Úvod: Karcinom pankreatu je jedním z nejagresivnějších maligních onemocnění vůbec se staticky prokazatelně stoupající tendencí a velmi špatnou prognózou. Příčiny sledujeme jednak v lokálních recidivách a jednak v časné diseminaci, a to buď cestou hematogenní, či lymfogenní. Konvenčními metodami nejsme schopni tyto buňky zachytit. Teprve moderní molekulárně-biologické metody, které umožňují stanovit tzv. minimální reziduální chorobu, tj. přítomnost izolovaných nádorových buněk v těle pacienta. Metodika: Do studie bylo zařazeno 52 pacientů operovaných na I. chirurgické klinice FN Olomouc pro karcinom pankreatu v různém stadiu. Metodou QRT-PCR byla stanovena exprese hTERT, EGFR 1 a CEA a to v periferní krvi, portální krvi, kostní dřeni, peritoneální laváži a v tumoru samotném. Výsledky a závěr: Výsledky této pilotní studie prokazují vysokou senzitivitu a specifitu PCR metodiky pro detekci cirkulujících nádorových buněk u pacientů s karcinomem pankreatu, rozšířením této metodiky budeme schopni stanovit prognostické hodnoty minimální reziduální choroby a její význam při indikaci k radikálnímu chirurgickému výkonu na pankreatu.
Introduction: Pancreatic cancer is one of the most aggressive malignity with the statistically shown an upward trend and a very poor prognosis. The causes we follow up in the local recurrence and in the early dissemination, either through hematogenic or lymphogenic way. Conventional methods are not able to capture these cells, just the modern molecular-biological methods make possible to fix the so-called minimal residual disease, that is the presence of isolated tumor cells in the patient’s body. Material and Methods: The study included 52 patients operated on the Clinic of Surgery I. in the University Hospital Olomouc for pancreatic cancer in different stages. QRT-PCR method was determined expression of hTERT, EGFR 1 and CEA both in peripheral blood, portal blood, bone marrow, peritoneal lavage and the tumor itself. Results and Conclusion: The results of this pilot study demonstrated a high sensitivity and specificity of the PCR method for detection of circulating tumor cells in patients with pancreatic cancer, extending this methodology, we are able to provide prognostic value of minimal residual disease and its significance for the indication of radical surgery for pancreatic cancer.

To define multiresistant bacterial strains in livestock breeding, to assess risks of transmission into human population by foods of animal origin (meat and meat products, milk and dairy products) and by livestock breeding personnel. Analysing DNA, and resistance genes in multiresistant bacterial strains from the biological material from human population (both community and hospital setting), livestock and animal-origin foods; creating a hypothesis about strain source and spread. To define the impact ofthe selection pressure of antibiotics (namely large-scale medication) in animals kept for food production (cattle, pigs, poultry) on the occurrence of multiresistant bacteria. Creating guidelines for veterinary antibiotics policy aiming at minimizing theoccurrence of multiresistant bacteria in livestock breeding and decreasing health risks for human population.
Definovat multirezistentní bakteriální kmeny v chovech hospodářských zvířat, vyhodnotit rizika průniku do lidské populace prostřednictvím potravin živočišného původu (maso a masné výrobky, mléko a mléčné výrobky) a prostřednictvím ošetřovatelů zvířat. Analýza DNA a genů rezistence u multirezistentních bakteriálních kmenů z biologického materiálu humánní populace (komunitní i nemocniční prostředí), hospodářských zvířat a potravin živočišného původu, vytvoření hypotézy o zdroji a šíření těchto kmenů. Definovat vliv selekčního tlaku antibiotik (zejména plošné medikace) u hospodářských zvířat (skot, prasata, drůbež) na výskyt multirezistentních bakterií. Vypracovat doporučení pro veterinární antibiotickou politiku s cílem minimalizovat výskyt multirezistentních bakterií v chovech hospodářských zvířat a snížit zdravotní rizika pro lidskou populaci.

This study focused on the prevalence and molecular biology of extended-spectrum beta-lactamase (ESBL)-positive Klebsiella pneumoniae isolates collected in the Czech Republic. Clinical material from patients hospitalised in 16 Czech hospitals in September 2004 was used to isolate K. pneumoniae strains. Strains were identified by standard identification procedures. Susceptibility of the strains to antibiotics was tested using a microdilution method. The double-disk synergy test and combination disk method were used to determine ESBL production. Molecular biology characteristics of ESBL-positive isolates were determined using genomic DNA isolation, XbaI restriction digestion and pulsed-field gel electrophoresis differentiation. The acquired restriction maps of individual isolates were compared using GelCompar II software and their relationships were determined. During the 3-week period, 483 K. pneumoniae strains causing clinically detectable diseases were isolated. Of these, 117 (24.2%) were determined to be ESBL-positive. The prevalence of ESBL-positive isolates was 38.9% in Intensive Care Units (ICUs) and 13.1% in standard wards. More than 50% of ESBL-positive isolates were treated effectively only with meropenem (98%), cefoperazone/sulbactam (61%) and amikacin (54%). Conversely, ESBL-negative isolates showed high susceptibility to all tested antibiotics (76-99%). Molecular biology analysis identified 18 clonal types containing two to six identical isolates. Seventeen clones usually contained isolates from only one hospital; isolates from two hospitals were identified only in one clone. Based on the abovementioned results, the prevalence of ESBL-positive K. pneumoniae isolates in the Czech Republic can be perceived as relatively high, especially in ICUs. Extensive spread of 'epidemic clones' within Czech hospitals and, to a limited extent, between them can be demonstrated.

