Jednoznačné je, že v budoucnu bude stoupat počet polymorbidních pacientů s dia-gnózou diabetes mellitus všech typů a zvýší se tedy i prevalence závažných diabetic-kých komplikací, mezi něž patří i chronické renální selhání. Nezbytná bude léčba nejendiabetu, ale i diabetických komplikací a komorbidit, kde se nabízí možnost využitímoderní terapie SGLT2 inhibitory, respektive glifloziny. Naše kazuistika dokládá po-zitivní efekt léčby za využití kanagliflozinu a dokladuje tak shodu s prokázanými vý-sledky klinické studie CREDENCE. Pozitivní terapeutický efekt byl verifikován jaksnížením HbA1c, tak snížením hodnot albumin/creatinin ratio a ostatními parametry.
It is obvious that the number of polymorbid patients with diabetes mellitus will in-crease in the future, resulting in a rising prevalence of serious diabetic complicationssuch as chronic kidney disease. It is necessary to treat not only diabetes itself, but allof the co-morbidities as well. The modern therapy with SGLT2 inhibitors (gliflozins)can be used for the treatment of renal impairment.The case report shows, in accordance with the results of the clinical trial CRE-DENCE, the positive effect of canagliflozin. The positive therapeutic effect was verifiedby decreasing of HbA1cand decreasing of albumin/creatinine ratio and other para-meters.

Nové třídy antidiabetik jsou nejen účinnou, ale také bezpečnou terapií diabetu. V klinických studiích s těmito léčivými látkami se prokázal jejich příznivý vliv na různé ukazatele renálního poškození. Agonisté receptoru pro glukagonu podobný peptid 1 (GLP‑1) dominantně kladně ovlivňují riziko vzniku a progrese albuminurie. Glifloziny (inhibitory SGLT2) zpomalují progresi poklesu odhadnuté glomerulární filtrace. První jednoznačný důkaz přinesla studie CREDENCE pro kanagliflozin. Všechna data však nasvědčují skutečnosti, že se jedná o tzv. efekt třídy. U velmi rizikové populace snižují také riziko terminálního renálního selhání.
New classes of antidiabetics offer not only efficient but also safe therapy of diabetes. The beneficial influence on different markers of renal impairment was determined in clinical trials with these medicinal products. Glukagon‑like peptide 1 (GLP‑1) receptor agonists show prominent positive effect on the risk of development and progression of albuminuria. Gliflozins (SGLT2 inhibitors) slow down the progression of estimated glomerular filtration decrease. The body of evidence suggests that these are class‑specific effects. In high‑risk population, these drugs also decrease the risk of terminal renal failure.

BACKGROUND/AIMS: We found recently that increasing renal epoxyeicosatrienoic acids (EETs) levels by blocking soluble epoxide hydrolase (sEH), an enzyme responsible for EETs degradation, shows renoprotective actions and retards the progression of chronic kidney disease (CKD) in Ren-2 transgenic hypertensive rats (TGR) after 5/6 renal ablation (5/6 NX). This prompted us to examine if additional protection is provided when sEH inhibitor is added to the standard renin-angiotensin system (RAS) blockade, specifically in rats with established CKD. METHODS: For RAS blockade, an angiotensin-converting enzyme inhibitor along with an angiotensin II type receptor blocker was used. RAS blockade was compared to sEH inhibition added to the RAS blockade. Treatments were initiated 6 weeks after 5/6 NX in TGR and the follow-up period was 60 weeks. RESULTS: Combined RAS and sEH blockade exhibited additional positive impact on the rat survival rate, further reduced albuminuria, further reduced glomerular and tubulointerstitial injury, and attenuated the decline in creatinine clearance when compared to 5/6 NX TGR subjected to RAS blockade alone. These additional beneficial actions were associated with normalization of the intrarenal EETs deficient and a further reduction of urinary angiotensinogen excretion. CONCLUSION: This study provides evidence that addition of pharmacological inhibition of sEH to RAS blockade in 5/6 NX TGR enhances renoprotection and retards progression of CKD, notably, when applied at an advanced stage.

