The use of oxygen therapy (high doses of oxygen - hyperoxia) in the treatment of premature infants results in their survival. However, it also results in a high incidence of chronic lung disease known as bronchopulmonary dysplasia, a disease in which airway hyper-responsiveness and pulmonary hypertension are well known as consequences. In our previous studies, we have shown that hyperoxia causes airway hyper-reactivity, characterized by an increased constrictive and impaired airway smooth muscle relaxation due to a reduced release of relaxant molecules such as nitric oxide, measured under in vivo and in vitro conditions (extra- and intrapulmonary) airways. In addition, the relaxation pathway of the vasoactive intestinal peptide (VIP) and/or pituitary adenylate cyclase activating peptide (PACAP) is another part of this system that plays an important role in the airway caliber. Peptide, which activates VIP cyclase and pituitary adenylate cyclase, has prolonged airway smooth muscle activity. It has long been known that VIP inhibits airway smooth muscle cell proliferation in a mouse model of asthma, but there is no data about its role in the regulation of airway and tracheal smooth muscle contractility during hyperoxic exposure of preterm newborns.

INTRODUCTION AND HYPOTHESIS: Animal models are useful for investigating the genesis of pelvic floor dysfunction and for developing novel therapies for its treatment. There is a need for an alternative large-animal model to the nonhuman primate. Therefore we studied the effects of the first vaginal delivery, ovariectomy and systemic hormonal replacement therapy (HRT) on the biomechanical and structural properties of the ovine vagina. METHODS: We examined the gross anatomical properties of nulliparous, primiparous, ovariectomized multiparous, and ovariectomized hormone-replaced multiparous sheep (six animals per group). We also harvested mid-vaginal and distal vaginal tissue to determine smooth muscle contractility and passive biomechanical properties, for morphometric assessment of the vaginal wall layers, to determine collagen and elastin content, and for immunostaining for α-smooth muscle actin and estrogen receptor-α. RESULTS: There were no regional differences in the nulliparous vagina. One year after the first vaginal delivery, stiffness and contractility of the distal vagina were decreased, whereas the elastin content increased. The mid-vagina of ovariectomized sheep was stiff, and its epithelium was thin and lacked glycogen. HRT decreased the stiffness of the mid-vagina by 45% but had no measurable effect on contractility or elastin content, and increased epithelial thickness and glycogen content. HRT also increased the epithelial thickness and glycogen content of the distal vagina. At this location, there were no changes in morphology or stiffness. CONCLUSION: In sheep, life events including delivery and ovariectomy affect the biomechanical properties of the vagina in a region-specific way. Vaginal delivery mainly affects the distal region by decreasing stiffness and contractility. HRT can reverse the increase in stiffness of the mid-vagina observed after surgical induction of menopause. These observations are in line with scanty biomechanical measurements in comparable clinical specimens.

PURPOSE: This work investigated the effects of diuresis, duration of dialysis and age on lower urinary tract function in urologically healthy males on the waiting list for kidney transplant. MATERIALS AND METHODS: The study included all men who had kidney transplants at our centre between January 2009 and December 2014 who had normal urological findings prior to inclusion on the list. Diuresis, the duration of haemodialysis, age, and parameters of function of the lower urinary tract as determined by filling and voiding cystometry were evaluated. RESULTS: The study included 127 men (median age, 59 years; median diuresis, 250 mL; median duration of dialysis, 469.5 days). We found that greater diuresis was accompanied by significantly higher FDV, FDV/Cmax, NDV,Cmax and compliance and by significantly lower Pdet.max, PdetQmax and BOOI. Longer duration of dialysis was accompanied by significantly lower FDV, NDV and Cmax, compliance and Qmax.p and with significantlyhigher Pdet.max, PdetQmax and BOOI. Older age was associated with significantly higher Pdet.max and with significantly lower compliance. Worsening of the basic parameters of the storage function of the lower urinary tract occurred when diuresis decreased to 500-750 mL, when the duration of dialysis was one year and when patients were older than 54 years. CONCLUSION: In healthy male patients on the waiting list for kidney transplant, there were connections between the occurrence of dysfunctions of the lower urinary tract and diuresis, duration of dialysis and age. Patients should be monitored for dysfunctions of the lower urinary tract before and after transplantation.

