The opportunistic fungal pathogen Cryptococcus neoformans is a major cause of mortality in immunocompromised individuals, resulting in more than 600,000 deaths per year. Many human fungal pathogens secrete peptidases that influence virulence, but in most cases the substrate specificity and regulation of these enzymes remains poorly understood. The paucity of such information is a roadblock to our understanding of the biological functions of peptidases and whether or not these enzymes are viable therapeutic targets. We report here an unbiased analysis of secreted peptidase activity and specificity in C. neoformans using a mass spectrometry-based substrate profiling strategy and subsequent functional investigations. Our initial studies revealed that global peptidase activity and specificity are dramatically altered by environmental conditions. To uncover the substrate preferences of individual enzymes and interrogate their biological functions, we constructed and profiled a ten-member gene deletion collection of candidate secreted peptidases. Through this deletion approach, we characterized the substrate specificity of three peptidases within the context of the C. neoformans secretome, including an enzyme known to be important for fungal entry into the brain. We selected a previously uncharacterized peptidase, which we term Major aspartyl peptidase 1 (May1), for detailed study due to its substantial contribution to extracellular proteolytic activity. Based on the preference of May1 for proteolysis between hydrophobic amino acids, we screened a focused library of aspartyl peptidase inhibitors and identified four high-affinity antagonists. Finally, we tested may1Δ strains in a mouse model of C. neoformans infection and found that strains lacking this enzyme are significantly attenuated for virulence. Our study reveals the secreted peptidase activity and specificity of an important human fungal pathogen, identifies responsible enzymes through genetic tests of their function, and demonstrates how this information can guide the development of high affinity small molecule inhibitors.

Od 2. 2. 1983 do 31. 12. 2011 bylo v transplantačním centru v Brně provedeno celkem 420 transplantací jater. Neobvyklé a závažné mykotické infekce, nezpůsobené kandidami či aspergily, jsme pozorovali celkem u pěti pacientů. Po jednom případě šlo o infekci Cladophialophora bantiana a Fonsecaea compacta. Ve třech případech byla infekce vyvolána Cryptococcus neoformans. Tři pacienti na uvedenou infekci zemřeli (3/5 = 60 %). Autoři popisují případ 44letého muže s mozkovými a plicními abscesy způsobenými černou plísní (Cladophialophora bantiana). I přes veškerou terapii antimykotickou a operační došlo k progresi infekce a následné smrti pacienta. U druhé nemocné, 54leté ženy, došlo ke vzniku recidivujících kožních infiltrátů, které byly histologicky, mikroskopicky a kultivačně vyšetřeny s průkazem Fonsecaea compacta. Ložiska byla pro recidivu opakovaně chirurgicky odstraněna, ale až zavedení antimykotické terapie vedlo k jejich definitivnímu vymizení. Pacientka je sedm let bez známek infekce. Autoři dále popisují případ 56letého muže s kožním hematogenním rozsevem mykotické infekce způsobené Cryptococcus neoformans. V tomto případě chirurgické odstranění kožních ložisek a dlouhodobá antimykotická terapie vedly k regresi infekce a stabilizaci pacienta. Ve čtvrtém případě autoři popisují případ 56letého muže po ortotopické transplantaci jater s časnou encefalitidou, bilaterální brochopneumonií, které vedly ke smrti pacienta 40. den po transplantaci. Etiologie infekce (Cryptococcus neoformans) byla prokázána post mortem mikroskopickým vyšetřením. Pátý nemocný byl 31letý muž s primární biliární cirhózou, pro kterou byla provedena transplantace jater s rekonstrukcí žlučových cest s Roux kličkou. Operace i časný pooperační průběh byly bez komplikací. Od 12. dne došlo ke ztrátě vědomí, v provedené lumbální punkci byl prokázán Cryptococcus neoformans a pacient přes zavedenou terapii za tři měsíce zemřel pod obrazem mozkového edému.
A total of 420 liver transplants were carried out at the transplant centre in Brno between 2 February 1983 and 31 December 2011. We observed an unusual and serious mycotic infection, which was not caused by candida or aspergillus, in a total of five patients. We found one case each of Cladophialophora bantiana and Fonsecaea compacta infection. In three cases the infection was caused by Cryptococcus neoformans. Three patients (3/5 = 60%) died of the aforementioned infection. The authors describe the case of a 44-year old man with abscesses of the lungs and brain, caused by black fungus (Cladophialophora bantiana). Despite the application of the full spectrum of antimycotic therapy and surgical procedures, the infection continued to progress, resulting in the death of the patient. The second patient, a 54-year old woman, had developed recidivising skin infiltrations, which were examined from a histological, microscopic and cultivatory point of view, with the presence of Fonsecaea compacta demonstrated. While repeated surgical interventions were required to remove deposits following recidivism, only the introduction of antimycotic therapy led to its complete disappearance. The patient has shown no signs of infection for seven years. The authors further describe the case of a 56-year old man with a haematogenic spread of mycotic infection of the skin caused by Cryptococcus neoformans. In this case, the surgical removal of skin deposits and long-term antimycotic therapy led to the regression of the infection and stabilisation of the patient. In the fourth case study, the authors describe the case of a 56-year old man after orthotopic liver transplantation and with early onset encephalitis, bilateral bronchial pneumonia, which led to the death of the patient on the 40th day following transplant. The etiology of the infection (Cryptococcus neoformans) was demonstrated following post mortem microscopic examination. The fifth patient was a 31-year old man with primary biliary cirrhosis, on whom a liver transplant was performed, accompanied by the reconstruction of the biliary tracts using a Roux loop. The operation and early post-operative period passed without complications. From the 12th day following surgery the patient suffered loss of consciousness and Cryptococcus neoformans was found in the lumbar puncture. Despite the introduction of therapy, the patient died of brain oedema within three months. Growth patterns of Cryptococcus neoformans submerged culture in different culture volumes, intensity of agitation and types of sealing were evaluated to better understand the physiological role of hypoxia response in this yeast. When low intensity agitation was set at high culture volumes and air exchange between the cultivation vessel and external environment was not abolished completely, the cells proliferated slowly but steadily. On the other hand, when the intensity of agitation was high but the vessel was withheld from fresh air supply, the cells first proliferated rapidly, then arrested completely and finally died. Therefore, the central strategy of C. neoformans here seems to lie in its proliferation-rate adjustment to the available oxygen levels and not in its capacity to survive under anoxia. The data support the opinion that the cultures grown under limited aeration (even though not completely withheld from fresh air supply) are much closer to the real cryptococcal life in human tissues than conventional well-aerated exponential cultures.
BACKGROUND: Cryptococcus neoformans is an obligate aerobic pathogenic yeast causing lung infection typically followed by spread to the central nervous system. During pathogenesis, it relies on well-established virulence factors. This review focuses on the emerging role of cryptococcal adaptation to hypoxia in pathogenesis. METHODS AND RESULTS: We examined the MedLine database for information on the cryptococcal hypoxia response. While several recent papers describe components of two presumable hypoxia-sensing pathways including description of their target genes, a link of this system to the hypoxic tuning of proliferation is still missing. In addition, an interpretation of this knowledge in respect to the general picture of microbial pathogenesis is lacking. CONCLUSIONS: There seems to be a striking parallel between biofilm formation in bacteria, which results in chronic dormant infection with the potential for acute outbreaks, and the dormant state of primary infection followed by secondary outbreaks in C. neoformans. We propose a hypothesis that cryptococcal response to hypoxia might be the driving force for developing a state of dormant infection which is characterized by slowed proliferation and extensive changes in transcriptome and phenotype. This state enables C. neoformans to survive in host and possibly develop life-threatening acute outbreaks later. Hence, conventional well-aerated planktonic culture is not a good in vitro model for studying the pathogenesis of infection and we advocate the development of a more adequate model. Our further conclusion is that the ability of the immune system and antifungal agents to cope with hypoxia-adapted cells is crucial for the successful eradication of cryptococcal infection.

