Hereditární nádorové syndromy s možnou manifestací v oblasti ženského vnitřního genitálu představují heterogenní skupinu onemocnění. Mezi dva nejčastější syndromy patří syndrom hereditárního karcinomu prsu a ovarií a Lynchův syndrom. Méně časté syndromy zahrnují syndrom predispozice k maligním rabdoidním nádorům, Cowdenův syndrom, komplex tuberózní sklerózy, DICER1 syndrom, syndrom névoidního bazocelulárního karcinomu, Peutz-Jeghersův syndrom, von Hippelova-Lindauova choroba a syndrom hereditární leiomyomatózy a renálního karcinomu. Cílem následujícího sdělení je podat přehled problematiky hereditárních nádorových syndromů s manifestací v oblasti ženského genitálu se zaměřením na jejich přehled, charakteristiky nádorů, které se v souvislosti s jednotlivými syndromy vyskytují, postup při vyšetřování profylakticky odstraněných tkání a orgánů a problematiku screeningu Lynchova syndromu.
Hereditary tumor syndromes with a possible manifestation in the female internal genital tract represent a heterogeneous group of diseases. The two most common entities are the hereditary breast and ovarian cancer syndrome, and the Lynch syndrome. The less common syndromes include the rhabdoid tumor predisposition syndrome, Cowden syndrome, tuberous sclerosis complex, DICER1 syndrome, nevoid basal cell carcinoma syndrome, Peutz-Jeghers syndrome, von Hippel-Lindau disease, and hereditary leiomyomatosis and renal cell cancer syndrome. The goal of this manuscript is to provide a comprehensive overview of those hereditary tumor syndromes which can manifest in the area of the female genital system, with an emphasis on their summary, the characteristics of the tumors which can develop in association with these syndromes, and the approach to the processing of prophylactically removed tissues and organs. The issue of Lynch syndrome screening is also discussed.

Phosphatase and tensin homologue (PTEN) is a tumour suppressor gene implicated in tumorigenesis of melanoma, with distinct cytoplasmic and nuclear functions. Cytoplasmic PTEN negatively regulates the PI3K/AKT/mTOR signalling pathway, while nuclear PTEN works as a tumour suppressor. Clinical data suggest that the loss of PTEN function in melanoma is associated with aggressive tumour behaviour. We performed a comprehensive analysis of PTEN in 112 primary cutaneous melanomas including immunohistochemical (IHC), fluorescent in situ hybridization (FISH), next-generation sequencing (NGS), and epigenetic analysis. The goal of our study was to: (a) correlate PTEN expression with selected clinico-pathological variables, and assess its prognostic significance; (b) correlate molecular aberrations with PTEN expression to consider the utility of immunohistochemical analysis of PTEN protein expression for screening PTEN genetic alterations; (c) review the literature and evaluate the PTEN expression level in melanoma with respect to possible therapeutic targeting. Our results showed that PTEN molecular alterations were present in 4/20 (20 %) cases with a loss of expression, 3/11 (27 %) cases with clonal-like expression, and 1/81 (1 %) cases with positive PTEN expression. No PTEN promoter methylation was found in any of the cases. Even though the value of our observation is limited by the low number of cases fully evaluated by IHC (112 cases), FISH (19 cases) and NGS (30 cases), our data suggest that IHC is not an appropriate method for the screening of PTEN genetic alterations. Our survival analysis suggests that patients with positive cytoplasmic PTEN expression show better disease-free survival (P < 0.05).