Inkontinence moči patří k nejčastějším zdravotním problémům zejména ženské populace, kterým trpí v celosvětovém měřítku stamilióny lidí. Zejména v posledních dvou desetiletích jsme svědky rostoucího zájmu o tuto problematiku jak ze strany odborné, tak i laické veřejnosti. Nejčastějšími formami inkontinence, se kterými se v praxi setkáváme, jsou stresová a urgentní inkontinence. Fyzioterapie hrála v léčbě inkontinence vždy spíše okrajovou úlohu a tradičně byla indikována pouze v léčbě stresové formy. Práce předkládá nový pohled na roli fyzioterapie v léčbě močové inkontinence. Díky pokrokům v posledních letech je dnes na správně indikovanou a kvalifikovaně prováděnou fyzioterapii pohlíženo jako na plnohodnotnou a široce použitelnou metodu léčby jak stresové, tak urgentní inkontinence a hyperaktivního měchýře (symptomový komplex zahrnující závažné urgence, zpravidla provázené frekvencemi a někdy též urgentní inkontinencí).
Urinary incontinence is one of the most common health problems especially in the female population. Globally hundreds of millions of people suffer from urinary incontinence. Over the last two decades, we have witnessed a growing interest in this field by both health care professionals and the general public. The main two forms of incontinence are stress urinary incontinence and urge incontinence. Physiotherapy played in the treatment of incontinence always rather marginal role and traditionally was recommended only for the treatment of stress urinary incontinence. This paper presents a new perspective on the role of physiotherapy in the treatment of urinary incontinence. Thanks to advances our knowledge in recent years, physical therapy is now regarded as a fully-fledged and widely applicable method of treatment for both stress and urinary incontinence, as well as an overactive bladder (symptom complex characterized by urgency, with or without urge incontinence, usually associated with frequency).

Hypoxic pulmonary vasoconstriction (HPV) is a beneficial mechanism that diverts blood from hypoxic alveoli to better ventilated areas of the lung, but breathing hypoxic air causes the pulmonary circulation to become hypertensive. Responses to airway hypoxia are associated with depolarization of smooth muscle cells in the pulmonary arteries and reduced activity of K(+) channels. As Kv7 channels have been proposed to play a key role in regulating the smooth muscle membrane potential, we investigated their involvement in the development of HPV and hypoxia-induced pulmonary hypertension. Vascular effects of the selective Kv7 blocker, linopirdine, and Kv7 activator, flupirtine, were investigated in isolated, saline-perfused lungs from rats maintained for 3-5 days in an isobaric hypoxic chamber (FiO2 = 0.1) or room air. Linopirdine increased vascular resistance in lungs from normoxic, but not hypoxic rats. This effect was associated with reduced mRNA expression of the Kv7.4 channel α-subunit in hypoxic arteries, whereas Kv7.1 and Kv7.5 were unaffected. Flupirtine had no effect in normoxic lungs but reduced vascular resistance in hypoxic lungs. Moreover, oral dosing with flupirtine (30 mg/kg/day) prevented short-term in vivo hypoxia from increasing pulmonary vascular resistance and sensitizing the arteries to acute hypoxia. These findings suggest a protective role for Kv7.4 channels in the pulmonary circulation, limiting its reactivity to pressor agents and preventing hypoxia-induced pulmonary hypertension. They also provide further support for the therapeutic potential of Kv7 activators in pulmonary vascular disease.

Úvod: Inkontinence stolice je výrazně deprimujícím a psychicky devastujícím postižením. Chirurgická léčba, jako metoda první volby, je indikována u inkontinence vzniklé v důsledku traumatického nebo iatrogenního postižení svěračového aparátu, zejména zevního svěrače. U idiopatické (neurogenní) inkontinence není indikována až na výjimky vůbec. Autoři předkládají pilotní studii ověřující možnost chirurgické léčby anální inkontinence podporou smyčky puborektálního svalu vstřebatelnou síťkou STRATASIS? TF . Materiál a metodika: V letech 2010–2012 bylo provedeno 8 experimentálních operačních výkonů. Indikovány byly ženy s anamnézou inkontinence III. stupně delší jednoho roku a EMG verifikovaným neurologickým postižením. Hodnotícím kritériem bylo Wexner skóre a změny levátorového a anorektálního úhlu získané z MR defekografického vyšetření provedeného před a 6 měsíců po operaci. Výsledky: U 6ti nemocných došlo k výraznému zlepšení obtíží. Ve 2 zbylých případech bylo zaznamenáno plné selhání metody, které bylo v obou případech spojeno se zánětlivou komplikací. U 1 pacientky autoři nevylučují i možnou chybnou indikaci. Wexner skóre pokleslo u zlepšených z 18 na 10. Změny levátorového i anorektálního úhlu nebyly signifikantní. Závěr: Výsledky potvrzují možnost úspěšného chirurgického ovlivnění inkontinence III. stupně korekcí levátorového hiátu pomocí biodegradabilní síťky. Zlepšení kontinence přetrvává i po přestavbě síťky v jizevnatou tkáň.
Introduction: Faecal incontinence is a significantly depressing and mentally devastating disability. Surgical treatment, as a first choice method, is indicated for incontinence originating as a result of traumatic or iatrogenic infliction of the sphincter apparatus, particularly of the external sphincter. In case of idiopathic (neurogenic) incontinence, it is indicated very exceptionally, if ever. The authors present a pilot study to verify the possibility of surgical treatment of anal incontinence with the support of a puborectal muscle loop by means of the absorbable STRATASIS? TF mesh. Material and methods: In the years 2010–2012, eight experimental surgical procedures were performed. Female patients with a history of faecal incontinence of the third stage longer than one year and with EMG-verified neurological lesion were recruited. The evaluating criteria were the Wexner score and changes in the levator and anorectal angle acquired from a MR defecographic examination performed before and six months after the operation. Results: The complaints improved distinctly in six patients; in the two remaining cases, the method failed completely. The failures were associated with an inflammatory complication in both cases. In one patient, the authors do not rule out an incorrect indication, too. The Wexner score decreased from 18 to 10 in improved patients. Changes in the levator and anorectal angle were not significant. Conclusion: The results confirm the possibility of successful surgical influence on incontinence of the third stage by correcting the levator hiatus with biodegradable mesh. Continence improvement persists even after the mesh transforms into scar tissue.

