To evaluate an influence of ascorbic acid on phenotype of CMT1A neuropathy. The cohort of sixty patients will be treated ( AA vs placebo) fot two years. The primary efficacy outcome will be an improvement on CMTNS, electro- physiological parameters, disability ( ONDS) and QoL ( SF 36).
Stanovení vlivu kyseliny askorbové na fenotyp osob s chorobou Charcot Marie Tooth 1A. Na souboru 60 osob bude sledován vliv kys.askorbové proti placebu během 24 měsíců. Hodnoceny jsou klinické škály CMTNS, dále EMG a dotazník SF 36. Cílem je změna fenotypu o 1 stupeň na škále CMTNS.

Distal hereditary motor neuropathy (HMN) is a clinically and genetically heterogeneous group of disorders affecting spinal alpha-motor neurons. Since 2001, mutations in six different genes have been identified for autosomal dominant distal HMN; glycyl-tRNA synthetase (GARS), dynactin 1 (DCTN1), small heat shock 27 kDa protein 1 (HSPB1), small heat shock 22 kDa protein 8 (HSPB8), Berardinelli-Seip congenital lipodystrophy (BSCL2) and senataxin (SETX). In addition a mutation in the (VAMP)-associated protein B and C (VAPB) was found in several Brazilian families with complex and atypical forms of autosomal dominantly inherited motor neuron disease. We have investigated the distribution of mutations in these seven genes in a cohort of 112 familial and isolated patients with a diagnosis of distal motor neuropathy and found nine different disease-causing mutations in HSPB8, HSPB1, BSCL2 and SETX in 17 patients of whom 10 have been previously reported. No mutations were found in GARS, DCTN1 and VAPB. The phenotypic features of patients with mutations in HSPB8, HSPB1, BSCL2 and SETX fit within the distal HMN classification, with only one exception; a C-terminal HSPB1-mutation was associated with upper motor neuron signs. Furthermore, we provide evidence for a genetic mosaicism in transmitting an HSPB1 mutation. This study, performed in a large cohort of familial and isolated distal HMN patients, clearly confirms the genetic and phenotypic heterogeneity of distal HMN and provides a basis for the development of algorithms for diagnostic mutation screening in this group of disorders.

Hereditární neuropatie jsou klinicky a geneticky heterogenní skupinou chorob a představují nejčastější dědičné neuromuskulární onemocnění. Prevalence je přibližně 17-40 : 100 000. Nejčastější forma se označuje jako choroba Charcot-Marie-Toothova (CMT) neboli hereditární motorická a senzitivní neuropatie (HMSN). Klinické příznaky se objevují nejčastěji v 1. a 2. dekádě, zahrnují atrofie a slabost svalů dolních končetin (peroneální atrofie), deformitu nohy typu pes cavus se zkrácením Achillovy šlachy, areflexii na dolních končetinách a poruchu čití punčochovitého typu. Základní klasifikace hereditárních neuropatií se opírá o elektrofyziologické studie, které rozdělují CMT chorobu na dva základní typy. Typ 1 - demyelinizační - s rychlostí vedení motorickými vlákny n. medianus na předloktí pod 38 m/s, a typ 2 - axonální - s rychlostí vedení nad 38 m/s. Molekulární genetici dosud identifikovali kauzální mutace ve 40 genech a významně přispěli k molekulárně genetické klasifikaci CMT choroby a k pochopení patogeneze nejčastějších forem hereditárních neuropatií. Dědičnost zahrnuje všechny typy monogenního mendelovského přenosu - autozomálně dominantní, autozomálně recesivní a gonozomálně dominantní. Protože kauzální terapie neexistuje, je současná léčebná strategie opřena o rehabilitační, protetickou a ortopedickou léčbu. Prognóza pacientů je v drtivé většině forem příznivá, neboť nezkracuje běžnou délku života, avšak významně ovlivňuje jeho kvalitu.
Hereditary neuropathy is a clinically and genetically heterogeneous group of diseases which represent most often hereditary neuromuscular disorder. The prevalence is approximately 17-40 per 100,000. The most frequent form is called Charcot-Marie-Tooth disease (CMT) or hereditary motor and sensitive neuropathy (HMSN). In most cases, the clinical symptoms appear in the 1st and 2nd decade and include atrophy and weakness of lower limb muscles (peroneal atrophy), the pes cavus deformity of the foot with a shortened Achilles tendon, areflexia in the lower limbs and the stocking type sensory disorder. The basic classification of hereditary neuropathies is based on electrophysiological studies which classify CMT disease into two basic types. Type 1 - demyelinisation - with the median nerve motor conduction velocity at forearm below 38 m/s, and type 2 - axonal - with a velocity above 38 m/s. Molecular geneticists have so far identified causal mutations in 40 genes and have made a major contribution to molecular genetic classification of CMT disease and to the understanding of pathogenesis of the most frequent forms of hereditary neuropathies. Heredity involves all types of monogenic Mendelian inheritance: autosomal dominant, autosomal recessive and gonosomal dominant. Because causal therapy does not exist, current therapeutic approach is based on rehabilitation, prosthetic and orthopedic treatment. In a vast majority of cases, the prognosis of patients is good as this condition does not impair their standard life expectancy, even though it has a major impact on the patients? quality of life.

