BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) still has a relatively high complication rate, underscoring the importance of high-quality training. Despite existing guidelines, real-world data on training conditions remain limited. This pan-European survey aims to systematically explore the perceptions surrounding ERCP training. METHODS: A survey was distributed through the friends of United European Gastroenterology (UEG) Young Talent Group network to physicians working in a UEG member or associated states who regularly performed ERCPs. RESULTS: Of 1035 respondents from 35 countries, 649 were eligible for analysis: 228 trainees, 225 trainers, and 196 individuals who regularly performed ERCP but were neither trainees nor trainers. The mean age was 43 years, with 72.1% identifying as male, 27.6% as female, and 0.3% as non-binary. The majority (80.1%) agreed that a structured training regimen is desirable. However, only 13.7% of trainees and 28.4% of trainers reported having such a structured program in their institutions. Most respondents (79.7%) supported the concept of concentrating training in centers meeting specific quality metrics, with 64.1% suggesting a threshold of 200 annual ERCPs as a prerequisite. This threshold revealed that 36.4% of trainees pursued training in lower-volume centers performing <200 ERCPs annually. As many as 70.1% of trainees performed <50 annual ERCPs, whereas only 5.0% of trainers performed <50 ERCPs annually. A low individual trainee caseload (<50 ERCPs annually) was more common in lower-volume centers than in higher-volume centers (82.9% vs. 63.4%). CONCLUSIONS: The first pan-European survey investigating ERCP training conditions reveals strong support for structured training and the concentration of training efforts within centers meeting specific quality metrics. Furthermore, this survey exposes the low availability of structured training programs with many trainees practicing at lower-volume centers and 71% of all trainees having little hands-on exposure. These data should motivate to standardize ERCP training conditions further and ultimately improve patient care throughout Europe.

OBJECTIVE: To evaluate the role of clinical exchange programs in postgraduate obgyn training using the International Federation of Gynecology and Obstetrics (FIGO)-World Association of Trainees in Obstetrics and Gynecology (WATOG) One World Exchange (OWE), a clinical exchange program held in France in October 2023, as a case-study. METHODS: This was a cross-sectional study. A 31-item structured questionnaire designed with Google Forms was electronically distributed to the 51 obgyn postgraduate trainees (OWE fellows) who participated in the OWE, to collect information about the exchange. Collected data was analyzed using IBM Statistical Product and Service Solutions (SPSS) Statistics for Windows. RESULTS: The survey response rate was 68.6%. The mean age of the respondents was 33.0 ± 4.0 years. Majority of the them were females (26, 74.3%), married (19, 54.3%), at least in their third year of training (30, 85.7%) and from Africa (11, 31.4%). During the period of the exchange program, fellows observed various obstetric and gynecologic procedures, including open and minimal access procedures, with more than one-fifth (8, 22.9%) of them reporting that they were allowed to assist in some of these procedures. The fellows noted salient differences in practice between their exchange hospitals and their home countries. An overwhelming majority (30, 85.7%) of the fellows believed the OWE was beneficial and would positively impact their clinical practices back in their home countries. CONCLUSION: Clinical exchange programs like the OWE provide valuable benefits in improving the clinical knowledge and skills of postgraduate obgyn trainees.

Tento článek se zaměřuje na zkušenosti získané ze zahraniční stáže během ošetřovatelské praxe. Popisuje nejen přípravu a průběh stáže, ale také nabízí pohled na finský systém zdravotní péče, který podporuje digitalizaci a modernizaci. Dalšími tématy jsou pracovní podmínky sester a konkrétní návrhy na zlepšení českého zdravotnictví inspirované finským přístupem.
This article focuses on the experience gained during a nursing internship abroad. It describes not only the preparation and the course of the internship, but also offers insights into the Finnish healthcare system that supports digitalization and modernization. Other topics include the working conditions of nurses and concrete suggestions for improving the Czech healthcare system, inspired by the Finnish approach.

