BACKGROUND: Acquired benign bronchoesophageal fistula (BEF) is rare and develops as a complication of other diseases, mostly of inflammatory processes and traumas of the chest. The treatment of choice is a surgical repair, which is considered definitive and leads to successful outcomes. However, incidence of recurrence after the primary repair based on limited data is up to 10% and its treatment is challenging. We report a surgical case of a patient with recurrent acquired benign BEF after primary resection and ensuing successful definitive repair with esophageal bypass surgery after temporary esophageal stenting. CASE REPORT: A 46-year-old male was referred to our department with a symptomatic left-sided bronchoesophageal fistula as a complication of severe acute necrotizing mediastinitis that originated from odontogenic abscess. Previously, several cervicotomies and bilateral thoracotomy were performed at an external medical facility to manage the acute condition. We performed resection of the fistula through re-thoracotomy. Postprocedural esophagography demonstrated a recurrence of bronchoesophageal communication. Postinflammatory adhesions excluded further repair through thoracotomy, therefore a stent was introduced in the esophagus for 12 weeks. Thereafter, an esophageal bypass surgery using a substernaly interposed gastric conduit was performed and resulted in an excellent long-term outcome. CONCLUSIONS: Esophageal bypass surgery using a substernaly interposed gastric conduit may be considered if the standard surgical repair of acquired benign bronchoesophageal fistula is not successful or feasible.

Prezentujeme kazuistiku 70leté pacientky, u které bylo diagnostikováno akutní krvácení z aortoezofageální píštěle, které bylo řešeno zavedením samoexpandibilního metalického stentu a po stabilizaci následným zavedením stentgraftu do aortálnímu oblouku. I přes úspěšné dosažení hemostázy pacientka podlehla komplikacím. Aortoezofageální píštěl je jednou z vzácných příčin krvácení z horního gastrointestinálního traktu. Je zatížena vysokou mortalitou i morbiditou. Nejčastější etiologií je hrudní aneuryzma, mezi další patří nádory jícnu, cizí těleso v jícnu, trauma či iatrogenní důvod. Jedná se o urgentní stav, který má často letální následky, zejména pro různě dlouhé bezpříznakové období, po kterém následuje masivní krvácení. Zlatým diagnostickým standardem je dnes počítačová tomografie s angiografií. Terapeutické řešení zahrnuje jednostupňový (operační řešení) nebo dvoustupňový přístup (endovaskulární terapie). Mortalita aortoezofageální píštěle je však stále vysoká a vyžaduje multioborovou spolupráci, vhodně zvolený terapeutický přístup i zvládnutí septických komplikací.
We present a case of a 70-year-old patient diagnosed with acute bleeding from an aortoesophageal fistula, which was treated by endoscopic insertion of a self-expandable stent. Although the haemostasis was successful, the patient died from complications. Aortoesophageal fistula is a rare cause of upper gastrointestinal bleeding with high mortality and morbidity. Thoracic aortic aneurysm is the most common aetiology among other causes such as oesophageal carcinoma, foreign body ingestion, trauma or iatrogenic causes. It is an urgent condition that has often lethal consequences, especially due to a symptoms-free period followed by a massive haemorrhage. Nowadays, the diagnostic standard is computed tomography with angiography. The therapeutic solution consists in either a one-step (surgery) or two-step approach (endovascular treatment). However, the mortality from of aortoesophageal fistula is still high and requires multidisciplinary team collaboration to ensure a well-chosen therapeutic approach, and management of septic complications.

BACKGROUND: Esophagopleural and bronchopleural fistulas represent a rare, but life-threatening complication after lung resections, most often after a right pneumonectomy. CASE STUDY: A 64 years old woman was indicated for right pulmectomy for local recurrence of initially stage IIB lung cancer treated by lower lobectomy. On the postoperative day 34, an esophagopleurobronchial fistula occurred. Further course required thoracostomy with closure of the bronchial stump and vacuum-assisted closure therapy and two-phase esophagectomy with 6 weeks interval to the esophageal reconstruction. Patient represents 2 years of disease-free survival with good functional results. CONCLUSION: The therapy of esophagopleural and bronchopleural fistula is long-term and complicated, requiring a multidisciplinary approach and several basic principles must be adhered to the management including treatment of infection and prevention of sepsis, local treatment of the fistula and pleural empyema, and adequate ventilation and nutritive care.

