Dědičné neuropatie (DěN) jsou heterogenní skupinou geneticky podmíněných onemocnění periferních nervů, projevující se u převážné většiny pacientů do 20. roku věku a typicky ve školním věku, mezi 6. a 15. rokem, pomalu progredující svalovou slabostí distálně a později i atrofiemi, dříve a více na dolních končetinách a většinou i různě závažnou poruchou čití s distálním maximem, někdy i trofickými změnami. Začátek obtíží s prvními příznaky onemocnění v předškolním věku nebo i po 20. roce života jsou vzácnější, ale také možné a souvisí to do velké míry s konkrétní genovou poruchou. DěN jsou nejčastějším geneticky podmíněným nervosvalovým onemocněním a v ČR je jimi postiženo asi 4 000 lidí. Dědičné neuropatie nezkracují normální délku života, ale mohou vést k tělesné invalidizaci se ztrátou nebo výrazným zhoršením samostatné chůze a výrazně ovlivňovat kvalitu života i samostatný pohyb i sebeobsluhu. Mentální úroveň je typicky normální. Závažnost postižení je do velké míry závislá na genetické příčině a také na délce trvání nemoci.
Hereditary neuropathies are a heterogeneous group of genetically determined peripheral nerve diseases. In the vast majority of patients, they occur by 20 years of age, most typically during school age between ages 6 and 15, and are manifested by slowly progressive muscle weakness distally and, later on, by atrophies, initially and more predominantly in the lower limbs, as well as by sensory disturbance of varying severity with a distal maximum, sometimes even by trophic changes. The onset of complaints with initial symptoms of the disease in preschool age or after 20 years of age is rarer, but also possible, and is, to a great degree, related to the particular gene disorder. Hereditary neuropathies are the most frequent genetically determined neuromuscular diseases, affecting approximately 4,000 people in the Czech Republic. Hereditary neuropathies do not reduce normal life expectancy, but may lead to physical disability with loss or substantial worsening of independent gait and significantly affect the quality of life as well as self-locomotion and self-care. The intellectual level is typically normal. The severity of disability largely depends on the genetic cause as well as on disease duration.
BACKGROUND AND OBJECTIVES: Charcot-Marie-Tooth disease (CMT) is the most common inherited neuropathy and often presents during childhood. Guidelines for the optimal management of common problems experienced by individuals with CMT do not exist, for either children or adults. We formed the Paediatric CMT Best Practice Guidelines Consortium to develop evidence and consensus-based recommendations for the clinical management of children and adolescents with CMT, with the primary objective of promoting optimal, standardised care globally. METHODS: Development of this clinical practice guideline involved a series of systematic reviews covering 10 clinical questions, modified Delphi methodology involving an international panel of clinicians to generate consensus where evidence did not exist, and application of the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) approach to evaluate the body of literature and formulate recommendations. RESULTS: The final guideline includes three evidence-based and 31 consensus-based recommendations. They encompass the management of muscle weakness, balance and mobility impairment, sensory symptoms, muscle cramps, impaired upper limb function, respiratory impairment, maintenance of joint range of motion and non-surgical management of joint deformity. Consensus was not achieved in some management areas, reflecting differences in practice between clinicians and healthcare settings, and highlighting the need for further research. CONCLUSIONS: This clinical practice guideline provides practical and implementable guidance on the management of common clinical problems experienced by children with CMT and advocates for improved access to multidisciplinary care. Successful dissemination and implementation of these recommendations will be critical in ensuring their application across multiple healthcare settings.

Hereditárne polyneuropatie predstavujú veľkú skupinu geneticky podmienených neurologických ochorení, prezentujúcich sa heterogénnym klinickým obrazom. Na tieto poruchy pomýšľame hlavne pri pozitívnej rodinnej anamnéze, manifestácii chronickej progredujúcej svalovej slabosti a atrofie svalov na končatinách či prítomnosti typických deformít nohy. V našej práci sme sa bližšie zamerali na najčastejšiu hereditárnu periférnu neuropatiu - Charcotovu-Marieovu-Toothovu chorobu (CMT) a urýchlenie progresie tohto ochorenia v kombinácii s chronickým abúzom alkoholu v kazuistike nášho pacienta.
Hereditary peripheral neuropathies represent a large group of genetic neurological diseases presenting with a heterogeneous clinical picture. We mainly think about these disorders at positive family history, presentation of chronic and progressive muscle weakness, atrophy of the limbs and by the presence of typical foot deformities appears. This article closely focuses on the most common hereditary neuropathy - Charcot-Marie-Tooth (CMT) disease and the accelerated progression of this disease in combination with chronic alcohol abuse in our patient's case report.

