BACKGROUND: Adipose lesions of nerve primarily include intra- and extraneural lipomas and lipomatosis of nerve (LN). This paper will summarize the advances that have been made in the past decade, particularly related to LN and nerve territory overgrowth that have improved our understanding of the natural history, genetic background, diagnosis, imaging features, and clinical management. METHODS AND MATERIALS: Articles about adipose lesions of nerve were reviewed from 2011, when the last comprehensive review on this topic was published. Papers reporting advances on natural history, genetic background, diagnosis, imaging features, and clinical management were screened using PubMed and Google Scholar databases and then analyzed. Case reports and small case series were included only if they reported model examples of discussed pathologies, as these types of articles were summarized in recent systematic reviews on intraneural lipomas and LN. All eligible papers were assessed by the authors, who are subject matter experts. RESULTS: The first screen revealed 404 articles. After careful evaluation, a total of 53 articles were analyzed which includes advances in diagnosis (especially imaging), classification of the lesions, the role of somatic mutations in PIK3CA in LN, and treatment approaches for all adipose lesions of the peripheral nerve. CONCLUSION: Many advances have been made in the understanding of adipose lesions of nerve in the past decade. These pathologic entities are more readily recognized as a spectrum of lesions that share common phenotypic features.

BACKGROUND: Inherited peripheral neuropathies (IPN) are the most common inherited neurological condition. It represents a highly heterogeneous group, both clinically and genetically. Targeted disease specific gene panel massively parallel sequencing (MPS) seems to be a useful tool in diagnosis of disorders with high genetic heterogeneity. METHODS: In our study, we have designed, validated and updated our own custom gene panel of all known genes associated with IPN. One hundred and ninety-eight patients have been tested so far. Only patients in whom mutations in more common causes or relevant genes have already been excluded were enrolled. Five consecutive panel designs were prepared according to recent literature search, the last one covering ninety-three genes. Each patient was tested only once. All data were evaluated with at least two different pipelines. RESULTS: In summary, causative mutation has been found in fifty-one patients (26 %). The results were inconclusive in thirty-one (16 %) patients. No variants of likely significance to IPN were found in one hundred and sixteen (58 %) patients. CONCLUSION: MPS gene panel enables testing of all known IPN causes at once with high coverage and at an affordable cost making it truly a method of choice also in IPN. Gene panel testing results in several interesting results and findings.

Inherited peripheral neuropathies are a genetically heterogeneous group of disorders characterized by distal muscle weakness and sensory loss. Mutations in genes encoding aminoacyl-tRNA synthetases have been implicated in peripheral neuropathies, suggesting that these tRNA charging enzymes are uniquely important for the peripheral nerve. Recently, a mutation in histidyl-tRNA synthetase (HARS) was identified in a single patient with a late-onset, sensory-predominant peripheral neuropathy; however, the genetic evidence was lacking, making the significance of the finding unclear. Here, we present clinical, genetic, and functional data that implicate HARS mutations in inherited peripheral neuropathies. The associated phenotypic spectrum is broad and encompasses axonal and demyelinating motor and sensory neuropathies, including four young patients presenting with pure motor axonal neuropathy. Genome-wide linkage studies in combination with whole-exome and conventional sequencing revealed four distinct and previously unreported heterozygous HARS mutations segregating with autosomal dominant peripheral neuropathy in four unrelated families (p.Thr132Ile, p.Pro134His, p.Asp175Glu and p.Asp364Tyr). All mutations cause a loss of function in yeast complementation assays, and p.Asp364Tyr is dominantly neurotoxic in a Caenorhabditis elegans model. This study demonstrates the role of HARS mutations in peripheral neuropathy and expands the genetic and clinical spectrum of aminoacyl-tRNA synthetase-related human disease.

Cíl: Cílem práce je charakterizovat klinické a elektrofyziologické nálezy dvou typů ulceromutilující dědičné neuropatie CMT2B a HSN1 u tří českých rodin s molekulárně geneticky objasněnou příčinou. Soubor: Popisujeme tři rodiny s dědičnou senzitivní neuropatií s celkem 16 postiženými. Metodika: Na základě neurologického a elektromyografického vyšetření, výskytu podobných obtíží u příbuzných byla stanovena diagnóza senzitivní, převážně axonální neuropatie. Následně bylo provedeno sekvenování genů SPTLC1 a RAB7 v rodinách A, B, C a u dalších 24 nepříbuzných pacientů s klinickým podezřením na dědičnou senzitivní neuropatii. Výsledky: V rodině B byla nálezem mutace p.C133Y v SPTLC1 genu prokázána hereditární senzitivní neuropatie typ 1 (HSN1). V rodinách A a C byla nálezem mutací p.L129F a p.V162M v genu RAB7 prokázána choroba Charcot‑Marie‑Tooth typ 2B (CMT2B). Všechny tři mutace již byly dříve popsány a fenotyp odpovídá popisu pacientů z jiných zemí. DNA vyšetření dalších 24 nepříbuzných pacientů však příčinu onemocnění neobjasnilo. Závěr: Ulceromutilující dědičné neuropatie CMT2B a HSN1 jsou v české populaci, podobně jako v jiných zemích vzácné, ale klinické projevy jsou jasně poznatelné, pokud jsou zohledněny a správně získány genealogické údaje – rodinná anamnéza. Tyto tři popsané rodiny jsou dosud jediné známé v ČR s objasněnou příčinou dědičné senzitivní neuropatie v důsledku mutací v RAB7 a SPTLC1. Objasnění příčiny ulceromutilující senzitivní neuropatie má význam nejen pro upřesnění genetické a klinické prognózy a pro cílenou genetickou prevenci, ale u pacientů s mutacemi v SPTLC1 genu možná i pro cílenou terapii se substitucí L‑serinem, která bude testována v klinické studii (Boston, USA)
Aim: The goal was to clinically and electrophysiologically characterize two types of ulceromutilating hereditary neuropathy CMT2B and HSN1 in three Czech families where molecular genetic cause was confirmed. Patients: We describe three families, overall 16 affected patients, with hereditary sensory neuropathy. Methods: The diagnosis of sensory, predominantly axonal neuropathy was done on the basis of neurological and electrophysiological examination. Sequencing of the SPTLC1 and RAB7 genes was done in families A, B, C and 24 unrelated patients with clinical suspicion for HSN. Results: Hereditary sensory neuropathy type 1 (HSN1) caused by the p.C133Y mutation in the SPTLC1 gene was confirmed in family B and the Charcot-Marie-Tooth type 2B (CMT2B) caused by p.L129F and p.V162M mutations in the RAB7 gene was confirmed in families A and C. All three mutations have been previously described. DNA examination of 24 unrelated patients did not reveal the cause of their disease. Conclusion: As in other countries, ulceromutilating hereditary neuropathies CMT2B and HSN1 are rare in the Czech population. However, clinical manifestations are clearly recognizable if correctly obtained genealogical data – family history – is properly taken into account. The three families described here are the only known families with hereditary neuropathies caused by mutations in RAB7 and SPTLC1 in the Czech Republic. Clarification of the cause of ulceromutilating sensory neuropathy is crucial for genetic and clinical prognosis, including targeted genetic prevention, but possibly also for an L-serin therapy in SPTLC1 mutation patients to be tested in a clinical study (Boston, USA).

