Aneurysms and pseudoaneurysms of the dorsalis pedis artery are rare vascular entities usually caused by traumatic injury or iatrogenic intervention, producing a focal soft pulsatile tissue mass in the dorsal foot. Gradual expansion of the fibrous capsule can produce secondary physical signs through the pressure exerted on surrounding structures. We report the case of a 57-year-old male with an idiopathic pseudoaneurysm of the dorsalis pedis artery. A focused history found a low-grade blunt trauma to the dorsal foot that had occurred 2 years previously. The diagnosis of a pseudoaneurysm was confirmed by ultrasound examinations and computed tomography angiograms. The pseudoaneurysm was treated with resection, and a short reversed great saphenous vein interposition graft was placed. He recovered well after surgery and remained free of symptoms 18 months postoperatively. The literature review of this uncommon vascular pathologic entity and the diagnosis and management are discussed.

OBJECTIVE: The goal of our study was to analyze the prevalence of branching pattern variations in the popliteal artery (PA) along with morphometrics of the PA to better address its importance in disease and vascular surgical procedures. METHODS: An extensive search for the PA and its anatomic variations was done in the major online medical databases. The anatomic data found were extracted and pooled for a meta-analysis. RESULTS: A total of 33 studies (N = 12,757 lower limbs) were included in the analysis. The most common variant was a division of the PA below the knee into the anterior tibial artery and a common trunk for the posterior tibial and peroneal arteries, with a prevalence of 92.6% (95% confidence interval [CI], 90.2-93.8). The second most common variation was a trifurcation pattern of all three branches dividing within 0.5 cm of each other, with a prevalence of 2.4% (95% CI, 1.4-3.5). Of the three studies that reported the diameter of the PA at the level of the subcondylar plane, a mean diameter of 8 mm (95% CI, 7.29-8.70) was found. CONCLUSIONS: The PA most commonly divides below the knee into the anterior tibial artery and the common trunk of the posterior tibial artery and the peroneal artery. Knowledge of the prevalence of possible variations in this anatomy as well as morphometric data is crucial in the planning and execution of any surgical intervention in the area of the knee.

Prezentujeme kompletní endovaskulární revaskularizaci aterotrombotického uzávěru popliteální tepny (APo) a bércových tepen u polymorbidního pacienta za použití několika endovaskulárních technik, a to lokální kontinuální trombolýzou, mechanickou trombektomií, aspirační trombektomií a perkutánní transluminální angioplastikou bércových tepen s implantací stentů.
We present a complete endovascular revascularisation of an atherothrombotic occlusion of the popliteal artery (APo) and below-the-knee arteries in a polymorbid patient, using several endovascular techniques, specifically local continual pharmacological thrombolysis, mechanical thrombectomy (Rotarex), aspiration thrombectomy and PTA of the below-the-knee arteries with stent deployment.

A 43-year-old man, injured during a factory explosion on January 4, 2010, was transported to the nearest hospital. Volume resuscitation started, subclavian vein cannulation, chest tube, and covering of burns were performed. The intubated and ventilated patient was transferred by helicopter to our hospital. The leading diagnoses were: haemorrhagic – traumatic and burns shock, haemoperitoneum, pneumothorax, subtotal amputation of the left distal crus/feet, burns over 40 % of the body surface, right calcaneus fracture etc. Preliminary circulatory stabilisation with discontinuation of norepinephrine infusion was achieved within 10 hours, by excessive positive fluid balance, which took three days (22, 10 and 9 litres). Preliminary blood lactate 7 mmol/l was normalised within 24 hours. Blood albumin level 15 g/l was tolerated without any artificial replacement. The second hit appeared on day 10, in a form of septic shock caused by Aspergillus fumigatus infection from the lacerated left lower limb. High-volume continuous haemodiafiltration because of hyperpyrexia was used, after the exarticulation in the left knee-joint. An excessive proteolysis (urea loss more than 1000 mmol/day) persisted 1.5 months, with energy expenditure over 2600 kcal/day. The patient underwent multiple surgical interventions. Metabolic support using combined parenteral and enteral nutrition was performed during the first month, enteral nutrition combined with food were consequently given for more than 3 months. The patient was hospitalised until April 26, 2010. These types of traumas are fully comparable with military battlefield environment injuries. Principles of war surgery (life and limb saving, damage control surgery) should be applied also in civilian environment, particularly in disaster situations.

