Hlavnými indikáciami na peroperačné vyšetrenie gastrointestinálneho traktu, pečene a pankreatobiliárneho systému sú zhodnotenie resekčných okrajov a histologická diagnóza lézií, u ktorých nie je z rôznych príčin známa predoperačná diagnóza. Špeciálnym vyšetrením je zhodnotenie prítomnosti steatózy alebo zápalových zmien v pečeni darcu, alebo prítomnosti gangliových buniek v stene hrubého čreva pred vykonaním anastomózy u pacientov s Hirschprungovou chorobou. Najväčšími diagnostickými pascami sú rozpoznanie duktálneho karcinómu pankreasu od chronickej pankreatitídy, odlíšenie karcinómu žlčníka a extrahepatálnych žlčovodov od reaktívnych zmien spôsobených zápalom a rozoznanie prítomnosti difúzneho adenokarcinómu na resekčnom okraji pažeráka a žalúdka.
The main indications for intraoperative consultation of gastrointestinal tract, liver, and pancreatobiliary system are to evaluate the resection margin and to make a tissue diagnosis of lesions for which preoperative histology is not aviable for various reasons. Special situations include the evaluation of liver donor biopsies for the presence of steatosis and inflamation, or determination that ganglion cells are present in the bowel wall at the level where the anastomosis will be placed in case of Hirschprung's disease. The most worrisome pitfalls include differentiating pancreatic ductal carcinoma from chronic pancreatitis, distinguishing biliary tree and gallbladder carcinoma from reactive changes caused by inflammation, and recognizing the presence of diffuse adenocarcinoma at the resection margin of the esophagus and stomach.

AIMS: Carpoglyphus lactis is a stored product mite infesting saccharide-rich stored commodities including dried fruits, wine, beer, milk products, jams and honey. The association with micro-organisms can improve the survival of mites on dried fruits. METHODS AND RESULTS: The microbial communities associated with C. lactis were studied in specimens originating from the packages of dried apricot, plums and figs and compared to the laboratory strain reared on house dust mite diet (HDMd). Clone libraries of bacterial 16S rRNA gene and fungal internal transcribed spacer (ITS) region were constructed and analysed by operational taxonomic unit (OTU) approach. The 16S rRNA gene libraries differed among the compared diets. The sequences classified to the genera Leuconostoc, Elizabethkingia, Ewingella, Erwinia, Bacillus and Serratia were prevailing in mites sampled from the dried fruits. The ITS library showed smaller differences between the laboratory strain on HDMd and the isolates from dried fruits packages, with the exception of the mite strain from dried plums. The population growth was used as an indirect indicator of fitness and decreased in the order from yeast diet to HDMd and dried fruits. CONCLUSIONS: The treatment and pretreatment of mites by antibiotics did not reveal the presence of antagonistic bacteria which might slow down the C. lactis population growth. The shifts of the microbial community in the gut of C. lactis were induced by the diet changes. The identified yeasts and bacteria are suggested as the main food source of stored product mites on dried fruits. SIGNIFICANCE AND IMPACT OF THE STUDY: The study describes the adaptation of C. lactis to feeding on dried fruits including the interaction with micro-organisms. We also identified potentially pathogenic bacteria carried by the mites to dried fruits for human consumption.

Krvácení do dolní části gastrointestinálního traktu představuje akutní nebo chronickou ztrátu krve distálně od Treitzova ligamenta. Jako akutní se označuje krvácení trvající méně než tři dny, které může mít za následek anémii a nestabilitu vitálních funkcí. Chronické krvácení je definováno jako pomalá ztráta krve v trvání dnů až týdnů, charakterizované malou enteroragií nebo intermitentní melénou. Okultní krvácení se projevuje hypochromní anémií a pozitivními testy stolice na přítomnost krve. Krvácení z nejasného zdroje je definováno jako krvácení, které přetrvává nebo recidivuje po iniciální negativní endoskopii.
Lower gastrointestinal bleeding (LGIB) is defined as blood loss distal to the ligament of Treitz. Acute LGIB is defined as having duration shorter than 3 days and it may result in instability of vital signs and anaemia. Chronic LGIB is a slow blood loss over a period of several days or longer with episodes of melena or enterorrhagia. Occult OGIB is manifested by recurrent iron-deficiency anaemia and/or recurrent positive faecal occult blood test results. Obscure GIB has been defined as bleeding of unknown origin that persists or recurs after an initial negative endoscopic evaluation.

To date, only morphological and anatomical descriptions of microwhip scorpions (Arachnida: Palpigradi) have been published. This very rare group is enigmatic not only in its relationships to other arachnids, but especially due to the fact that these animals dwell only underground (in caves, soil, and interstitial spaces). We observed the curious feeding habit of the microwhip scorpion Eukoenenia spelaea over the course of one year in Ardovská Cave, located in Slovakia's Karst region. We chose histology as our methodology in studying 17 specimens and based it upon Masson's triple staining, fluorescent light and confocal microscopy. Single-celled cyanobacteria (blue-green algae) were conspicuously predominant in the gut of all studied palpigrades. Digestibility of the consumed cyanobacteria was supported by the presence of guanine crystals, glycogen deposits and haemocytes inside the palpigrade body. Cyanobacteria, the oldest cellular organisms on Earth, are very resistant to severe conditions in caves, including even darkness. Therefore, the cyanobacteria are able to survive in dark caves as nearly heterotrophic organisms and are consumed by cave palpigrades. Such feeding habit is extraordinary within the almost wholly predacious orders of the class Arachnida, and particularly so due to the type of food observed.

