The article discusses issues associated with the manifestations of aggressive behavior in an individual diagnosed with moderate intellectual disability and behavioral disorders (according to ICD-10; F7; F711-moderate intellectual disability, significant impairment of behavior requiring attention or treatment). In the research survey, we focused on a client corresponding with relevant features. The research was carried out at the beginning of hospitalization, ongoing hospitalization, and the end of hospitalization, followed by a recommendation to limit the legal capacity of the client and his placement in a residential care home. The case study points out individual approaches to special education and psychology and outlines the key steps in the cooperation of selected helping professions suggesting conclusions and recommendations for practice regarding these selected issues. Upon the termination of our investigation, there was a rapid deterioration of the client being admitted to the intensive care unit with a follow-up treatment in a long-term intensive care unit.

The purpose of the study was to assess factor structure of Bosnian version of Behaviour Rating Inventory of Executive Function (BRIEF). This instrument is a rating scale used for assessment of executive functions in children and adults of diff erent ages and sample category. Executive functions are crucial for early development of cognitive and social capacities, which are very important for children with intellectual disabilities and very often serve as a basis of their future integration into society. However, benefi ts of well-developed executive functions, its relationships and eff ects to diff erent aspects of life in this population, have started to be investigated relatively recently. Therefore, beside increasing knowledge about executive functions in children with intellectual dis ability, the study aimed to test feasibility and stability of BRIEF in this population with future goal of its application in schools for development of educational plans for children of this population. The sample consisted of 104 children with intellectual disability (62 boys and 42 girls) from 7 to 18 years of age. There were 49 children with mild and 55 with moderate intellectual disability. The BRIEF – teacher version was completed by 15 special education teachers of children participating in the study. The goal was to examine factor struc ture of 8 BRIEF scales: inhibit, shift , emotional control, initiate, working memory, organization of materials, plan/organize and monitor. Teachers were not considered to be participants, but only supporters of its implementation. The study confi rmed original two factor structure of the BRIEF with 80.6 % of total variance explained. It has shown that BRIEF is feasible and reliable in application with this population and it can be used for school assessment as a basis for educational planning and development of executive functions.

Porucha autistického spektra je závažné pervazivní postižení, které ovlivňuje všechny klíčové oblasti vývoje dítěte. Projevy bývají v raném věku diagnosticky nespecifické, působení poruchy na chování dítěte začíná být patrné často až okolo 12 měsíců věku. V 18 měsících, kdy děti běžně využívají dovednosti sdílené pozornosti, sledují vzdálený ukázaný objekt, pochlubí se a počkají si na zrakový kontakt s dospělým, reagují na jméno, perfektně a s oblibou imitují a mají předstíranou hru "jako", je rozdíl mezi běžným vývojem a vývojem dítěte vykazujícím rizikové faktory již diagnosticky významný. Včasnost záchytu rizika této poruchy je důležitým kritériem pro možné výsledky rehabilitační péče. Pravidelná a dobře mířená stimulace dovedností pro autismus směrodatných dokáže propojit oblasti mozku, které by bez řízené stimulace pracovaly jen omezeně nebo by nebyly aktivovány vůbec. U PAS je třeba vytvářet propojení vycházející ze spontaneity dítěte, podporovat přirozenou frekvenci a kvalitu očního kontaktu, probouzet zájem o lidskou řeč (předává sociální informace stejně jako výrazy tváře). Intervence, postavená na posilování pozitivních interakcí rodič-dítě (rodič hlavní terapeut), může optimalizovat vývoj dítěte, změnit jeho chování a vnímání, pomáhat aktivaci zrcadlových neuronů (zodpovědných za schopnost uzavírání děje do jednoho celku: "gestalt" designu), rozvoji exekutivních funkcí a zlepšení schopnosti centrální koherence. Takto se dá terapeuticky působit i u dětí starších 3 let, ale u těch se nová propojení tvoří na pozadí starých vzorců chování, které je nutné reedukovat, přepsat pamětní mapu mozku. U starších dětí je efektivita rehabilitace nízká, je třeba dlouhodobé intenzivní působení a jen obtížně se daří nové chování stabilizovat a dovednosti fixovat. Odborná intervence započatá u dětí raného věku, optimálně do 2 let, může optimalizovat vývoj dítěte v mnohem širším měřítku prostřednictvím stimulace klíčových dovedností, které aktivují důležité psychické procesy. Výsledky pilotní longitudinální studie intervenční metody O.T.A. u 16 dětí ukazují, že nízkonákladová terapie za využití VTI může být účinná.
Autism spectrum disorder is a severe and pervasive disability that affects all key areas of child development. Symptoms are diagnostically nonspecific at an early age, the effects of disorder on a child´s behavioral begin to be apparent often around 12 months of age. At the 18-months, when children commonly use the skills of joint attention, they follow shown distant object, boast and wait for visual contact with the adult, respond to the name, they are perfectly imitating and engage in pretend play, the difference between the typical development and the development of a child exhibiting risk factors is already diagnostically significant. Early detection of this disorder is an important criterion for possible outcomes of rehabilitation care. Regular and well-targeted stimulation of skills decisive for autism can connect areas of the brain that would work only limited or were not activated at all. It is important to create a connection based on the spontaneity of the child with ASD, to support the natural frequency and quality of eye contact, to raise interest in human speech (which transmits social information as well as facial expressions). Intervention, based on positive parent-child interactions (parent as main therapist), can optimize child development, changing the behavior and perception of the child. Helps activate mirror neurons (responsible for the ability of an individual to close the story into a whole: "gestalt" design), the development of executive functions and improving the central coherence capability. This way is therapeutically effective in children older than 3 years, but in the case of these new connections, they are based on old patterns of behavior, which must be reeducated, rewritten a memory map of the brain. The effectiveness of rehabilitation is low in the elderly children, long-term intense care is needed and it is difficult to stabilize new behavior and fixing skills. Professional intervention initiated in early-age children, optimally within 2 years, can optimize the development of the child on a much wider scale by stimulating key skills that trigger important psychological processes. The results of a longitudinal pilot study of the intervention method O.T.A. in 16 children show that low-cost therapy using VIG can be effective.

