Východiska: Incidence karcinomu pankreatu (pancreatic ductal adenocarcinoma – PDAC) má zejména v rozvinutých zemích zvyšující se tendenci. V roce 2021 bylo celosvětově diagnostikováno 496 000 nových případů PDAC. Incidence v ČR je jedna z nejvyšších na světě a za rok 2018 bylo zachyceno 2 332 nových pacientů. Vzhledem k absenci symptomů v časných stadiích je přibližně 50 % pacientů vstupně diagnostikováno se vzdálenými metastázemi. Mortalita je nepatrně nižší než incidence a navzdory výrazným pokrokům v onkologickém výzkumu zůstává PDAC stále fatální diagnózou. Zajímavým přístupem, a to nejen u PDAC, je studium mikrobiomu. Ten je definován jako soubor všech mikroorganizmů (mikrobiota, tedy bakterie, houby, viry archea a protozoa) a jejich genomu v určitém prostředí. Za fyziologických podmínek je střevní mikrobiom v symbióze s osídleným organizmem, a udržuje tak rovnováhu metabolizmu, slizniční imunomodulaci a reguluje proces trávení. Při dysregulaci počtu či funkce střevních mikroorganizmů nastává dysbióza. Ta pak vede ke vzniku metabolických a kardiovaskulárních chorob, k poruchám nervového systému, indukci zánětů střeva či kancerogenezi. Mikrobiota mohou indukovat kancerogenezi několika způsoby, a to zejména vyvoláním zánětlivé odpovědi, snížením schopnosti imunitního systému eliminovat poškozené buňky a v neposlední řadě mohou metabolity mikrobů vést k deregulaci genomu osídleného organizmu. Tato deregulace vede k aktivaci proapoptotických a proproliferativních proteinů. Dosavadní výzkum prokazuje, že na rozvoji PDAC se může podílet právě střevní či orální mikrobiom. Jednou z nejvíce studovaných bakterií je Porphyromonas gingivalis. I u dalších bakterií, jako jsou Fusobacteria, Enterobacter, Klebsiella, Prevotella či Rothia, byla prokázána role při vzniku PDAC. Cíl: Cílem tohoto přehledového článku je poukázat na jeden z možných mechanizmů vzniku PDAC. Ten by mohl být ovlivnitelný, což může znamenat snížení incidence a zlepšení prognózy tohoto agresivního onemocnění.
Background: The incidence of pancreatic cancer (pancreatic ductal adenocarcinoma – PDAC) is increasing, especially in developed countries. In 2021, 496,000 new PDAC cases were diagnosed worldwide. In the Czech Republic, the incidence is one of the highest in the world, with 2,332 new PDAC patients diagnosed in 2018. Due to the absence of symptoms in the early stages, approximately 50% of patients are initially diagnosed with distant metastases. Mortality is slightly lower than the incidence count and, despite significant advances in cancer research, PDAC remains a fatal diagnosis. However, microbiome seems to be an interesting approach, and not only in PDAC patients. Microbiome is defined as the set of all microorganisms (microbiota, i.e. bacteria, fungi, viruses, archaea, and protozoa) and their genome in a certain environment. In a physiological setting, the gut microbiome is in symbiosis with the host organism, maintaining the balance of metabolism, mucosal immunomodulation and regulating the digestion process. When dysregulation of the number or function of intestinal microorganisms occurs, dysbiosis is developed. It may lead to metabolic and cardiovascular diseases, nervous system disorders, induction of intestinal inflammation, or carcinogenesis. Microbiota can induce carcinogenesis in multiple ways, such as by activating an inflammatory response, reducing the immune system‘s ability to eliminate damaged cells, and deregulation of the host genome by microbial metabolites. This deregulation may lead to an activation of pro-apoptotic and pro-proliferative proteins. To date, research shows that the gut or oral microbiome may be involved in the development of PDAC. One of the most studied bacteria is Porphyromonas gingivalis. Other bacteria, such as Fusobacteria, Enterobacter, Klebsiella, Prevotella, and Rothia, have also been shown to play a role in PDAC. Purpose: The aim of this review article is to point out one of the possible mechanisms of cancerogenesis in PDAC patients and its therapeutic influence to reduce the incidence and improve the prognosis of this aggressive disease.
