PURPOSE OF REVIEW: Oral drug absorption after bariatric surgery is likely to be altered, but the impact of different bariatric surgery procedures on individual drugs is not uniform. The aim of this article is to describe factors influencing the bioavailability of orally administered drugs after bariatric surgery and to provide readers with practical recommendations for drug dosing. We also discuss the medications that may be harmful after bariatric surgery. RECENT FINDINGS: The fundamental factors for enteral drug absorption are the production of gastric acid; the preserved length of the intestine, i.e., the size of the absorption surface and/or the preserved enterohepatic circulation; and the length of common loop where food and drugs are mixed with digestive enzymes and bile acids. Bypassing of metabolizing enzymes or efflux pumps and changes in intestinal motility can also play an important role. Significant changes of drug absorption early after the anatomic alteration may also be gradually ameliorated due to gradual intestinal adaptation. The most affected drugs are those with low or variable bioavailability and those undergoing enterohepatic circulation. Attention should also be paid to oral drug formulations, especially in the early postoperative period, when immediate-release and liquid formulations are preferred. The changes in oral bioavailability are especially clinically meaningful in patients treated with drugs possessing narrow therapeutic index (e.g., oral anticoagulants, levothyroxine, and anticonvulsants) or in acute conditions (e.g., anti-infectives); nevertheless, it may also influence the therapeutic value of chronic therapy (e.g., antidepressants. antihypertensives, antiplatelets, statins, PPIs, contraceptives, and analgesics); therapeutic effect of chronic therapy is further influenced by pharmacokinetic alterations resulting from weight loss. Therapeutic drug monitoring, periodical clinical evaluation, and adequate dose adjustments are necessary. Due to safety reasons, patients should avoid oral bisphosphonates, regular use of non-steroidal anti-inflammatory drugs, and, if possible, corticosteroids after bariatric surgery.

Obezita je závažné onemocnění, jehož incidence nadále celosvětově roste. Onemocnění asociovaná s obezitou vedou ke snížení kvality života a vyšší mortalitě (vysoký krevní tlak, dyslipidemie, diabetes mellitus 2. typu a další). Léčba obezity je multidisciplinární a komplexní proces, který vyžaduje důslednou spolupráci pacienta. Změna životního stylu je stavebním kamenem úspěšné terapie obezity. Farmakologická terapie zažívá v posledních letech renesanci a její role, jak už v mo- noterapii, tak i v kombinaci s jinými metodami, bude postupně nabývat na významu. Endoskopická terapie je rychle se vyvíjecí podobor digestivní endoskopie, která nabízí celou řadu nových metod redukujících například objem žaludku nebo bypass tenkého střeva. Metody jsou efektivní a zároveň mají nízké riziko komplikací. Chirurgická terapie je na druhou stranu vysoce účinná, nicméně je zatížena vyšším výskytem komplikací. Bariatrická chirurgie je určena především pro pacienty s pokročilým stadiem obezity s komorbiditami. Cílem této publikace je dát souhrnný přehled dosavadních endoskopických a chirurgických metod v léčbě obezity.
Obesity is a serious disease whose incidence continues to rise worldwide. Obesity-associated diseases lead to reduced quality of life and increased mortality (high blood pressure, dyslipidemia, type 2 diabetes mellitus, etc.). The treatment of obesity is a multidisciplinary and complex process that requires consistent patient cooperation. Lifestyle change is the cornerstone of successful obesity treatment. Pharmacological therapy has experienced a renaissance in recent years and its role, both in monotherapy and in combination with other methods, will gradually become more important. Endoscopic therapy is a rapidly evolving subfield of digestive endoscopy that offers a variety of new methods to reduce, for example, gastric volume or small bowel bypass. The methods are both effective and have a low risk of complications. Surgical therapy, on the other hand, is highly effective, but is burdened with a higher incidence of complications. Bariatric surgery is primarily for patients with advanced obesity with comorbidities. The aim of this publication is to give a comprehensive review of the current endoscopic and surgical methods in the treatment of obesity.

Úvod: Bariatrie se stala v dnešní době vzhledem k nárůstu obezity relativně rozšířeným odvětvím chirurgie. Je to jeden ze způsobů, jak lze dosáhnout dlouhodobějších výsledků při hubnutí obézních pacientů. Stejně jako jiné operace má bariatrie mnohá rizika včetně vzniku ileózního stavu a volvulu. Volvulus je akutní stav v chirurgii, který se nejčastěji řeší operační revizí. Kazuistika: Zde se zabýváme 58letou ženou, která před 8 lety podstoupila minigastrický bypass laparoskopickou technikou. Po primárním výkonu byl průběh bez komplikací. Pacientka zhubla asi 50 kilogramů. Na naše pracoviště byla přijata 2. den po plánované TEP kyčelního kloubu se známkami ileózního stavu a dle CT vyšetření se známkami volvulu na tenkém střevě. Byla jí indikována akutní operační revize se zrušením původní anastomózy, derotací střeva, desuflací tlustého střeva a konstrukcí nové enteroenteroanastomózy. Pooperačně došlo k pomalejší obnově pasáže, ale bez chirurgické komplikace. Závěr: Diagnostika volvulu není jednoduchá vzhledem k nespecifickým klinickým příznakům. Přitom ale právě časnost diagnózy výrazně ovlivňuje pacientovu prognózu. V této kazuistice se volvulus demaskoval až 8 let po primární bariatrické operaci. Příznaky se rozvinuly až jako následek paralytického ileu po náhradě kyčelního kloubu.
Introduction: Bariatric surgery is a widespread branch of surgery because of the increase in obesity in population. It is one way to achieve long-term weight loss effect in obese patients. Like other surgeries, bariatric surgery has many complications including ileus and volvulus in small intestine. It is an acute state in surgery and usually leads to a revision surgery. Case report: 58 years old woman who underwent mini-gastric bypass in 2014. She was admitted to our department because of manifestation of ileus on the second day after TEP of the hip joint. There was a typical sign of volvulus on the CT scan. She was operated on the same day. The reinsertion of enteroenteroanastomosis and denotation of the small intestine, desufflation of the large intestine, and reconstruction of new enteroenteroanastomosis was needed. After the surgery, the patient was without any complications. The bowel function recovery was slower postoperatively. Conclusion: Diagnosis of volvulus is not easy because of non-specific clinical symptoms. In this case report, the volvulus occurred 8 years after the primary surgery. Symptoms developed because of paralytic ileus after hip replacement.