Cíl. Cílem práce je zhodnotit naše zkušenosti s léčbou benigních pištěli jícnu (pooperačních pištěli a perforací jícnu) krytými biodegradabilními stenty. Metoda. V období od listopadu 2008 do prosince 2010 jsme léčili šest mužů s pooperační pištěli jícnu v anastomóze nebo s benigní perforací jícnu implantací krytého biodegradabilního Ella-BD stentu. Průměrný věk pacientů byl 60 let (38-74 let). Pět pacientů bylo léčeno pro pooperační pištěl v anastomóze (jeden po exstirpaci jícnu, jeden po resekci divertiklu, jeden po sutuře jícnu, dva po gastrektomii). Jeden nemocný byl léčen pro perforaci jícnu komplikující léčbu ruptury jícnu kovovým stentem (k ruptuře jícnu došlo během balónkové dilatace benigní stenózy jícnu). Výsledky. Celkem bylo u šesti pacientů implantováno 9 stentů. Primární technická úspěšnost byla 100%. Klinického úspěchu (zhojení píštěle) bylo dosaženo u pěti ze šesti nemocných (83 %). K migraci stentu došlo u čtyř nemocných. U dvou nemocných byla pištěl v době migrace stentu již zhojena, reintervence tedy nebyla nutná. U dvou nemocných byl implantován další stent. Závěr. Použití biodegradabilních krytých stentů v léčbě benigních pooperačních pištěli nebo perforací jícnu je technicky možné a bezpečné. Počáteční výsledky jsou slibné, nicméně je potřeba většího počtu pacientů k zhodnocení účinnosti. Zásadní je také použití biodegradabilního materiálu pro krytí stentu.
Aim. To evaluate our experience with the treatment of postoperative anastomotic leaks and benign esophageal perforations with covered biodegradable stents. Methods. From 2008 to 2010 we treated 6 men with either an anastomotic leak or benign esophageal perforation by implanting of covered biodegradable Ella-BD stents. The average age of the patients was 60 years (range 38-74). Postoperative anastomotic leaks were treated in 5 patients (1 after esophagectomy 1 after resection of the diverticulum, 1 after suture of esophagus, 2 after gastrectomy). In one patient perforation occurred as a complication of the treatment of an esophageal rupture (which occurred during a balloon dilatation of benign stenosis) with a metallic stent. Results. Nine covered biodegradable stents were implanted in 6 patients. Primary technical success was 100%. Clinical success (leak sealing) was achieved in 5 out of the 6 patients (83%). Stent migration occurred in 4 patients. In 2 of these patients the leak had been sealed by the time of the stent migration, therefore no re-intervention was necessary. In two patients an additional stent had to be implanted. Conclusion. The use of biodegradable covered stents in the treatment of anastomotic leaks or esophageal perforations is technically feasible and safe. The initial results are promising, however, large numbers of patients will be required to evaluate the capability of these biodegradable stents in the future. The use of biodegradable material for the coverage of the stent is essential.

Retrospektivně jsme sledovali soubor konsekventně léčených nemocných s píštělemi horní části gastrointestinálního traktu, tedy píštělemi s vnitřním ústím v oblasti jícnu, žaludku a orální části tenkého střeva, hospitalizovaných na našem pracovišti (septická jednotka intenzivní péče Chirurgické kliniky, Fakultní nemocnice v Plzni) v době od 1. ledna 2006 do 31. prosince 2008. Statisticky jsme vyhodnotili výsledky naší léčby a pokusili jsme se vysledovat příčinnou souvislost mezi charakteristikou píštělí, případně léčebným postupem a mortalitou resp. morbiditou nemocných.
The authors performed a retrospective analysis in a group of consecutively treated patients with upper GIT fistules (ie. fistules with internal orifices in the esophageal, gastric and proximal small intestine region), hospitalized in the Plzen hospital Surgical Clinic Septic Intensive Care Unit, from January 1, 2006 to December 31, 2008. The author assessed the results using statistical methods and he studied any causative relationship between the characteristics of the fistules, the treatment methods and the patients morbidity/mortality rates.