Between July 1, 2002 and December 31, 2003, rectal swabs from both hospitalized patients and community subjects in the Czech Republic were taken to ascertain the prevalence of vancomycin-resistant enterococci (VRE). The swabs were used for isolating and identifying enterococci and their susceptibility to antibiotics. Vancomycin resistance phenotypes were verified by PCR detection of vanA, vanB, vanC1 and vanC2 genes. A molecular biology analysis was performed in Enterococcus faecium VanA strains. During the observed period, 2691 rectal swabs from the hospitalized patients and 6529 rectal swabs from the subjects in community setting were examined. In total, 31 VRE of hospital origin and 13 community-population strains were isolated. The prevalence of VRE in the gastrointestinal tract was 1.9% in the hospitalized patients and 0.4% in the community subjects. The prevailing strains were Enterococcus faecium VanA (61.3%) in the VRE of hospital origin and Enterococcus gallinarum VanC (46.2%) in the community VRE. Mutual comparison between the hospital and community Enterococcus faecium VanA strains showed no similarity.

This study describes the first molecular characterisation of clinical isolates of vancomycin-resistant enterococci (VRE) in the Czech Republic. Of 2647 patient isolates of Enterococcus spp. from 1997-2002, 121 (4.6%) were identified as VRE. The most common isolates were VanA+ Enterococcus faecium (78%) and VanB+ Enterococcus faecalis (10%). In addition, five VanA+ E. faecium isolates were obtained from environmental and staff sampling. Macrorestriction analysis of SmaI restriction fragment length polymorphism was performed for 54 VanA+ E. faecium clinical isolates and the five VanA+ E. faecium environmental isolates. Thirty-two unique restriction endonuclease patterns were identified, including two predominant clonal types represented by five or more isolates. Two environmental VanA+ E. faecium isolates were closely related to two patient isolates, which had an identical SmaI macrorestriction pattern. The results indicated potential survival of strains in the hospital environment and possible subsequent transmission to hospitalised patients.

Vliv selekčního tlaku antimikrobních preparátů na výskyt vankomycin-rezistentních enterokoků a možnosti jejich prevence v rámci racionální antobiotické politiky. Zavedení metodiky, umožňující rychlou a spolehlivou detekci determinant rezistence typu VanA a VanB pomocí PCR.amplifikace specifických genů kódujících rezistenci k vankomycinu.

In the period between September 2002 and May 2004, a total of 6023 rectal swabs from humans in the Czech Republic were evaluated and 821 Enterococcus spp. strains were isolated. Nine strains (1.1 %) were identified as vancomycin-resistant enterococci (VRE). Two strains were VanA Enterococcus faecium, one strain was VanB Enterococcus faecalis and six strains were VanC Enterococcus casseliflavus. In total, 527 Enterococcus spp. strains were isolated from poultry breeds of which 11 (2.1 %) were VRE. Most (54.5 %) were identified as VanA E. faecium. Cluster analysis of SmaI-generated macrorestriction patterns showed high variability in both human and animal VRE strains and no relatedness between strains from the two sources.

Sepsis is a greatly feared complication of total joint arthroplasty. One key question is how to prevent perioperative bacterial adherence, and therefore the potential for infectious complications. The objective of our study was to appraise the emerging capacity of staphylococcal survival on prosthetic materials and to analyze the in vitro effects of gentamicin and vancomycin loaded polymethylmethacrylate (PMMA) cement on bacterial adherence and growth. Hospital acquired staphylococcal strains were systematically inoculated on four orthopedic materials (ultrahigh molecular weight polyethylene, PMMA without antibiotic, commercially produced PMMA loaded with gentamicin, and manually mixed PMMA loaded with gentamicin and vancomycin). Staphylococci were identified using culture and biochemical tests. The inoculated material was allowed to incubate in a liquid broth growth media and subsequently prepared for scanning electron microscopy and bacterial growth quantification. Materials without antibiotics showed evidence of staphylococcal growth. PMMA loaded with only gentamicin grew methicillin-resistant Staphylococcus aureus. Gentamicin-vancomycin loaded PMMA completely inhibited any bacterial growth. Low-dose gentamicin-vancomycin loaded PMMA prevents staphylococcal colonization better than commercially manufactured PMMA loaded with gentamicin. We recommend this combination in high-risk procedures and revision surgeries requiring bone cement.

The diagnosis of low grade prosthetic joint infection is difficult and time consuming. Nested-PCR for universal bacterial DNA segments detection of "orthopaedic" bacteria was tested in a laboratory setting. This method is based on amplification of the 16S bacterial ribosomal RNA coding sequences. 11 species of the most frequent bacterial pathogens (Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus pyogenes, Streptococcus agalactiae, Enterococcus faecium, Enterococcus faecalis, Klebsiella pneumoniae, Escherichia coli, Proteus mirabilis, Pseudomonas aeruginosa, Serratia marcescens) involved in prosthetic joint infections were studied. All could be detected rapidly and sensitively by this method.
