The molecular basis for the propensity of a small number of environmental proteins to provoke allergic responses is largely unknown. Herein, we report that mite group 13 allergens of the fatty acid-binding protein (FABP) family are sensed by an evolutionarily conserved acute-phase protein, serum amyloid A1 (SAA1), that promotes pulmonary type 2 immunity. Mechanistically, SAA1 interacted directly with allergenic mite FABPs (Der p 13 and Blo t 13). The interaction between mite FABPs and SAA1 activated the SAA1-binding receptor, formyl peptide receptor 2 (FPR2), which drove the epithelial release of the type-2-promoting cytokine interleukin (IL)-33 in a SAA1-dependent manner. Importantly, the SAA1-FPR2-IL-33 axis was upregulated in nasal epithelial cells from patients with chronic rhinosinusitis. These findings identify an unrecognized role for SAA1 as a soluble pattern recognition receptor for conserved FABPs found in common mite allergens that initiate type 2 immunity at mucosal surfaces.

Necrotizing enterocolitis (NEC) is a severe gastrointestinal disease affecting mainly preterm newborns. It is characterized by unexpected onset and rapid progression with specific diagnostic signs as pneumatosis intestinalis or gas in the portal vein appearing later in the course of the disease. Therefore, we analyzed diagnostic and prognostic potential of the markers of early NEC pathogenesis, such as excessive inflammatory response (serum amyloid A (SAA)) and gut epithelium damage (intestinal and liver fatty acid-binding protein (I-FABP and L-FABP, respectively) and trefoil factor-3 (TFF-3)). We used ELISA to analyze these biomarkers in the urine of patients with suspected NEC, either spontaneous or surgery-related, or in infants without gut surgery (controls). Next, we compared their levels with the type of the disease (NEC or sepsis) and its severity. Already at the time of NEC suspicion, infants who developed NEC had significantly higher levels of all tested biomarkers than controls and higher levels of I-FABP and L-FABP than those who will later develop sepsis. Infants who will develop surgery-related NEC had higher levels of I-FABP and L-FABP than those who will develop sepsis already during the first 6 hours after the abdominal surgery. I-FABP was able to discriminate between infants who will develop NEC or sepsis and the SAA was able to discriminate between medical and surgical NEC. Moreover, the combination of TFF-3 with I-FABP and SAA could predict pneumatosis intestinalis, and the combination of I-FABP, L-FABP, and SAA could predict gas in the portal vein or long-term hospitalization and low SAA predicts early full enteral feeding. Thus, these biomarkers may be useful not only in the early, noninvasive diagnostics but also in the subsequent NEC management.

BACKGROUND: Obstructive sleep apnoea (OSA) is considered an important risk factor of cardiovascular diseases (CVDs). Epicardial fat (EF) thickness and adipocyte fatty acid-binding protein (A-FABP) may be important links to accelerated atherosclerosis observed in patients with OSA. The aim was to evaluate the relationship between EF thickness and A-FABP levels in patients with OSA. METHODS: 66 patients (of which, 60 were males) of average age 55.6 ± 8.8 years, with newly diagnosed OSA were enrolled in this study. All patients underwent a sleep study, anthropometric parameters were measured, laboratory analysis and echocardiography with EF thickness measurements were collected. Patients were divided into two groups: Group 1: EF < 1mm; Group 2: EF > 1mm. RESULTS: Epicardial fat was present in 51 patients (77.3%). A positive correlation was found between A-FABP levels and % of body fat (r=0.452, p=0.0002). After adjusting to % of body fat, there was no significant difference found in A-FABP levels in the two groups divided. CONCLUSIONS: This study found a positive correlation between serum A-FABP level and % of body fat in patients with moderate to severe obstructive sleep apnoea. No significant difference was found between both groups.