Postižení trávicího ústrojí u diabetiků je časté a představuje závažný problém kvůli četnosti výskytu, vlivu na snížení kvality života a vlivu na kompenzaci diabetu. Po 20 letech trvání diabetu se vyskytuje u 30–60 % pacientů. Gastroparéza je stav významně opožděného vyprazdňování potravy ze žaludku do duodena, kdy příčinou není mechanická obstrukce žaludečního antra či pyloru. Je to velmi heterogenní skupina závažných motorických poruch žaludku různé etiologie a asi třetinu všech případů představuje diabetes mellitus (DM). Symptomy doprovázející diabetickou gastroparézu se vyskytují asi u 5–12 % diabetiků. Poruchy motility trávicí trubice jsou podmíněny diabetickou autonomní neuropatií v zažívacím traktu. K vyšetření rychlosti evakuace žaludku je k dispozici scintigrafie evakuace žaludku, která je považována za zlatý standard. Retence více než 60 % testační stravy v žaludku po dvou hodinách a 10 % po čtyřech hodinách je považována za důkaz významně opožděného vyprazdňování žaludku. Dále je k dispozici dechový test s kyselinou 13C-oktanovou nebo s 13C-Na-acetátem. Tyto dvě zmíněné metody nevyšetřují identický děj. V optimálním případě se tedy oba testy doplňují. K posouzení myoelektrické aktivity žaludku je k dispozici elektrogastrografie (EGG). EGG zatím není zavedena do běžné praxe a její praktické využití v klinické praxi je zatím otázkou budoucnosti.
Involvement of the gastrointestinal tract in patients with diabetes is common and is a serious problem due to the frequency of occurrence, the impact on the reduced quality of life and the effect on glycemic control. After 20 years of diabetes it occurs in 30 to 60% of patients. Gastroparesis is a disorder characterised by a significant delay in gastric emptying after a meal in the absence of a mechanical obstruction of gastric antrum or pylorus. Gastroparesis accounts for a very heterogeneous group of severe motor gastric disorders of various aetiology and diabetes mellitus accounts for about one third of all cases. The symptoms of gastroparesis occur in 5% to 12% patients of diabetes. Motility disorders of the gastrointestinal tract are subject to diabetic autonomy neuropathy in the intestinal tract. The gold standard test to diagnose gastroparesis is scintigraphy. Retention of more than 60% of the test meal in the stomach after two hours and 10% after four hours is considered evidence of a significant delay in gastric emptying. Another alternative method for assessing gastric function includes the gastric emptying breath test using 13C-octanoic acid or 13C-Na-acetate. These two methods do not investigate the same action. Ideally, therefore the two tests complement each other. The electrogastrography (EGG) is a method for testing the myoelectrical activity of the stomach. EGG has not yet been introduced into routine practice and its practical application in clinical practice still remains a question for the future. Both divisions of the autonomic nervous system are involved in regulation of urinary bladder function. Several substances, other than noradrenaline and acetylcholine, seem to play important roles in physiology and pathophysiology of lower urinary tract. In the current study, we aimed to examine if there exist interplays between nitric oxide (NO) and autonomic transmitters and if such interactions vary in different parts of the urinary bladder in healthy and cyclophosphamide (CYP)-induced cystitic rats; when administered to the animals (100 mg/kg; i.p.), the cytotoxic CYP metabolite acrolein induces bladder inflammation. In the current study a series of in vitro functional studies were performed on detrusor muscle strip preparations. Stimulation with electrical field stimulation (EFS), methacholine, adenosine 5´-triphosphate (ATP), and adrenaline evoked contractile responses in isolated bladder preparations that were significantly reduced in cyclophosphamide (CYP)-treated rats. While the nitric oxide synthase inhibitor N(omega)-nitro-L-arginine (L-NNA; 10(-4) M) did not affect contractile responses in normal, healthy strip preparations, it significantly increased the contractile responses to EFS, methacholine and adrenaline, but not to ATP, in the bladders from the CYP-treated rats. In the CYP-treated rats, the ATP-evoked relaxatory part of its dual response (an initial contraction followed by a relaxation) was 6-fold increased in comparison with that of normal preparations, whereas the isoprenaline relaxation was halved in the CYP-treated. While L-NNA (10(-4) M) had no effect on the isoprenaline-evoked relaxations, it reduced the ATP-evoked relaxations in strip preparations from the bladder body of CYP-treated rats. Stimulation of beta(2)- and beta(3)-adrenoceptors evoked relaxations and both responses were reduced in cystitis, the latter to a larger extent. In the trigone, the reduced ATP-evoked contractile response in the inflamed strips was increased by L-NNA, while L-NNA had no effect on the ATP-evoked relaxations, neither on the relaxations in healthy nor on the larger relaxations in the inflamed trigone. The study shows that both contractile and relaxatory functions are altered in the state of inflammation. The parasympathetic nerve-mediated contractions of the body of the bladder, evoked by the release of ATP and acetylcholine, were substantially reduced in cystitis. The relaxations to beta-adrenoceptor and purinoceptor stimulation were also reduced but only the ATP-evoked relaxation involved NO.

