PURPOSE: Endovascular treatment (EVT) of tandem lesion (TL) in anterior circulation (AC) acute ischemic stroke (AIS) represents still a clinical challenge. We aimed to evaluate selected factors related to EVT and assess other possible predictors of good clinical outcome besides the generally known ones. METHODS: AIS patients with TL in AC treated with EVT were enrolled in the multicenter retrospective ASCENT study. A good three-month clinical outcome was scored as 0-2 points in modified Rankin Scale (mRS) and achieved recanalization using the TICI scale. Symptomatic intracerebral hemorrhage (SICH) was assessed using the SITS-MOST criteria. Logistic regression analysis was used for the assessment of possible predictors of mRS 0-2 with adjustment for potential confounders. RESULTS: In total, 300 (68.7% males, mean age 67.3 ± 10.2 years) patients with median of admission NIHSS 17 were analyzed. Recanalization (TICI 2b-3) was achieved in 290 (96.7%) patients and 176 (58.7%) had mRS 0-2. Besides the age, admission NIHSS and SICH, admission glycemia (p = 0.005, OR: 0.884) the stent patency within the first 30 days after EVT (p = 0.0003, OR: 0.219), dual antiplatelet therapy (DAPT) started within 12 h after EVT (p < 0.0001, OR: 5.006) and statin therapy started within 24 h after stenting (p < 0.0001, OR: 5.558) were found as other predictors. CONCLUSION: Admission glycemia, start of DAPT within 12 h and statin therapy within 24 h after EVT, and stent patency within the first 30 days after EVT were found as other predictors of good three-month clinical outcome in AIS patients treated with EVT for TL.

Cíl: Zjistit, jaké jsou životní zkušenosti pacientky po prodělané ischemické cévní mozkové příhodě (iCMP) v mladém věku a jaký vliv mělo toto onemocnění na kvalitu jejího dalšího života (Health Related Quality of Life, HRQoL). Metodika: Pilotní studie měla charakteristiku interpretativní fenomenologické případové studie. Sběr dat byl realizován prostřednictvím hloubkového polostrukturovaného rozhovoru se zaměřením na oblasti zkoumaného fenoménu. Přepis rozhovoru byl analyzován na základě interpretativní fenomenologické analýzy (IPA). Participantka: 45letá žena, 2 roky po prodělané iCMP, před nemocí OSVČ provozovatelka baru. V důsledku přetrvávajících následků po iCMP (únava, expresivní fatická porucha, lehká paréza PHK a deprese) je v invalidním důchodu. Výsledky: Bylo identifikováno celkem osm témat, která popisují pacientčinu zkušenost s následky iCMP. Tato témata jsme charakterizovali jako: Bylo to mimo mě – včasné a adekvátní poskytnutí zdravotní péče; Byla jsem nikdo – následky těžké afázie; Byla jsem živá, veselá, divoká – ztráta integrity; Únava – změna denního rytmu; Byla jsem všude, teď pořád doma – sociální izolace; Naučila jsem se už chodit a mluvit – přínos intenzivní RHB a logopedické péče; Úspěchy po malých krůčcích – postupný návrat do normálního života; Partner a rodina jako pomoc, podpora a motivace – nový rozměr vztahů. Závěr: I přes významné zlepšení klinického stavu po iCMP mohou být její dopady pro kvalitu života velmi výrazné. U referované mladé pacientky trvalo její zotavení řadu měsíců až let i přes intenzivní rehabilitační, logopedickou a psychologickou podporu. Dlouhodobě přetrvává únava, depresivní ladění, abulie, společenská izolace. Pro adekvátní péči o tyto pacienty je potřeba vysoce individuálního přístupu multidisciplinárních týmů.