Cryptoccocus neoformans patří mezi oportunní patogeny osob s hlubokým imunodeficitem různého původu. Do zavedení antiretrovirové terapie a antimykotika flukonazolu se vyskytoval až u 10 % osob v rozvinutém stádiu HIV infekce. V současné době onemocnění postihuje především pacienty s do té doby nediagnostikovaným, tedy neléčeným závažným imunodeficitem. V prezentovaném případě se jednalo o pacienta s dosud nezjištěnou HIV infekcí, postiženi však mohou být také pacienti v těžké indukované imunosupresi, zejména po transplantacích. Pro onemocnění je typický pozvolný nástup nespecifických obtíží. Jediným projevem onemocnění ve většině případů mohou být pouze bolesti hlavy a zvýšená teplota, a proto často dochází k oddálení diagnózy u pacientů s nezjištěným imunodeficitem. Diagnózu potvrzuje průkaz kapsulárního kryptokokového antigenu latexovou aglutinací a také kultivace. Lékem první volby je amfotericin B. Poměrně časté nežádoucí účinky bývají důvodem ke změně léčby na méně účinný flukonazol. Mezi nepříznivé prognostické faktory patří mimo jiné poruchy smyslů a poruchy vědomí, které se objevily u našeho pacienta. Prognóza onemocnění je vždy závažná, přes řádně zavedenou terapii je letalita 10 %. Možné jsou relapsy i latentní přežívání patogenu v organismu.
Cryptococcus neoformans is an opportunistic pathogen, which affects individuals who are severely immunosuppressed for any number of reasons. Before the introduction of antiretroviral therapy and antimycotic Fluconazole this pathogen had occurred in 10 % of patients with advanced HIV infection. Nowadays, this disease primarily affects patients with so far undiagnosed, thus untreated, severe immunosuppression. The patient outlined here presented with so far undiagnosed HIV infection, nevertheless, patients can be affected following severe induced immunosuppression - particularly following transplantation. The disease is typified by a gradual onset of non-specific symptoms. Often, a headache or a temperature can be the only manifestation of the disease. For this reason, a delay can frequently occur in diagnosing patients with undiagnosed immunosuppression. The diagnosis is verified by confirmation of the pathogen by latex agglutination of cryptococcal capsular antigen and by cultivation. Amphotericin B is the first-choice therapy. Relatively frequent side-effects are the reason for the change of therapy to the less effective Fluconazole. Sensory dysfunction and impaired consciousness are two of the unfavourable prognostic factors that were noticed in the presented patient. The prognosis of the disease is always severe, and in spite of properly introduced therapy, the mortality rate is 10 %. Relapses and the latent survival of the pathogen in the prostate gland are possible.

Infekcie, ktorých póvodcom je Cryptococcus neoformans, sa vyskytujú převážné u pacientov s poruchami funkcie T-lymfocytov, a najma u osob s AIDS.Autoři sa domnievajú, že touto infekciou sú ohrození tiež fudia vyššieho veku, a to aj relativné zdraví. Zaznamenali fatálnu kryptokokovú meningoencefalitídu s fulminantným priebehom u 76-ročnej pacientky bez prejavov imunodeficitu v anamnéze.
Infections caused by Cryptococcus neoformans affect mainly patients with impaired functions oř T-cells, and in particular individuals with AIDS.The authors suppose also older subjects in relatively good health to be endangered by this infection. A čase of fatal cryptococcal meningoencephalitis with fulminant coarse was detected in a 76-year female patient without evident immunodeficiency.
