Hypothalamic Adult Neurogenesis (hAN) has been implicated in regulating energy homeostasis. Adult-generated neurons and adult Neural Stem Cells (aNSCs) in the hypothalamus control food intake and body weight. Conversely, diet-induced obesity (DIO) by high fat diets (HFD) exerts adverse influence on hAN. However, the effects of anti-obesity compounds on hAN are not known. To address this, we administered a lipidized analogue of an anti-obesity neuropeptide, Prolactin Releasing Peptide (PrRP), so-called LiPR, to mice. In the HFD context, LiPR rescued the survival of adult-born hypothalamic neurons and increased the number of aNSCs by reducing their activation. LiPR also rescued the reduction of immature hippocampal neurons and modulated calcium dynamics in iPSC-derived human neurons. In addition, some of these neurogenic effects were exerted by another anti-obesity compound, Liraglutide. These results show for the first time that anti-obesity neuropeptides influence adult neurogenesis and suggest that the neurogenic process can serve as a target of anti-obesity pharmacotherapy.
BACKGROUND: Patients with heart failure (HF) with preserved ejection fraction (HFpEF) and obesity experience a high burden of symptoms and functional impairment, and a poor quality of life. In the STEP-HFpEF trial (Research Study to Investigate How Well Semaglutide Works in People Living With Heart Failure and Obesity), once-weekly semaglutide 2.4 mg improved symptoms, physical limitations, and exercise function, and reduced inflammation and body weight. This prespecified analysis investigated the effects of semaglutide on the primary and confirmatory secondary end points across the range of the Kansas City Cardiomyopathy Questionnaire (KCCQ) scores at baseline and on all key summary and individual KCCQ domains. METHODS: STEP-HFpEF randomly assigned 529 participants with symptomatic HF, an ejection fraction of ≥45%, and a body mass index of ≥30 kg/m2 to once-weekly semaglutide 2.4 mg or placebo for 52 weeks. Dual primary end points change in KCCQ-Clinical Summary Score (CSS) and body weight. Confirmatory secondary end points included change in 6-minute walk distance, a hierarchical composite end point (death, HF events, and change in KCCQ-CSS and 6-minute walk distance) and change in C-reactive protein. Patients were stratified by KCCQ-CSS tertiles at baseline. Semaglutide effects on the primary, confirmatory secondary, and select exploratory end points (N-terminal pro-brain natriuretic peptide) were examined across these subgroups. Semaglutide effects on additional KCCQ domains (Total Symptom Score [including symptom burden and frequency], Physical Limitations Score, Social Limitations Score, Quality of Life Score, and Overall Summary Score) were also evaluated. RESULTS: Baseline median KCCQ-CSS across tertiles was 37, 59, and 77 points, respectively. Semaglutide consistently improved primary end points across KCCQ tertiles 1 to 3 (estimated treatment differences [95% CI]: for KCCQ-CSS, 10.7 [5.4 to 16.1], 8.1 [2.7 to 13.4], and 4.6 [-0.6 to 9.9] points; for body weight, -11 [-13.2 to -8.8], -9.4 [-11.5 to -7.2], and -11.8 [-14.0 to -9.6], respectively; Pinteraction=0.28 and 0.29, respectively); the same was observed for confirmatory secondary and exploratory end points (Pinteraction>0.1 for all). Semaglutide-treated patients experienced improvements in all key KCCQ domains (estimated treatment differences, 6.7-9.6 points across domains; P≤0.001 for all). Greater proportion of semaglutide-treated versus placebo-treated patients experienced at least 5-, 10-, 15-, and 20-point improvements in all KCCQ domains (odds ratios, 1.6-2.9 across domains; P<0.05 for all). CONCLUSIONS: In patients with HFpEF and obesity, semaglutide produced large improvements in HF-related symptoms, physical limitations, exercise function, inflammation, body weight, and N-terminal pro-brain natriuretic peptide, regardless of baseline health status. Tirzepatid – duální agonista receptorů pro glukagon-like peptid-1 (GIP) a glukózo-dependentní inzulinotropní polypeptid (GIP) je účinným antidiabetikem s významným efektem na redukci hmotnosti, u kterého je předpokládaný potenciál ovlivnit i kardiovaskulární riziko pacientů. Tento přehled se věnuje jeho vlivu na lipidové spektrum, srovnává jej s účinky selektivních agonistů GLP-1 receptoru a poskytuje pohled na vývoj dalších inkretinových agonistů a jejich metabolické působení.
Tirzepatide - a dual agonist of receptors for glucagon-like peptide-1 (GIP) and glucose-dependent insulinotropic polypeptide (GIP) is an effective antidiabetic drug associated with significant weight loss, which is expected to have the potential to influence the cardiovascular risk of patients. This review focuses on its effects on the lipid spectrum, compares it to the effects of selective GLP-1 receptor agonists, and provides insight into the development of other incretin agonists and their metabolic actions.