Objective: To find out the life experiences of a patient after an ischemic stroke (IS) at a young age and what effect this disease had on the quality of her further life (Health Related Quality of Life, HRQoL). Methodology: The pilot study had the characteristics of an interpretative phenomenological case study. Data collection was carried out through an in-depth semi-structured interview focusing on the areas of the investigated phenomenon. The interview transcript was analysed based on interpretative phenomenological analysis (IPA). Participant: 45-year-old woman, two years after IS, self-employed bar operator before the illness. As a result of the lingering consequences after IS (fatigue, expressive phatic disorder, slight right arm paresis and depression), she is on disability pension. Results: A total of 8 themes were identified that describe the patient's experience with the consequences of IS. We characterized these themes as: It was beyond me – timely and adequate provision of health care; I was nobody – the consequences of severe aphasia; I was lively, cheerful, wild – loss of integrity; Fatigue – change in daily rhythm; I was everywhere, now always at home – social isolation; I have already learned to walk and talk – the benefit of intensive RHB and speech therapy care; Successes in small steps – a gradual return to normal life; Partner and family as help, support and motivation – a new dimension of relationships. Conclusion: Despite the significant improvement of the clinical condition after IS, its impact on the quality of life can be very significant. In the case of the referred young patient, her recovery took many months to years despite intensive rehabilitation, speech therapy and psychological support. Fatigue, depressive mood, abulia, social isolation persist for a long time. Adequate care for these patients requires a highly individualized approach by multidisciplinary teams.

BACKGROUND: Acute ischemic stroke (AIS) due to anterior circulation tandem lesion (TL) remains a technical and clinical challenge for endovascular treatment (EVT). Conflicting results from observational studies and missing evidence from the randomized trials led us to report a recent real-world multicenter clinical experience and evaluate possible predictors of good outcome after EVT. METHODS: We analyzed all AIS patients with TL enrolled in the prospective national study METRICS (Mechanical Thrombectomy Quality Indicators Study in Czech Stroke Centers). A good 3-month clinical outcome was scored as 0-2 points in modified Rankin Scale (mRS), achieved recanalization using the Thrombolysis In Cerebral Infarction (TICI) scale and symptomatic intracerebral hemorrhage (sICH) according to the Safe Implementation of Thrombolysis in Stroke-Monitoring Study (SITS-MOST) criteria. RESULTS: Of 1178 patients enrolled in METRICS, 194 (19.2%) (59.8% males, mean age 68.7±11.5 years) were treated for TL. They did not differ in mRS 0-2 (48.7% vs 46.7%; p=0.616), mortality (17.3% vs 22.7%; p=0.103) and sICH (4.7% vs 5.1%; p=0.809) from those with single occlusion (SO). More TL patients with prior intravenous thrombolysis (IVT) reached TICI 3 (70.3% vs 50.8%; p=0.012) and mRS 0-2 (55.4% vs 34.4%; p=0.007) than those without IVT. No difference was found in the rate of sICH (6.2% vs 1.6%; p=0.276). Multivariate logistic regression analysis showed prior IVT as a predictor of mRS 0-2 after adjustment for potential confounders (OR 3.818, 95% CI 1.614 to 9.030, p=0.002). CONCLUSION: Patients with TL did not differ from those with SO in outcomes after EVT. TL patients with prior IVT had more complete recanalization and mRS 0-2 and IVT was found to be a predictor of good outcome after EVT.

Úvod: V posledních letech celosvětově dochází k nárůstu incidence, prevalence a hospitalizací pro ischemickou CMP (iCMP) u pacientů do 65 let. To vede k nárůstu počtu jedinců, kteří žijí poměrně dlouhou dobu svého dalšího aktivního života s následky prodělané iCMP. S tím souvisí narůstající potřeba detailně zkoumat a objektivně hodnotit dopady iCMP na kvalitu života pacientů do 65 let. Cíl: Cílem práce bylo identifikovat relevantní specifické nástroje k hodnocení kvality života a dlouhodobých následků iCMP u pacientů do 65 let. Metodika: K identifikaci (vyhledávání a třídění) relevantních hodnoticích nástrojů byly použity metodiky PRISMA a Joanna Briggs Institute for scoping review. Vyhledávání bylo provedeno v elektronických databázích Web of Science, MEDLINE, PsycInfo, ScienceDirect, Scopus a ProQuest Science Database s použitím klíčových slov a Booleovských operátorů. Výsledky: V elektronických databázích bylo nalezeno 1 198 studií, z nichž bylo identifikováno celkem pět relevantních nástrojů specifických pro iCMP: Stroke Specific Quality of Life scale (SS-QoL), Stroke Impact scale 3.0 (SIS 3.0), Mapping Young persons with Stroke (MYS), Young Stroke Questionnaire (YSQ) a Quality of Life in Neurological Disorders (NeuroQoL); a tři generické nástroje: Short Form Health Survey s 36 položkami (SF 36), World Health Organization Quality of Life Brief Version (WHOQOL BREF) a EQ-5D, které byly použity pro hodnocení u pacientů do 65 let. Pouze YSQ byl validován u mladých pacientů po iCMP a jen u SS-QoL byla hodnocena jeho spolehlivost v cílové skupině mladých pacientů. Psychometrické vlastnosti vybraných hodnocených nástrojů byly velmi rozdílné. Nejsilnější důkazy pro použití k hodnocení kvality života u mladých pacientů byly zjištěny pro nástroj YSQ. Závěr: Použití dostupných specifických nástrojů k objektivnímu hodnocení kvality života po iCMP je u pacientů do 65 let dosud značně limitované. Nejsilnější důkazy pro použití k hodnocení kvality života byly zjištěny pro nástroj YSQ.
