BACKGROUND: The pharmacokinetics of tacrolimus (TAC) and mycophenolic acid (MPA) are highly variable. An impact of single-nucleotide polymorphisms (SNPs) of the genes coding for enzymes and transporters involved in the pharmacokinetics of TAC and/or MPA is intuitively conceivable. Accordingly, we sought to analyze the influence of different SNPs on TAC and MPA exposure in pediatric renal transplant recipients. METHODS: A subpopulation of 37 patients (median age: 12.8 years, range 2.2-18.3 years) participating in the TWIST study was included in the analysis of SNPs of CYP3A5, ABCB1 (MDR1), ABCG2, SLCO1B3 (coding for OATP2), ABCC2 (coding for cMOAT), and UGT1/2. TAC trough concentrations and abbreviated area under the concentration-time curves (AUC) of MPA were measured on days 7, 28, 91, and 183 after transplant. Both of these were adjusted to the respective dose the patient received. RESULTS: The allele frequencies of analyzed SNP's were comparable to those reported previously for white populations. Dose-adjusted trough concentrations of TAC were approximately 60% lower in patients with the CYP3A5*1/*3 allele as compared with the CYP3A5*3/*3 allele (P = 0.004). Steroid-free patients in CYP3A5*3/*3 and CYP3A5*1/*3 carrier subgroups had comparable dose-adjusted TAC concentrations to the subgroup on steroids (P = 0.13). Patients younger than 10 years had a significantly lower median dose-adjusted TAC C0 concentration than patients older than 10 years; this age effect was comparable in heterozygous and homozygous CYP3A5 carriers as well as in patients on and off steroid medication. As for MPA, the genetic variability of transporters or enzymes had no impact on dose-adjusted MPA-AUC due to the low allele frequencies. Patients off steroids had a higher dose-adjusted MPA-AUC (0.18 mg·h/L per mg/m, 0.012-0.27) compared with patients on steroids (0.12 mg·h·L·mg, 0.09-0.19; P = 0.04). CONCLUSIONS: Genetic variability of CYP3A5 has an impact on TAC metabolism in pediatric renal transplant recipients, contributing partly to the variability of TAC exposure. Therefore, adjusting initial TAC dosing to the genotype of CYP3A5 might be of clinical benefit.

Aims: Tacrolimus is the mainstay of transplant immunosuppression in kidney graft recipients. It is most commonly used in combination with mycophenolate mofetil. Literature data on the existence and significance of drug-drug interaction of these two drugs is contradictory and inconclusive for kidney transplant patients. The aim of the study was to confirm and quantify the interaction in kidney transplant patients. Methods: A total of 4,220 tacrolimus level measurements spanning 5 years in 181 renal graft recipients in a single transplant center were analyzed. Change in dose needed to achieve a unit concentration was used as a surrogate for drug clearance variability. A regression multivariate model was constructed to identify significant predictors of tacrolimus dose required to reach a unit concentration. Results: The model identified significant predictors of tacrolimus dose, including hematocrit, liver function, body weight, prednisone dose, and age. The coefficient for mycophenolate mofetil dose was -8.76e-04 (standard error 1.35e-04, p < 0.001), i.e. each 1000 mg increase of mycophenolate dose lead on average to a 15.1 % reduction in the dose of tacrolimus required to reach the same concentration. Conclusions: Based on our analysis, the interaction between mycophenolate mofetil and tacrolimus reported previously in liver transplant patients is present in kidney transplant patients as well. After prospective validation, a pharmacokinetic model could be used to predict tacrolimus level changes following adjustment of mycophenolate mofetil doses.
Cíle: Takrolimus je základem imunosuprese u příjemců transplantované ledviny. Nejčastěji je užíván v kombinaci s mykofenolát mofetilem. Údaje o existenci a významu interakce těchto dvou léků u pacientů po transplantaci ledvin jsou protichůdné a nejednoznačné. Cílem této studie bylo potvrdit a kvantifikovat tuto interakci u pacientů po transplantaci ledvin. Metody: Bylo analyzováno celkem 4 220 měření hladin takrolimu v období 5 let u 181 pacientů s renálním štěpem v jednom transplantačním centru. Změna dávky potřebné k dosažení jednotkové koncentrace byla použita jako náhradní parametr pro popis variability clearance léku. Regresní multivariační model byl sestaven tak, aby identifikoval významné prediktory dávky takrolimu potřebné k dosažení jednotkové koncentrace. Výsledky: Model identifikoval významné prediktory dávky takrolimu jako hematokrit, jaterní funkce, tělesnou hmotnost, věk pacienta a jeho dávku prednisonu. Koeficient dávky mykofenolátmofetilu byl -8,76e-04 (standardní chyba 1,35e-04, p < 0,001). Každé zvýšení dávky mykofenolátu o 1 000 mg tedy vedlo v průměru k 15,1% snížení dávky takrolimu potřebné k dosažení stejné koncentrace. Závěr: Na základě naší analýzy je interakce mezi mykofenolát mofetilem a takrolimem, identifikovaná již dříve u pacientů po transplantaci jater, přítomna také u pacientů po transplantaci ledviny. Po prospektivní validaci by pro stanovení změn hladiny takrolimu po úpravě dávky mykofenolát mofetilu mohl být použit farmakokinetický model.