Nevelký nebo dočasný hmotnostní úbytek, kterého se dosahovalo s většinou antiobezitik, která byla v minulosti k dispozici, nesplnil očekávání jak pacientů, tak lékařů. Kombinované léky a analoga gastrointestinálních hormonů představují nový, účinnější a bezpečnější přístup k léčbě obezity a s ní spojených zdravotních rizik. Analoga intestinálních hormonů, jako je např. analog glucagon like peptidu-1 (GLP-1) liraglutid, byla zprvu úspěšně použita v léčbě diabetu 2. typu a až později v léčbě obezity. Článek informuje o klinických studiích 3. fáze s novými analogy intestinálních hormonů, o agonistovi GLP-1 semaglutidu a o duálním agonistovi intestinálních hormonů tirzepatidu; jeho jedna molekula cílí na receptory dvou střevních hormonů GLP-1 a GIP (glucose-dependent insulinotropic polypeptide). Jak semaglutid, tak tirzepatid se aplikují podkožně jednou týdně. Hmotnostní redukce docílená s těmito léky, zvláště s tirzepatidem, je srovnatelná s tou, kterou pozorujeme po méně invazivních bariatrických výkonech a je provázena významným zlepšením kardiometabolických rizik a příznivým bezpečnostním profilem. Gastrointestinální nežádoucí účinky po těchto lécích jsou obvykle přechodného charakteru a jsou mírné až střední závažnosti. Semaglutid byl již schválen k dlouhodobé léčbě obezity Evropskou lékovou agenturou (EMA), zatímco schválení tirzepatidu se očekává. Klinické studie s orálním semaglutidem, trojitým inkretinovým agonistou retatrutidem, nepeptidickým orálním agonistou GLP-1 orforglipronem a kombinací cagrilintidu se semaglutidem přinesla rovněž nadějné výsledky v redukčním režimu. Na druhé straně setmelanotid je lék, který působí na melanokortinový receptor 4 a je vyhrazen pro léčbu velmi vzácných monogenních onemocnění, která se projevují těžkou obezitou manifestující se v raném dětství, např. při deficitu proopiomelanokortinu nebo leptinového receptoru. Individualizovaná léčba antiobezitiky by měla být integrální součástí komplexní léčby obezity zahrnující dietu, pohybovou aktivitu a kognitivně behaviorální intervenci. Řádně vyškolení lékaři by měli být garantem úspěšnosti léčby antiobezitiky. Navíc, ministerstvo zdravotnictví a zdravotní pojišťovny by měly vzít v úvahu, že cena nových antiobezitik zabraňuje této účinné léčbě u většiny indikovaných pacientů, a proto by měla být přijata opatření k zajištění patřičné dostupnosti těchto léků.
Only modest or transient weight loss achieved with most anti-obesity medications available in the past failed to meet expectations of both patients and physicians. Drug combinations and gut hormone analogues represent a new, more efficient and safe approach to the treatment of obesity and related health risks. Initially, gut hormone analogues, e.g. glucagon like peptide-1 (GLP-1) agonist liraglutide, have successfully been applied for the treatment of type 2 diabetes and later on for the treatment of obesity. The paper reports on the phase 3 clinical trials in obesity management with novel gut hormone analogues, GLP-1 agonist semaglutide and dual gut hormone agonist tirzepatide, a single molecule which targets receptors of two gut hormones, GLP1-1 and glucose-dependent insulinotropic polypeptide (GIP). Both semaglutide and tirzepatide are applied subcutaneously once-weekly. Weight loss achieved with these new drugs, particularly with tirzepatide, is comparable to that observed with less invasive bariatric procedures and is accompanied by significant improvements in cardiometabolic risks and favorable safety profile. Observed gastrointestinal adverse events with these medications were usually transient and mild to moderate in severity. Semaglutide has been approved as an anti-obesity drug for long-term weight management by the European Medicines Agency (EMA) while tirzepatide is waiting for its approval. Clinical studies with oral semaglutide, triple incretin agonist retatrutide, non-peptide GLP-1 oral agonist orforglipron and cagrilintide/semaglutide combination also provided promising results in the weight management. On the other hand, setmelanotide is a drug that acts on the melanocortin-4 receptor (MC4R) and is designated for the treatment of very rare monogenic disorders exhibiting severe obesity in early childhood, e.g. deficiency of proopiomelanocortin or leptin receptor. Individually tailored treatment with antiobesity drugs should be an integral part of the comprehensive obesity management which includes diet, exercise and cognitive behavioral intervention. Well educated physicians should ensure the success of treatment by antiobesity drugs. In addition, health authorities and health insurance companies should consider that the cost of new antiobesity drugs prevents such efficient treatment in most of indicated patients and therefore the measures to arrange appropriate availability of these drugs should be adopted.

In the STEP-HFpEF trial, semaglutide improved symptoms, physical limitations and exercise function and reduced body weight in patients with obesity phenotype of heart failure and preserved ejection fraction (HFpEF). This prespecified analysis examined the effects of semaglutide on dual primary endpoints (change in Kansas City Cardiomyopathy Questionnaire-Clinical Summary Score (KCCQ-CSS) and body weight) and confirmatory secondary endpoints (change in 6-minute walk distance (6MWD), hierarchical composite (death, HF events, change in KCCQ-CSS and 6MWD) and change in C-reactive protein (CRP)) across obesity classes I-III (body mass index (BMI) 30.0-34.9 kg m-2, 35.0-39.9 kg m-2 and ≥40 kg m-2) and according to body weight reduction with semaglutide after 52 weeks. Semaglutide consistently improved all outcomes across obesity categories (P value for treatment effects × BMI interactions = not significant for all). In semaglutide-treated patients, improvements in KCCQ-CSS, 6MWD and CRP were greater with larger body weight reduction (for example, 6.4-point (95% confidence interval (CI): 4.1, 8.8) and 14.4-m (95% CI: 5.5, 23.3) improvements in KCCQ-CSS and 6MWD for each 10% body weight reduction). In participants with obesity phenotype of HFpEF, semaglutide improved symptoms, physical limitations and exercise function and reduced inflammation and body weight across obesity categories. In semaglutide-treated patients, the magnitude of benefit was directly related to the extent of weight loss. Collectively, these data support semaglutide-mediated weight loss as a key treatment strategy in patients with obesity phenotype of HFpEF. ClinicalTrials.gov identifier: NCT04788511 .