Introduction: Currently, incidence, prevalence and hospitalizations for ischemic stroke (IS) are increasing in patients under 65 years of age. This leads to a greater number of individuals living a relatively long period of their active life with sequels after IS. This results in a growing need to reliably investigate and evaluate impacts of IS on the quality of life in patients under 65 years of age. Aim: The aim of our study was to identify relevant specific tools for the evaluation of quality of life and long-term sequels in IS patients under 65 years of age. Methods: The PRISMA and Joanna Briggs Institute for scoping review methods were used for the identification (searching and sorting) of relevant evaluation tools. The searching was performed in the following electronic databases: Web of Science, MEDLINE, PsycInfo, ScienceDirect, Scopus and ProQuest Science Database using key words and Boolean operators. Results: Out of 1,198 studies found in the electronic databases, the following five relevant tools specific for IS: Stroke Specific Quality of Life scale (SS-QoL), Stroke Impact scale 3.0 (SIS 3.0), Mapping Young persons with Stroke (MYS), Young Stroke Questionnaire (YSQ), and Quality of Life in Neurological Disorders (NeuroQoL), and three generic instruments: Short Form Health Survey with 36 items (SF 36), World Health Organization Quality of Life Brief Version (WHOQOL BREF) and EQ-5D, were identified as they all were used for the evaluation in patients under 65 years of age. YSQ was the only tool validated for young IS patients and the reliability for the target group of young patients was only assessed in SS-QoL. Psychometric parameters of identified and evaluated tools differed substantially. The strongest evidence for using these in young patients to evaluate the quality of life was found for the tool YSQ. Conclusion: Using available specific tools for the objective evaluation of the quality of life after IS in patients under 65 years of age still remains very limited. The strongest evidence for its use to evaluate the quality of life was found for the tool YSQ.

Cíl: Mechanická trombektomie (MT) symptomatického uzávěru mozkové tepny u pacientů s akutní ischemickou cévní mozkovou příhodou (iCMP) se stala standardní léčbou. Cílem studie METRICS (MechanicalThrombectomy Quality Indicators Study in Czech Stroke Centers) bylo ověřit, zda centra v České republice provádějící endovaskulámí léčbu iCMP plní indikátory kvality navržené multioborovým konsenzem, a tak v mezinárodním kontextu ověřit kvalitu péče napříč naší republikou. Metodika: Národní prospektivní observační studie METRICS probíhala v České republice v roce 2019. Studie se zúčastnilo 12 z 15 center provádějících mechanickou trombektomii. Všechna technická a klinická data byla sbírána prospektivně a následně byla srovnána s doporučenými hodnotamijednotlivých parametrů multioborového konsenzu. Výsledky: V práci jsou analyzována data z 12 center, ve kterých bylo mechanickou trombektomii ošetřeno celkem 1178 pacientů (50,3% mužů) s mediánem věku 72 roky (18-98 let). Intravenóznitrombolýzou (IVT) bylo před MT léčeno 827 nemocných (70,2%) a 445 (37,8%) bylo sekundárně transponováno k MT z jiného centra. Získaná data z národní prospektivní observační studie METRICS byla hodnocena pro jednotlivá centra. Shoda výsledků péče s doporučeními navrženými multioborovým konsenzem byla velmi vysoká. Pět center splnilo parametry všech doporučení. Ostatní centra nesplnila pouze ojedinělá kritéria a ve většině případů jen hraničně. Závěr: Výsledky studie METRICS ukazují, že lze splnit doporučované hodnoty parametrů uvedených v mezinárodním multioborovém konsenzu i v podmínkách běžné klinické praxe napříč Českou republikou.
