OBJECTIVE: The management of neurofibromatosis type 2 (NF2)-associated meningiomas is challenging. The role of Gamma Knife radiosurgery (GKRS) in the treatment of these tumors remains to be fully defined. In this study, the authors aimed to examine the role of GKRS in the treatment of NF2-associated meningiomas and to evaluate the outcomes and complications after treatment. METHODS: Seven international medical centers contributed data for this retrospective cohort. Tumor progression was defined as a ≥ 20% increase from the baseline value. The clinical features, treatment details, outcomes, and complications were studied. The median follow-up was 8.5 years (range 0.6-25.5 years) from the time of initial GKRS. Shared frailty Cox regression was used for analysis. RESULTS: A total of 204 meningiomas in 39 patients treated with GKRS were analyzed. Cox regression analysis showed that increasing the maximum dose (p = 0.02; HR 12.2, 95% CI 1.287-116.7) and a lower number of meningiomas at presentation (p = 0.03; HR 0.9, 95% CI 0.821-0.990) were predictive of better tumor control in both univariable and multivariable settings. Age at onset, sex, margin dose, location, and presence of neurological deficit were not predictive of tumor progression. The cumulative 10-year progression-free survival was 94.8%. Radiation-induced adverse effects were noted in 4 patients (10%); these were transient and managed medically. No post-GKRS malignant transformation was noted in 287 person-years of follow-up. CONCLUSIONS: GKRS achieved effective tumor control with a low and generally acceptable rate of complications in NF2-associated meningiomas. There did not appear to be an appreciable risk of post-GKRS-induced malignancy in patients with NF2-treated meningiomas.

Primary central nervous system (CNS) post-transplant lymphoproliferative disorder (PTLD) in childhood is rare. Twenty-five patients were retrieved from nine European Intergroup for Childhood Non-Hodgkin's Lymphoma and/or international Berlin-Frankfurt-Münster Study Group members. Types of allografts included kidney (n = 11), liver (n = 4), heart (n = 5), bowel (n = 1) and haematopoietic stem cells (n = 4). Eighteen were male, 16 ≥ 10 years old, 21 had monomorphic disease and 24 solid intracranial tumour masses. Four-year event-free and overall survival rates were 50% ± 10% and 74% ± 9% respectively. This report represents the largest paediatric series of CNS PTLD reported to date, showing favourable survival odds following systemic and intrathecal chemotherapy and rituximab administration.

Tuberous sclerosis complex (TSC) is a genetic disorder characterized by frequent noncancerous neoplasia in the brain, which can induce a range of severe neuropsychiatric symptoms in humans, resulting from out of control tissue growth. The causative spontaneous loss-of-function mutations have been also identified in rats. Herein, we studied histopathological and molecular changes in brain lesions of the Eker rat model carrying germline mutation of the tsc2 gene, predisposed to multiple neoplasias. Predominant subcortical tumors were analyzed, along with a rare form occurring within the pyriform lobe. The uniform composition of lesions supports the histochemical parity of malformations, with immunofluorescence data supporting their neuro-glial origin. Massive depletion of mature neurons and axonal loss were evident within lesions, with occasional necrotic foci implying advanced stage of pathology. Enrichment of mesenchymal-derived cell markers with hallmarks of neurogenesis and active microglia imply enhanced cell proliferation, with local immune response. The depletion of capillaries within the core was complemented by the formation of dense mesh of nascent vessels at the interface of neoplasia with healthy tissue, implying large-scale vascular remodeling. Taken as a whole, these findings present several novel features of brain tumors in Eker rat model, rendering it suitable for studies of the pathobiology and progression of primary brain tumors, with therapeutic interventions.