BACKGROUND: Diseases caused by atherosclerosis play the most important role in mortality and morbidity worldwide. Serum adipocyte fatty acid binding protein (A-FABP) seems to be a new promising marker to determine the risk of atherosclerosis. OBJECTIVE: The aim of this study was to evaluate relationships between serum A-FABP levels in studied individuals and to assess the possibility of modeling the intima media thickness of the common carotid artery (C-IMT) using A-FABP levels and other observed characteristics. METHODS: Seventy two Caucasian individuals were enrolled and divided into 3 groups: dyslipidemic patients with or without metabolic syndrome (MetS+, n=17; MetS-, n= 34) and controls (n=21). RESULTS: There was confirmed the well-established risk profile of individuals with MetS (unfavorable lipid and lipoprotein profile, as well as increased parameters of insulin resistence and C-IMT). A-FABP concentrations in this group were significantly higher in comparison with both MetS- and controls. CONCLUSION: Using multiple linear regression models of C-IMT values for all individual data, healthy controls and dyslipidemic patients without metabolic syndrome (MetS-) A-FABP levels were not revealed as an important predictor of C-IMT in our model. In contrast, age, gender, waist circumference, nonHDL cholesterol levels and ApoB/ApoA1 ratio were important repressors of C- IMT in study individuals. This finding may be attributed to the overwhelming effect of other more robust risk factors for atherosclerosis in these individuals.

The aim of our study was to assess the presence and degree of intestinal leakage in subjects suffering from short bowel syndrome (SBS) and its modification by parenteral nutrition. To this end we assessed circulating levels of selected makers of intestinal permeability including zonulin, fatty acid binding protein 2 (FABP-2), citrulline and glucagon-like peptide 2 (GLP-2). We also measured lipopolysaccharide binding protein (LBP) as a marker of circulating levels of lipopolysaccharide acting through the CD14 molecule. Eleven SBS and 10 age- and BMI-matched control subjects were included into the study. The effect of parenteral nutrition was assessed after 14 days, 6 and 12 months from its initiation, respectively. At baseline, SBS patients had increased gut permeability as measured by zonulin (47.24+/-2.14 vs. 39.48+/-1.20 ng/ml, p=0.006) and LBP (30.32+/-13.25 vs. 9.77+/-0.71 microg/ml, p<0.001) compared to healthy controls. Furthermore, SBS subjects had reduced FABP-2, unchanged citrulline and increased sCD14 and GLP-2 relative to control group. Throughout the whole study period the administered parenteral nutrition had no significant effect on any of the studied parameters. Taken together, our data show that patients with short bowel syndrome have increased intestinal permeability that is not affected by parenteral nutrition.

OBJECTIVES: Asians (including Chinese, Japanese and Koreans), who generally have a relatively smaller body size and a lower mean body mass index (BMI), have a relatively higher risk of developing android-type obesity than westerners. Substitution of alanine for threonine (Ala54Thr) on the FABP2 gene (rs 1799883) is related to insulin resistance and obesity. However, few studies have examined this substitution in Koreans, and the number of Korean subjects in those studies is limited. For this reason, we investigated the differences between the FABP2 Ala54Thr polymorphism and obesity, hemodynamic variables, blood lipid profile results, and insulin resistance among middle-aged Korean women with abdominal obesity. METHODS: We studied 243 middle-aged community-dwelling Korean women with abdominal obesity from Gyeonggi Province, Republic of Korea, who had no history of taking chronic medications. We examined each subject (n = 243) for the presence of FABP2 Ala54Thr polymorphism using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). Subjects were also examined for obesity hemodynamic variables (n = 243), lipid profiles (n = 142), and insulin resistance (n = 142). RESULTS: Of the 243 subjects, 117 had AA ("normal") homozygotic genotype, 100 had AT heterozygotic genotype, and 26 had TT homozygotic genotype for the FABP2 Ala54Thr polymorphism. The AT heterozygotic individuals had a significantly higher mean waist-to-hip ratio, abdominal fat area, and visceral fat area than individuals with other genotypes. TT homozygotic individuals had higher mean triglyceride and fasting glucose levels than individuals with other genotypes. CONCLUSIONS: The results of this study show that the FABP2 Ala54Thr polymorphism was associated with central obesity and obesity-related metabolic syndrome among middle-aged Korean women.

BACKGROUND/PURPOSE: We analyzed the capacity of urinary Intestinal fatty acid-binding protein (I-FABP) to quantify the degree of mucosal injury in neonates with gastroschisis (GS) and to predict the speed of their clinical recovery after surgery. METHODS: In this prospective study, we collected urine during the first 48h after surgery from neonates operated between 2012 and 2015 for GS. Neonates with surgery that did not include gut mucosa served as controls for simple GS and neonates with surgery for intestinal atresia served as control for complex GS patients. The I-FABP levels were analyzed by ELISA. RESULTS: Urinary I-FABP after the surgery is significantly higher in GS newborns than in control group; I-FABP in complex GS is higher than in simple GS. I-FABP can predict subsequent operation for ileus in patients with complex GS. Both ways of abdominal wall closure (i.e. primary closure and stepwise reconstruction) led to similar levels of I-FABP. None of the static I-FABP values was useful for the outcome prediction. The steep decrease in I-FABP after the surgery is associated with faster recovery, but it cannot predict early start of minimal enteral feeding, full enteral feeding or length of hospitalization. CONCLUSION: Urinary I-FABP reflects the mucosal damage in gastroschisis but it has only a limited predictive value for patients' outcome.