BACKGROUND: The painless form of chronic pancreatitis is one of the rarer forms of the disease. While 80% to 90% of all chronic pancreatitis cases have abdominal pain as their clinical symptom, a smaller proportion of persons with chronic pancreatitis do not report typical pain. This form of the disease is often associated with exocrine and endocrine pancreatic insufficiency and weight loss, but the absence of pain symptoms may initially lead to misdiagnosis. METHODS: In a cohort of 257 people with chronic pancreatitis, the painless form was diagnosed in 30 individuals (11.6%), with an average age of 56 years and a predominance of men (71.4%). Thirty-eight percent were non-smokers and 47.6% of patients smoked up to 10 cigarettes per day. Alcohol intake of less than 40 g per day was reported by 61.9% of subjects. A quarter were moderately overweight, with a mean BMI of 26.5. Newly diagnosed diabetes mellitus had 25.7% of the subjects. RESULTS: A frequent finding was the demonstration of morphological changes, with calcifications found in 85,7% and dilatation of the pancreatic duct greater than 6.0 mm in 66%. A surprising finding was the presence of metabolic syndrome in 42.8% and the most frequent finding was the demonstration of decreased external pancreatic secretion (90%). CONCLUSION: Painless chronic pancreatitis is usually treated conservatively. We demonstrate a subset of 28 patients with painless chronic pancreatitis treated surgically. Most frequent indications were benign stenosis of the intrapancreatic bile duct and stenosis of the pancreatic duct. Although approximately 1 in 10 people with chronic pancreatitis present with a painless form of it, so that the form of the disease is described as rare, this does not change the fact that management of these people is still not optimal.

BACKGROUND: Through regulation of signaling pathways, microRNAs (miRNAs) can be involved in sepsis and associated organ dysfunction. The aims of this study were to track the 7-day time course of blood miRNAs in patients with sepsis treated with vancomycin, gentamicin, or a non-nephrotoxic antibiotic and miRNA associations with neutrophil gelatinase-associated lipokalin (NGAL), creatinine, procalcitonin, interleukin-6, and acute kidney injury (AKI) stage. METHODS: Of 46 adult patients, 7 were on vancomycin, 20 on gentamicin, and 19 on another antibiotic. Blood samples were collected on days 1, 4, and 7 of treatment, and miRNAs were identified using quantitative reverse transcription PCR. RESULTS: The results showed no relationship between miRNA levels and biochemical variables on day 1. By day 7 of gentamicin treatment miR-15a-5p provided good discrimination between AKI and non-AKI (area under curve, 0.828). In patients taking vancomycin, miR-155-5p and miR-192-5p positively correlated with creatinine and NGAL values, and miR-192-5p and miR-423-5p positively correlated with procalcitonin and interleukin-6 in patients treated with a non-nephrotoxic antibiotic. In patients together we found positive correlation between miR-155-5p and miR-423-5p and all biochemical markers. CONCLUSION: The results suggest that these four miRNAs may serve as diagnostic or therapeutic tool in sepsis, renal injury and nephrotoxic treatment. Virus SARS-CoV-2 je etiologickým faktorem řady chorob, které jsou WHO označeny jako onemocnění covid-19. Mezi možné oblasti, postižené infekcí covid-19, patří trávicí trakt, včetně onemocnění slinivky břišní. Mezi hlavní, resp. nejčastější symptomy covid-19 patří v 71,6 % horečka, v 68,9 % kašel a subjektivní pocit nedostatku dechu v 71,2 %. Mezi symptomy postižení trávicího traktu patří bolesti břicha, nauzea až zvracení anebo průjem. Vztah infekce a postižení slinivky břišní není v literatuře popisován jednoznačně. Častým nálezem je hyperlipazemie, event. hyperamylazemie, které jsou většinou buď zcela bez, nebo s minimálními histomorfologickými změnami, tj. změnami známými pod obrazem edematózního prosáknutí žlázy; naprosto výjimečně byla popsána nekrotická forma. Přesto při srovnání mortality osob s akutní pankreatitidou a pozitivních na covid-19 infekci, byla u této skupiny vyšší než u covid-19 negativních jedinců. Z daných výsledků je možno hovořit o vztahu covid-19 infekce a slinivky břišní ve smyslu akutního postižení žlázy. Poškození exokrinní i endokrinní funkce žlázy virem SARS-CoV-2 je multifaktoriální. Je diskutabilní, zda je postižení žlázy v přímém vztahu k přítomnosti viru, nebo zda se jedná o epifenomén. Významnou roli v indukci pankreatického poškození hrají angiotenzin konvertující enzym 2 (ACE-2) receptory, zánětlivé změny (především beta pankreatických buněk), ale také vysoká hladina IL-18 anebo virem navozená lipotoxicita či imunologická dysregulace. Pankreatopatie v éře infekce covid-19 jsou stále velmi živou tematikou a jsou intenzivně studovány.
