Jsou porovnány výsledky manipulační léčby vertebrogenních algických syndromů bez a dále s podporou preparátu Myolastan. Myolastan se ukazuje jako vhodná medikamentózní podpora manipulační léčby zejména u chronických neurotizovaných pacientů. U akutních stavů je prokázáno zvýšení pohyblivosti.
The author compares results of manipulation treatment of vertebrogenic algic syndromes without and with the support of the preparation Myolastan. Myolastan proved suitable medicamentous support of manipulation treatment in particular in chronic neurotic patients. In acute conditions increased mobility was demonstrated.
Cervikokraniální syndrom zahrnuje bolest hlavy způsobenou poruchou v oblasti krční páteře. Terapie cervikokraniálního syndromu vyžaduje komplexní multioborový přístup s využitím farmakologické léčby i nefarmakologických postupů vč. rehabilitační a myoskeletální medicíny. Mobilizace a manipulace byly vyhodnoceny jako postupy přínosné při léčbě poruch krční páteře. Cílem práce bylo zhodnotit účinky manipulace krční páteře podle principů myoskeletální medicíny. Do studie bylo zařazeno 103 probandů s diagnostikovaným cervikokraniálním syndromem, kteří byli náhodně rozděleni do kontrolní a experimentální skupiny. V kontrolní skupině probandi absolvovali standardní rehabilitační myoskeletální léčbu. V experimentální skupině byla ke standardní myoskeletální rehabilitační léčbě přidána vstupně jednorázová nárazová manipulace krční páteře. Naše studie prokázala, že manipulace krční páteře vede bezprostředně po zákroku k signifikantnímu zlepšení hybnosti. Po 6týdenní komplexní terapii pozorujeme zlepšení hybnosti a zmírnění bolesti krční páteře v obou sledovaných skupinách. Lepší efekty pozorujeme v experimentální skupině, avšak tyto rozdíly jsou u našeho souboru pacientů nesignifikantní.
The symptoms of cervicocranial syndrome include headache caused by cervical spine disorder. The treatment of cervicocranial syndrome requires a comprehensive multidisciplinary approach using pharmacological treatment and non-drug procedures, including physiotherapy and myoskeletal therapy. There is a consensus that the physiotherapy procedures of mobilization and manipulation are beneficial in the treatment of cervical spine disorders. The aim of the study was to evaluate the effects of manipulation therapy of the cervical spine according to the principles of myoskeletal medicine. The study included 103 patients with cervicocranial syndrome, who were randomly divided into a control and an experimental group. The control group underwent standard physiotherapy myoskeletal treatment. In addition to the standard physiotherapy myoskeletal treatment, the experimental group received one initial thrust manipulation of the cervical spine at the very beginning of the treatment. The results of our study showed that the manipulation of the cervical spine led to a significant increase in immediate mobility after the therapy. An improvement of cervical spine mobility and pain reduction were observed in both groups after six weeks of comprehensive physiotherapy myoskeletal treatment. The differences between the experimental group and controls were shown to be non-significant.

Manipulační terapie krční páteře je jedna z nejčastěji používaných forem léčby bolestivých stavů krční páteře. Vertebrobazilární cévní poškození je jedna z nejčastěji citovaných komplikací manipulací krční páteře. Autoři se v literárním přehledu zaměřili na shrnutí dosavadních publikovaných názorů a pozorování o této problematice. Z dosavadních údajů je zřejmé, že výskyt cerebrovaskulární léze následkem manipulace krční páteře je diskutabilní, nepotvrzený. Je-li zde souvislost, je výskyt této komplikace v porovnání s počtem provedených manipulací statisticky naprosto zanedbatelný. Neexisuje žádný specifický test k odhalení nebezpečí vzniku cerebrovaskulární komplikace.
Prezentujeme kazuistiku 37leté ženy, u které bezprostředně po manipulaci došlo k rozvoji neurologické symptomatologie. Diagnostikovaný uzávěr a. basilaris byl úspěšně vyřešen akutní implantací stentu. Vertebrobazilární disekce po manipulaci krční páteře je vzácná komplikace této léčebné metody. Přesná incidence vzniku disekce ve vertebrobazilárním povodí a následně iktu není známa. Evidentní zhoršení bolestí v šíji a vznik neurologické symptomatologie v návaznosti na manipulaci vyžaduje okamžité neurologické vyšetření. Nejdostupnější zobrazovací metodou v diagnostice uzávěru bazilární tepny je CT angiografie. V posouzení viability tkáně má dominantní postavení vyšetření magnetickou rezonancí – DWI (diffusion-weighted imaging) a PWI (perfusion-weighted imaging) vyšetření. Diagnostikovaný uzávěr bazilární tepny je indikací k akutnímu endovaskulárnímu řešení.
The authors present the case of a 37-year-old female who presented with severe acute dizziness with subsequent neurological deficit. A diagnosis of basilar artery occlusion was made and successfully treated with acute stenting.Vertebrobasilar dissection after manipulation of the cervical spine is a rare complication of this therapeutic modality. The exact incidence of vertebrobasilar dissection and subsequent stroke is not known. The significant worsening of neck pain and the onset of neurological symptoms after the manipulation indicates urgent neurological examination. CT angiography (CTA) is the most available imaging method in the diagnostics of basilar artery occlusion. Diffusion-weighted MRI (DWI) in combination with perfusion- weighted MRI (PWI) has proved wisely useful in the selection of patients suitable for acute therapy, should a mismatch between these procedures suggest viable penumbral tissue. A diagnosis of occlusion of the basilar artery is a strong indication for urgent endovascular intervention.