BACKGROUND: In this study we compared levels of selected adipokines between patients with type 2 diabetes (T2D) and healthy individuals and we determined their relationship with early vascular damage markers. METHODS: Seventy-seven subjects: 56 patients with T2D (34 men and 22 women) and 21 healthy controls (8 men and 13 women) were examined in this cross-sectional study. Selected adipokines [adiponectin, adipocyte fatty acid-binding protein (A-FABP), fibroblast growth factor 21 (FGF-21), C1q/TNF-related protein 9 (CTRP-9), and allograft inflammatory factor-1 (AIF-1)] with possible cardiovascular impact were measured in all participants. To identify markers of vascular damage von Willebrand factor (vWF), plasminogen activator inhibitor-1 (PAI-1) and arterial stiffness parameters were examined in all the subjects. RESULTS: When compared with healthy controls, T2D had significantly higher levels of A-FABP [50.0 (38.1-68.6) vs. 28.6 (23.6-32.9) ng/mL, P < 0.0001] and lower levels of adiponectin [5.9 (4.3-9.0) vs. 11.3 (8.7-14.8) μg/mL, P < 0.0001]. Differences in other adipokines were not statistically significant. Adiponectin level correlated negatively with vWF levels (ρ = -0.29, P < 0.05) and PAI-1 (ρ = -0.36, P < 0.05) and A-FABP positively with vWF (ρ = 0.61, P < 0.05) and PAI-1 (ρ = 0.47, P < 0.05) and augmentation index (ρ = 0.26, P < 0.05). Multivariate regression analysis showed independent association between A-FABP and vWF (b = 0.24, P < 0.05). CONCLUSIONS: Patients with T2D have significantly higher levels of A-FABP and lower levels of adiponectin. These adipokines correlate with indicators of vascular damage and could contribute to cardiovascular risk in patients with T2D. A-FABP may participate in direct endothelium damage.

BACKGROUND: Adipocyte fatty acid-binding protein (A-FABP) is a promising link between metabolic syndrome and atherosclerosis. Epicardial fat (EPI) is an independent risk factor for cardiovascular disease (CVD). OBJECTIVE: The aim of this pilot study was to evaluate the correlation between EPI and A-FABP in asymptomatic patients with a family history of CVD. METHODS: 59 subjects (39 males) (median = 54 years old) were enrolled in the study and their EPI thickness and A-FABP levels were assessed. RESULTS: EPI was found in 46 patients (77.9%). There were positive correlations between EPI and A-FABP (r=0.336; P=0.010), age (r=0.526; P<0.001), fibrinogen (r=0.304; P=0.023) and systolic blood pressure (r=0.279; P=0.034). A positive correlation was found between EPI and A-FABP in a subgroup of overweight and obese patients (0.389; P=0.041, 0.407; P=0.004) and in the subgroup of patients with excluded CVD (r=0.368; P=0.006). CONCLUSIONS: We found a positive correlation between EPI and A-FABP in a group of patients with a family history of CVD and in subgroups of overweight and obese patients.

Adipokines are peptides that signal the functional status of adipose tissue to the brain and other target organs. In adipose tissue dysfunction, adipokine secretion is altered, and this can contribute to a spectrum of obesity-associated conditions including cardiovascular disease. Some adipokines have anti-inflammatory and cardioprotective effects (omentin, apelin, adiponectin). Others are pro-inflammatory with negative impact on cardiovascular function (leptin, visfatin, resistin, adipocyte fatty-acid-binding protein). In the first part, this article reviews the endocrine functions of adipose tissue in general, effects of the distribution and composition of fat tissue, and the roles of cortisol and the renin-angiotensin-aldosterone system in the development of the inflammatory state of addipose tissue. In the second part, the known cardiovascular effects of different adipokines and their clinical potential are discussed in detail.
