Intoxikace vodou, která vede k hyponatremii, případně až hemolýze, je relativně vzácný, ale potenciálně smrtelný stav, jenž bývá nejčastěji pozorován u psů, kteří si rádi hrají ve vodě, je ale zaznamenána také např. u telat nebo lidí. Příznaky intoxikace vodou jsou nespecifické a nastupují poměrně rychle. Pokud nedojde k rychlému zásahu, jedinec upadá v důsledku edému mozku do kómatu a umírá.
Water intoxication that leads to hyponatraemia or even hemolysis is a relatively rare but potentially fatal condition that is most commonly observed in dogs who like to play in water, but is also described, for example, in calves or humans. The symptoms of water intoxication are non-specific and occur quite quickly. If there is no rapid intervention, the individual falls into a coma due to brain edema and dies.

OBJECTIVES: A novel method of long-term telemetric monitoring of mean arterial pressure (MAP) and intracranial pressure (ICP) for the determination of current cerebral perfusion pressure (CPP) and the time course of ICP in freely moving rats under physiological conditions and with increased ICP due to the induced cerebral edema were studied. METHODS: The brain edema, that caused volume enlargement and ICP elevation was achieved in entirely experimental conditions without any parallel pathological process. Vasogenic/extracellular edema was induced by osmotic blood-brain barrier disruption (BBBd) and for induction of cytotoxic/intracellular edema the water intoxication model (WI) was used. RESULTS: The results showed significantly elevated values of ICP both in conditions of osmotic blood-brain barrier disruption (BBBd model) and cytotoxic/intracellular edema (WI model) compared to intact rats. The average values of ICP were significantly higher in WI model compared to osmotic BBBd model. Distinct pattern of elevated ICP, related to the selected way of experimental brain edema induction, was found. In the experimental model of osmotic BBB disruption, the elevation of ICP started earlier but was of very short duration. In WI model the elevation of ICP was present during the whole period of monitoring. CONCLUSION: Our results indicate that purely experimental models of brain edema (WI, BBBd) without any parallel pathological process can compromise the basic brain homeostatic activity.

Otrava vodou patří mezi poměrně vzácně se vyskytující diagnózy na interních odděleních. Jedná se o život ohrožující stav, při kterém se může rozvinout mozkový edém s neurologickou symptomatologií až smrtí. Patofyziologickým podkladem je hypoosmolární hyperhydratace, s přesunem vody do buněk. Stav může mít řadu patologických příčin, vyskytovat se jako nežádoucí účinek některých léků, v rámci jiných onemocnění, z iatrogenních příčin, nebo je důsledkem extenzivního příjmu hypoosmolárních tekutin u jinak zdravých lidí.
Water intoxication is a relatively rare occurrence in departments of internal medicine. It is a life-threatening condition in which cerebral oedema with neurological symptomatology or even death can occur. The pathophysiology involves hypo-osmolar hyperhydration, with a shift of water into the cells. The condition can have a number of pathological causes, occur as an adverse effect of certain medications, as part of other diseases, due to iatrogenic causes, or as a result of excessive intake of hypo-osmolar fluids in otherwise healthy individuals.

AIM: Paediatric hyponatraemia is usually caused by an excess of antidiuretic hormone and may lead to serious neurological complications. It is challenging for clinicians to differentiate between conditions causing excess water and salt loss. This review analyses individual causes of hyponatraemia and focuses on optimal diagnostic algorithms and treatment strategies. CONCLUSION: Correct evaluation of hyponatraemia requires proper understanding of the aetiology and appropriate management calls for a detailed history, physical examination and specific laboratory investigations.

Diabetická nefropatie patří mezi časté komplikace diabetu mellitu 1. i 2. typu a je nejčastější příčinou chronického renálního selhání. V kazuistice je popsán případ diabetičky s již rozvinutými makro- i mikrovaskulárními komplikacemi. Zajímavým nálezem je ložisko v játrech, které zprvu imponuje jako subkapsulární hematom. V diferenciální diagnostice se uvažuje i o parazitární cystě, zejména vzhledem k velmi výraznému počtu eozinofilů v krevním obraze. Biopsií se ovšem diagnostikuje jaterní absces. Následně jsou nalezeny i nezralé abscesy plic a ovaria. Jak je vidět, je kromě typických infekcí spojených s diabetem přítomna u naší pacientky řada dalších infekčních komplikací majících fatální následky.
Diabetic nephropathy is one of the common complications of diabetes mellitus 1st and 2nd type, and is the leading cause of chronic renal failure. In the case report is described a case of diabetic patient with macro- and microvascular complications already developed. An interesting finding is liver focus which is initially impressed as a subcapsular hematoma, in the differential diagnosis is also considered parasitic cyst mainly due to a significant number of eosinophils in the blood. However, biopsy-diagnosed liver abscess. Subsequently, they are also found immature lung abscesses and ovarian abscess. As you can see, in addition to typical infections associated with diabetes present in our patient series of other infectious complications with fatal consequences.

Jedním z problémů hemodialyzovaných pacientů je kvantitativní zhodnocení bilance tekutin a dosažení normohydratace. Metoda BCM (body composition monitor) nám umožnuje neinvazivně a přesně kvantifikovat množství tělesné vody, stanovit optimální hmotnost pacienta a následně udržet „fyziologický“ stav hydratace.
One of the problems of hemodialysis patients is a quantitative assessment of fluid balance and achieve normohydratation. BCM method (body composition monitor) allows us to noninvasively and accurately quantify the amount of body fluid, determine the optimal weight of the patient and then conserve the „physiological“ state of hydration.

There are two distinct facets of adequate fluid balance control in haemodialysis patients--estimation of dry weight (DW) as the target and adequate ultrafiltration (UF) strategy, i.e. the way to reach the target in a possibly symptom-free way. The article reviews the continuous blood volume monitoring (CBVM) based procedures to deal with the former facet-DW determination. The existing approaches are divided in three groups--methods defining certain alert value of relative blood volume (RBV) reduction, methods working with RBV response to constant UF rate, and methods evaluating dynamics of RBV response to UF pulse or chain of UF pulses. While the first and the third approaches are relatively easy to automate, the second group of methods are suitable mainly for observational evaluations only. All the discussed methods, without exception, need large-scale verification, as they all were evaluated in the majority by their authors only and on small patient cohorts.
