Residual mitral regurgitation after MitraClip implantation is a progressively increasing problem. Progression of left ventricular dysfunction, deterioration of valvular and subvalvular function, or suboptimal MitraClip implantation all increase the risk of progression of residual regurgitation. In this case report, we describe a patient with severe degenerative mitral regurgitation managed with repeated implantation of MitraClips followed by implantation of Amplatzer Vascular Plugs between the clips, with a significant reduction in regurgitation and functional normalization of the patient's complaints.
Reziduální mitrální regurgitace po implantaci MitraClipů je postupně narůstající problém. Progrese dysfunkce levé komory, zhoršení funkce chlopně a subvalvulárního aparátu či suboptimální implantace MitraClipů zvyšuje riziko progrese reziduální regurgitace. V kazuistice popisujeme pacienta s těžkou degenerativní mitrální regurgitací řešenou opakovanou implantací MitraClipů a následnou implantací Amplatzer vascular plugů mezi klipy s výraznou redukcí regurgitace a funkční normalizací potíží pacienta.

Pacientka s častými komorovými extrasystolami s arytmogenní dysfunkcí levé komory. Nejprve pacientka prodělala endokardiální a epikardiální katétrovou ablaci. Po výkonu dle 24hodinového EKG holterovského vyšetření došlo k záchytu 36 % komorových extrasystol (KES). Po důsledném zvážení byla u nemocné indikována alkoholová ablace retrográdně v terciární větvi. Již den po výkonu došlo k významné redukci celkového počtu KES. Kontrolní echokardiografi cké vyšetření při propuštění ukázalo zlepšenou systolickou funkci levé komory.
Female patient with frequent premature ventricular contraction (PVC) with arrhythmia induced cardiomyopathy with left ventricle systolic dysfunction. At fi rst patient underwent endocardial end epicardial catheter ablation via the coronary sinus. After these procedures a total amount of 36% of PVC activity was verifi ed with 24 hour EKG holter. Therefore after detailed consideration and due to numerous publications we chose retrograde coronary venous ethanol ablation in third branches. AIMS: Optimal ECG monitoring in detecting recurrences of atrial fibrillation (AF) or atrial tachycardia (AT) after catheter ablation has not been well established. The purpose of this prospective study was to compare the utility of daily ECG monitoring with episodic card recorder (ECR) vs. periodic monitoring with episodic loop recorder (ELR) for the detection of post-blanking AF/AT recurrences during early (Months 4-6) and late (Months 7-12) periods after catheter ablation for paroxysmal AF. METHODS: The study included 105 consecutive patients, who received ECR for 12 months and were instructed to send at least 2 random ECG recordings daily with extra-recordings during symptoms. The patients were simultaneously monitored for one week with ELR at the end of each period (Months 6 and 12). RESULTS: Thirty-one and 12 patients with AF/AT recurrence were identified by means of ECR and ELR, respectively. In patients with complete and valid data, ELR technology was inferior to ECR by detecting AF/AT in 5 (31%) of 16 and 5 (26%) of 19 patients with arrhythmia identified by ECR in the early and late period, respectively. Overall, ELR had a sensitivity of 8/23 (35%) for detecting AF/AT recurrence. There was no single patient with AF/AT recurrence on ELR that would not be known from ECR monitoring. Only 2 patients with arrhythmia recurrence were completely asymptomatic throughout the study period. CONCLUSION: Daily ECG monitoring with ECR was better than periodic monitoring with ELR in detecting AF/AT recurrences during the follow-up periods. Entirely asymptomatic patients with AF/AT recurrences were rare.

Left atrial (LA) volume (LAV) is used for the selection of patients with atrial fibrillation (AF) to rhythm control strategies. Calculation of LAV from the LA diameters and areas by two-dimensional (2D) echocardiography may result in significant error. Accuracy of atrial volume assessment has never been studied in patients with long-standing persistent AF (LSPAF) and significant atrial remodeling. This study investigated correlation and agreement between 2D echocardiographic (Simpson method) and electroanatomic (CARTO, Biosense Webster) left and right atrial (RA) volumes (LAV(ECHO) vs. LAV(CARTO) and RAV(ECHO) vs. RAV(CARTO)) in patients undergoing catheter ablation for LSPAF. The study enrolled 173 consecutive subjects (females: 21 %, age: 59+/-9 years). There was only modest correlation between LAV(ECHO) (92+/-31 ml) and LAV(CARTO) (178+/-37 ml) (R=0.57), and RAV(ECHO) (71+/-29 ml) and RAV(CARTO) (173+/-34 ml) (R=0.42), respectively. LAV(ECHO) and RAV(ECHO) underestimated LAV(CARTO) and RAV(CARTO) with the absolute bias (+/-1.96 standard deviation) of -85 (-148; -22) ml and -102 (-169; -35) ml, respectively, and with the relative bias of -48 (-75; -21) % and -59 (-88; -30) %, respectively (all P<0.000001 for their mutual difference). Significant confounders of this difference were not identified. In patients with LSPAF, 2D echocardiography significantly underestimated both LA and RA volumes as compared with electroanatomic reference. This disagreement was independent of clinical, echocardiographic and mapping characteristics.

Hypertrofická kardiomyopatie je v současnosti chápána jako skupina onemocnění s hypertrofií levé komory, která nevznikla na podkladě adaptačních mechanizmů. V 1. části přehledového článku byly podrobně rozebrány možnosti zobrazení srdce pomocí magnetické rezonance v rámci diagnostiky sarkomerické formy hypertrofické kardiomyopatie, 2. část bude věnována možnostem rozlišení sarkomerické formy od jejích fenokopií.
Hypertrophic cardiomyopathy is currently understood as a group of diseases with left ventricular hypertrophy, which are not based on adaptive mechanisms. The first part of the review details the possibility of cardiac magnetic resonance in the diagnosis of sarcomeric forms of hypertrophic cardiomyopathy, the second part will focus on the possibilities of distinguishing the sarcomeric forms from their phenocopies.

Transkatétrová implantace aortální chlopně (TAVI) je preferovanou metodou léčby u neoperabilních anebo vysoce rizikových nemocných se závažnou aortální stenózou. Základní princip krimplovatelné bioprotézy a její implantace do stenotické nativní chlopně zůstává neměnný, došlo však k vývoji několika rozdílných konceptů bioprotéz i přístupových cest. Všechny koncepty prokázaly svoji bezpečnost a účinnost, existují však limitovaná data porovnávající jednotlivé chlopně či přístupové cesty mezi sebou. Cílem článku je poskytnout přehled o běžně používaných přístupových cestách k TAVI a blíže je specifikovat.
The transcatheter aortic valve implantation (TAVI) is a preferred treatment option among the patients with severe aortic stenosis who were considered to be non-operable or at high surgical risk. The basic principle of the crimped valve and the catheter-based implantation to the stenotic native valve remains unchanged, but several different concepts of bioprosthesis and approaches of valve implantation have been developed. All the concepts have proven their safety and efficiency, however, there are only limited data available comparing one approach to another. The objective of this paper is to offer an overview and more detailed specification of current TAVI access techniques.
