BACKGROUND: Preoperative FOLFIRINOX chemotherapy is increasingly administered to patients with borderline resectable (BRPC) and locally advanced pancreatic cancer (LAPC) to improve overall survival (OS). Multicenter studies reporting on the impact from the number of preoperative cycles and the use of adjuvant chemotherapy in relation to outcomes in this setting are lacking. This study aimed to assess the outcome of pancreatectomy after preoperative FOLFIRINOX, including predictors of OS. METHODS: This international multicenter retrospective cohort study included patients from 31 centers in 19 European countries and the United States undergoing pancreatectomy after preoperative FOLFIRINOX chemotherapy (2012-2016). The primary end point was OS from diagnosis. Survival was assessed using Kaplan-Meier analysis and Cox regression. RESULTS: The study included 423 patients who underwent pancreatectomy after a median of six (IQR 5-8) preoperative cycles of FOLFIRINOX. Postoperative major morbidity occurred for 88 (20.8%) patients and 90-day mortality for 12 (2.8%) patients. An R0 resection was achieved for 243 (57.4%) patients, and 259 (61.2%) patients received adjuvant chemotherapy. The median OS was 38 months (95% confidence interval [CI] 34-42 months) for BRPC and 33 months (95% CI 27-45 months) for LAPC. Overall survival was significantly associated with R0 resection (hazard ratio [HR] 1.63; 95% CI 1.20-2.20) and tumor differentiation (HR 1.43; 95% CI 1.08-1.91). Neither the number of preoperative chemotherapy cycles nor the use adjuvant chemotherapy was associated with OS. CONCLUSIONS: This international multicenter study found that pancreatectomy after FOLFIRINOX chemotherapy is associated with favorable outcomes for patients with BRPC and those with LAPC. Future studies should confirm that the number of neoadjuvant cycles and the use adjuvant chemotherapy have no relation to OS after resection.

Trifluridin/tipiracil (TRI/TIP) se na základě výsledků studie III. fáze RECOURSE stal standardní součástí terapie metastatického kolorektálního karcinomu. Je používán ve 3. linii léčby u pacientů, u kterých selhala předchozí terapie. Ve studii III. fáze SUNLIGHT přidání bevacizumabu k TRI/TIP zlepšilo medián celkového přežití ve srovnání s léčbou samotným trifluridin/tipiracilem na 10,8 měsíce u kombinovaného režimu s bevacizumabem oproti 7,5 měsíce u samotného TRI/TIP (HR: 0,61; 95% CI: 0,49–0,77; p < 0,001). Studie zahrnovala pacienty s histologicky potvrzeným metastatickým kolorektálním karcinomem v pokročilém stadiu, kteří byli léčeni 1–2 liniemi chemoterapie. Přidání bevacizumabu nezvýšilo riziko závažných nežádoucích účinků nebo nežádoucích účinků vedoucích k přerušení léčby. Přínosem je také prodloužení mediánu doby do zhoršení výkonnostního stavu, který dosáhl u kombinované léčby 9,3 měsíce proti 6,3 měsíce u monoterapie (HR: 0,54; 95% CI: 0,43–0,67).
Trifluridine/tipiracil (TRI/TIP) has become a standard part of the treatment of metastatic colorectal cancer based on the results of the phase III RECOURSE trial. It is used as a third-line treatment for patients in whom previous therapy has failed. In the phase III SUNLIGHT trial, the addition of bevacizumab to TRI/TIP improved median overall survival compared with trifluridine/tipiracil alone, 10,8 months for the bevacizumab combination regimen versus 7,5 months for TRI/TIP alone (HR: 0,61; 95% CI: 0,49–0,77; p < 0,001). The study included patients with histologically confirmed metastatic colorectal cancer who were treated with 1 to 2 lines of chemotherapy for advanced disease. The addition of bevacizumab did not increase the risk of serious adverse events or adverse events leading to treatment discontinuation. Another benefit is the prolongation of the median time of deterioration in performance status, which reached 9,3 months with combination treatment versus 6,3 months with monotherapy (HR: 0,54; 95% CI: 0,43–0,67).