CONTEXT.—: Ultrastaging of sentinel lymph nodes (SLNs) is a crucial aspect in the approach to SLN processing. No consensual protocol for pathologic ultrastaging has been approved by international societies to date. OBJECTIVE.—: To provide a review of the ultrastaging protocol and all its aspects related to the processing of SLNs in patients with cervical cancer. DATA SOURCES.—: In total, 127 publications reporting data from 9085 cases were identified in the literature. In 24% of studies, the information about SLN processing is entirely missing. No ultrastaging protocol was used in 7% of publications. When described, the differences in all aspects of SLN processing among the studies and institutions are substantial. This includes grossing of the SLN, which is not completely sliced and processed in almost 20% of studies. The reported protocols varied in all aspects of SLN processing, including the thickness of slices (range, 1-5 mm), the number of levels (range, 0-cut out until no tissue left), distance between the levels (range, 40-1000 μm), and number of sections per level (range, 1-5). CONCLUSIONS.—: We found substantial differences in protocols used for SLN pathologic ultrastaging, which can impact sensitivity for detection of micrometastases and even small macrometastases. Since the involvement of pelvic lymph nodes is the most important negative prognostic factor, such profound discrepancies influence the referral of patients to adjuvant radiotherapy and could potentially cause treatment failure. It is urgent that international societies agree on a consensual protocol before SLN biopsy without pelvic lymphadenectomy is introduced into routine clinical practice.
Hodnocení zánětlivé celulizace (tumor infiltrujících lymfocytů – TIL) by u maligního melanomu mělo být standardní součástí bioptického vyšetření. Nejčastěji používaný způsob hodnocení podle Clarka však není optimální a je snaha o nalezení alternativního systému. V následujícím sdělení podáváme přehled možných přístupů zahrnujících pět různých způsobů hodnocení z barvení hematoxylin-eozinem včetně recentního návrhu sjednocené koncepce hodnocení TIL pro všechny solidní nádory. Diskutována je problematika metodiky, prognostického a prediktivního významu stanovení TIL a také význam imunohistochemické subtypizace zánětlivého infiltrátu.
The evaluation of inflammatory infiltrate (tumor infiltrating lymphocytes – TIL) should be a standard part of biopsy examination for malignant melanoma. Currently, the most commonly used assessment method according to Clark is not optimal and there have been attempts to find an alternative system. Here we present an overview of possible approaches involving five different evaluation methods based on hematoxylin-eosin staining, including the recent suggestion of unified TIL evaluation method for all solid tumors. The issue of methodology, prognostic and predictive significance of TIL determination as well as the importance of immunohistochemical subtyping of inflammatory infiltrate is discussed.

Metodika: Provedli jsme revizi recentní i starší literatury s intencí sestavit souhrnné sdělení o hamartomech dutiny břišní a retroperitonea. Úvod: Hamartom v dutině břišní a retroperitoneu je vzácné onemocnění, jehož problematice je věnován relativně malý prostor. Výsledky: Nejčastěji postiženými orgány bývají játra, slezina, trávicí trubice, pankreas a ledviny. Svou biologickou povahou je hamartom benigní léze s dobrou prognózou. Může však dojít k neoplastické transformaci a je třeba jej pokládat za potenciálně maligní. Pacienti bývají velmi často asymptomatičtí. Znaky a příznaky se odvíjejí od orgánové lokalizace a různou měrou se na nich podílejí obstrukce, útlak okolní tkáně nebo ruptura či krvácení z tumoru. V diagnostice zobrazovacími metodami není ve většině případů možné bezpečně odlišit hamartom od maligní léze, proto bývá pacient raději indikován k chirurgické revizi (či endoskopickému odstranění) za účelem histologické verifikace a zároveň definitivní léčby. Závěr: Hamartomy dutiny břišní a retroperitonea jsou relativně vzácná, benigní a často asymptomatická onemocnění, která se ovšem mohou projevit útlakem, obstrukcí, perforací, krvácením a dokonce maligním zvratem. V závěru uvedená recentní kazuistika hamartomu sleziny dokazuje, že chirurgické řešení je často jedinou racionální možností definitivního potvrzení diagnózy.
Methods: We have reviewed recent as well as older literature with the intention of compiling a summary report on hamartomas of the abdominal cavity and retroperitoneum. Introduction: Hamartoma of the abdominal cavity and retroperitoneum is a rare condition which has received relatively little attention. Results: The most commonly affected organs are the liver, spleen, digestive tract, pancreas and kidneys. By its nature, hamartoma is a benign lesion with a good prognosis. However, neoplastic transformation may occur and hamartomas need to be regarded as potentially malignant. Patients are very often asymptomatic. Signs and symptoms depend on the organ involved and are caused, to different degrees, by obstruction, surrounding tissue compression, rupture or tumor bleeding. In imaging diagnostics, it is usually not possible to safely distinguish between a hamartoma and a malignant lesion. Therefore, the patient is preferably indicated for surgery (or endoscopic removal) for the purpose of histological verification and definitive treatment. Conclusion: Hamartomas of the abdominal cavity and retroperitoneum are relatively rare, benign and often asymptomatic diseases which, however, may manifest as compression, obstruction, perforation, bleeding or even malignant transformation. The recent case report of splenic hamartoma presented at the end of this review proves that surgical management is often the only rational option to obtain definitive confirmation of the diagnosis.

