OBJECTIVE: Genetic and environmental factors interact in the development of major depressive disorder (MDD). While neurobiological correlates have only partially been elucidated, altered levels of calcitonin gene-related peptide (CGRP)-like immunoreactivity (LI) in animal models and in the cerebrospinal fluid of depressed patients were reported, suggesting that CGRP may be involved in the pathophysiology and/or be a trait marker of MDD. However, changes in CGRP brain levels resulting from interactions between genetic and environmental risk factors and the response to antidepressant treatment have not been explored. METHODS: We therefore superimposed maternal separation (MS) onto a genetic rat model (Flinders-sensitive and -resistant lines, FSL/FRL) of depression, treated these rats with antidepressants (escitalopram and nortriptyline) and measured CGRP-LI in selected brain regions. RESULTS: CGRP was elevated in the frontal cortex, hippocampus and amygdala (but not in the hypothalamus) of FSL rats. However, MS did not significantly alter levels of this peptide. Likewise, there were no significant interactions between the genetic and environmental factors. Most importantly, neither escitalopram nor nortriptyline significantly altered brain CGRP levels. CONCLUSION: Our data demonstrate that increased brain levels of CGRP are present in a well-established rat model of depression. Given that antidepressants have virtually no effect on the brain level of this peptide, our study indicates that further research is needed to evaluate the functional role of CGRP in the FSL model for depression.

Erenumab je prvním přípravkem ze zcela nové skupiny léčiv pro epizodickou a chronickou migrénu, která se nyní dostávají do klinické praxe. Jedná se o skupinu humanizovaných nebo humánních protilátek proti receptoru pro CGRP (calcitonin gene‑related peptide) nebo proti samotnému neuropeptidu CGRP. Přípravek se dostává na trh pod názvem Aimovig jak v zemích Evropské unie, tak v USA. Erenumab je humánní monoklonální protilátka, která se váže na receptor CGRP nacházející se v místech relevantních pro patofyziologii migrény, jako je například ganglion trigeminale. CGRP je neuropeptid, který moduluje nociceptivní signalizaci, a vazodilatátor spojovaný s patofyziologií migrény. Během ataky migrény byl pozorován významný nárůst koncentrací CGRP v krvi z vena jugularis. Pro klinickou účinnost a bezpečnost podání erenumabu svědčí výsledky dvou aktuálně publikovaných studií. Podle Souhrnu údajů o přípravku je erenumab určen k profylaxi migrény u dospělých, kteří jí trpí nejméně čtyři dny v měsíci. Je podáván v subkutánních injekcích obvykle jednou za čtyři týdny. Nežádoucími účinky jsou nejčastěji reakce v místě aplikace hodnocené podle intenzity jako mírné nebo střední.
Erenumab is a novel, first‑in‑class drug for the treatment of episodic and chronic migraine, now entering the clinical practice. This new class of drugs is formed by humanized or human antibodies against the CGRP (calcitonin gene‑related peptide) receptor or against the neuropeptide CGRP itself. Erenumab, manufactured as Aimovig, is now being launched in both European Union and the U.S.A. It is a human monoclonal antibody targeting the CGRP receptor at sites relevant for the pathophysiology of migraine such as the trigeminal ganglia. CGRP is a neuropeptide modulating nociceptive signaling and also a vasodilating agent associated with the pathophysiology of migraine. Significant rise in the level of CGRP has been detected in blood samples taken from the jugular vein during migraine attacks. The clinical effectiveness and safety of erenumab have been demonstrated in two recently published trials. According to SPC, erenumab can be used for the prophylaxis of migraine in adults suffering from migraine attacks at least 4 days a month. It is applied subcutaneously, usually once in 4 weeks. Adverse effects may consist of application site reactions, usually of mild or moderate severity.