BACKGROUND: Charcot-Marie-Tooth disease type 1A (CMT1A) is the most common inherited neuropathy, a debilitating disease without known cure. Among patients with CMT1A, disease manifestation, progression and severity are strikingly variable, which poses major challenges for the development of new therapies. Hence, there is a strong need for sensitive outcome measures such as disease and progression biomarkers, which would add powerful tools to monitor therapeutic effects in CMT1A. METHODS: We established a pan-European and American consortium comprising nine clinical centres including 311 patients with CMT1A in total. From all patients, the CMT neuropathy score and secondary outcome measures were obtained and a skin biopsy collected. In order to assess and validate disease severity and progression biomarkers, we performed qPCR on a set of 16 animal model-derived potential biomarkers in skin biopsy mRNA extracts. RESULTS: In 266 patients with CMT1A, a cluster of eight cutaneous transcripts differentiates disease severity with a sensitivity and specificity of 90% and 76.1%, respectively. In an additional cohort of 45 patients with CMT1A, from whom a second skin biopsy was taken after 2-3 years, the cutaneous mRNA expression of GSTT2, CTSA, PPARG, CDA, ENPP1 and NRG1-Iis changing over time and correlates with disease progression. CONCLUSIONS: In summary, we provide evidence that cutaneous transcripts in patients with CMT1A serve as disease severity and progression biomarkers and, if implemented into clinical trials, they could markedly accelerate the development of a therapy for CMT1A.

Hereditární motorické neuropatie (HMN), někdy též nazývané distální spinální svalové atrofie (dSMA), jsou charakterizované selektivním postižením motorické části periferního nervového systému. Představují asi 10 % všech hereditárních neuropatií. Typickým znakem jsou symetrické svalové atrofie, které postihují distální svaly končetin a vykazují tzv. na délce nervu závislý (length dependent) vzorec poruchy. Představují heterogenní skupinu motorických neuropatií s významnou variabilitou fenotypu a velkou genetickou heterogenitou s různými typy dědičnosti. Aktuálně bylo popsáno více než 12 různých genů, jejichž mutace jsou kauzální pro různé formy HMN. Jednotlivé formy se liší věkem nástupu příznaků, primárním postižením buď horních nebo dolních končetin a kombinací s dalšími příznaky např. s pyramidovými jevy, minoritními senzitivními poruchami, neuromyotonií, parézou hlasivek. Diagnostika se vedle kliniky a elektrodiagnostiky opírá o molekulárně genetické testy a nálezy kauzálních mutací. Diferenciální diagnostika je důležitá proti CMT nemoci, nemocem periferního motoneuronu, včetně juvenilní familární formy amyotrofické laterální sklerózy nebo získané multifokální motorické neuropatie. Kauzální terapie není k dispozici a základem je rehabilitační a lázeňská léčba, dále protetické a zdravotní pomůcky. V případě stanovení kauzální mutace je důležité genetické poradenství a v některých případech i zajištění prenatální či preimplantační diagnostiky.
The hereditary motor neuropathies (HMN), sometimes called distal spinal muscular atrophies (dSMA), are characterized by selective involvement of peripheral motor nervous system. They affect about 10% of all patients suffering from hereditary neuropathies. The typical clinical features are symmetric atrophies and weakness of distal muscles of all extremities in lenght dependent pattern. They are clinically and genetically heterogeneous group of motor neuropathies with great diversity of phenotype and genotype. This group was subdivided into different types according to age of onset, muscle weakness distribution at upper or lower limbs, minor sensory impairment, neuromyotonia or vocal cord paralysis. Recently, more than 12 genes were discovered and are considered as a cause of different type of HMN. The diagnostic algorithm include clinical symptoms, electrophysiology and molecular genetic testing. The differential diagnosis is very important and similar conditions are considered, g.e. Charcot-Marie-Tooth disease, motor neuron diseases, including juvenile forms of amyotrophic lateral sclerosis or acquired multifocal motor neuropathy. The causative treatment is not available. The physiotherapy, orthotics and orthopedic surgery are important. The genetic counselling, prenatal or preimplant genetics testing are very important in the case of known causative mutation.