Intravezikální aplikace BCG vakcíny je zavedená, relativně jednoduchá a bezpečná terapeutická metoda užívaná v léčbě povrchových karcinomů močového měchýře. Obvykle bývá dobře tolerována, nežádoucí účinky bývají mírné. Prezentujeme případ 76letého imunokompetentního pacienta, u kterého vedla tato léčba k vzácné komplikaci ve formě granulomatozní hepatitidy a pneumonitidy. V diagnostice tohoto stavu byla použita také elastografie jater.
Intravesical BCG application is an established and relatively simple treatment for superficial carcinomas of the urinary bladder. It is usually well tolerated and has only mild side effects. We report a case of a 76-year-old immunocompetent man in whom this treatment led to a rare complication characterized by granulomatous hepatitis and pneumonitis. Elastography of the liver was used during diagnosis.

Mantle cell lymphoma (MCL) is an aggressive type of B-cell non-Hodgkin lymphoma (NHL) associated with poor prognosis. Animal models of MCL are scarce. We established and characterized various in vivo models of metastatic human MCL by tail vein injection of either primary cells isolated from patients with MCL or established MCL cell lines (Jeko-1, Mino, Rec-1, Hbl-2, and Granta-519) into immunodeficient NOD.Cg-Prkdc(scid) Il2rg(tm1Wjl)/SzJ mice. MCL infiltration was assessed with immunohistochemistry (tissues) and flow cytometry (peripheral blood). Engraftment of primary MCL cells was observed in 7 out of 12 patient samples. The pattern of engraftment of primary MCL cells varied from isolated involvement of the spleen to multiorgan infiltration. On the other hand, tumor engraftment was achieved in all five MCL cell lines used and lymphoma involvement of murine bone marrow, spleen, liver, and brain was observed. Overall survival of xenografted mice ranged from 22 ± 1 to 54 ± 3 days depending on the cell line used. Subsequently, we compared the gene expression profile (GEP) and phenotype of the engrafted MCL cells compared with the original in vitro growing cell lines (controls). We demonstrated that engrafted MCL cells displayed complex changes of GEP, protein expression, and sensitivity to cytotoxic agents when compared with controls. We further demonstrated that our MCL mouse models could be used to test the therapeutic activity of systemic chemotherapy, monoclonal antibodies, or angiogenesis inhibitors. The characterization of MCL murine models is likely to aid in improving our knowledge in the disease biology and to assist scientists in the preclinical and clinical development of novel agents in relapsed/refractory MCL patients.

Využití molekulárně biologických a cytogenetických metod a rozvoj poznatků týkající se molekulární podstaty melanocytárních nádorů je v posledních letech na vzestupu. Okruhy zájmu zahrnují nejen diagnostiku a oblast prediktivní patologie, ale např. i genetické poradenství a molekulární klasifikaci melanocytárních lézí. V následujícím textu jsou probrány současné poznatky týkající molekulární podstaty maligního melanomu, využití molekulárně biologických a zejména cytogenetických metod v diferenciální diagnostice sporných melanocytárních lézí a problematika prediktivního testování.
The use of molecular biology and cytogenetic methods and the development of knowledge regarding the molecular basis of melanocytic tumors in recent years has been on the rise. Topics of interest include not only diagnostic and predictive pathology, but also genetic counseling and molecular classification of melanocytic lesions. The following text discusses the current knowledge on the molecular basis of malignant melanoma, the use of molecular biology and cytogenetic methods, especially in the differential diagnosis of melanocytic lesions, and the difficulties of predictive testing.
