The International Working Group on the Diabetic Foot (IWGDF) has published evidence-based guidelines on the prevention and management of diabetic foot disease since 1999. This is the first guideline on the diagnosis and treatment of active Charcot neuro-osteoarthropathy in persons with diabetes published by the IWGDF. We followed the GRADE Methodology to devise clinical questions in the PACO (Population, Assessment, Comparison, Outcome) and PICO (Population, Intervention, Comparison, Outcome) format, conducted a systematic review of the medical literature, and developed recommendations with the rationale. The recommendations are based on the evidence from our systematic review, expert opinion when evidence was not available, and also taking into account weighing of the benefits and harms, patient preferences, feasibility and applicability, and costs related to an intervention. We here present the 2023 Guidelines on the diagnosis and treatment of active Charcot neuro-osteoarthropathy in persons with diabetes mellitus and also suggest key future topics of research.

BACKGROUND: There are uncertainties regarding the diagnostic criteria, optimal treatment methods, interventions, monitoring and determination of remission of Charcot neuro-osteoarthropathy (CNO) of the foot and ankle in people with diabetes mellitus (DM). The aims of this systematic review are to investigate the evidence for the diagnosis and subsequent treatment, to clarify the objective methods for determining remission and to evaluate the evidence for the prevention of re-activation in people with CNO, DM and intact skin. METHODS: We performed a systematic review based on clinical questions in the following categories: Diagnosis, Treatment, Identification of Remission and Prevention of Re-Activation in people with CNO, DM and intact skin. Included controlled studies were assessed for methodological quality and key data from all studies were extracted. RESULTS: We identified 37 studies for inclusion in this systematic review. Fourteen retrospective and observational studies relevant to the diagnosis of active CNO with respect to clinical examination, imaging and blood laboratory tests in patients with DM and intact skin were included. We identified 18 studies relevant to the treatment of active CNO. These studies included those focused on offloading (total contact cast, removable/non-removable knee high devices), medical treatment and surgical treatment in the setting of active CNO. Five observational studies were identified regarding the identification of remission in patients who had been treated for active CNO. We did not identify any studies that met our inclusion criteria for the prevention of re-activation in patients with DM and intact skin who had been previously treated for active CNO and were in remission. CONCLUSIONS: There is a paucity of high-quality data on the diagnosis, treatment, and prognosis of active CNO in people with DM and intact skin. Further research is warranted to address the issues surrounding this complex disease.

Diabetes mellitus významně zvyšuje riziko koronární, mozkové a periferní aterosklerózy, jejímž klinickým důsledkem je rozvoj srdečního infarktu, cévní mozkové příhody, končetinové ischemie a vyšší mortalita u osob s diabetem. Snížení rizika rozvoje kardiovaskulárních onemocnění u diabetiků je možné pouze komplexní a vytrvalou kontrolou všech rizikových faktorů aterosklerózy, jež představují nikotinová abstinence, kontrola diabetu, hypertenze a dyslipidemie a podávání léků s antiagregačním působením. U pacientů s pokročilou ischemií je nutné zvážit revaskularizaci tepen k odvrácení rizika těžkých forem onemocnění.
Diabetes markedly increases the risk of coronary, cerebral and peripheral atherosclerosis and the clinical consequences of myocardial infarction, stroke, limb ischemia, and death. Aggressive medical management directed at optimizing glucose control, achieving normal blood pressure, correcting dyslipidemia, and inhibiting platelet function reduces the likelihood of these adverse cardiovascular events. In patients with severe atherosclerosis, revascularization is often necessary to avert the risk of end‑organ damage.

Předním úkolem současné podiatrie je zvýšení povědomí o správné diagnostice i terapii syndromu diabetické nohy a o zásadách jeho prevence mezi lékaři nejrůznějších oborů. Spoluprací ošetřujících lékařů s podiatrickými ambulancemi pro diabetiky je postupně možné docílit snížení výskytu invalidizujících následků syndromu diabetické nohy – amputací. Předpokladem snížení výskytu ulcerací i jejich recidiv jsou podiatrická edukace pacientů a jejich dispenzarizace podle stupně rizika tohoto onemocnění. Včasná léčba, spojená podle potřeby i s urgentní hospitalizací, je účinná nejen u ulcerací na nohou způsobených diabetem a jeho komplikacemi, které jsou nejčastějším projevem syndromu diabetické nohy, ale i u hlubokých infekcí typu flegmóny nebo osteomyelitidy a také u postižení kostí nohy při neuropatické Charcotově osteoartropatii. Pro zkvalitnění podiatrické péče je nutné zvýšit kapacitu podiatrických ambulancí v regionech a zajistit podmínky pro jejich funkci.
The challenge of the current podiatry is to raise awareness of the correct diagnosis and therapy of diabetic foot syndrome and of its prevention among doctors of various specialties. It is possible to reduce gradually the disabling diabetic foot amputation by cooperation of physicians with foot clinics. The reduction in the incidence of ulceration and their recurrence may be provided by patient's education in foot care and by regular long‑term follow up of patients according to degree of risk. Early treatment, as necessary and urgent hospitalisation are effective in not only in the ulceration caused by diabetes and its complications, but also in cellulitis or osteomyelitis and in neuropathic Charcot osteoartrhopathy. It is necessary to increase capacity of the foot clinics in the regions and to ensure the conditions for their function for improving podiatric care.

Stanovení diagnózy u monoartritidy může být poměrně složitý proces, neboť na rozdíl od polyartritidy máme často k dispozici jen málo klinických informací a přitom diagnostická škála je velmi obšírná. Diagnostika zánětlivých revmatických onemocnění je obyčejně v kompetenci revmatologa, nicméně z praxe víme, že je nezbytné, aby základní diferenciálně diagnostická rozvaha byla provedena co nejdříve a mohla tak být co nejdříve zahájena optimální léčba. V prvním kontaktu se s tímto problémem setkávají především praktičtí lékaři a ortopedi. Monoartritidy můžeme dělit na nezánětlivé, kam řadíme nejčastěji aktivovanou osteoartrózu a traumata, a na zánětlivé, kam patří dnavá artritida, chondrokalcinóza, infekční artritida, juvenilní idiopatická artritida, spondylartritida, počínající revmatoidní artritida a mnoho dalších. Článek se dále věnuje managementu pacientů s monoartritidou, kde je k diferenciálně diagnostické rozvaze nezbytná podrobná anamnéza, pečlivé klinické vyšetření kloubu a zhodnocení charakteru artritidy. Rozvahu nám dále usnadní zobrazovací metody, artrocentéza s analýzou synoviální tekutiny a samozřejmě laboratorní vyšetření.
Making the diagnosis of monoarthritis can be quite difficult, because in contrast with polyarthritis just a few clinical data is available and wide diagnostic spectrum is present. The diagnosis of inflammatory rheumatic disease is usually in responsibility of a rheumatologist, but we know from experience that is necessary to carry out the basal differential diagnostic assessment as soon as possible to begin the optimal therapy. General practitioners and orthopaedists are usually first to face this problem. Monoarthritis can be divided into non-inflammatory arthritis where activated osteoarthritis and trauma belongs and inflammatory arthritis, which include gouty arthritis, chondrocalcinosis, infectious arthritis, juvenile idiopathic arthritis, spondylitis, incipient rheumatoid arthritis and many others. The article also focuses on the management of patients with monoarthritis where detailed history, careful clinical joint examination and the nature of arthritis assessment is necessary for differential diagnostic considerations. Our balance sheet further facilitate imaging, arthrocentesis with the analysis of synovial fluid and of course the laboratory examination.