This article, an output of the 2016 International Summit on Intellectual Disability and Dementia, examines familial caregiving situations within the context of a support-staging model for adults with intellectual disability (ID) affected by dementia. Seven narratives offer context to this support-staging model to interpret situations experienced by caregivers. The multidimensional model has two fundamental aspects: identifying the role and nature of caregiving as either primary (direct) or secondary (supportive); and defining how caregiving is influenced by stage of dementia. We propose staging can affect caregiving via different expressions: (1) the "diagnostic phase," (2) the "explorative phase," (3) the "adaptive phase," and (4) the "closure phase." The international narratives illustrate direct and indirect caregiving with commonality being extent of caregiver involvement and attention to the needs of an adult with ID. We conclude that the model is the first to empirically formalize the variability of caregiving within families of people with ID that is distinct from other caregiving groups, and that many of these caregivers have idiosyncratic needs. A support-staging model that recognizes the changing roles and demands of carers of people with ID and dementia can be useful in constructing research, defining family-based support services, and setting public policy.

People with intellectual disabilities are exposed to particular challenges within the healthcare system. In particular, elderly people with intellectual disabilities have special needs and require physical activity to prevent age-related diseases. Health Literacy could help people with intellectual disabilities to take participative health-related decisions. To be able to generate knowledge in this area, a literature search was carried out. For this the databases PubMed, Scopus, ERIC, CINAHL, PsycINFO, and Web of Science were searched. The research shows a research gap in the field of health literacy among people with intellectual disabilities. Nevertheless, three categories have been systematized: Barriers for people with intellectual disabilities, health literacy promotion for people with intellectual disabilities, and studies concerning the concept of health literacy for people with intellectual disabilities. The studies found emphasize the importance of a target group specific health literacy concept for people with intellectual disabilities and a need to involve health professionals and adequate communication. In addition, conceptualisation considerations should take into account the specific skills and social context factors of people with intellectual disabilities. Furthermore, the results show that there is an urgent need for a target group-specific definition and measuring instruments.