Spinocelulární karcinom anu představuje vzácné onemocnění gastrointestinálního traktu. Díky přítomnosti časných příznaků, mezi které patří krvácení, bolestivost a pocit rezistence v konečníku, bývá toto onemocnění diagnostikováno v časných stadiích. V současné době je standardním léčebným postupem multimodální přístup, který kombinuje radioterapii a chemoterapii na bázi 5-fluorouracilu s mytomicinem nebo cisplatinou. Prognóza je u většiny pacientů s onemocněním lokalizovaným pouze na anus velmi příznivá, přesto je léčba spjata s jistými riziky. Tato kazuistika popisuje managment širokého spektra komplikací včetně kardiotoxicity spojené s 5-fluorouracilem, rozvoje heparinem indukované trombocytopenie a trombotických komplikací u jinak standardně léčené pacientky pro spinocelulární karcinom anu.
Squamous cell carcinoma of the anus is a rare gastrointestinal tract disease. Due to the early presence of symptoms, which include bleeding, soreness and a feeling of resistance in the anus, this disease is usually diagnosed in its initial stages. The standard treatment consists of a multimodal approach combining radiotherapy and 5-fluorouracil-based chemotherapy with mitomycin or cisplatin. The prognosis is very favorable for most patients with the disease localized only to the anus, yet there are some risks associated with the treatment. This case report describes the management of a wide range of complications, including 5-fluorouracil-associated cardiotoxicity, development of heparin-induced thrombocytopenia, and thrombotic complications in a patient otherwise standardly treated for squamous cell carcinoma of the anus.

Popisujeme kazuistiku lokální aplikace 5-fluoruracilu na aktinické keratózy vertexu u 77letého pacienta. Aktinická keratóza je velmi častá diagnóza, se kterou se v běžné praxi setkáváme, jedná se o epiteální tumor, kožní prekancerózu, ze které se může vyvinout spinocelulární karcinom, a proto je léčba aktinické keratózy důležitá.
This case report focuses on the local application of 5-fluoruracil to actinic keratosis of the vertex with a 77 year-old patient. Actinic keratosis is a very common diagnosis frequently encountered in general practice. It is an epithelial tumor, a skin precancerous lesion, that can potentially develop into squamous cell carcinoma and therefore, the treatment of actinic keratosis is important.
PURPOSE: Napabucasin is an investigational, orally administered reactive oxygen species generator bioactivated by intracellular antioxidant NAD(P)H:quinone oxidoreductase 1 that has been evaluated in various solid tumors, including metastatic colorectal cancer (mCRC). Phosphorylated signal transducer and activator of transcription 3 (pSTAT3) is hypothesized to predict response in napabucasin-treated patients with mCRC. PATIENT AND METHODS: In the multi-center, open-label, phase III CanStem303C (NCT02753127) study, adults with histologically confirmed mCRC that progressed on first-line fluoropyrimidine plus oxaliplatin ± bevacizumab were randomized to twice-daily napabucasin plus FOLFIRI (napabucasin) or FOLFIRI alone (control). The primary endpoint was overall survival (OS) in the general study population and in patients with pSTAT3-positive tumors (biomarker-positive). RESULTS: In the general study population (napabucasin, n = 624; control, n = 629), median OS was 14.3 months for napabucasin and 13.8 months for control (hazard ratio [HR], 0.976, one-sided P = .74). Overall, 44% of patients were biomarker-positive (napabucasin, n = 275; control, n = 272). In the biomarker-positive population, median OS was 13.2 months for napabucasin and 12.1 months for control (HR, 0.969; one-sided P > .99). In the control arm, median OS was shorter for biomarker-positive versus biomarker negative patients (12.1 vs. 18.5 months; HR, 1.518; nominal 2-sided P = .0002). The most common treatment-emergent adverse events (TEAEs) were diarrhea (napabucasin, 84.6%; control, 53.9%), nausea (60.5%, 50.5%), vomiting (41.2%, 29.3%), and abdominal pain (41.0%, 25.2%). Grade ≥3 TEAEs occurred in 73.8% of napabucasin-treated and 66.7% of control-treated patients, most commonly diarrhea (21.2%, 7.0%), neutrophil count decreased (13.7%, 19.2%), and neutropenia (13.3%, 15.2%). Safety was similar in biomarker-positive patients. CONCLUSION: In patients with previously treated mCRC, adding napabucasin to FOLFIRI did not improve OS. Results from the control arm indicate that pSTAT3 is an adverse prognostic factor in mCRC.