The aim of the present study was to investigate the relaxatory function of adenosine receptor subtypes in rat urinary bladder, and if it is altered in the state of inflammation. The in vitro responses to the P1 receptor agonist adenosine were investigated in the presence of the general P2 receptor antagonist pyridoxal-phosphate-6-azophenyl-2',4'-disulfonic acid (PPADS; 1*10(-4)M). Experiments were performed on preparations from normal (healthy) rats and rats with cyclophosphamide (CYP; 100mg kg(-1) i. p.)-induced cystitis. The specific P1A(1) antagonist 1,3-dipropyl-8-cyclopentylxanthine (DPCPX; 1*10(-5)M) decreased the adenosine relaxatory response in normal bladders (-60%), but not in preparations from CYP pre-treated rats. Immunohistochemical findings support the hypothesis that the expression of P1A(1) receptors in the rat urinary bladder is decreased during cystitis. The adenosine-evoked relaxation was not affected by the specific P1A(2A) antagonist SCH 58261 (3*10(-7)M), neither in normal nor in CYP pre-treated rats. The relaxation to adenosine was, however, significantly increased by the specific P1A(3) antagonist MRS 1523 (1*10(-5)M) in preparations from both normal and CYP pre-treated rats, suggesting P1A(3) to be mediating bladder contraction. Thus, in the rat urinary bladder the relaxation to adenosine is mainly due to the P1A(1) receptor, while the P1A(3) receptor seems to be responsible for contractile responses. The DPCPX-resistant part of the relaxation is possibly due to the P1A(2B) receptor, the fourth subtype of the adenosine receptor family.

Botulotoxin je v současné době významným přínosem v léčbě celé řady poruch motoriky – spasticity, dystonie, hyperkinéz, tremoru. V menším rozsahu se využívá botulotoxinu při poruchách hladkého svalstva (hyperfunkční syndromy močového měchýře, achalazie jícnu) a při hyperfunkčních autonomních syndromech (hyperhidróza, hypersalivace). Rozvoj terapeutického využití botulotoxinu povede k ovlivnění dalších motorických hyperfunkčních projevů, k indikaci u viscerálních poruch, dysfunkcí autonomního nervového systému a v indikaci bolestivých stavů. V práci jsou shrnuty dosavadní indikace, mechanizmy působení i předpokládaný další rozvoj klinického využití botulotoxinu.
Botulinum toxin nowadays represents an important contribution to the treatment of several motor disorders – spasticity, dystonia, hyperkinetic syndromes, tremor. In a lesser extent botulinum toxin is used in smooth muscle dysfunction (e. g. urine bladder hyperfunctional syndromes, oesophagus achalazia) and in hyperfuctional autonomic syndromes (e.g. hyperhidrosis, hypersalivation). We suppose that the therapeutical use of botulinum toxin will include new indications of motor system hyperfunctional syndromes, in visceral disorders, dysfunctions of autonomic nervous system and new indications in various pain syndromes. In this review article we summarize the most up-to date indications, mechanisms of action and presumed future development of clinical use of botulinum toxin.
