Authors examined the results of surgical treatment for 123 breast age patients with Acute Paraproctitis (AP) being cured for the last 10 years. Was offered the less invasive, less traumatic, defensive way of fistulectomy at AP treatment in breast fed infants, reducing possible relapse up to minimum. The suggested approach provides full desquamation and removal of fistula channel epithelium lining rests, and therefore the rectum sphincter is not damaged and there is no much damage in mucus. Examining with scanning electron microscopy (SEM) of micro drainages used for micro sanitation in the form of nylon line with knots, has found that their use contributes to removal from the fistula cavity not only detritus, fibrin and desquamated cells, but pathogen content in the form of microorganisms such as sticks, and structures fungi. It was determined that it is necessary to use not only antibacterial and anti fungous therapy. The advantage of the new operational way over the traditional approach is prevention of wound reinfection. Of the 52 breast patients, 51 obtained results of treatment were quite good in the near and distant periods of illness. Application of minimally invasive fistulocriptoectomy at AP infants maximizes the improvement of the results of surgical treatment, and allows to minimize complications.

Uretrorektální píštěle jsou vzácným, ale závažným onemocněním. Získané píštěle vznikají nejčastěji jako komplikace chirurgické léčby nemocí prostaty a rekta, jako následek zranění, nebo jako komplikace zánětlivých onemocnění v této oblasti, jejich úprava je vždy velmi obtížná a většinou vyžaduje řešení v několika dobách. Kromě klasických operací se v posledních letech objevují i úspěšné pokusy o minimálně invazivni léčbu, jsou však vhodné spíše pro malé uretrorektální píštěle. U našeho nemocného s rozsáhlou uretrorektální pištěli po transuretrálni prostatektomii byl uzávěr píštěle komplikován obrovskou obezitou a hypotonií análního svěrače. To si vynutilo kombinovaný uzávěr přední stěny rekta z rektotomie s následnou transuretrální resekcí píštěle s aplikací tkáiíového lepidla.
Urethrorectal fistulae are rare but serious conditions. Acquired fistulae occur most frequently as a complication of the surgical treatment of the diseases of prostate and reaum, as a result of an injury or as a complication of the inflamatory diseases in this area. Their correction is always very difficult and demands a gradual process in several steps. Besides the established procedures new and successul trials to minimize the invasivity of treatment have appeared in the last several years. The closure of the extensive urethrorectal fistula after transurethral prostatectomy in our patient were complicated by extreme obesity and the hypotonia of the anal sphincter. This made necessary the combined closure of the anterior wall of the reaum and subsequent transurethral resection of the fistula with the application of tissue glue.