Cíl: Cílem výzkumné studie bylo zjistit, zda lze využít robotickou rehabilitaci chůze k pozitivnímu ovlivnění psychosomatických a sociálních potíží dětí s poruchou na úrovni lehké mentální retardace. V případě mentální retardace a léčby souvisejících kauzálních neurologických poruch a onemocnění patří k aktuálním trendům stimulace a případný rozvoj nepostižených nebo zachovaných schopností jedince a zaměření rehabilitace na integraci jedince do společnosti. K výzkumné studii nás inspirovaly výsledky studií, které se týkají účinnosti robotické rehabilitace chůze v případě dětské mozkové obrny, která je často s mentální retardací spojena. Soubor a metodika: Šetřený soubor o celkovém počtu 65 pacientů byl rozdělen do tří skupin. Vyšetřeno bylo 24 dětí s dětskou mozkovou obrnou, 21 s teratogenní etiologií a 20 s postnatální etiologií u lehké mentální retardace. Sledováno bylo 42 chlapců a 23 dívek ve věku 3–6 let na začátku léčby a po 6 týdnech. Sledovány byly psychosomatické indikátory. Jako metodologická východiska byly použity materiály Mezinárodní klasifikace funkčních schopností (MKF) a Hodnocení funkční nezávislosti (FIM). Výsledky: Statisticky významný pozitivní rozdíl byl zaznamenán u somatických indikátorů v případě dětské mozkové obrny (p < 0,05). V případě teratogenní etiologie bylo zaznamenáno, ač statisticky nevýznamné, ale také zlepšení u somatických ukazatelů. O pozitivním posunu u všech sledovaných skupin můžeme hovořit v případě sociální adaptability. V případě dětí s dětskou mozkovou obrnu bylo zjištění opět statisticky významné (p < 0,05). Závěr: Výsledky šetření ukazují, že robotická terapie chůzí může být vhodnou rehabilitační i podpůrnou metodou také u mentálně retardovaných osob. Při jejím využití můžeme doporučit zohlednění původní etiologii poruchy. Jednoznačně lze doporučit preferenci této metody u mentálně retardovaných osob s dětskou mozkovou obrnou po hypoxii.
Aim: The aim of study was to find out the effect of robotic gait rehabilitation on psychosomatic indicators at people with different etiology of mental retardation. A stimulation and eventual development of unaffected or preserved abilities of individuals belong among current trends in the case of mental retardation and the treatment to related causal neurological disorders and diseases. The rehabilitation focused on environmental factors with aim to integrate individual into society is another current trend. For a decision to realize this research study we have been inspired by results of numerous studies, reported, for example, about the results of influence of robotic rehabilitation on psychosomatically health in the case of cerebral palsy. Material and methods: A total of 65 children in age from 3 to 6 years (42 boys, 23 girls) were observed in the beginning of rehabilitation and then after 6 weeks. Investigated patients were selected into three groups. 21 patients were with cerebral palsy, 21 with teratogenic and 20 with postnatal etiology. Psychosomatic indicators were monitored. The International Classification of Functioning, Disability and Health (ICF), and Functional Independence Measure were used as a methodological background. Results: We observed statistically significant positive differences in somatic indicators in the cause of cerebral palsy. In the case of teratogenic etiology was observed, although not statistically significant, but also an evident improvement of somatic indicators. In the case of all observed groups of patients we can talk about positive improvement in social adaptability. Statistically significant (p < 0.05) were findings again in case of cerebral palsy. Conclusions: Results of the research indicate that the robotic gait rehabilitation may be usable as a rehabilitative and supportive method also in the treatment to mentally retarded persons. In the case of its use it is appropriate to take into account of the origin etiology of the disorder. Definitely we can recommend a preference for this method for mentally retarded persons with cerebral palsy after hypoxia.

Kazuistika přináší poznatky z praxe o příznacích autizmu v dětském věku a ukazuje na nezbytnost medikovat tyto pacienty zavedeným způsobem léčby, a tím poskytnout pomocnou ruku rodičům v péči o takto nemocné dítě. Uvádí stručný přehled projevů a možnosti léčby u desetileté pacientky s dětským autizmem a přidruženou mentální retardací.
The case report presents knowledge from the practice of the symptoms of autism in childhood and highlights the need to medicate these patients with an established method of treatment, thus giving the parents a helping hand in caring for a child with this condition. A brief overview is provided of the manifestations and treatment options in a 10-year-old female patient with childhood autism and associated mental retardation.

Academic self-concept could significantly affect academic achievement and self-confidence in children with epilepsy. However, limited attention has been devoted to determining factors influencing academic self-concept of children with epilepsy. We aimed to analyze potentially significant variables (gender, frequency of seizures, duration of epilepsy, intellectual disability, learning disability and attention deficit hyperactivity disorder) in relation to academic self-concept in children with epilepsy and to additional domains of their quality of life. The study group consisted of 182 children and adolescents aged 9-14 years who completed the SPAS (Student's Perception of Ability Scale) questionnaire determining their academic self-concept and the modified Czech version of the CHEQOL-25 (Health-Related Quality of Life Measure for Children with Epilepsy) questionnaire evaluating their health-related quality of life. Using regression analysis, we identified learning disability as a key predictor for academic-self concept of children with epilepsy. While children with epilepsy and with no learning disability exhibited results comparable to children without epilepsy, participants with epilepsy and some learning disability scored significantly lower in almost all domains of academic self-concept. We moreover found that children with epilepsy and learning disability have significantly lower quality of life in intrapersonal and interpersonal domains. In contrast to children with epilepsy and with no learning disability, these participants have practically no correlation between their quality of life and academic self-concept. Our findings suggest that considerable attention should be paid to children having both epilepsy and learning disability. It should comprise services of specialized counselors and teaching assistants with an appropriate knowledge of epilepsy and ability to empathize with these children as well as educational interventions focused on their teachers and classmates.