BACKGROUND: A major concern of patients who have stereotactic radiosurgery is the long-term risk of having a secondary intracranial malignancy or, in the case of patients with benign tumours treated with the technique, the risk of malignant transformation. The incidence of stereotactic radiosurgery-associated intracranial malignancy remains unknown; therefore, our aim was to estimate it in a population-based study to assess the long-term safety of this technique. METHODS: We did a population-based, multicentre, cohort study at five international radiosurgery centres (Na Homolce Hospital, Prague, Czech Republic [n=2655 patients]; Ruber International Hospital, Madrid, Spain [n=1080], University of Pittsburgh Medical Center, Pittsburgh, PA, USA [n=1027]; University of Virginia, Charlottesville, VA, USA [n=80]; and NYU Langone Health System, New York, NY, USA [n=63]). Eligible patients were of any age, and had Gamma Knife radiosurgery for arteriovenous malformation, trigeminal neuralgia, or benign intracranial tumours, which included vestibular or other benign schwannomas, WHO grade 1 meningiomas, pituitary adenomas, and haemangioblastoma. Patients were excluded if they had previously had radiotherapy or did not have a minimum follow-up time of 5 years. The primary objective of the study was to estimate the incidence of stereotactic radiosurgery-associated intracranial malignancy, including malignant transformation of a benign lesion or development of radiation-associated secondary intracranial cancer, defined as within the 2 Gy isodose line. Estimates of age-adjusted incidence of primary CNS malignancies in the USA and European countries were retrieved from the Central Brain Tumor Registry of the United States (CBTRUS) and the International Agency for Research on Cancer (IARC) Global Cancer statistics. FINDINGS: Of 14 168 patients who had Gamma Knife stereotactic radiosurgery between Aug 14, 1987, and Dec 31, 2011, in the five contributing centres, 4905 patients were eligible for the analysis (had a minimum follow-up of 5 years and no history of previous radiation therapy). Diagnostic entities included vestibular schwannomas (1011 [20·6%] of 4905 patients), meningiomas (1490 [30·4%]), arteriovenous malformations (1089 [22·2%]), trigeminal neuralgia (565 [11·5%]), pituitary adenomas (641 [13·1%]), haemangioblastoma (29 [0·6%]), and other schwannomas (80 [1·6%]). With a median follow-up of 8·1 years (IQR 6·0-10·6), two (0·0006%) of 3251 patients with benign tumours were diagnosed with suspected malignant transformation and one (0·0002%) of 4905 patients was considered a case of radiosurgery-associated intracranial malignancy, resulting in an incidence of 6·87 per 100 000 patient-years (95% CI 1·15-22·71) for malignant transformation and 2·26 per 100 000 patient-years (0·11-11·17) for radiosurgery-associated intracranial malignancy. Two (0·0004%) of 4905 patients developed intracranial malignancies, which were judged unrelated to the radiation field. Overall incidence of radiosurgery-associated malignancy was 6·80 per 100 000 patients-years (95% CI 1·73-18·50), or a cumulative incidence of 0·00045% over 10 years (95% CI 0·00-0·0034). The overall incidence of 6·8 per 100 000, which includes institutions from Europe and the USA, after stereotactic radiosurgery was found to be similar to the risk of developing a malignant CNS tumour in the general population of the USA and some European countries as estimated by the CBTRUS and IARC data, respectively. INTERPRETATION: These data show that the estimated risk of an intracranial secondary malignancy or malignant transformation of a benign tumour in patients treated with stereotactic radiosurgery remains low at long-term follow-up, and is similar to the risk of the general population to have a primary CNS tumour. Although prospective cohort studies with longer follow-up are warranted to support the results of this study, the available evidence suggests the long-term safety of stereotactic radiosurgery and could support physicians counselling patients on Gamma Knife stereotactic radiosurgery. Gliomy představují heterogenní skupinu primárních mozkových nádorů, jejichž prognóza a léčba se mohou významně lišit. WHO klasifikace je založena na morfologických kritériích, což se ukazuje jako nedostatečné. Nicméně v poslední době se do popředí stále více dostávají molekulárně genetické markery, které ovlivňují prognózu, terapeutickou účinnost a celkové přežití daleko více než samotná histologická kritéria. Především mutace IDH1/2 a kodelece 1p/19q představují v současné době nejvýznamnější molekulární znaky pro stanovení prognózy u gliomů nižších stupňů. U anaplastických astrocytomů hraje významnou roli v prognóze exprese ATRX, u oligodendrogliomů je to vedle kodelece 1p/19q mutace promotoru TERT. Glioblastomy i nadále představují terapeuticky obtížně ovlivnitelné onemocnění, hlavním markerem prognózy zůstává metylace promotoru MGMT a případně mutace EGFRvIII. V nedávné době byla odhalena i role jednobodových polymorfizmů –SNP. Jedná se o záměny bazí, které se vyskytují i ve zdravé populaci. V současné době známe více takovýchto polymorfních úseků, u kterých byl vysledován vztah k predispozici nebo prognóze pacientů s gliomy.