SARS-CoV-2 is an etiological factor in several diseases that the WHO designated covid-19. Covid-19 infection may affect the digestive tract, including the pancreas. The main/most common symptoms of covid-19 include fever (71.6%), cough (68.9%) subjective feeling of shortness of breath (71.2%). The gastrointestinal symptoms include abdominal pain, nausea and vomiting and/or diarrhea. The relationship of covid-19 and pancreas affection is not clearly described. A common finding is hyperlipasemia, eventually hyperamylasemia. Both conditions are usually associated with either no or minimal histomorphological changes, i.e. changes referred to as edematous glandular infiltration. Rarely, a necrotic form was reported. Nevertheless, the mortality of patients with acute pancreatitis who were covid-19 positive was higher than in covid-19 negative individuals. The available data indicate there is a link between covid-19 infection and the pancreas presenting as an acute disorder of the gland. SARS-CoV-2 impairment of both exocrine and endocrine function of the pancreas is multifactorial. It is debatable whether the involvement of the gland is directly related to the presence of the virus or whether it is an epiphenomenon. Angiotensin-converting enzyme 2 (ACE-2) receptors, inflammatory changes (especially in pancreatic beta cells), together with high IL-18 levels, virus-induced lipotoxicity and immunological dysregulation play an important role in the induction of pancreatic damage. During the covid-19 pandemic, pancreatopathies represent a very lively topic and are being studied intensively.

BACKGROUND AND AIMS: Primary sclerosing cholangitis (PSC) is a rare cholestatic liver disease. Differential diagnostics can confuse it with immunoglobulin (Ig) G4-related sclerosing cholangitis (SC), an IgG4-related disease with clearly proven autoimmune origin. Differential diagnosis is made even more challenging because PSC with increased IgG4 levels (PSC-increased IgG4) also occurs. In order to facilitate their differential diagnosis, we reviewed recent literature regarding the etiologies, identifying characteristics, the most useful diagnostics, treatment, and the progression of these partially similar diseases. It is clear that PSC's pathogenesis differs from that of IgG4-related SC. In any differential diagnosis between PSC and PSC-increased IgG4, high IgG1 and low or normal IgG2 levels are characteristic for patients with PSC. Histological examination of the biliary tree wall in patients with IgG4-related SC typically reveals such changes as storiform fibrosis, obliterative phlebitis, and venulitis. These are absent in PSC-increased IgG4, which is characterized by a typical circular thickness in different parts of the biliary ducts. Finally, PSC is associated with inflammatory bowel disease, which is rare in IgG4-related SC, and more frequently is associated with cholangiocarcinomas and colon cancers. As distinct from IgG4-related SC, PSC is not a primary autoimmune disease.