STUDY DESIGN: Retrospective study of 175 patients with hereditary motor and sensory neuropathy (HMSN), i.e., Charcot-Marie-Tooth (CMT) disease. OBJECTIVE: To investigate the frequency, age of onset, character, familial, and genotypical incidence of spinal deformities among HMSN patients. SUMMARY OF BACKGROUND DATA: Prior studies addressing HMSN discuss the associated spinal deformities. However, these data vary significantly while inconsistently including genotypes within the classification framework. METHODS: Plain-film radiographic spine studies of 175 HMSN patients were performed to determine the incidence, character, and severity of spinal deformity. The degree of the spinal deformity was evaluated measuring Cobb's angle of the main curve. The results of the entire cohort were initially assessed before being classified by genotype. RESULTS: The incidence of spinal deformity for the entire group was 26%. Of these, 58% demonstrated scoliosis, 31% had kyphoscoliosis, and 11% had thoracic hyperkyphosis; 73% of patients with spinal deformity were classified as HMSN Type I with confirmed duplication of the PMP 22 (peripheral myelin protein) gene on chromosome 17. The incidence of spinal deformity by genotype was: duplication of the PMP 22 gene: 29% (25 of 87); deletion of the PMP 22 gene: 0% (0 of 15); Cx32 (connexin 32) gene mutation: 24% (8 of 34); and MPZ (myelin protein zero) gene mutation: 100% (6 of 6). Familial incidence of spinal deformity was found in "MPZ gene mutation" and "duplication of PMP 22 gene" subgroups. CONCLUSION: This study demonstrates a 26% incidence of spinal deformity among HMSN patients. Spinal deformity was most frequently observed in patients with the MPZ gene mutation, where the most common familial incidence was also found.