Charcot-Marie-Tooth je geneticky podmíněná periferní neuropatie s rozvojem svalových dysbalancí a kontraktur svalů a vazivových struktur. S věkem dochází též k rozvoji deformit kostí a kloubů nohy a ruky. Průběh onemocnění je variabilní, klinický nález kolísá i v rámci jedné rodiny od asymptomatických až po těžce postižené jedince. Diagnostika je neurologická (elektrodiagnostika), molekulárněgenetická (analýza DNA) a klinická. Kauzální terapie CMT choroby neexistuje. Symptomatická léčba zahrnuje terapii medikamentózní, rehabilitační, protetickou a operační v závislosti na tíži a prognóze postižení a subjektivních potížích pacienta. Ortopedická operační léčba CMT se na našem pracovišti týká převážně oblasti nohy. Na základě dominujícího patologického obrazu provádíme kombinované operační výkony na měkkých tkáních, skeletu a kloubech. Taktika léčby vychází z individuality a potřeb pacienta a prognózy postižení. Pro strategii je charakteristická kombinace operací od jednodušších (měkké tkáně, jednoduché osteotomie) po složitější (artrodézy), tzv. krok za krokem, se snahou o maximální korekci deformity v jednom operačním sezení.
Charcot-Marie-Tooth disease is a genetically determined polyneuropathy presenting with muscle imbalance and contractures. It is often associated with bone and joint deformities of the feet and hands. The progress of the disease is very individual and clinical pictures can vary even within a single family, with some CMT individuals being almost asymptomatic while others being quite severely affected. CMT diagnosis is based on electromyographic studies and DNA analysis. Curative therapy is unknown. Symptomatic therapy involves medication, rehabilitation, prosthetic care and orthopaedic surgical procedures depending on actual clinical picture, subjective symptoms and progression. Orthopaedic surgical treatment mostly concerns feet deformities. Depending on clinical picture and imaging studies, soft tissue, bone and/or joint procedures can be done. Surgical treatment approach must be strictly individual based on expected progression of the disease. Our surgical treatment approach is to start with easier procedures done on soft tissues or simple osteotomies, later more complicated joint procedures and finally arthrodesis is often indicated. It is advisable to proceed step by step aiming for maximum bone and joint deformity correction within one surgical procedure.
This study evaluates primary and secondary clinical outcome measures in Charcot-Marie-Tooth disease type 1A (CMT1A) with regard to their contribution towards discrimination of disease severity. The nine components of the composite Charcot-Marie-Tooth disease Neuropathy Score and six additional secondary clinical outcome measures were assessed in 479 adult patients with genetically proven CMT1A and 126 healthy controls. Using hierarchical clustering, we identified four significant clusters of patients according to clinical severity. We then tested the impact of each of the CMTNS components and of the secondary clinical parameters with regard to their power to differentiate these four clusters. The CMTNS components ulnar sensory nerve action potential (SNAP), pin sensibility, vibration and strength of arms did not increase the discriminant value of the remaining five CMTNS components (Ulnar compound motor action potential [CMAP], leg motor symptoms, arm motor symptoms, leg strength and sensory symptoms). However, three of the six additional clinical outcome measures - the 10m-timed walking test (T10MW), 9 hole-peg test (9HPT), and foot dorsal flexion dynamometry - further improved discrimination between severely and mildly affected patients. From these findings, we identified three different composite measures as score hypotheses and compared their discriminant power with that of the CMTNS. A composite of eight components CMAP, Motor symptoms legs, Motor symptoms arms, Strength of Legs, Sensory symptoms), displayed the strongest power to discriminate between the clusters. As a conclusion, five items from the CMTNS and three secondary clinical outcome measures improve the clinical assessment of patients with CMT1A significantly and are beneficial for upcoming clinical and therapeutic trials.

To evaluate an influence of ascorbic acid on phenotype of CMT1A neuropathy. The cohort of sixty patients will be treated ( AA vs placebo) fot two years. The primary efficacy outcome will be an improvement on CMTNS, electro- physiological parameters, disability ( ONDS) and QoL ( SF 36).
Stanovení vlivu kyseliny askorbové na fenotyp osob s chorobou Charcot Marie Tooth 1A. Na souboru 60 osob bude sledován vliv kys.askorbové proti placebu během 24 měsíců. Hodnoceny jsou klinické škály CMTNS, dále EMG a dotazník SF 36. Cílem je změna fenotypu o 1 stupeň na škále CMTNS.