Terapeutické možnosti u mCRC ve 3. a vyšší linii se neustále rozšiřují jak ve smyslu nových kombinací stávající léčby, tak o nové cílené malé molekuly. Výsledky dosavadních studií dokazují, že i vysoce předléčeným pacientům lze vhodnou terapeutickou intervencí prodloužit život při zachování jeho kvality. Současný rozvoj klinické onkologie nás nutí mít přehled nejen o aktuálně schválené léčbě, ale i o formujících se léčebných přístupech ve vyšších liniích léčby u pokročilého onemocnění.
Terapeutic possbilities in mCRC in 3rd and higher line of treatment are constantly expanding not only by new combinations of actual treatment possibilities, but also by discoveries of new small targeted molecules. Results of clinical studies prove that by right therapeutic intervention, we can prolong lives of highly pre-treated patients without worsening their quality of life. Ever evolving clinical oncology require us to have constant knowledge of actual therapeutic regimes and formation of new approach in higher lines of treatment in advanced illness.

Prezentujeme kazuistiku 40letého pacienta s lézí na glans penis, primárně histologicky potvrzena jako penilní intraepiteliální neoplazie (PeIN). V první linii na sektorovém dermatovenerologickém pracovišti byla zvolena lokální terapie 5‐fluorouracilem. Následná opakovaná biopsie glandu histologicky prokazuje perzistenci PeIN. U pacienta byla provedena cirkumcize a parciální resurfacing glandu s použitím bukálního štěpu s dosažením příznivého kosmetického i funkčního efektu.
We present a case report of a 40 year old patient presenting with lession on glans penis. First biopsy histologically confirmed penile intraepithelial neoplasia (PeIN) associated with HPV p16. Local treatment with 5-fluorouracil provided by dermatologist was ineffective and another biopsy of glans penis again confirmed persistence of PeIN. The patient underwent a circumcision and partial glans resufacing using a buccal graft with favourable aesthetical and functional outcome.

INTRODUCTION: The exposures to hazardous antineoplastic drugs (AD) represent serious risks for health care personnel but the exposure limits are not commonly established because of the no-threshold effects (genotoxic action, carcinogenicity) of many ADs. In this study, we discussed and derived practically applicable technical guidance values (TGV) suitable for management of AD risks. METHODS: The long-term monitoring of surface contamination by eight ADs was performed in pharmacies and hospitals in the Czech Republic and Slovak Republic in 2008-2021; in total 2,223 unique samples were collected repeatedly in 48 facilities. AD contamination was studied by LC-MS/MS for cyclophosphamide, ifosfamide, methotrexate, irinotecan, paclitaxel, 5-fluorouracil and gemcitabine and by ICP-MS for total Pt as a marker of platinum-based ADs. RESULTS: The study highlighted importance of exposure biomarkers like 5-fluorouracil and especially carcinogenic and persistent cyclophosphamide, which should be by default included in monitoring along with other ADs. Highly contaminated spots like interiors of laminar biological safety cabinets represent a specific issue, where monitoring of contamination does not bring much added value, and prevention of staff and separated cleaning procedures should be priority. Rooms and surfaces in health care facilities that should be virtually free of ADs (e.g., offices, kitchenettes, daily rooms) were contaminated with lower frequency and concentrations but any contamination in these areas should be carefully examined. DISCUSSION AND CONCLUSIONS: For all other working places, i.e., majority of areas in pharmacies and hospitals, where ADs are being prepared, packaged, stored, transported, or administered to patients, the study proposes a generic TGV of 100 pg/cm2. The analysis of long-term monitoring data of multiple ADs showed that the exceedance of one TGV can serve as an indicator and trigger for improvement of working practices contributing thus to minimizing of unintended exposures and creating a safe work environment.