Kazuistika popisuje případ pacientky s diabetes mellitus 2. typu na intenzivní inzulínové terapii a sitagliptinu, s náhlým zhoršením renálních funkcí při infekci močových cest s minimálními příznaky, která nebyla dostatečně razantně zaléčena. Vlivem podcenění příznaků infekce močových cest nemocnou a jejím okolím došlo kmanifestaci diabetické nefropatie, poklesu funkce ledvin, minerálovému rozvratu a dekompenzaci diabetu.
The case study describes the case of a type 2 diabetes mellitus patient on IIT and sitagliptin with a sudden deterioration of renal function in urinary tract infections with mimic symptoms that have not been sufficiently robust. Due to underestimation of symptoms of urinary tract infection and its surroundings, diabetic nephropathy, de - creased renal function, mineral disruption, and diabetes decompensation have occurred.

Background: Emapticap pegol (NOX-E36) is a Spiegelmer® that specifically binds and inhibits the pro-inflammatory chemokine C-C motif-ligand 2 (CCL2) (also called monocyte-chemotactic protein 1). The objective of this exploratory study was to evaluate the safety and tolerability as well as the renoprotective and anti-diabetic potential of emapticap in type 2 diabetic patients with albuminuria. Methods: A randomized, double-blind, placebo-controlled Phase IIa study was initiated in 75 albuminuric type 2 diabetics. Emapticap at 0.5 mg/kg and placebo were administered subcutaneously twice weekly for 12 weeks to 50 and 25 patients, respectively, followed by a treatment-free phase of 12 weeks. Results: Twice weekly subcutaneous treatment with emapticap over 3 months was generally safe and well tolerated and reduced the urinary albumin/creatinine ratio (ACR) from baseline to Week 12 by 29% (P < 0.05); versus placebo a non-significant ACR reduction of 15% was observed (P = 0.221). The maximum difference, 26% (P = 0.064) between emapticap and placebo, was seen 8 weeks after discontinuation of treatment. At Week 12, the HbA1c changed by −0.31% in the emapticap versus +0.05% in the placebo group (P = 0.146). The maximum difference for HbA1c was observed 4 weeks after the last dose with −0.35% for emapticap versus +0.12% for placebo (P = 0.026). No relevant change in blood pressure or estimated glomerular filtration rate was seen between the treatment groups throughout the study. A post hoc analysis with exclusion of patients with major protocol violations, dual RAS blockade or haematuria increased the ACR difference between the two treatment arms to 32% at Week 12 (P = 0.014) and 39% at Week 20 (P = 0.010). Conclusions: Inhibition of the CCL2/CCL2 receptor axis with emapticap pegol was generally safe and well tolerated. Beneficial effects on ACR and HbA1c were observed in this exploratory study, which were maintained after cessation of treatment. Taken together, emapticap may have disease-modifying effects that warrant further investigation in adequately powered confirmatory studies.

OBJECTIVE: We evaluated the therapeutic effectiveness of a new, orally active epoxyeicosatrienoic acid analog (EET-A) in rats with angiotensin II (ANG II)-dependent malignant hypertension. METHODS: Malignant hypertension was induced in Cyp1a1-Ren-2 transgenic rats by activation of the renin gene using indole-3-carbinol (I3C), a natural xenobiotic. EET-A treatment was started either simultaneously with I3C induction process (early treatment) or 10 days later during established hypertension (late treatment). Blood pressure (BP) (radiotelemetry), indices of renal and cardiac injury, and plasma and kidney levels of the components of the renin-angiotensin system (RAS) were determined. RESULTS: In I3C-induced hypertensive rats, early EET-A treatment attenuated BP increase (to 175 ± 3 versus 193 ± 4 mmHg, P < 0.05, on day 13), reduced albuminuria (15 ± 1 versus 28 ± 2 mg/24 h, P < 0.05), and cardiac hypertrophy as compared with untreated I3C-induced rats. This was associated with suppression of plasma and kidney ANG II levels (48 ± 6 versus 106 ± 9 and 122 ± 19 versus 346 ± 11 fmol ml or g, respectively, P < 0.05) and increases in plasma and kidney angiotensin (1-7) concentrations (84 ± 9 versus 37 ± 6 and 199 ± 12 versus 68 ± 9 fmol/ml or g, respectively, P < 0.05). Remarkably, late EET-A treatment did not lower BP or improve renal and cardiac injury; indices of RAS activity were not affected. CONCLUSION: The new, orally active EET-A attenuated the development of experimental ANG II-dependent malignant hypertension, likely via suppression of the hypertensiogenic axis and augmentation of the vasodilatory/natriuretic axis of RAS.
