BACKGROUND: In the phase 3 ALCYONE study, daratumumab plus bortezomib/melphalan/prednisone (D-VMP) versus bortezomib/melphalan/prednisone (VMP) significantly improved progression-free survival (PFS) and overall survival (OS) in transplant-ineligible, newly diagnosed multiple myeloma (NDMM) patients. We present a subgroup analysis of ALCYONE by patient frailty status. PATIENTS AND METHODS: Frailty assessment was performed retrospectively using age, Charlson comorbidity index, and baseline Eastern Cooperative Oncology Group performance status score. Patients were classified as fit (0), intermediate (1), or frail (≥2); a nonfrail category combined fit and intermediate patients. RESULTS: Among randomized patients (D-VMP, n = 350; VMP, n = 356), 391 (55.4%) were nonfrail (D-VMP, 187 [53.4%]; VMP, 204 [57.3%]) and 315 (44.6%) were frail (163 [46.6%]; 152 [42.7%]). After 40.1-months median follow-up, nonfrail patients had longer PFS and OS than frail patients, but benefits of D-VMP versus VMP were maintained across subgroups: PFS nonfrail (median, 45.7 vs. 19.1 months; hazard ratio [HR], 0.36; P < .0001), frail (32.9 vs. 19.5 months; HR, 0.51; P < .0001); OS nonfrail (36-month rate, 83.6% vs. 74.5%), frail (71.4% vs. 59.0%). Improved greater than or equal to complete response and minimal residual disease (10-5)-negativity rates were observed for D-VMP versus VMP across subgroups. The 2 most common grade 3/4 treatment-emergent adverse events were neutropenia (nonfrail: 39.2% [D-VMP] and 42.4% [VMP]; frail: 41.3% and 34.4%) and thrombocytopenia (nonfrail: 32.8% and 36.9%; frail: 36.9% and 39.1%). CONCLUSION: Our findings support the clinical benefit of D-VMP in transplant-ineligible NDMM patients enrolled in ALCYONE, regardless of frailty status.

INTRODUCTION: The overall survival of patients with multiple myeloma has improved with the advent of novel agents; however, multiple myeloma remains incurable. Combinations of standard-of-care agents such as immunomodulators, proteasome inhibitors, and anti-CD38 monoclonal antibodies are increasingly used in earlier lines of therapy. Patients with disease that is refractory to multiple novel agents represent a population with high unmet medical need and for whom therapies with new mechanisms of action could be beneficial. Melphalan flufenamide (melflufen) has demonstrated encouraging activity in patients with relapsed and refractory multiple myeloma. AREAS COVERED: This review provides an overview of the mechanism of action of melflufen, a first-in-class peptide-drug conjugate that targets aminopeptidases and rapidly delivers alkylating agents into tumor cells. It reviews key Phase I and II clinical trial data for melflufen in combination with dexamethasone as well as in triplet combinations with daratumumab or bortezomib. The safety profile of melflufen, which is characterized primarily by clinically manageable hematologic adverse events, is described. EXPERT OPINION: Melflufen has potential to fill a gap in the myeloma treatment landscape by providing a new mechanism of action with clinically meaningful efficacy and a favorable safety profile in patients refractory to multiple novel agents.

Východiska: Izolovaná perfuze končetin s použitím TNF-α a melfalanu (TM-ILP), v kombinaci s následnou kompletní resekcí tumoru, je efektivní metoda léčby u nemocných s neresekovatelným sarkomem měkkých tkání končetin, spojená se záchranou končetiny ve více než 80 % případů. Metoda ILP je indikována též u nemocných s maligním melanomem končetin, s četnými in-tranzitními metastázami. Autoři prezentují technickou variantu kanylace pro mimotělní oběh u vybraných pa cientů s postižením horní končetiny. Soubor pa cientů: Z celkového počtu 55 ILP provedených na našem pracovišti od roku 2009 bylo devět výkonů na horní končetině (v obdobném poměru v indikaci sarkom měkkých tkání (n = 4) i maligní melanom (n = 4 + 1× reoperace)). Standardní technika kanylace brachiální žíly u malého kompartmentu horní končetiny vzhledem k nižším průtokům v systému a relativně velké ploše vzhledem k hmotnosti přináší obtíže s nedostatečným a déle trvajícím ohřátím perfundovaného kompartmentu horní končetiny. Jednoduchá technika „Y“ kanylace žilního systému cestou hluboké brachiální žíly a současně povrchního systému cestou v. basilica přináší až o 20 % vyšší minutový průtok v systému mimotělního oběhu, rychlejší ohřátí horní končetiny a stabilní teplotní prostředí po celou dobu ILP horní končetiny, což je základní podmínkou úspěchu léčby. Závěr: Uvedená technika se vzhledem ke svým přednostem stala na našem pracovišti standardní při ILP horní končetiny.
Background: The authors present a technical variation of the standard cannulation for cardiopulmonary bypass perfusion during hyperthermic isolated limb perfusion (ILP) procedures in selected patients with unresectable soft tissue sarcoma or malignant melanoma. Patients: Of 55 ILP procedures performed at our institution since the procedure was established in 2009, nine were performed at the upper extremity. Standard single venous cannulation was used in fi ve cases, and extended, double venous cannulation in the last four. The standard technique for brachial vein cannulation in a small compartment of the upper extremity entails a problematic and longer perfusion of the upper extremity. This is due to the lower fl ow rate in the venous system and relatively large surface area with respect to weight. We present a simple technique based on a “Y” cannulation of the venous system via the deep brachial vein and superfi cial venous system via the basilic vein, delivering a 20% increase in fl ow rate in the extracorporeal circulation. Faster heating of the upper extremity and a stable thermal environment throughout upper-extremity ILP are essential for successful treatment. Conclusion: Extended technique of venous cannulation for extracorporeal circulation setting, due to their advantages, became standard in the upper limb ILP procedure at our institution.