Problém nedostupnosti léčiv a výpadků je častým problémem současnosti. Omezení dodávek může ohrozit některé léčebné režimy onkologicky nemocných. Článek podává systematický přehled studií, které hodnotí účinnost a bezpečnost chemoterapeutických režimů kombinujících fluorouracil a leukovorin ve standardní a snížené dávce v terapii kolorektálního karcinomu. Celkem bylo do přehledu zahrnuto 13 prospektivních i retrospektivních prací. Nebyly zahrnuty metaanalýzy a přehledové práce. Z přehledu je zřejmé, že nižší dávka leukovorinu zásadně neovlivňuje účinnost režimů kombinujících 5-fluorouracil s leukovorinem v terapii pacientů s kolorektálním karcinomem. Podobně ani v případě porovnání bezpečnosti nemělo snížení dávky leukovorinu zásadní vliv na frekvenci a tíži sledovaných nežádoucích účinků. Naopak, ve třech studiích se překvapivě některé nežádoucí účinky objevovaly častěji u vyšší dávky leukovorinu. Dále článek předkládá výsledky dotazníkového průzkumu managementu výpadku leukovorinu na odděleních přípravy cytostatik (N = 46) v rámci České republiky. Zpětnou vazbu poskytlo 35 pracovišť. V 17 případech musela být na odděleních přípravy cytostatik v České republice přijata omezení stran podávání plné dávky leukovorinu. Tato omezení spočívala ve snížení dávky injekční formy leukovorinu, změně chemoterapeutického režimu, podávání perorální formy kalcium folinátu, vynucené terapeutické přestávce nebo kombinaci těchto přístupů.
The problem of unavailability of drugs and shortages have been a common problem in recent years. Shortages may threaten some treatment regimens for oncological patients. This article presents a systematic review of studies evaluating the efficacy and safety of chemotherapy regimens combining fluorouracil and standard or low-dose leucovorin in treating colorectal cancer. A total of 13 prospective and retrospective studies were included in the review. Meta-analyses and review papers were excluded. It is apparent from the systematic review that a lower dose of leucovorin does not fundamentally affect the efficacy of regimens combining 5-fluorouracil with leucovorin in treating patients with colorectal cancer. Similarly, even in the case of safety, reducing the dose of leucovorin did not influence the frequency and severity of observed adverse effects. Surprisingly, in three studies, some of the adverse effects occurred more often with the higher dose of leucovorin. Furthermore, the article presents the results of a questionnaire survey of the management of leucovorin shortages at the departments of preparation of cytostatics (N = 46) within the Czech Republic. In total, 35 workplaces provided feedback. In 17 cases, the departments for the preparation of cytostatics in the Czech Republic had to accept restrictions on administering the full dose of leucovorin. These restrictions consisted of reducing the dose of the injectable form of leucovorin, changing the chemotherapy regimen, administering the oral form of calcium folinate, forcing a therapeutic break, or a combination of these approaches.

Liposomální irinotecan (Onivyde) je první léčivo specificky schválené pro použití v kombinaci s 5-fluorouracilem/leucovorinem v léčbě metastazujícího adenokarcinomu pankreatu u pacientů, u nichž došlo k progresi po terapii založené na gemcitabinu. Tato kazuistika pojednává o prvních vlastních zkušenostech s tímto přípravkem ve druhé linii léčby tohoto onemocnění.
Liposomal irinotecan (Onivyde) is the first drug specifically approved for use in combination with 5-fluorouracil/leucovorin in the treatment of metastatic pancreatic adenocarcinoma in patients who have progressed after gemcitabine-based therapy. This case report discusses the first in-house experience with this agent in the second-line treatment of this disease.