Hereditární neuropatie jsou nejčastější geneticky podmíněné nervosvalové onemocnění a představují klinicky, histologicky a geneticky heterogenní skupinu. Prevalence je přibližně 1:2500. Nejčastější forma, která postihuje senzitivní i motorické nervy se nazývá choroba Charcot-Marie-Toothova (CMT) neboli hereditární motorická a senzitivní neuropatie (HMSN). Manifestace nemoci je nejčastěji v 1. a 2. dekádě, zahrnuje atrofie a slabost distálních svalů dolních končetin, deformity nohou (pes cavus), často areflexii na dolních končetinách a lehké poruchy čití punčochovitého typu. Hlavní diagnostickou procedurou je elektromyografie. Klasifikace hereditárních neuropatií je založená na molekulárně genetickém průkazu kauzální mutace (dosud více než 40 známých genů). Léčba je rehabilitační, protetická a ortopedická. Prognóza pacientů je sice příznivá, neboť neuropatie nezkracují běžnou délku života, ale významně ovlivňují jeho kvalitu.
Hereditary neuropathy is the most common inherited neuromuscular disease. The prevalence is approximately 1:2500. The most common subtype is called Charcot-Marie-Tooth disease and affects motor and sensory nerve fibres. In most cases clinical symptoms involve atrophies of distal feet muscles, feet deformities and absent ankle reflexes. The classification is based on nerve conduction velocity - type 1 - demyelinating - motor conduction velocity less than 38 m/s and type 2 – axonal – above 38 m/s. The molecular geneticists discovered more than 40 genes with different causative mutations and have made a major contribution to molecular genetics classification of the CMT disease. The current therapeutic approach is based on physiotherapy, prosthetic and orthopaedic treatment because causative treatment does not exist. Prognosis of CMT patients is relatively good because the standard life expectancy is not impaired but has a big impact on the quality of life.

The aim of this study was to evaluate the utility of the Charcot-Marie-Tooth Neuropathy Score (CMTNS) for evaluation of disease severity in young children with Charcot-Marie-Tooth type 1A. Current validated scoring scales for Charcot-Marie-Tooth are the CMTNS and the Neuropathy Impairment Score (NIS). Both work well for adult patients, and usually also for children over 10 years of age. There is no validation of scales for young children. Children with genetically proven Charcot-Marie-Tooth type 1A disease (n = 20, aged 3 to 10 years) were examined clinically, followed by electrophysiologic examination, and were scored under the CMTNS scale. The clinical symptoms were mild; the two most frequent symptoms were difficulty in heel walking and lower limb areflexia. The score was maximally abnormal in four of the nine categories. Categories for sensation, sensory symptoms, and motor symptoms of the arms were normal in all cases. The score was below 8 for all tested children. To conclude, the CMTNS in children aged 10 years and younger has limited sensitivity; out of nine categories, only four are useful. Thus, evaluation of disease severity and progression in young children with Charcot-Marie-Tooth disease remains limited, and there is need for other, effective scoring systems.

Dědičná polyneuropatie se manifestuje nejčastěji v dětství nebo dospívání, má progresivní charakter a zatím je kauzálně neléčitelná. V rámci komplexní péče je nutné věnovat dostatečnou pozornost pohybovým a sportovním aktivitám těchto jedinců. Vhodně volená posilovací cvičení a další typy pohybových aktivit nevedou ke zhoršení svalového oslabení, ale naopak mohou zpomalit progresi či dokonce zmírnit stupeň paréz a zlepšit výkon běžných denních činností u pacientů s tímto onemocněním. Nutné je stanovit individuální cvičební plán dle aktuálního stavu, aby cvičení nevedlo k přetížení se zhoršením svalového oslabení, bolestem a prohloubení únavy. Literatura naznačuje, že cvičení může mít lepší efekt při současném perorálním příjmu kreatin monohydrátu. Přiměřený typ pohybových aktivit by měl být doporučen všem pacientům s dědičnou polyneuropatií s různým stupněm postižení.
Inherited polyneuropathy is currently an incurable disease with progressive symptoms affecting young individuals. Complex case management must include education and instruction in appropriate movement and sporting activities. Moderate strengthening exercises have not been found to be harmful and may reduce the level of muscle paresis while increasing strength and improving the performance of activities of daily living. Customized exercise plans must be regularly reviewed and updated in order to avoid overuse leading to greater muscular weakness, pain and extreme post-exercise fatigue. The current literature suggests that exercise may be more beneficial if a creatine monohydrate peroral supplement is added to the diet. Current clinical management requires the application of appropriate movement activities for every individual with inherited polyneuropathy, even those whose conditions have progressed to the level of extreme disability.
