Prostate cancer is the most common malignancy in men, mostly affecting older men who harbor an increased prevalence of cardiovascular disease and metabolic syndrome. Androgen deprivation therapy (ADT), the standard therapy for various stages of prostate cancer, further increases the risk for cardiovascular disease and for metabolic syndrome. Therefore, screening for cardiovascular risk factors should be performed prior to the initiation of ADT, and, if necessary, cardiological evaluation and interdisciplinary management should be provided during and after completion of ADT. Moreover, the use of a gonadotropin-releasing hormone (GnRH) antagonist may help reduce cardiovascular risk in patients with cardiovascular disease.

Androgen deprivační léčba s využitím analogů gonadoliberinu je základním prvkem léčby u indikovaných případů karcinomu prostaty. Agonisté se využívají častěji, ale v úvodu pro potlačení flare fenomenu je nutné zajištění antiandrogenní terapie, pokles hladiny testosteronu je pozdější. Antagonisté mají rychlý nástup účinku s poklesem testosteronu, zdá se i snížené riziko kardiovaskulárních komplikací a p. o. relugolix není zatížen bolestí a reakcemi v místě s. c. injekce jako degarelix. Pozornost je nutné věnovat rizikovým lékovým interakcím.
Androgen deprivation therapy with gonadoliberin analogues are an essential element of treatment in indicated cases of prostate cancer. Agonists are more often used, but in the beginning, antiandrogen therapy is necessary to suppress the flare phenomenon, and the decrease in testosterone levels is later. The antagonists have a rapid onset of testosterone level decrease, there appears to be a reduced risk of cardiovascular complications, and p.o. relugolix is not burdened with pain and reactions at the site of s.c. injection like is in degarelix. Attention should be paid to risky drug interactions.

Androgen-deprivační (hormonální) léčba je využívána v terapii karcinomu prostaty (KP) již téměř 50 let. Lze ji použít u pacientů s lokalizovaným KP s vyšším rizikem, jednoznačně indikována je pak u nemocných s pokročilým a metastatickým KP. Mezi formy hormonální léčby patří chirurgická kastrace (bilaterální orchiektomie), která je již prováděna výrazně méně často, a farmakologická kastrace, tj. podávání agonistů či antagonistů LHRH, jež vede ke srovnatelným (kastračním) hladinám testosteronu. Novou formu této léčby představuje relugolix, který je prvním antagonistou LHRH podávaným perorálně. V případě metastatického onemocnění by vždy, kdy je to možné, měla být hormonální terapie kombinována s novými hormonálními léčivy (léky cílené na androgenní receptor) nebo chemoterapií. Cílem je dosažení a udržení co nejnižších hodnot PSA, které má jasný vliv na prognózu nemocných.
Androgen-deprivation (hormonal) therapy has been used in the treatment of prostate cancer for almost 50 years. It can be used in patients with localized prostate cancer with higher risk, and it is clearly indicated in patients with advanced and metastatic prostate cancer. Forms of hormone therapy include surgical castration (bilateral orchiectomy), which is now performed significantly less frequently, and pharmacological castration, i.e. administration of LHRH agonists or antagonists, which leads to comparable (castration) testosterone levels. A new form of this treatment is relugolix, which is the first LHRH antagonist to be administered orally. In case of metastatic disease, hormonal therapy should be combined with new hormonal drugs (androgen receptor-targeted agents) or chemotherapy whenever possible. The aim is to achieve and maintain the lowest possible PSA values, which has a clear impact on the prognosis of patients.

Cíl: Ilustrační přehled indikací relugolix kombinované terapie (RCT) při řešení symptomů spojených s děložní myomatózou. Metodika: Soubor komentovaných kazuistik z ambulantní a klinické praxe. Výsledky: Soubor zahrnuje neinvazivní metodiku definice excesivního menstruačního krvácení pomocí obrázkového záznamu (PBAC – pictorial bleeding assessment chart). Dále předkládá použití RCT jako fertilitu šetřícího postupu před plánovanou myomektomií a řešení istmického myomu jako děložního faktoru neplodnosti. Komentovány jsou případy RCT adenomyózy u primární sterility a u extragenitálních forem endometriózy. Náhlé příhody spojené s komplikacemi myomatózy v graviditě reprezentuje kazuistika o nekrotizující difuzní myomatóze v puerperiu. Diferenciálně diagnostické rozpaky adnexální patologie a myomatózy, RCT jako finální řešení neúspěšné farmakoterapie a alternativa hysterektomie v premenopauze dokreslují pestré spektrum indikací farmakologické léčby, vč. možností duální terapie RCT s aGnRH. Závěr: Relugolix kombinovaná terapie jako účinná a bezpečná kauzální léčba rozšiřuje terapeutické spektrum a možnosti specialistů na reprodukční medicínu a registrujících gynekologů. Dostupnost konzervativní léčby v kombinaci s chirurgickou léčbou vede k optimalizaci a vyšší efektivitě terapeutických postupů a zvýšení kvality života žen s myomatózou.
Objective: An illustrative review of the indications for relugolix combination therapy (RCT) in the management of symptoms associated with uterine myomatosis. Methods: A set of annotated case reports from outpatient and clinical practice. Results: The file includes a non-invasive methodology for defining excessive menstrual bleeding using the pictorial bleeding assessment chart (PBAC). It also presents the use of RCT as a fertility-sparing procedure prior to elective myomectomy and the management of isthmic fibroids as an uterine factor of infertility. Cases of RCT of adenomyosis in primary sterility and in extragenital forms of endometriosis are commented. Emergent events associated with complications of myomatosis in pregnancy are represented by a case report of necrotizing diff use myomatosis in puerperium. The differential-diagnostic confusion of adnexal pathology and myomatosis, RCT as a final solution to failed pharmacotherapy, and the alternative of hysterectomy in premenopause illustrate the diverse spectrum of indications for pharmacological treatment, including the possibility of dual therapy with RCT and aGnRH. Conclusion: Relugolix combination therapy as an effective and safe causal treatment expands the therapeutic spectrum and options for reproductive medicine specialists and registering gynaecologists. The availability of conservative treatment in combination with surgical treatment leads to optimalization and greater effectiveness of therapeutic procedures and increased quality of life for women with myomatosis.