Kazuistika popisuje překvapivý podíl ischemie dolních končetin na vzniku syndromu diabetické nohy u mladé pacientky s diabetes mellitus 1. typu. Při dlouhodobě špatné metabolické kompenzaci došlo ve věku 42 let k manifestaci výrazné aterosklerózy periferních tepen se syndromem diabetické nohy.
The case describes a surprising contribution of ischemia of lower extremities on a development of the diabetic foot syndrome in a young female patient with type 1 diabetes mellitus. Following a long-term poor metabolic compensation a significant atherosclerosis of the peripheral arteries with the diabetic foot syndrome manifested at her age of 42.

Angioplastika tepen dolních končetin se postupně stala metodou volby v terapii obliterujících tepenných lézí. K angioplastice jsou obecně indikovány zúžení tepen a kratší uzávěry. V mnoha případech je angioplastika doplněna implantací stentu. Článek rozebírá jednotlivé oblasti tepen dolních končetin včetně indikací a výsledku léčby. Aorta: indikace jsou stenózy a krátké uzávěry, vždy je implantován stent, dlouhodobá průchodnost je téměř 100%. lliacké tepny: indikace: stenózy i delší uzávěry, většinou je implantován stent, dlouhodobá průchodnost je 75-94 %. stehenní tepny: stenózy i uzávěry až do 15 cm, stent je implantován při selhání PTA, dlouhodobá průchodnost 50 %. Bércové tepny: indikace (při kritické končetinové ischémii): stenózy a uzávěry do 10 cm, dlouhodobá průchodnost je neznámá, ale roční záchrana končetiny je 80-85 %.

PTA bércových tepen je v současné době v našem ústavu rutinně prováděným léčebným postupem, který je uplatňován při léčbě ischemického postižení dolních končetin. Tyto výkony jsou zatíženy rizikem restenózy stejně tak, jako ostatní perkutánní i chirurgické výkony na tepnách. V našem souboru 805 výkonů u 638 pacientů jsme u 12 % pacientů prováděli opakovaný výkon na infrapopliteálních tepnách v časovém rozpětí od 3 týdnů až do 6 let od prvního výkonu. Na distálních bypassech jsme celkově provedli 21 intervencí u 16 nemocných, 13x se jednalo o uzávěr bypassu, 8x se jednalo pouze o stenózy. Celkově jsme 19x dosáhli normalizace průsvitu bypassu. PTA, eventuálně rePTA, je bezpečná a efektivní léčba restenóz a reobliterací tepen i bypassů.
PTA and rePTA of infrapopliteal arteries and distal bypasses is highly efficient method in critical lower limb ischemia treatment and has positive effect in more than 70-80% of jeopardised lower limbs. Restenosis after PTA and after distal bypass are widely known and in case of symptoms recurrence rePTA can be attempted. In our group of 638 patients we performed rePTA in 12% of patients in the period of time from 21 days to 6 year after PTA. We performed 21 PTA of surgical distal bypasses ( 13 PTA of occluded bypass), 19 PTA were successful.

