Cíl studie: Přehled problematiky hirsutismu, excesivního růstu ochlupení u žen. Přestože na první pohled se jedná o kosmetický problém, většinou je příčinou endokrinní porucha. Typ sudie: Přehledový článek. Materiál a metodika: Hirsutismus je klinická diagnóza definovaná nadměrným růstem ochlupení v androgen-senzitivních oblastech postihující 5–10 % žen v reprodukčním věku. Hirsutismus může být počátečním i jediným příznakem hyperandrogenního stavu. Závěr: Nejčastější příčinou hirsutismu je syndrom polycystických ovarií. Obvykle znamená kromě kosmetického dopadu i vážný psychologický problém pro pacientky, jenž vyžaduje celkovou léčbu. V případě rychle progredujícího a extenzivního hirsutismu, s vysokými hladinami androgenů, je třeba v první řadě vyloučit nejvážnější příčinu, androgen-secernující tumor.
Objective: Overview of excessive hair growth in women, hirsutism. Although women with hirsutism typically present because of cosmetic concerns, the majority also have an underlying endocrine disorder. Design: Review article. Setting: Department of Gynecology and Obstetrics, General Faculty Hospital and 1st Faculty of Medicine of Charles Universtity, Prague. Material and methods: Hirsutism is a clinical diagnosis defined by the presence of excess terminal hair growth (dark, coarse hairs) in androgen-sensitive areas. It affects between five and ten percent of women of reproductive age. It may be the initial, and possibly only, sign of an underlying androgen disorder. Conclusions: The most common cause of hirsutism is polycystic ovary syndrome (PCOS). In some cases, hirsutism is mild and requires only reassurance and local (nonsystemic) therapy, while in others it causes significant psychological distress and requires more extensive therapy. In case of rapid progressive hair growth should be first exclude androgen-secerning tumour (ovarian, adrenal) as the most serious condition.

Prezentujeme kazuistiku šestiletého chlapce s germinomem v neobvyklé lokalizaci thalamu, který se klinicky manifestoval předčasnou pseudopubertou na podkladě ektopické sekrece humánního choriogonadotropinu (hCG). Nádor byl potvrzen ze stereotaktické biopsie, následná chemoterapie a radioterapie vedla ke kompletní remisi. Je diskutováno o mechanizmu vzniku předčasné pseudopuberty, klinických i léčebných aspektech germinomů mozku u dětí.
We describe a case of a 6 years old boy with germinoma of an unusual thalamic localization that presented with precocious pseudopuberty based on ectopic human chorionic gonadotropin (hCG) secretion. The tumour was confirmed with stereotactic biopsy; subsequent chemotherapy and radiotherapy led to complete remission. The mechanism of precocious pseudopuberty and clinical and treatment aspects of brain germinomas in children arediscussed.