Aim: Mechanical thrombectomy (MT) of symptomatic occlusion of cerebral artery has become a standard treatment in acute ischemic stroke (IS) patients. The aim of the presented study was to evaluate if centers performing endovascular treatment of acute ischemic stroke meet the recommendations established by the international multi-society consensus. Methods: The national prospective observational multicenter study METRICS (Mechanical Thrombectomy Quality Indicators Study in Czech Stroke Centers) was conducted in Czech Republic in year 2019. Twelve of 15 centers performing mechanicaltrombectomy were participating on study. Alltechnicaland clinical data were collected prospectively and achieved results were subsequently compared with the recommendations established by international multi-society consensus. Results: In 12 centers 1178 (86%) patients (50.3% males, median of age 72 years, range 18-98 years) by MT were treated in 2019 and the collected data of these patients were analyzed. IV thrombolysis prior MT was performed in 827 (70.2%) patients and 445 (37.8%) patients were transferred secondarily for MT from primary stroke centers to dedicated comprehensive centers. Achieved data from the study METRICS were subsequently evaluated for individual participating centers. The concordance of results with the recommendations of multi-society consensus was very high. Five centers met parameters of all recommendations. Remaining centers did not meet sporadic criteria only and most parameters had borderline values. Conclusion: The results of METRICS study showed that it is possible meet the recommended values of parameters stated in the international multi-society consensus even in a real world practice across the Czech Republic.

Úvod: Hodnocení kvality života u pacientů po prodělané ischemické CMP (iCMP) je nezbytné pro objektivní posouzení následků v běžném životě a umožňuje lépe identifikovat všechny aspekty postižení a omezení. V ČR však validovaný specifický nástroj pro hodnocení kvality života u pacientů po iCMP chybí. Cíl: Validace české verze nejčastěji používaného nástroje na světě – Stroke Impact Scale verze 3.0 (SIS 3.0), která umožní její standardizované použití v české populaci. Soubor a metodika: Do validační studie byli zařazeni konsekutivní pacienti s iCMP bez kognitivního deficitu ve věku 18–80 let. Vnitřní konzistence byla zjišťována pomocí Cronbachova koeficientu alfa. Na hodnocení konstrukční a kriteriální validity SIS byly použity škály SIS 3.0, WHOQOL-BREF, National Institutes of Health Stroke Scale, modifikovaná Rankinova škála, index Barthelové a škála hodnocení úzkosti a deprese při hospitalizaci (Hospital Anxiety and Beck Depression Inventory). Výsledky: Do studie bylo zařazeno celkem 150 pacientů (58,4 % mužů, průměrný věk 57,6 ± 13,7 let) průměrně 3,8 ± 0,4 měsíce po vzniku iktu, přičemž 96,7 % z nich dosáhlo dobrého klinického výsledku (mRS 0–2). Cronbachův koeficient alfa jednotlivých domén se pohyboval v rozmezí domén od 0,750 do 0,934. Signifikantní korelace byly zjištěny mezi doménami SIS 3.0, WHOQOL-BREF a ostatními testy námi použité neuropsychologické baterie. Závěr: V prezentované studii byly ověřeny psychometrické vlastnosti a potvrzena dostatečná kriteriální a konstrukční validita české verze SIS 3.0.
Introduction: Assessment of the quality of life in patients after ischemic stroke (IS) is essential to objectively assess the consequences in everyday life and to better identify all aspects of disability and limitations. However, there is no validated specific tool for quality-of-life assessment in patients after IS in the Czech Republic. Aim: To validate the Czech version of the Stroke Impact Scale (SIS) 3.0 – the most used tool worldwide in stroke survivors, which will allow its standardized use in the Czech population. Patients and methods: Consecutive patients with IS without cognitive deficit aged 18–80 years were included in the validation study. Internal consistency was examined using the Cronbach’s alpha coefficient. The following scales were used to determine the construct and criterion validity of the Czech version of SIS 3.0: WHOQOL-BREF, the National Institutes of Health Stroke Scale, modified Rankin Scale, Barthel Index, and Hospital Anxiety and Beck Depression Inventory. Results: A total of 150 patients (58.4% men, mean age 57.6 ± 13.7 years) were enrolled in the study at a mean follow-up of 3.8 ± 0.4 months after the onset of stroke, with 96.7% achieving good functional independence (mRS 0–2). Cronbach’s alpha coefficient of particular domains ranged from 0.750 to 0.934. Significant correlations were found between SIS 3.0 domains, WHOQOL-BREF and other used tools of the neuropsychological battery. Conclusion: In the presented study, the psychometric properties were verified and adequate criteria and construct validity of the Czech version of SIS 3.0 were confirmed.