Celosvetovo patrí obezita k najrozšírenejším chronickým ochoreniam ako v dospelej, tak aj v detskej a adolescentnej populácii. V súčasnosti je jedným z najvýznamnejších problémov verejného zdravia nielen kvôli narastajúcej prevalencii, ale najmä asociácii so širokým spektrom ďalších chronických a život ohrozujúcich ochorení. Na Slovensku sa prevalencia nadhmotnosti pohybuje v rozmedzí okolo 63 %, prevalencia obezity okolo 29 %. V júni 2024 v Nature Medicine publikovala a zaviedla Európska spoločnosť pre štúdium obezity (European Association for the Study of Obesity – EASO) rámec na zosúladenie diagnostiky, hodnotenia závažnosti a liečby obezity so štandardmi iných chronických ochorení. Ciele liečby chronického ochorenia – (pre)obezity by mali byť holistické, mali by ísť „za“ pokles hmotnosti v kilogramoch, čo prináša so sebou dlhodobé prínosy pre zdravie, duševnú pohodu, fyzické fungovanie a zlepšenie kvality života. Holistické ciele môžeme dosiahnuť zmenou životného štýlu (behaviorálne, nutričné a pohybové intervencie). Zhodnotenie závažnosti ochorenia ovplyvňuje individuálnu liečbu (personalizovaná medicína) a v súčasnosti máme možnosť využívať kombináciu zmeny životného štýlu s farmakoterapiou, prípadne aj bariatrickými chirurgickými postupmi. V ostatnom čase nám pribudlo veľa nových informácií, výsledkov zo zaujímavých klinických štúdií týkajúcich sa farmakologického manažmentu založeného na báze inkretínov. V blízkej budúcnosti sa dočkáme aj ďalších noviniek zacielených na chronický manažment obezity.
Worldwide, obesity is one of the most widespread chronic diseases in the adult, child and adolescent population. It is currently one of the most significant public health problems not only due to its increasing prevalence, but especially due to its association with a wide range of other chronic and life-threatening diseases. In Slovakia, the prevalence of overweight is around 63 %, and the prevalence of obesity is around 29 %. In June 2024, the European Association for the Study of Obesity (EASO) published and introduced a framework to align the diagnosis, assessment of severity and treatment of obesity with the standards of other chronic diseases. The goals of treatment of chronic disease – (pre)obesity should be holistic, they should go “beyond” weight loss in kilograms, which brings with it long-term benefits for health, mental well-being, physical functioning and improved quality of life. Holistic goals can be achieved by changing lifestyle (behavioral, nutritional and exercise interventions). The assessment of the severity of the disease influences individual treatment (personalized medicine) and currently we have the opportunity to use a combination of lifestyle changes with pharmacotherapy, or even bariatric surgical procedures. Recently, we have received a lot of new information, results from interesting clinical studies related to pharmacological management based on incretins. In the near future we will also see other news aimed at chronic management of obesity.

Práca sa venuje otázkam prevalencie, diagnostiky a manažmentu obezity a s obezitou asociovaných komorbidít v populácii detí a adolescentov. Okrem štandardného spektra terapeutických možností (individuálna alebo skupinová modifikácia životného štýlu, nutričné odporúčania, modifikácia fyzickej aktivity, kognitívno-behaviorálna terapia a psychologická intervencia) od roku 2022 je na Slovensku možnosť medikamentóznej liečby liraglutidom u adolescentov s obezitou. Vzhľadom na stúpajúci trend ochorenia sa do popredia dostáva otázka vytvorenia novej infraštruktúry zdravotnej starostlivosti o dieťa či adolescenta s obezitou. Európska spoločnosť pre štúdium obezity (EASO) publikovala kritériá pre centrá manažmentu obezity s cieľom zlepšiť starostlivosť o pacientov s obezitou.
This paper addresses prevalence, diagnosis and management of obesity and obesity-associated comorbidities in the child and adolescent population. In addition to the standard spectrum of therapeutic options (individual or group lifestyle modification, nutritional recommendations, physical activity modification, cognitive-behavioral therapy and psychological intervention) the drug treatment with liraglutide has been available in Slovakia for adolescents with obesity since 2022. Due to the increasing trend of the disease, up to fourth comes the need of creating a new healthcare infrastructure for adolescents with obesity. The European Association for the Study of Obesity (EASO) has published criteria for obesity management centers with the aim of improving the care of patients with obesity.

BACKGROUND: The education of healthcare professionals, including nurses, represents a critical intersection with health systems science (HSS), which is often considered the third pillar of healthcare education alongside basic and clinical sciences. Despite the amount of research on nursing education during the COVID-19 pandemic, there remains a gap in analysis from an HSS perspective. METHODS: A Comparative Education Approach involving undergraduate nursing programs (UNPs) across Europe, with longitudinal data collection from 2002 to 2023. The aim of the study was threefold: (a) to summarize the overall changes in UNPs during the COVID-19 pandemic; (b) to identify the changes retained in the post-pandemic era; and (c) to identify the impact of the pandemic on nursing education as perceived by nurse educators across nine European countries. RESULTS: This study compares the changes in nursing education in European countries during and after the COVID-19 pandemic using a qualitative approach with data from nine universities. The COVID-19 pandemic had a significant impact on education provision. During the first wave, government restrictions forced a complete shift to online learning for theory classes, clinical training and laboratories. In subsequent waves, a hybrid format was chosen that combined online and face-to-face sessions. A major challenge was the placement of nursing students alongside general university students. This approach neglected their need for practical clinical training, which is crucial for their future careers. To compensate for the lack of clinical hours, various alternative teaching methods were introduced. Students were also offered the opportunity to volunteer in large-scale public health initiatives such as vaccination and testing campaigns, although bedside care for COVID-19 patients remained limited. The pandemic has also left its mark in the post-pandemic period. Some UNPs have retained elements of online education, notably lectures, research seminars, meetings, consultations and even online exams. Interestingly, an initial increase in applicant numbers was observed at six of the nine participating UNPs. CONCLUSIONS: The COVID-19 pandemic has widened the gap between university-educated nurses and the clinical setting, i.e. between theory and practise, underlining the importance of HSS in nursing education. Rebuilding strong partnerships is crucial, but simply returning to the pre-pandemic model is not enough. To ensure uninterrupted education during future crises, proactive planning, including the creation of predefined protocols for collaboration, is essential. The pandemic underscores the need for closer alignment between the two sectors, which would better equip future nurses with the skills they need to thrive in the nursing workforce and ensure they are prepared for the challenges of the 21st century.