Autoři předkládají případ pacienta s progredující vlhkou ggangrénou pravé nohy indikovaného na základě nepříznivého angiografického nálezu a po vyčerpání možností konzervativní terapie k amputaci končetiny. S přihlédnutím ke skutečnosti, že pacient je již po amputaci druhostranné končetiny v bérci, rozhodnuto ještě o revizi art. tibialis posterior (ATP). Peroperační angiografie nepotvrzuje původní nepříznivý angiografický nález. Po implantaci distálního krurálního žilního bypasu (ATP supramalleolámě) dochází během relativně krátké doby ke zhojení končetiny. Autori se pripojují k názoru, že distální krurální a pedální bypassy nelze vždy indikovat na základě běžně prováděné angiografie. Revize bércových tepen a eventuálm' peroperační angiografie mohou zvrátit do té doby neřešitelnou situaci a vést tak k záchraně končetiny.
The authors present a case of a patient with a progredient moist gangrene of the right foot. In this patient, amputation of the lower limb was indicated because of an unfavourable angiographic finding, after unsuccessful conservative treatment. With regard to the fact, that the patient had imdergone amputation of the other lower limb in the cms, revision of the posterior tibial artery was decided. Peroperative angiography did not confirm the previous unfavourable angiographic finding. Aňer the implantation of a distal crural venous bypass (posterior tibial artery - supramalleolar part), healing of the extremity occurred in a relatively short time period. The authors agree with the opinion that distal crural and pedal bypasses cannot be always indicated on the basis of routinely performed angiography. Revision of crural arteries and possible peroperative angiography can change the situation and lead to the salvage of the limb.

Cíl: Retrospektivně zhodnotit výsledky perkutánní aspirační trombektomie (PAT) v popliteálním a bércovém řečišti u primárních a iatrogenních embolizaci a pri intraarteriální trombolýze. Materiál a metoda: PAT byla použita u 41 nemocného (42 tepen), 12x primárně u trombembolického obliterujícího onemocněm' tepen dolních končetin, 14x v průběhu trombolýzy, 16x po iatrogenní embolizaci po perkutánní angioplastice. Výkon proveden vždy prográdnim pristupem do společné femorální tepny, nejčastěji byl použit 7F aspirační katétr. Aspirace byla provedena u 40 nemocných z oblasti a. poplitea a proximálních úseků bércových tepen. Výsledky: Úplné aspirace bylo celkem dosaženo v 67 %, částečné aspirace ve 23 %, aspirace nebyla úspěšná v 10 %. U iatrogennich distálnich embolizaci byla PAT úspěšná v 88 %, u trombolyzovaných nemocných byla zcela Úspěšná ve 40 %, částečně úspěšná v 50 %, neúspěšná v 10 %. Primárně prováděná aspirace pri trombemholickém obliterujícím onemocnění tepen DK byla úspěšná u 50 % nemocných. Nebyly zaznamenány žádné závažné komplikace. Závěr: PAT je vysoce účinná metoda odstranění embolizovaného materiálu u iatrogennich embolizaci po endovaskulámích výkonech, účinná pomocná metoda při intraarteriální trombolýze. Může být použita primárně U akutních uzávěrů v oblasti trifurkace a. poplitea.
Purpose: Retrospective analysis of results of percutaneous aspiration thrombectomy (PAT) in the popliteal and tibioperoneal arteries in patients with primary and iatrogenic peripheral embolization and during intraarterial trombolysis. Material and method: Percutaneous aspiration thrombectomy was used in 41 patients (42 arteries). In 12 cases this method was used as the first choice in treatment of the peripheral artery disease, in 14 cases during intraarterial thrombolysis and 16 times after iatrogenic embolization secondary to balloon angioplasty. An antegrade approach to the common femoral artery was performed in all cases. In 40 patients the aspiration thrombectomy was performed from the popliteal region and proximal parts of tibioperoneal arteries. Results: Overall success (n = 42) was achieved in 67 %, partial success in 23 %. Aspiration thrombectomy was unsuccessful in. 10 %. PAT was successful in 88 % after iatrogenic periferal embolization (n = 16), in patients during trombolysis (n = 14) a good result was achieved in 40 %, partial success in 50 % and this method failed in 10 %. Primarily performed aspiration thrombectomy (n = 12) in occlusion of peripheral arteries was successful in 50 %. No major complications were recorded. Conclusion: PAT is a highly effective method in treatment of iatrogenic peripheral embolizations after endovascular procedures and effective accessory method in combination with intraarterial thrombolysis. This method can be used primarily in acute occlusion of distal popliteal artery.