Background The benefit of intravenous thrombolysis is time dependent. It remains unclear, however, whether dramatic shortening of door-to-needle time (DNT) among different types of hospitals nationwide does not compromise safety and still improves outcome. Methods and Results Multifaceted intervention to shorten DNT was introduced at a national level, and prospectively collected data from a registry between 2004 and 2019 were analyzed. Generalized estimating equation was used to identify the association between DNT and outcomes independently from prespecified baseline variables. The primary outcome was modified Rankin score 0 to 1 at 3 months, and secondary outcomes were parenchymal hemorrhage/intracerebral hemorrhage (ICH), any ICH, and death. Of 31 316 patients treated with intravenous thrombolysis alone, 18 861 (60%) had available data: age 70±13 years, National Institutes of Health Stroke Scale at baseline (median, 8; interquartile range, 5-14), and 45% men. DNT groups 0 to 20 minutes, 21 to 40 minutes, 41 to 60 minutes, and >60 minutes had 3536 (19%), 5333 (28%), 4856 (26%), and 5136 (27%) patients. National median DNT dropped from 74 minutes in 2004 to 22 minutes in 2019. Shorter DNT had proportional benefit: it increased the odds of achieving modified Rankin score 0 to 1 and decreased the odds of parenchymal hemorrhage/ICH, any ICH, and mortality. Patients with DNT ≤20 minutes, 21 to 40 minutes, and 41 to 60 minutes as compared with DNT >60 minutes had adjusted odds ratios for modified Rankin score 0 to 1 of the following: 1.30 (95% CI, 1.12-1.51), 1.33 (95% CI, 1.15-1.54), and 1.15 (95% CI, 1.02-1.29), and for parenchymal hemorrhage/ICH: 0.57 (95% CI, 0.45-0.71), 0.76 (95% CI, 0.61-0.94), 0.83 (95% CI, 0.70-0.99), respectively. Conclusions Ultrashort initiation of thrombolysis is feasible, improves outcome, and makes treatments safer because of fewer intracerebral hemorrhages. Stroke management should be optimized to initiate thrombolysis as soon as possible optimally within 20 minutes from arrival to a hospital.

BACKGROUND AND AIMS: Fabry disease (FD) is a rare X-linked lysosomal storage disorder caused by disease-associated variants in the alpha-galactosidase A gene (GLA). FD is a known cause of stroke in younger patients. There are limited data on prevalence of FD and stroke risk in unselected stroke patients. METHODS: A prospective nationwide study including 35 (78%) of all 45 stroke centers and all consecutive stroke patients admitted during three months. Clinical data were collected in the RES-Q database. FD was diagnosed using dried blood spots in a stepwise manner: in males-enzymatic activity, globotriaosylsphingosine (lyso-Gb3) quantification, if positive followed by GLA gene sequencing; and in females GLA sequencing followed by lyso-Gb3. RESULTS: 986 consecutive patients (54% men, mean age 70 years) were included. Observed stroke type was ischemic 79%, transient ischemic attack (TIA) 14%, intracerebral hemorrhage (ICH) 7%, subarachnoid hemorrhage 1% and cerebral venous thrombosis 0.1%. Two (0.2%, 95% CI 0.02-0.7) patients had a pathogenic variant associated with the classical FD phenotype (c.1235_1236delCT and p.G325S). Another fourteen (1.4%, 95% CI 0.08-2.4) patients had a variant of GLA gene considered benign (9 with p.D313Y, one p.A143T, one p.R118C, one p.V199A, one p.R30K and one p.R38G). The index stroke in two carriers of disease-associated variant was ischemic lacunar. In 14 carriers of GLA gene variants 11 strokes were ischemic, two TIA, and one ICH. Patients with positive as compared to negative GLA gene screening were younger (mean 60±SD, min, max, vs 70±SD, min, max, P = 0.02), otherwise there were no differences in other baseline variables. CONCLUSIONS: The prevalence of FD in unselected adult patients with acute stroke is 0.2%. Both patients who had a pathogenic GLA gene variant were younger than 50 years. Our results support FD screening in patients that had a stroke event before 50 years of age.