PURPOSE: To identify the underlying molecular genetic cause in a Czech family with optic atrophy, deafness, ptosis, ophthalmoplegia, polyneuropathy and ataxia transmitted as an autosomal dominant trait. METHODS: Ophthalmological and neurological examination followed by molecular genetic analyses. RESULTS: Seven family members were clinically affected. There was a variable but progressive visual, hearing and neurological disability across the family as a whole. The majority of subjects presented with impairment of visual function and a variable degree of ptosis and/or ophthalmoplegia from the first to the third decade of life. Deafness, neuropathy and ataxia appeared later, in the third and fourth decade. Migraine, tachycardia, intention tremor, nystagmus and cervical dystonia were observed in isolated individuals. A significant overall feature was the high level of neurological disability leading to 3 of 4 members being unable to walk or stand unaided before the age of 60 years. A novel missense mutation c.1345A>C (p.Thr449Pro) in OPA1 segregating with the disease phenotype over three generations was detected. In silico analysis supported pathogenicity of the identified sequence variant. CONCLUSION: Our work expands the spectrum of mutation in OPA1, which may lead to severe multisystem neurological disorder. The molecular genetic cause of dominant optic atrophy in the Czech population is reported for the first time. We propose that regular cardiac follow-up in patients diagnosed with dominant optic atrophy and widespread neurological disease should be considered.

Identification of nuclear genetic determinants of inborn isolated defects of mitochondrial ATPase using methods of molecular genetics and cell biology and unique collection of genomic DNA, fibroblast cell lines and tissue samples from patients with nuclear genetic defects of ATPase. Analysis of biological function of deficient proteins and characterization of the mechanism downregulating biogenesis of ATPase, analysis of compensatory processes at mitochondrial and cellular levels. Článek přibližuje problematiku nervosvalových onemocnění dětského věku, poruch některé ze struktur tzv. periferního nervového systému (PNS), nebo jinak řečeno motorické jednotky. Ta začíná motoneuronem v předních rozích míšních, pokračuje axonem a přes nervosvalovou ploténku má spojení s buňkou kosterního svalu. Součástí periferního nervového systému jsou i nervová vlákna senzitivní a autonomní. Na všech těchto úrovních může dojít k lézi, buď vrozené, na genetickém podkladě (což ale neznamená, že se projeví hned při narození), a dále z mnoha dalších příčin – ischemie, zánět, úraz, toxické působení a podobně. Včasná a správná diagnóza je nezbytná ke stanovení léčebné strategie, a tam, kde je to možné, kauzální léčby, případně ke genetické konzultaci u hereditárních nemocí.
The article deals with neuromuscular diseases in childhood, i.e. disorders of some of the structures of the peripheral nervous system (PNS) or, in other words, of the motor unit. This starts with the motor neuron in the anterior horns of the spinal cord, continues with an axon, and synapses with a skeletal muscle cell at the motor end-plate. Sensory and autonomic nerve fibres are also a part of the peripheral nervous system. At all these levels, a lesion can develop that can be either congenital or of a genetic nature (which does not mean, however, that it will be manifested immediately at birth) as well as due to numerous other causes, including ischaemia, inflammation, trauma, toxic effects, and others. A timely and correct diagnosis is necessary to determine the therapeutic strategy and, whenever possible, causal

To continue the long term project of elucidation of possible causes of inherited peripheral neuropathies by the extended analysis of recently discovered candidate genes - mainly LITAF, GDAP1, PRX and NEFL in a large previously collected cohort of at least 300 Czech Charcot-Marie-Tooth disease families with already excluded main mutations - CMT1A duplication/ HNPP deletion, Cx32, MPZ and PMP22 gene mutations.
Zajistit pokračování projektu vyšetření molekulárních příčin dědičných neuropatií rozšířením vyšetřovacích možností o analýzu nově objevených kandidátních genů - zejména LITAF, GDAP1, PRX a NEFL v již shromážděném souboru nejméně 300 rodin s chorobami Charcot-Marie-Tooth s již vyloučenými nejčastějšími genovými mutacemi - CMT1A duplikace/ HNPP delece, Cx32, MPZ a PMP22.