Immune reconstitution after high-dose chemotherapy and stem cell transplantation plays a key role in restoring immunocompetence including defense against infection, immune regulation, and onco-immune surveillance. In this work, we examined the recovery of mucosal-associated invariant T (MAIT) cells, recently discovered innate-like T cells, after various types of myeloablative chemotherapy and autologous peripheral blood stem cell transplantation in 29 patients. We show that MAIT cells are relatively resistant to myeloablative conditioning. The median amount of MAIT cells rises to 43 % around day +30 and is sustained through further measurements on days +60 and +100. Moreover, MAIT cell recovery reaches 100 % of pre-treatment values in 33 % of patients already by day +60. The only factor affecting recovery of MAIT cells is age, younger age being associated with earlier MAIT cell recovery. The pre-treatment quantity of MAIT cells carries a prognostic impact on the early post-transplantation course. Patients with high levels of MAIT cells pre-treatment have significantly lower peak CRP levels (79.45 vs. 150 mg/L) post-treatment, reflecting a clinical trend of less severe infectious complications (less febrile days and less days on intravenous antibiotics). Altogether these data suggest that a high proportion of MAIT cells survive myeloablative chemotherapy and maintain their capacity to fight against infections probably on mucosal surfaces.

Since refractory celiac disease (RCD) may cause severe complications and is associated with a poor prognosis in a portion of patients, early diagnosis and treatment is of importance. Current diagnostics include complex PCR-based molecular pathology and FACS techniques in order to differentiate type I and type II RCD, the latter being associated with the development of an enteropathy-associated T cell lymphoma (EATL). Recently established treatment options also include high-dose chemotherapeutic regimens for RCD type II which have significantly improved survival. However, treatment strategies are highly individual targeting also complications of malabsorption and should be started in the RCD phase of the disease. Therefore, adequate diagnostics are pivotal for patient outcome.

V našich experimentech jsme se zaměřili na studium CD138+ a CD138- buněk izolovaných z kostní dřeně pacientů s diagnostikovaným mnohočetným myelomem (MM). Studovali jsme epigenetické modifi kace u myelomových buněčných linií, které byly ovlivněny klinicky využívanými cytostatiky. Chromatinová imunoprecipitace (ChIP) v kombinaci s polymerázovou řetězovou reakcí (PCR) nám umožnila studovat epigenetické profi ly, jako je H3K9 acetylace a H3K9 dimethylace v promotorech a exonech genů, které jsou zodpovědné za patogenezi MM. Epigenetické modifi kace těchto lokusů byly rovněž analyzovány u myelomových buněčných linií po ovlivnění vybranými cytostatiky. Pomocí ChIP-on-chip microarray analýzy jsme studovali v promotorech stovek genů zastoupení H3K9 acetylace, která je spojována se zvýšenou genovou expresí. Studium epigenetických změn u nádorových buněk je v současné době otázkou základního výzkumu. Naše výsledky ukazují originální výstupy z ChIP-PCR a ChIP-on-chip analýz u buněk pacientů s diagnostikovaným MM a buněčných linií ovlivněných cytostatiky. Naše experimenty poskytují užitečný návod, jak testovat a posoudit terapeutický potenciál cytostatik, které významně ovlivňují histonový kód.
Epigenetic changes such as histone acetylation and di-methylation at promoters and coding regions of genes which play an important role in the pathogenesis of multiple myeloma (MM) can be analyzed using methods such as chromatin immunoprecipitation (ChIP) in combination with polymerase chain reaction (PCR). We have studied epigenetic changes in MM cells treated by selected cytostatics that are used in MM therapy. ChIP-on-chip or with microarrays (chip) analyses of histone acetylation enabled us to study epigenetic status of hundreds of genes after cytostatic treatment. This type of epigenetic modifi cation is associated with increased gene expression. We have applied the ChIP-PCR and ChIP-on-chip methods to study epigenetics in MM cell lines as well as in clinical samples. Our experiments have led to the development of a useful method of assessing the therapeutic potential of cytostatic agents which have a signifi cant impact on the histone code.

The nuclear arrangement of promyelocytic leukaemia nuclear bodies (PML NBs) was studied in vitro after the cell treatment by clinically used agents such as all-trans retinoic acid (RA) in human leukaemia and cytostatics or gamma radiation in multiple myeloma cells. In addition, the influence of phorbol ester (PMA) on PML NBs formation was analyzed. A reduced number of PML bodies, which led to relocation of PML NBs closer to the nuclear interior, mostly accompanied RA- and PMA-induced differentiation. Centrally located PML NBs were associated with transcriptional protein RNAP II and SC35 regions, which support importance of PML NBs in RNA processing that mostly proceeds within the nuclear interior. Conversely, the quantity of PML NBs was increased after cytostatic treatment, which caused re-distribution of PML NBs closer to the nuclear envelope. Here we showed correlations between the number of PML NBs and average Centre-to-PML distances. Moreover, a number of cells in S phase, especially during differentiation, influenced number of PML NBs. Studying the proteins involved in PML compartment, such as c-MYC, cell-type specific association of c-MYC and the PML NBs was observed in selected leukaemic cells undergoing differentiation, which was accompanied by c-MYC down-regulation.