Imunoglubulin G4 asociované nemoci tvoří skupinu chorob, které charakterizuje vysoká hladina imunoglobulinu G4 (IgG4) v krevním séru, zvýšený obsah lymfocytů a plasmatických buněk s IgG4 v parenchymu některých orgánů a storiformní fibróza. Nejčastěji postiženým orgánem je pankreas. Hovoříme o autoimunitní formě pankreatitidy, která se rozlišuje na dva typy. 1. typ s vysokým obsahem IgG4 v pankreatickém parenchymu je významně častější než 2. typ a má zásadní odlišnost, i pokud jde o přítomnost extrapankreatických postižení. V obecné rovině platí, že chronický zánět patří mezi rizikové faktory vzniku karcinomu. Chronická pankreatitida je tak akceptovaným rizikovým faktorem vzniku pankreatického karcinomu. Je otázkou, zda toto platí i o autoimunitní pankreatitidě (AIP), která má některé mediátory zánětu shodné s pankreatitidou sporadickou, a jakou roli hraje přítomnost imunoglobulinu G4. Naprostá většina prací s touto tematikou jsou kazuistická sdělení, přesto však, a to i na základě vlastních zkušeností, si dovolujeme tvrdit, že existuje vztah mezi autoimunitní formou pankreatitidy a pankreatickým karcinomem, který se objevuje obvykle v prvních dvou letech od stanovení diagnózy AIP. Významný je rovněž fakt, že u skupiny osob s AIP, která je klinickou manifestací IgG4 asociovaných onemocnění, byl nalezen dokonce vyšší výskyt nádorových onemocnění extrapankreatických, než tomu bylo v pankreatu samotném. Diferenciální diagnostika AIP od karcinomu pankreatu může být někdy problematická, protože tato onemocnění se mohou projevovat obě jako fokální pankreatické léze. IgG4 je považován za užitečný pro diagnostiku AIP, avšak hladiny IgG4 mohou být mírně zvýšené i v případě adenokarcinomu pankreatu. Hladiny IgG4 přesahující dvojnásobek horní hranice jsou u pacientů s adenokarcinomem pankreatu vzácné. Byly však zdokumentovány případy současného projevu karcinomu pankreatu a AIP, a proto ani tato možnost by neměla být opomíjena. AIP patří mezi stav, jenž vyžaduje pravidelnou dispenzarizaci, a to i z pohledu možné kancerogeneze.
Immunoglobulin G4-related diseases (IgG4-RD) are a group of diseases characterized by high serum levels of immunoglobulin G4 (IgG4), increased lymphocyte and plasma cell with IgG4 positivity in the parenchyma of some organs, and storiform fibrosis. The most frequently affected organ is the pancreas. This is an autoimmune form of pancreatitis, which can be divided into two types: Type 1, which is significantly more common than Type 2, is high in IgG4 in the pancreatic parenchyma and shows a fundamental difference in the noted presence of extra‑pancreatic disorders. In general, chronic inflammation is a risk factor in the development of carcinomas. Chronic pancreatitis is an accepted risk factor for the development of pancreatic cancer. The question is whether this also applies to autoimmune pancreatitis (AIP), which has some mediators of inflammation in common with sporadic pancreatitis, and what role the presence of IgG4 plays. The vast majority of the work on this topic consists of case reports, yet, even based on our own experience, we would like to say that there is a relationship between the autoimmune form of pancreatitis and pancreatic cancer, which usually occurs in the first two years after diagnosis of AIP. Also significant is the fact that the group of people with AIP, who is a clinical manifestation of IgG4-RD, was found to have an even higher incidence of extrapancreatic cancer than in the pancreas itself. Differentiating AIP from pancreatic cancer can sometimes be problematic since these diseases can both present as focal pancreatic lesions. IgG4 has been considered useful for AIP diagnosis, however, IgG4 levels can be slightly elevated, as in the case with pancreatic adenocarcinoma. IgG4 levels of over twice the upper limit are rare among patients with pancreatic adenocarcinoma. However, cases of simultaneous presentation of pancreatic cancer and AIP have been documented and should not be neglected. AIP is a condition where regular follow‑up is mandatory, including from the perspective of possible cancerogenesis.