Podle nejnovější mezinárodní klasifikace bolestí hlavy ICHD‑3 (International Classification of Headache Disorders‑3) publikované letos v lednu se bolesti hlavy s malými obměnami nadále dělí do tří skupin: na primární bolesti hlavy, sekundární bolesti hlavy a bolestivé kraniální neuropatie a jiné bolesti obličeje a hlavy. Na rozdíl od sekundárních bolestí hlavy, které mají svou jasnou etiologickou příčinu (z velké škály například cévní nebo nádorovou), je podklad primární bolesti hlavy čistě patofyziologický a příznačný pro danou podskupinu. Následující text je věnován přehledu léčby nejznámější primární bolesti hlavy – migrény – v současnosti a blízké budoucnosti.
According to the latest International Classification of Headache Disorders‑3 (ICHD‑3) published this January, headaches are with small variations still divided into 3 groups: primary headaches, secondary headaches and painful cranial neuropathies, other facial pains and other headaches. As secondary headaches have their clear etiological causes (largely for example vascular or tumorous), primary headaches have purely pathophysiological background which is characteristic for each subgroup. The following text is a review of the treatment of the most common primary headache – migraine – in the present and near future.

Ciguatoxins (CTXs) are marine toxins that cause ciguatera fish poisoning, a debilitating disease dominated by sensory and neurological disturbances that include cold allodynia and various painful symptoms as well as long-lasting pruritus. Although CTXs are known as the most potent mammalian sodium channel activator toxins, the etiology of many of its neurosensory symptoms remains unresolved. We recently described that local application of 1 nM Pacific Ciguatoxin-1 (P-CTX-1) into the skin of human subjects induces a long-lasting, painful axon reflex flare and that CTXs are particularly effective in releasing calcitonin-gene related peptide (CGRP) from nerve terminals. In this study, we used mouse and rat skin preparations and enzyme-linked immunosorbent assays (ELISA) to study the molecular mechanism by which P-CTX-1 induces CGRP release. We show that P-CTX-1 induces CGRP release more effectively in mouse as compared to rat skin, exhibiting EC50 concentrations in the low nanomolar range. P-CTX-1-induced CGRP release from skin is dependent on extracellular calcium and sodium, but independent from the activation of various thermosensory transient receptor potential (TRP) ion channels. In contrast, lidocaine and tetrodotoxin (TTX) reduce CGRP release by 53-75%, with the remaining fraction involving L-type and T-type voltage-gated calcium channels (VGCC). Using transgenic mice, we revealed that the TTX-resistant voltage-gated sodium channel (VGSC) NaV1.9, but not NaV1.8 or NaV1.7 alone and the combined activation of the TTX-sensitive VGSC subtypes NaV1.7 and NaV1.1 carry the largest part of the P-CTX-1-caused CGRP release of 42% and 34%, respectively. Given the contribution of CGRP to nociceptive and itch sensing pathways, our findings contribute to a better understanding of sensory symptoms of acute and chronic ciguatera that may help in the identification of potential therapeutics.

The survey deals with all important aspects of migraine. At the beginning the epidemiology, impact of the disease on the individual and the whole society and clinical picture are mentioned. In the next part the pathophysiology of migraine is described including cortical spreading depression, trigeminal vascular system, vasoactive neuropeptides, especially calcitonin gene-related peptide, vasodilatation and sterile neurogenic inflammation, transmission of the pain along the trigeminal pathway, mechanisms of peripheral and central sensitization, significance of brainstem generator, serotonin and nitric oxide. Genetics of migraine and provoking factors are described. Menstrual migraine is specially mentioned. Then the pharmacotherapy of migraine follows, which is divided into the acute and prophylactic treatment. From the acute therapy except other medications especially triptans are described in detail. From the prophylactic treatment all drug groups used in migraine are listed. The paper continues with the treatment of menstrual migraine, migraine in pregnancy and pediatric migraine. The last part is dedicated to the future therapy which includes receptor antagonists of calcitonin gene-related peptide, glutamate receptor antagonists, especially memantine, then tonabersat, some antiepileptic drugs and inhibitors of nitric oxide synthase.