Intracerebroventricular (ICV) administration of estradiol benzoate (E2B) and progesterone (P) induces intense lordosis behavior in ovariectomized rats primed peripherally with E2B. The present study tested the hypothesis that the Kisspeptin (Kiss) and melanin-concentrating hormone (MCH) pathways regulate female sexual behavior induced by these steroid hormones. In Experiment 1, we tested the relevance of the Kiss pathway by ICV infusion of its inhibitor, kiss-234, before administration of E2B or P in estrogen-primed rats. Lordosis induced by E2B alone or with the addition of P was reduced significantly at 30, 120, and 240 min. In Experiment 2, ICV infusion of MCH 30 min before E2B or P significantly reduced lordosis in rats primed with E2B alone. These data support the hypothesis that the Kiss and MCH pathways, which can release or modulate gonadotropin-releasing hormone (GnRH), are involved in E2B- and P-induced lordosis.

CONTEXT: Degarelix is associated with high rates of injection site reaction. The US Food and Drug Administration approved relugolix, an oral gonadotropin-releasing hormone (GnRH) antagonist, for the treatment of advanced prostate cancer patients. OBJECTIVE: This systematic review and network meta-analysis aimed to compare the efficacy and safety of relugolix versus degarelix. EVIDENCE ACQUISITION: A systematic search was performed using major web databases for studies published before January 30, 2021, according to the Preferred Reporting Items for Systematic Review and Meta-analyses (PRISMA) extension statement for a network meta-analysis. Studies that compared the efficacy (12-mo castration rate with testosterone ≤50 ng/dl) and safety (adverse events [AEs]) of relugolix or degarelix and of the control group (GnRH agonists) were included. We used the Bayesian approach in the network meta-analysis. EVIDENCE SYNTHESIS: Four studies (n = 2059) met our eligibility criteria. The main efficacy analysis was conducted for two different treatments (relugolix and all doses of degarelix vs GnRH agonists); relugolix (risk ratio [RR] 1.09, 95% credible interval [CrI]: 0.95-1.23) and degarelix (RR 0.98, 95% CrI: 0.91-1.06) were not associated with different 12-mo castration rates. In the subgroup analysis, degarelix 480 mg was significantly associated with a lower castration rate (RR 0.46, 95% CrI: 0.07-0.92). In all efficacy ranking analyses, relugolix achieved the best rank. The safety analyses showed that relugolix (RR 0.99, 95% CrI: 0.6-1.6 and RR 0.72, 95% CrI: 0.4-1.3, respectively) and degarelix (RR 1.1, 95% CrI: 0.75-1.35 and RR 1.05, 95% CrI: 0.42-2.6, respectively) were not associated with either all AE or serious AE rates. In the ranking analyses, degarelix achieved the worst rank of all AEs and the best rank of serious AEs. Relugolix (RR 0.44, 95% CrI: 0.16-1.2) and degarelix (RR 0.74, 95% CrI: 0.37-1.52) were not associated with different cardiovascular event (CVE) rates; both were associated with lower CVE rates than GnRH agonists in the ranking analyses. CONCLUSIONS: We found that the efficacy and safety of relugolix are comparable with those of degarelix, albeit with no injection site reaction. Such data should be interpreted with caution until large-scale direct comparison studies with a longer follow-up are available. PATIENT SUMMARY: We found that relugolix, an oral gonadotropin-releasing hormone (GnRH) antagonist, has comparable efficacy and safety with degarelix, a parenteral GnRH antagonist, for the treatment of advanced prostate cancer patients.