BACKGROUND: The European Association for Endoscopic Surgery Bariatric Guidelines Group identified a gap in bariatric surgery recommendations with a structured, contextualized consideration of multiple bariatric interventions. OBJECTIVE: To provide evidence-informed, transparent and trustworthy recommendations on the use of sleeve gastrectomy, Roux-en-Y gastric bypass, adjustable gastric banding, gastric plication, biliopancreatic diversion with duodenal switch, one anastomosis gastric bypass, and single anastomosis duodeno-ileal bypass with sleeve gastrectomy in patients with severe obesity and metabolic diseases. Only laparoscopic procedures in adults were considered. METHODS: A European interdisciplinary panel including general surgeons, obesity physicians, anesthetists, a psychologist and a patient representative informed outcome importance and minimal important differences. We conducted a systematic review and frequentist fixed and random-effects network meta-analysis of randomized-controlled trials (RCTs) using the graph theory approach for each outcome. We calculated the odds ratio or the (standardized) mean differences with 95% confidence intervals for binary and continuous outcomes, respectively. We assessed the certainty of evidence using the CINeMA and GRADE methodologies. We considered the risk/benefit outcomes within a GRADE evidence to decision framework to arrive at recommendations, which were validated through an anonymous Delphi process of the panel. RESULTS: We identified 43 records reporting on 24 RCTs. Most network information surrounded sleeve gastrectomy and Roux-en-Y gastric bypass. Under consideration of the certainty of the evidence and evidence to decision parameters, we suggest sleeve gastrectomy or laparoscopic Roux-en-Y gastric bypass over adjustable gastric banding, biliopancreatic diversion with duodenal switch and gastric plication for the management of severe obesity and associated metabolic diseases. One anastomosis gastric bypass and single anastomosis duodeno-ileal bypass with sleeve gastrectomy are suggested as alternatives, although evidence on benefits and harms, and specific selection criteria is limited compared to sleeve gastrectomy and Roux-en-Y gastric bypass. The guideline, with recommendations, evidence summaries and decision aids in user friendly formats can also be accessed in MAGICapp: https://app.magicapp.org/#/guideline/Lpv2kE CONCLUSIONS: This rapid guideline provides evidence-informed, pertinent recommendations on the use of bariatric and metabolic surgery for the management of severe obesity and metabolic diseases. The guideline replaces relevant recommendations published in the EAES Bariatric Guidelines 2020.

Obstrukční spánková apnoe je nejběžnější poruchou dýchání ve spánku s vysokou prevalencí, která je spojena s nárůstem obezity. Primární terapií u většiny pacientů, zejména pak u pacientů se středně těžkou až těžkou obstrukční spánkovou apnoí, je přetlak v dýchacích cestách (positive airway pressure, PAP). Compliance k PAP však není ani zdaleka ideální a pro optimalizaci péče je nezbytný individualizovaný léčebný přístup.
Obstructive sleep apnea is the most common respiratory sleep disorder, with a high prevalence that is linked to the increase in obesity. The primary therapy for most patients, especially those with moderate to severe obstructive sleep apnea, is positive airway pressure (PAP). However, compliance with PAP is far from perfect. An individualized treatment approach that emphasizes chronic disease management that improves sleep-related health outcomes is necessary to optimize care.

AIM: We aimed to determine whether the sodium/glucose cotransporter family member SGLT3, a proposed glucose sensor, is expressed in the intestine and/or kidney, and if its expression is altered in mouse models of obesity and in humans before and after weight-loss surgery. MAIN METHODS: We used in-situ hybridization and quantitative PCR to determine whether the Sglt3 isoforms 3a and 3b were expressed in the intestine and kidney of C57, leptin-deficient ob/ob, and diabetic BTBR ob/ob mice. Western blotting and immunohistochemistry were also used to assess SGLT3 protein levels in jejunal biopsies from obese patients before and after weight-loss Roux-en-Y gastric bypass surgery (RYGB), and in lean healthy controls. KEY FINDINGS: Sglt3a/3b mRNA was detected in the small intestine (duodenum, jejunum and ileum), but not in the large intestine or kidneys of mice. Both isoforms were detected in epithelial cells (confirmed using intestinal organoids). Expression of Sglt3a/3b mRNA in duodenum and jejunum was significantly lower in ob/ob and BTBR ob/ob mice than in normal-weight littermates. Jejunal SGLT3 protein levels in aged obese patients before RYGB were lower than in lean individuals, but substantially upregulated 6 months post-RYGB. SIGNIFICANCE: Our study shows that Sglt3a/3b is expressed primarily in epithelial cells of the small intestine in mice. Furthermore, we observed an association between intestinal mRNA Sglt3a/3b expression and obesity in mice, and between jejunal SGLT3 protein levels and obesity in humans. Further studies are required to determine the possible role of SGLT3 in obesity.