Dostupná imunosupresiva jsou užívaná u transplantací orgánů, u autoimunitních onemocnění, astma bronchiale, revmatoidní artritidy a v mnoha dalších indikacích. Zástupci skupiny imunosupresiv mají relativně úzké terapeutické rozmezí a významnou toxicitu závislou na podané dávce. Tento přehledový článek je věnován farmakodynamice, biotransformaci, lékovým interakcím a nežádoucím účinkům kalcineurinových inhibitorů, mTOR inhibitorů, mykofenolátu, cyklofosfamidu a metotrexátu.
The currently available immunosuppressive agents have been used for the management of patient with a wide range of medicinal issues, such as organ transplantation, autoimmune diseases, asthma, and rheumatoid arthritis, among others. All of the agents have relatively narrow therapeutic ranges with significant dose-limiting toxicities. This review discusses pharmacodynamic properties, biotransformation, drug interactions, and side effects of calcineurin inhibitors, mTOR inhibitors, mycophenolat, cyclophosphamide, and methotrexate.

Úspěšná transplantace ledviny závisí především na chirurgickém zákroku a potlačení rejekce štěpu imunosupresivní léčbou. Úkolem transplantologů je nalézt rovnováhu mezi účinnou imunosupresí potlačující rejekci a imunosupresí spojenou s vedlejšími účinky, jakými jsou vznik infekcí, novotvarů, metabolických změn a nefrotoxicita.
Successful kidney transplantation depends mostly on surgical intervention and graft rejection by suppression by immunosuppressive treatment. Tranplantologists‘ task is to find out the balance between effective rejection suppressing immunosuppression and immunosuppression connected with side effects like infections development, neoplasms, metabolic changes and nephrotoxicity.

The aim of our study was to evaluate the efficacy of FK506, mycophenolate mofetil (MM) and aminoguanidine (AMG) on infiltration of macrophages (MPHs), neutrophils (NPHs) and dendritic cells (DC) into corneal grafts during the early phases after transplantation (Tx). Tx was performed in mice (C57BL/10 to BALB/c). Therapy included FK506 (0.2 mg/kg), MM (30 mg/kg) or AMG (0.1 g/kg), started at the day of Tx and was injected i.p. daily. Corneas were excised on the 3rd and 7th day after Tx. Immunohistological evaluation using antibodies against MPHs, NPHs and DC was performed and corneal grafts were assessed in the periphery and in central part of the cornea separately. On the 3rd day after Tx, a massive infiltration of MPHs and NPHs into corneal grafts was revealed; the DC infiltration was lower in all treated groups. Treatment with FK506 and MM led to a significant reduction of NPHs in the centers of the grafts, but not of MPHs. In contrast, AMG significantly reduced MPHs migration into allografts on the third day after Tx, whereas NPHs infiltration has not been attenuated. However, immunosuppressants had no influence on the infiltration of DC during early phases after Tx.

Efficacy and safety of mycophenolate mofetil (MMF) may be optimized with individualized doses based on therapeutic monitoring of its active metabolite, mycophenolic acid (MPA). In this 12-month study, 137 renal allograft recipients from 11 French centers receiving basiliximab, cyclosporine A, MMF and corticosteroids were randomized to receive either concentration-controlled doses or fixed-dose MMF. A novel Bayesian estimator of MPA AUC based on three-point sampling was used to individualize doses on posttransplant days 7 and 14 and months 1, 3 and 6. The primary endpoint was treatment failure (death, graft loss, acute rejection and MMF discontinuation). Data from 65 patients/group were analyzed. At month 12, the concentration-controlled group had fewer treatment failures (p = 0.03) and acute rejection episodes (p = 0.01) with no differences in adverse event frequency. The MMF dose was higher in the concentration-controlled group at day 14 (p < 0.0001), month 1 (p < 0.0001) and month 3 (p < 0.01), as were median AUCs on day 14 (33.7 vs. 27.1 mg*h/L; p = 0.0001) and at month 1 (45.0 vs. 30.9 mg*h/L; p < 0.0001). Therapeutic MPA monitoring using a limited sampling strategy can reduce the risk of treatment failure and acute rejection in renal allograft recipients 12 months posttransplant with no increase in adverse events.