The ingestion of chromogenic or fluorescent substrates for protease detection enables the visualization of digestive processes in mites in vivo due to their transparent bodies. The substrates for protease detection were offered to Lepidoglyphus destructor, and the resulting signals were observed in specimens under a compound microscope. The protease activity was successfully localized using chromogenic substrates (azoalbumin, AAPpNA, SAAPFpNA, elastin-orcein, SA(3) pNA, ZRRpNA, ArgpNA, and MAAPMpNA) and fluorescent substrates (casein-fluorescein, albumin-fluorescein, AAPAMC, BAAMC, ZRRAMC, ArgAMC, and AGPPPAMC). No activity was detected using the keratin azure and BApNA substrates. In the mesodeum, trypsin-like activity generated by hydrolysis of the BApNA substrate was not observed, but the BAAMC substrate allowed the visualization of trypsin-like activity in food boli in the posterior mesodeum. The results indicate that cathepsins B, D, and G and cathepsin H or aminopeptidase-like activities are present in the midgut of L. destructor. Among these activities, cathepsin D-like activity was identified for the first time in the gut of L. destructor. All proteases mentioned are produced in the mesodeal lumen and form the food bolus together with ingested food, afterward passing through the gut to be defecated. The method used enables the visualization of protease activities in the gut of transparent animals.
Odběr slizničních vzorků pro bioptické vyšetření je standardní součástí endoskopického vyšetření gastrointestinálního traktu. Zatímco určení lokalizace biopsie při vyšetření nádorových (neoplastických) lézí nepředstavuje většinou příliš závažný problém, u zánětlivých onemocnění není optimální strategie pro odběr materiálu zdaleka tak jednoznačná. Příčinou těchto obtíží je diskontinuální až ložisková distribuce morfologických změn, topické rozdíly v intenzitě zánětlivých změn a konečně i snaha o zastižení časných vývojových fází nádorů, které vznikají na podkladě některých chronických zánětů. Navíc, přestože pro některá onemocnění (např. Barrettův jícen nebo chronická gastritida) jsou doporučené postupy pro odběr bioptických vzorků zpracovány, klinická praxe je často jiná, zejména pokud jde o množství odebraného materiálu, respektive počet míst, z nichž byly pořízeny biopsie. Nedostatečný odběr však může paradoxně vést ke zvýšené zátěži pacienta v důsledku nutnosti opakování endoskopických vyšetření.
Obtaining mucosal biopsies has become a standard part of endoscopic investigations of the gastrointestinal tract. Although targeting biopsies of tumoriform (neoplastic) lesions does not usually constitute a difficult task, investigating inflammatory disorders may necessitate more extensive tissue harvesting due to the discontinuous or even patchy distribution of morphological changes, topical differences in the intensity of inflammatory changes and finally, in attempts to detect early phases of inflammation-associated tumour development. Furthermore, even if clinical guidelines exist (e.g. Barrett's oesophagus or chronic gastritis), they are usually not followed in practice, especially regarding the amount of retrieved tissue or number of sites investigated by biopsy. However, an inappropriate number of tissue samples may paradoxically cause harm to the patient due to the necessity of repeating the biopsy.

Digesce a resorpce hrají v metabolizmu organizmu zásadní roli. Poruchy, ať už postihují pouze jednu či obě výše uvedené funkce, významně ovlivňují organizmus jako celek. V případě onemocnění jsou spoluurčující pro další vývoj nemoci, a to jak z pohledu zajištění celkové energetické potřeby, tak i z pohledu optimálního složení přijímaných substrátů v konkrétní situaci. Omezení funkce digesce i resorpce by mělo vést k indikaci náhradní nutrice, a to jak přípravky ve formě sipingu či ve formě speciálních enterálních výživ. Náhradní nutrice vybranými preparáty v klinické praxi, aby měla očekávaný efekt, musí odpovídat fyziologickým požadavkům za situací omezené digesce a resorpce gastrointestinálního traktu.
Digestion and resorption play an essential role in the body's metabolism. Disorders involving one or both of the functions mentioned above significantly affect the organism as a whole. In the case of disease, they codetermine the further course of the disease both in terms of providing the total energy needs and in terms of the optimal composition of the substrates ingested in a particular situation. Impaired function of digestion and resorption should be an indication for nutritional support, both in the form of sipping and in the form of special enteral feeding. In order to have the expected effect, nutritional support with selected products in the clinical practice must meet the physiological needs in the setting of impaired digestion and resorption by the gastrointestinal tract.