Gliomas represent a heterogenous group of primary brain tumors that can significantly vary in prognosis and treatment. The WHO Brain Tumors classification is based on morphological criteria that seem inadequate as they do not reflect new molecular markers. These new markers affect prognosis, overall survival and treatment more than histological diagnosis. IDH1/2 mutation and 1p/19q codeletion are the most important molecular markers predicting patient prognosis in lower grade gliomas and secondary glioblastomas. IDH1/2 mutation is a marker of better prognosis than wild type IDH. Similarly, ATRX plays an important role in anaplastic astrocytomas and loss of ATRX expression positively affects treatment response in these tumors. Clinical studies suggest that 1p/19q codeletion is the most relevant prognostic marker in oligodendroglial tumors, probably because of tumor chemosensitivity. Besides 1p/19q codeletion, TERT promoter mutation is another important factor in overall survival. Patients with gliomas carrying combined IDH1/2 mutation and 1p/19q codeletion have the best prognosis compared to those with wild type IDH. Glioblastomas are highly malignant glial tumors. Despite the progress in understanding of tumor formation and development, they are still difficult to treat. Nevertheless, MGMT promoter methylation in GBM is the most important predictor of good treatment response. A specific EGFRvIII mutation is another potential treatment target. Recently, single nucleotide polymorphisms (SNP) were found as another, in this case inherited, factor that can have an impact on a patient´s prognosis and treatment response. In particular, rs55705857 in anaplastic oligodendroglioma G allele carriers have much better prognosis in comparison to those who carry A allele. These new findings confirm that the prognosis is affected by multiple factors, including inherited predisposition.

Komplex tuberózní sklerózy je autosomálně dominantně dědičné onemocnění vyskytující se s prevalencí jednoho nemocného na 10 000 osob. Projevuje se tvorbou nezhoubných tumorů typu hamartomů potenciálně v jakémkoli orgánu. Nejčastěji trpí nemocní postižením mozku, kůže a ledvin. Patofyziologickým podkladem tohoto děje je porucha regulace buněčného dělení, způsobená mutacemi v tumor supresorových genech TSC-1 a TSC-2, kódujících proteiny hamartin a tuberin, jejichž chybné produkty vedou k nedostatečné inhibici mTOR kinázy, centrální křižovatky buněčného růstu a dělení. Cílem tohoto přehledového článku je seznámit čtenáře se základními poznatky o onemocnění s důrazem na problematiku renálního postižení.
Tuberous sclerosis complex (TSC) is an autosomal dominant inherited disorder with prevalence of 1 : 10 000 characterized by formation of benign tumours in virtually any organ system. Brain, skin and kidneys are the most affected organs. The disease is caused by mutations in tumour suppressor genes either TSC-1 or TSC-2 leading to dysfunction of their protein transcripts – hamartin and tuberin, respectively. Both of them form a heterodimer with the ability to inhibit activation of mTOR kinase, which acts as the main regulator of the cell proliferation. The aim of this article is to summarize known facts about TSC with emphasis on renal disease and treatment possibilities.
