST, která je upřesněním a vylepšením stále nejvíce používané klasifikace TOAST. SSS-TOAST rozděluje ischemické ikty stále na pět základních typů – kardioembolizační, ateroskleróza velkých tepen, onemocnění malých tepen, jiná určená a neurčená. Podle úrovně důkazu pak vymezuje tři síly etiologické klasifikace – jasná, pravděpodobná a možná. V článku jsou rozebrány vyšetřovací metodiky, které originální publikace klasifikačního systému vyžaduje. Součástí článku je schematické rozdělení vyšetřovacích taktik do tří panelů – základního (pro všechny pacienty s iCMP), pokročilého (pokud je negativní základní panel a u cévně mladších pacientů) a detailního (vysoce výběrového při významném klinickém podezření na vzácnou etiologii iCMP).
The presented article summarizes the algorithm for the diagnostic evaluation of patients with ischemic stroke. The general principles of stroke etiology classification, i.e. causative and phenotypic, are discussed. The classification system deemed the most practical for daily clinical practice SSS-TOAST is described in detail. SSS-TOAST is an upgrade of the most widely used classification system TOAST. It divides the ischemic strokes into five etiology subgroups (cardioembolic, large artery atherosclerosis, small artery occlusion, other etiology and undetermined). Based on the weight of evidence, each subtype is subdivided into three subcategories as evident, probable, or possible. The algorithm defines three diagnostic panels – basic (for ischemic stroke patients), advanced (if the basic panel examinations are negative or in all young stroke patients), and detailed (highly selective examinations for the rare causes of ischemic stroke).

BACKGROUND The aim of the study was to investigate the role of von Willebrand factor (vWF), the vWF-cleaving protease, ADAMTS13, the composition of thrombus, and patient outcome following mechanical cerebral artery thrombectomy in patients with acute ischemic stroke. MATERIAL AND METHODS A prospective cohort study included 131 patients with ischemic stroke (<6 hours) with or without intravenous thrombolysis. Interventional procedure parameters, hemocoagulation markers, vWF, ADAMTS13, and histological examination of the extracted thrombi were performed. The National Institutes of Health Stroke Scale (NIHSS) score was used on hospital admission, after 24 hours, at day 7; the three-month modified Rankin Scale score was used. RESULTS Mechanical thrombectomy resulted in a Treatment in Cerebral Ischemia (TICI) score of 2-3, with recanalization in 89% of patients. Intravenous thrombolysis was used in 101 (78%). Patients with and without intravenous thrombolysis therapy had a good clinical outcome (score 0-2) in 47% of cases (P=0.459) using the three-month modified Rankin Scale. Patients with a National Institutes of Health Stroke Scale (NIHSS) score ≥15 had significantly increased vWF levels (P=0.003), and a significantly increased vWF: ADAMTS13 ratio (P=0.038) on hospital admission. Significant correlation coefficients were found for plasma vWF and thrombo-embolus vWF (r=0.32), platelet (r=0.24), and fibrin (r=0.26) levels. In the removed thrombus, vWF levels were significantly correlated with platelet count (r=0.53), CD31-positive cells (r=0.38), and fibrin (r=0.48). CONCLUSIONS In patients with acute ischemic stroke, mechanical cerebral artery thrombectomy resulted in a good clinical outcome in 47% of cases, with and without intravenous thrombolysis therapy.

BACKGROUND AND PURPOSE: Higher admission serum glucose levels have been associated with poor outcomes in patients with acute ischemic stroke (AIS) treated with IV thrombolysis. We sought to evaluate the association of admission serum glucose with early outcomes of patients with emergent large vessel occlusion (ELVO) treated with mechanical thrombectomy (MT). METHODS: Consecutive AIS patients due to ELVO treated with MT in three tertiary stroke centers were evaluated. The following outcomes were documented using standard definitions: symptomatic intracranial hemorrhage (sICH), complete reperfusion, mortality, functional independence (modified Rankin Scale (mRS) score of 0-2), and functional improvement (shift in mRS score) at 3 months. The association of admission serum glucose and admission hyperglycemia (>140 mg/dL) with outcomes was evaluated using univariable and multivariable binary and ordinal logistic regression models. RESULTS: 231 AIS patients with ELVO (mean age 62±14 years, 51% men, median admission National Institute of Health Stroke Scale score 16 points (IQR 12-21), median admission serum glucose 125 mg/dL (IQR 104-162)) were treated with MT. Admission hyperglycemia was associated with a lower likelihood of functional improvement (common OR 0.53; 95% CI 0.31 to 0.97; p=0.027) and higher odds of 3 month mortality (OR 2.76; 95% CI 1.40 to 5.44; p=0.004) in multivariable analyses adjusting for potential confounders. A 10 mg/dL increase in admission blood glucose was associated with a higher likelihood of sICH (OR 1.07; 95% CI 1.01 to 1.13; p=0.033) and 3 month mortality (OR 1.07; 95% CI 1.02 to 1.12; p=0.004) in multivariable models. There was no association between admission serum glucose or hyperglycemia and complete reperfusion. CONCLUSIONS: Higher admission serum glucose and admission hyperglycemia are independent predictors of adverse outcomes in ELVO patients treated with MT.
