We present the case report of an unvaccinated Czech child with tetanus. The child had not received any vaccines due to its parent's refusal. The disease originated from the wound in the nose caused by a small flat battery. The typical onset of tetanus followed after two weeks, rapidly progressing to respiratory failure with the need for mechanic ventilation despite intensive treatment. The child spent five weeks in the hospital. Mild long-term sequelae persisted 5 months.
BACKGROUND: Transplacentally transferred antibodies induced by maternal pertussis vaccination interfere with infant immune responses to pertussis primary vaccination. We evaluated whether this interference remains in toddlers after booster vaccination. METHODS: In a prior phase IV, observer-blind, placebo-controlled, randomized study (NCT02377349), pregnant women in Australia, Canada and Europe received intramuscular tetanus-reduced-antigen-content diphtheria-three-component acellular pertussis vaccine (Tdap group) or placebo (control group) at 270/7-366/7 weeks' gestation, with crossover immunization postpartum. Their infants were primed (study NCT02422264) and boosted (at 11-18 months; current study NCT02853929) with diphtheria-tetanus-three-component acellular pertussis-hepatitis B virus-inactivated poliovirus/Haemophilus influenzae type b vaccine (DTaP-HepB-IPV/Hib) and 13-valent pneumococcal conjugate vaccine. Immunogenicity before and after booster vaccination, and reactogenicity and safety of the booster were evaluated descriptively. RESULTS: 263 (Tdap group) and 277 (control group) toddlers received a DTaP-HepB-IPV/Hib booster. Pre-booster vaccination, observed geometric mean concentrations (GMCs) for the three pertussis antigens and diphtheria were 1.4-1.5-fold higher in controls than in the Tdap group. No differences were observed for the other DTaP-HepB-IPV/Hib antigens. One month post-booster vaccination, booster response rates for pertussis antigens were ≥ 92.1% and seroprotection rates for the other DTaP-HepB-IPV/Hib antigens were ≥ 99.2% in both groups (primary objective). Higher post-booster GMCs were observed in controls versus the Tdap group for anti-filamentous hemagglutinin (1.2-fold), anti-pertussis toxoid (1.5-fold) and anti-diphtheria (1.4-fold). GMCs for the other DTaP-HepB-IPV/Hib antigens were similar between groups. Serious adverse events were reported for three toddlers (controls, not vaccination-related). One death occurred pre-booster (Tdap group, not vaccination-related). CONCLUSIONS: As a consequence of interference of maternal pertussis antibodies with infant immune responses to pertussis primary vaccination, pertussis antibody concentrations were still lower in toddlers from Tdap-vaccinated mothers before DTaP-HepB-IPV/Hib booster vaccination. After the booster, antibody concentrations were lower for filamentous hemagglutinin and pertussis toxoid but not for pertactin. Východiska: Tetanus je významné infekční onemocnění, které se vyznačuje vysokou smrtností. Mnoho desítek let je k dispozici účinná očkovací látka proti tomuto onemocnění, jejímž používáním bylo dosaženo eliminace této nebezpečné infekce. Cíl: Prokázat, že v současné době je tetanus mizející infekce, proti které je ve všech věkových skupinách, včetně nejstarších osob, dobrá imunita. Metody: Sérologicky, metodou ELISA, byly prokázány ochranné protilátky proti tetanu i u osob starších 60 let. Výsledky: Vyšetření starších osob v rámci sérologických přehledů v České republice v letech 1986 – 2002 prokázalo ochranné titry protilátek. Skupina 13 osob ve věku 61 – 93 let, které měly interval od posledního očkování delší než 10 let, měla ve většině případů ochranné titry protilátek proti tetanu. Závěry: Starší osoby v České republice mají dobrou a akceptabilní imunitu proti tetanu, jak potvrdila řada vyšetření protilátek v rámci sérologických přehledů nebo jiných vyšetření. Nepřímým důkazem, ale vysoce validním, je také epidemiologická situace ve výskytu tetanu v České republice. Od roku 1990 byly zaznamenány jen ojedinělé případy, a to při stále vysoké expozici celé populace.
Background: Tetanus is an important infectious disease that is characterized by high lethality. For many decades, an effective vaccine against this disease has been available, the use of which has achieved the elimination of this dangerous infection. Objective: To prove that at present tetanus is a disappearing infection, against which there is good immunity in all age groups, including the oldest individuals. Methods: Protective antibodies against tetanus have been demonstrated in persons over 60 years of age using the ELISA method. Results: Examination of elderly persons in serological reviews in the Czech Republic between 1986 and 2002 showed protective antibody titres. In most cases, a group of 13 people aged 61-93 years who had an interval of more than 10 years from the last vaccination had tetanus antibody protection titres. Conclusions: Elderly people in the Czech Republic have good and acceptable immunity to tetanus as confirmed by a number of antibody tests within serological reviews or other examinations. The epidemiological situation in tetanus in the Czech Republic is also circumstantial evidence, but highly valid. Since 1990, only isolated cases have been recorded, with still high exposure of the entire population.
Tetanus is a disease caused by tetanotoxin produced in necrotic wounds by Clostridium tetani. It is a very rare disease in Czechia due to successful and effective population-wide vaccination programme, despite the fact that spores of C. tetani are permanently present in the environment. Groups with the highest risk of clinical tetanus include elderly people, immunocompromised individuals, residents of foreign origin with unclear vaccination history, and unvaccinated children. We present four case studies of severe and mild form of tetanus, wound infection with the presence of C. tetani without the development of clinical tetanus in a fully vaccinated individual, and unexpected risk of tetanus in an unvaccinated child. Due to the rare occurrence of tetanus in Czechia, the clinical awareness of the risk of tetanus decreases as well as the clinical experience with diagnosis of early or mild forms of tetanus. Communication skills during the management of contaminated wounds play a critical role in the decision who should get tetanus anatoxin only and who should get antitetanus immunoglobulin along with the active immunization by tetanus anatoxin. Onemocnění tetanem je v České republice díky dobré proočkovanosti raritní. Neočkovaným nebo nedostatečně očkovaným osobám ale nákaza tetanem v případě rizikového poranění stále hrozí. U dětí, které nebyly řádně očkovány, musí být proto důsledně prováděna postexpoziční vakcinace, případně i pasivní imunizace. V článku upozorňujeme na potřebu imunoprofylaxe tetanu po poranění u dětí jako na novou problematikou pro chirurgy, kteří poraněné děti ošetřují, a také pro pediatry jako garanty očkování u dětí.
Tetanus disease is rare in the Czech Republic due to good vaccination coverage. However, non-vaccinated or insufficiently vaccinated persons are still at risk of tetanus infection when injured. Therefore, post-exposure vaccinations and, if necessary, passive immunizations must be performed consistently in children. In this article, we draw attention to the need for immunoprophylaxis of tetanus after injury in children as a new issue for surgeons who treat injured children, and for pediatricians as guarantors of vaccination in children.
